  • The Metabolic syndrome is a constellation of lipid and non-lipid factors of metabolic origin. The presence of any 3 of the following factors is considered sufficient for diagnosis : hypertension(BP <130/85mmHg), low HDL-cholesterol (<40mg% in men, <50mg% in woman), high triglyceride(>150mg%) and abdominal obesity(abdominal girth >102cm in men, 88cm in woman). The major adverse consequence of the metabolic syndrome is cardiovascular disease. Several studies have shown an association between metabolic syndrome and increased cardiovascular events. In Korea, the prevalence of the factor of metabolic syndrome has been increasing since 20 years previously when the Korean economy began to grow rapidly, with a resultant change in lifestyle, toward that of western countries. Thus, the management of the metabolic syndrome is an important social and medical issue in terms of the national health problem. This review will consider each factor in turn, providing insight for health care providers in an effort to prevention of cardiovascular events and maintenance of quality of life in persons with metabolic syndrome was discussed.

중풍환자에서 대사증후군과 적혈구변형능의 관련성 (The Relationship between Metabolic Syndrome and Erythrocyte Deformability in Small Vessel Disease Stroke Patients)

  • Objectives : The aim of this study was to assess the relationship between metabolic syndrome and erythrocyte deform ability in acute stroke patients. Methods : Among 88 of the recruited patients, 52 were diagnosed as metabolic syndrome. We assessed their general characteristics, risk factors. We compared the assessed variables between metabolic syndrome and control group. We analyzed the relationship between metabolic syndrome and erythrocyte deform ability. We analyzed relationship between cardiovascular risk factors and erythrocyte deformability. Results : The general characteristics waist and hip circumference, waist/hip ratio were higher in metabolic syndrome group. The metabolic syndrome group was also diagnosed with hypertension, DM, and hyperlipidemia more often than the control group. The blood test metabolic syndrome group showed higher triglycerides, total lipids, fasting blood sugar, and 2 hours postprandial plasma glucose level and lower HDL-cholesterol than the control group. There were more patients diagnosed with Dampness-Phlegm in the metabolic syndrome group. There were more patients showing lower erythrocyte deform ability in the metabolic syndrome group. The plasma homocysteine level was negatively correlated with erythrocyte deform ability. Conclusion : The results reconfirmed that the risk factors are more in metabolic syndrome group. The results indicated that metabolic syndrome lead to a lower erythrocyte deform ability in small vessel disease stroke patients. The Plasma homocysteine level was negatively correlated with erythrocyte deform ability.

  • Purpose: This study was to investigate the sex differences in risk of cardiovascular disease(CVD), depression and self-care activities in type 2 diabetes with metabolic syndrome. Methods: The descriptive correlational design was conducted using a convenient sample. One hundred and twenty-nine diabetic patients with metabolic syndrome were recruited in a university hospital. The data were analyzed using descriptive statistics, the Pearson correlation coefficient, Students' t-test, one way ANOVA, and stepwise multiple regression with SPSS/WIN 12.0. Results: The risk of CVD in diabetic patients with metabolic syndrome indicated a significantly negative correlation to self-care activities and age, and positive correlation to waist_C, SBP, DBP and TG. The metabolic syndrome is associated with an approximate 1.7-fold increase in the relative risk in CVD in diabetic women. The main significant predictors influencing CVD risk of diabetes with metabolic syndrome were age, waist_C, SBP and TG, which explained about 29.7%. Conclusion: These results indicate that patients with diabetes with metabolic syndrome with a high degree of waist_C, SBP and TG are likely to be high in risk of CVD.

제2형 당뇨병환자에서 혈중 글루카곤 농도와 심혈관 질환 위험도의 관계 (The relationship between glucagon levels and cardiovascular risk in patients with type 2 diabetes)

  • Glucagon regulates glucose and fat metabolism as well as being involved in the production of ketone bodies. The new antidiabetic drug, a sodium-glucose co-transporter-2 inhibitor, increases glucagon, and reduces the risk of cardiovascular death and hospitalization due to heart failure. The presence of metabolic syndrome is an important risk factor for cardiovascular diseases(CVD) in type 2 diabetes(T2DM) patients. We, thus, investigated the association between glucagon levels and metabolic syndrome in T2DM patients. This cross-sectional study involved 317 T2DM patients. Fasting and postprandial (30 min after ingestion of a standard mixed meal) glucagon levels were measured. Metabolic syndrome was defined according to the criteria of the International Diabetes Federation. A multiple regression logistic analysis was employed for statistical evaluation. A total of 219 (69%) subjects had metabolic syndrome. The fasting and postprandial glucagon levels did not differ between the group with metabolic syndrome and the group without. Postprandial glucagon levels increased significantly with the increase in the number of metabolic syndrome components, but the fasting levels did not. However, a hierarchical logistic regression analysis revealed that the postprandial glucagon levels did not contribute significantly to metabolic syndrome even after adjusting for other covariates. Fasting and postprandial glucagon levels are not associated with metabolic syndrome in T2DM patients. However, further studies are needed to investigate the relationship between glucagon and cardiovascular risk in patients with T2DM.

