제조업 남자 근로자의 대사증후군 관리를 위한 참여형 개선기법(PAOT) 프로그램을 적용한 3년간의 효과분석 (Effectiveness of Participatory Action-Oriented Training (PAOT) Methods for the Management Metabolic Syndrome in Korean Manufacture Workers for Three-years)

  • Purpose: The purpose of this study was to examine the effectiveness of Participatory Action-Oriented Training (PAOT) programs in korean manufacture workers with metabolic syndrome for three years. Methods: A total of 890 workers with risks of metabolic syndrome were recruited from one workplace. The experimental group (n=51) received PAOT program for the preventive management on metabolic syndrome, while the control group (n=51) received conventional program. Results: In the experimental group who participated in PAOT program, percentage of achieved action plans was 73.4% at three months, 75.8% at six months, 82.8% at three years. After three years, the rate of risk factor in the experimental group has significantly decreased from $1.94{\pm}1.77$ to $0.76{\pm}1.25$ for moderate exercise, and from $1.72{\pm}2.01$ to $1.04{\pm}1.59$ for the strenuous exercise. Systolic and diastolic blood pressure has significantly decreased from $125.06{\pm}11.83$ to $117.65{\pm}19.94$, from $83.45{\pm}13.38$ to $76.39{\pm}8.09$ mmHg. In addition, risk factor score also decreased from $3.31{\pm}0.61$ to $1.41{\pm}0.89$. Conclusion: To improve the effectiveness of PAOT program for the management of risk factors of metabolic syndrome, there is a need to continue implement programs and analyze the long-term effects are required in the workplace.

간호사의 대사증후군 유발 위험요인과 건강증진행위간의 관계 (The Relationship between Metabolic Syndrome Risk Factors and Health Promotion Behavior in General Hospital Nurses)

  • 본 연구의 목적은 간호사의 대사증후군 위험 요인과 건강 증진 행위와의 관계를 알아보고자 하였다. 자료는 경기도에 있는 일 종합병원의 450명 간호사들이 구조화된 설문지를 이용하였다. 자료수집 기간은 2014년 10 월 6 일부터 10 월 31일까지였으며 분석은 SPSS/WIN 21.0 통계검증을 하였다. 병원에 근무하는 간호사의 대사증후군 유병률은 8.0%이었으며 건강증진행위 점수는 평균2.53점(범위 1.22~3.44)이었다. 대사증후군의 유병률이 높은 위험그룹은 나이가 26세~30세, 중환자실근무간호사, 음주를 주 1~2회하는 그룹이었다. 병원관리자는 대사증후군 유병률의 위험 개선과 예방을 위한 간호사들의 직업환경, 특히 대사증후군 유병률이 높은 위험군으로 분류된 그룹의 간호사들에게 집중관리가 이루어져야할 것으로 사료된다.

한국 성인의 대사증후군 및 그 구성요소가 성별에 따른 건강관련 삶의 질 손상에 미치는 영향: 2016-2018 국민건강영양조사 (The Influence of Metabolic Syndrome and its Components on the Impaired Health-related Quality of Life by Gender in Korean Adults: The 2016-2018 Korea National Health and Nutrition Examination Survey)

  • 본 연구의 목적은 국민건강영양조사 자료를 활용하여 한국 성인의 대사증후군 및 그 구성요소가 성별에 따른 건강관련 삶의 질 손상에 미치는 영향을 알아보기 위한 것이다. 참가자는 19세 이상 성인 16,657명이었으며, 대사증후군은 NCEP-ATP III에 따라 정의되었고, HRQOL는 EQ-5D를 사용하여 평가되었다. 연구 대상자의 대사증후군 유병율은 34.3%였고 남성이 여성보다 높았다. 건강관련 삶의 질 평균 점수는 남성이 96.7점, 여성이 94.5점으로 남성이 높았다. 인구학적, 건강행위, 건강관련특성을 보정변수로 통제 한 후 다중 로지스틱 회귀분석을 실시한 결과, 대사 증후군이 있는 여성은 대사증후군이 없는 여성에 비해 건강관련 삶의 질 손상의 교차비가 1.27(95% CI: 1.09-1.46)이었다. 대사증후군의 구성요소 중 복부비만(OR=1.72, 95% CI: 1.50-1.97)과 고혈압(OR=1.26, 95% CI: 1.06-1.50)은 건강관련 삶의 질 손상과 연관이 있었다. 반면, 남성에서는 유의하지 않았다. 결론적으로 여성의 건강관련 삶의 질 향상을 위해서는 여성의 특성을 고려한 대사증후군 위험요인을 감소시키는 접근전략이 필요함을 시사한다.

The Metabolic Syndrome is Associated with More Aggressive Prostate Cancer

  • Purpose: The aim of this study was to analyze any association between the metabolic syndrome (MetS) and risk of prostate cancer (PCa) and cancer grade among men undergoing radical prostatectomy for PCa. Materials and Methods: 50 patients with MetS and 50 patients without MetS who undervent radical prostatectomy (RP) were included in the study. Age at biopsy, height, weight, digital rectal examination (DRE), pre-biopsy PSA levels, prostate volume, histopathologic diagnosis after surgery and gleason scores were collected data from all patients. Histologic material obtained at biopsy was given a Gleason score; tumours with a Gleason score ${\geq}7$ were considered high grade and <7 were considered low grade. Results: The mean age at the time of biopsy was $63.7{\pm}5.94$ in patients with MetS and $61.6{\pm}6.14$ in patients without MetS. Men with MetS had significantly lower PSA levels (p=0.01) ($7.21{\pm}2.74$ and $8.81{\pm}2.72$, respectively). Also, the men with MetS had higher RP tumor grade (p=0.04). Conclusions: Men with MetS undergoing RP have lower PSA levels and have significantly higher grade PCa. We must be careful for screening PCa in patients with MetS. Although the patients had lower PSA levels, they may have high grade disease.

한열변증 점수를 이용한 한증과 열증의 건강 상태와 미병 특징 비교 (Comparison of Health Status and Mibyeong Characteristics between Cold Syndrome and Heat Syndrome by Cold Heat Syndrome Differentiation Score)

  • Objectives: The objective of this study is to develop the diagnostic tool to distinguish between cold syndrome (CS) and heat syndrome (HS). Methods: A total of 1,753 subjects were divided into three groups, those are CS group, intemediate group, and HS group, by the mean and standard deviation of the cold heat syndrome differentiation score using 7 point scale consisting of 9 items. Demographic characteristics, diseases history, health status, Mibyeong, syndrome differentiation were analyzed. Results: CS is characterized by women, elderly, and low body mass index. CS has a history of thyroid disease, cataract, depression, osteoporosis, and HS has a history of prostatomegaly. CS receives less social psychosocial stress than HS, and the quality of life associated with health status is lower than HS. CS group has the tendency to be tired, painful, sleepless, dyspeptic and anxious. Conclusions: CS is a set of symptoms associated with decreased energy metabolism and decreased metabolic function, and is more likely to be unhealthy than HS.

제2형 당뇨병 환자의 식사요법 실천 정도, 식이섭취 상태, 신체활동과 대사증후군 유병률과의 관계 (The Relations between Diabetic Dietary Compliance, Dietary Intake, and Physical Activity and the Prevalence of Metabolic Syndrome (MS) in Type 2 Diabetic Patients)

  • Objectives: The purpose of this study was to investigate the relations between diabetic dietary compliance and dietary intake, physical activity and prevalence of metabolic syndrome (MS) in type 2 diabetic patients. Methods: Seventy five subjects diagnosed with type 2 diabetes visiting the D hospital in Dongducheon from May 2014 to Dec 2014 were included in this study. The subjects were divided into two groups according to their diabetic dietary compliance score (median 39); low diabetic dietary compliance (LDDC) group (n=44) and high diabetic dietary compliance (HDDC) group (n=31). Survey data collection was carried out by direct interview method. The nutrient intake, food intake and KDDS (Korean's dietary diversity score), DVS (dietary variety score) and GMVDF (grain, meat, vegetable, dairy and fruit) were analyzed using data from the 24-recall method. Metabolic parameters were obtained from the hospital records. Data was analyzed using Chi-square test and general linear model adjusted for sex. Results: The prevalence of MS was 59.7% in the participating diabetic patients. The prevalence of MS of the HDDC (39.3%) was significantly lower than that of the LDDC (72.7%). The distribution of physical activity showed a significant difference between the groups (p=0.006). The intakes of seeds and nuts of the HDDC were significantly lower than those of the LDDC. Fat and vegetable fat intakes and % fat of energy in the HDDC were significantly lower than those in the LDDC. But, carbohydrate (CHO) and potassium intake and %CHO of energy in the HDDC were significantly higher than those in the LDDC. KDDS and GMVDF showed significant differences between groups (p=0.033; p=0.012). Conclusions: Continuous intervention by specialized nutritionists for long-term self-regulation is needed for diabetic patients, and the high compliance to diabetic diet and increasing physical activity may be effective in the prevention of metabolic syndrome.

걷기운동과 행동수정 프로그램이 비만여중생의 체조성, 체력 및 대사증후군 관련인자에 미치는 영향 (Effects of Walking and Behavior Modification Program on Body Composition, Physical Fitness and Metabolic Syndrome Related Factors in Obese Girls)

  • 비만 여중생을 대상으로 하여 12주간 행동수정 프로그램을 포함한 걷기운동을 실시한 결과 비만 여중생의 체조성, 체력, 대사증후군의 유병률 및 대사증후군 위험인자에서 긍정적인 변화를 가져오는 것을 확인하였다. 또한 다중선형회 귀분석 결과 체조성의 상완위팔근육둘레, 허리둘레, 신체발달지수, 체수분, 체지방량의 개선과 체력의 50m달리기 향상이 BMI, WHR, HDL-C, 공복혈당, 수축기혈압, HOMA지수를 개선시킬 수 있는 것으로 나타나 유산소성 걷기운동과 행동수정 프로그램에 의한 식사량조절과 식습관 변화가 비만청소년에 있어서 대사증후군에 의한 심혈관질환과 당뇨병, 심리적 우울증에 효과적인 프로그램이 될 수 있다고 생각된다. 그러나 피험자 수가 적고, 실험기간이 짧아 실험결과를 일반화하기에는 주의가 필요하며, 실험이후에 학생들의 지속적 운동 습관을 유지하기 위한 프로그램의 개발이 필요하다고 생각된다.

생활습관 요인이 대사증후군 유병 위험에 미치는 영향 (Impact of Life Style Characteristics on Prevalence Risk of Metabolic Syndrome)

  • Purpose: The goal of this study was to evaluate the impact of life style characteristics on the prevalence risk of metabolic syndrome (MS). Methods: A total of 581 adults were recruited from a cardiovascular outpatient clinic. A newly developed comprehensive life style evaluation tool for MS patients was used, and patient data related to the MS diagnosis were reviewed from the hospital records. Results: The overall prevalence of MS was 53.2%, and the mean of MS score was 2.6 for patients at a cardiovascular outpatient clinic (78% of the patients had hypertension). Dietary habits among the life style characteristics had significant influence on the prevalence risk of MS and MS scores. And also interestingly, the classification and regression tree (CART) model suggested that the high prevalence risk groups for MS were older adults (61.5$\leq$age<79.4), and adults between 48.5 and 61.5 yr of age with bad dietary habits. Conclusion: This study indicates that nurses should focus on dietary habits of patients (especially patients classified as high prevalence risk for MS) for improvement and prevention of MS prevalence risk.

대사증후군 대상자의 폐쇄성수면무호흡증 위험성 관련요인 (Factors Associated with Obstructive Sleep Apnea Risk in Patients with Metabolic Syndrome)

  • Purpose: This study was conducted to identify the factors associated with obstructive sleep apnea (OSA) risk in patients with metabolic syndrome (MS). Methods: Patients with MS between 30 and 74 years of age were recruited in an outpatient clinic of a cardiovascular center in Seoul, South Korea. MS and the risk of OSA were evaluated by Berlin questionnaire survey, the medical records of the participants were reviewed and a comprehensive lifestyle survey was performed. SPSS WIN 21.0 was used for statistical analysis. Results: BMI (OR: 1.31, CI: 1.14-1.51, p<.001) and lifestyle score (OR: 0.96, CI: 0.93-0.99, p=.028) were associated with the risk of OSA. Physical activity, weight control and diet were specifically associated with the risk of OSA after controlling for age, gender and BMI. Conclusion: This study demonstrated that lifestyle was an important factor associated with OSA risk in patients with MS.

Flavonoids Fraction of Mespilus Germanica Alleviates Insulin Resistance in Metabolic Syndrome Model of Ovariectomized Rats via Reduction in Tumor Necrosis Factor-α

  • Objectives: The rate of metabolic syndrome (MetS) in women diagnosed as they age is one of the main concerns of health cares. Recently new strategies used to prevent progressions of MetS toward the diagnosis of diabetes have focused on plant flavonoids. This study was aimed to investigate the beneficial effects of flavonoids fraction of Mespilus germanica leaves (MGL) on MetS in ovariectomized (OVX) rats. Methods: Twenty-four adult female Wistar rats, weighing 200 to 250 g, were divided into 3 groups: Sham surgery, OVX + Salin, or OVX + Flavonoid. Three weeks after ovariectomy, animals displayed MetS criteria received flavonoid injection (10 mg/kg, intraperitoneally) for 21 days. Then the body weight, body mass index, waist circumference, visceral fat, fasting blood glucose, serum insulin, lipid profiles and tumor necrosis $factor-{\alpha}$ ($TNF-{\alpha}$) were measured. Results: Treatment with flavonoids fraction of MGL significantly decreased serum level of insulin (P = 0.011), glucose (P = 0.024), $TNF-{\alpha}$ (P = 0.010), also MetS Z score (P = 0.020) and homeostasis model assessment of insulin resistance (P = 0.007). Lipid profiles and visceral fat showed insignificant reduction. Conclusions: Flavonoids of MGL attenuates some of the MetS components possibly via reduction in $TNF-{\alpha}$ inflammatory cytokine.