Age at Menarche and Brachial-ankle Pulse Wave Velocity in Women with Metabolic Syndrome

  • Early age at menarche, which is indicator of early biological maturity, has been shown to be associated with increased adult body mass index. Early menarche has also been associated with many cardiovascular disease risk factors and metabolic syndrome. To evaluate the impact of menarche to cardiovascular risk factor, we assessed by age at menarche, brachial-ankle pulse wave velocity (baPWV), which represents arterial stiffness, in women with or without metabolic syndrome. The subjects recruited for this study were three hundred one women. Relatively early menarche and relatively late menarche were classified according to less than $50^{th}$ percentile for relatively early menarche, and great than the $50^{th}$ percentile for relatively late menarche. Subject were divided four group, 1) women who had not adulthood metabolic syndrome and relatively early menarche, 2) women who had not adulthood metabolic syndrome and relatively late menarche, 3) women who had adulthood metabolic syndrome and relatively early menarche, 4) women who had adulthood metabolic syndrome and relatively late menarche. Women who had a relatively early menarche with adulthood metabolic syndrome had significantly high levels of blood pressure, triglyceride, fasting insulin and homeostatic model assessment of insulin resistance (HOMA-IR) levels than women with late menarche with adulthood metabolic syndrome, and had significantly lower HDL-cholesterol levels. And also, women who underwent a relatively early menarche with metabolic syndrome had highest level of baPWV in adult. In this study we found effect of age at menarche on adulthood metabolic risk factors for cardiovascular disease (e.g., baPWV, insulin resistance, hyperlipidemia) in Korean women.

제2형 당뇨병환자에서 대사증후군 동반에 따른 영양소 섭취상태 비교 및 심혈관질환과의 관련성 (Comparative of the Nutritional Status and Correlation of Cardiovascular Disease in Type Ⅱ Diabetes Mellitus Patients with Metabolic Syndromes)

  • This study was designed to investigate the correlation between nutrition status and cardiovascular disease in type 2 diabetes patients with metabolic syndrome. The subjects were 66 patients and divided into Non MS (a group without metabolic syndrome, n = 37) and MS (a group with metabolic syndrome, n = 29). The percentage of patients accompanying metabolic syndrome was 43.9% and family history such as DM, skipping meal and eating speed were higher in MS (P < 0.05) The average values of BMI, body fat (%), waist circumference were significantly higher in patients of MS than that of Non MS. For hamatological values, MS showed higher FRS, HOMA-IR, LDL-Cholesterol, CRP. Percentage of FRS was 21.63% in MS that is relatively higher in comparison with 16.81% in Non MS. Moreover, the incidence of cardiovascular disease appeared 13.8% in MS that is higher than 2.7% in Non MS. The intake of sodium and vitamin E were higher, but the intake of fat, vitamin A and zinc were lower in MS than in Non MS. Close correlations were elucidated among FRS, occurrence of cardiovascular disease, weight, waist circumference, Total-Cholesterol, LDLCholesterol, sodium in both groups. In conclusion, cardiovascular disease risk factors would be higher in type 2 diabetes patients with metabolic syndrome and there were distinctive patterns that were associated with hamatological values, nutrition intake risk factors. This result should be considered when designing nutrition study and intervetion programs.

우리나라 성인의 고감도 C-반응성 단백과 대사증후군의 관련성: 성별 분석 (Association between High Sensitivity C-Reactive Protein and Metabolic Syndrome in South Korea: A Gender-Specific Analysis)

  • Background: Metabolic syndrome has been known as a risk of cardiovascular disease. Meanwhile, high sensitivity C-reactive protein (hs-CRP) is used as a predictor of cardiovascular disease. In this paper, we aimed to investigate the association between hs-CRP and metabolic syndrome. Method: A total of 7,633 were chosen as the study population from the 7th Korea National Health and Nutrition Examination Survey dataset (2016-2017). Our dependent variable was whether an individual had metabolic syndrome or not, and the independent variable of interest was hs-CRP which was categorized into three groups. The chi-square tests and hierarchical logistic regression analyses reflecting survey characteristics were conducted. All analyses were stratified by gender. Results: According to the adjusted model with all covariates, compared to individuals having the low risk of hs-CRP, those having its average risk were more likely to have metabolic syndrome in men (odds ratio [OR], 1.41; 95% confidence interval [CI], 1.12-1.76) and women (OR, 1.69; 95% CI, 1.33-2.16). Individuals having the high risk was not significantly different in men; however, they were more likely to have metabolic syndrome in women (OR, 2.03; 95% CI, 1.28-3.23). Conclusion: In an upcoming aging society, it is important to reduce the risk of metabolic syndrome to improve population health. This study suggests that hs-CRP may be used as a marker of the risk of metabolic syndrome in a gender-specific way, thereby contributing to enhancing awareness of the risk of metabolic syndrome among the general public.

Gender difference in the association of metabolic syndrome with hs-CRP Concentration of Blood

  • The high sensitivity C-reactive protein (hs-CRP) as one of the typical acute phase reactants is used for predictive factor of the cardiovascular disease and diabetes mellitus. In addition, there are claims that must be included as factors of metabolic syndrome. This research examined the relationship between the concentration of hs-CRP in blood and risk factors of the metabolic syndrome by gender, and the rates of metabolic syndrome depending on the hs-CRP level based on the general public who took the comprehensive medical check-up at Chonbuk National University Hospital in the Jeonbuk province. The subjects aged 17-87 years were participated, and 2,000 people were included as the final subjects except the persons with more than 10 mg/L of the hs-CRP of blood level. The hs-CRP concentrations increased according to the number of risk factors of metabolic syndrome in both men and women. In regards to the risk ratio of metabolic syndrome based on hs-CRP level in blood according to gender, the risk ratio increased by 3.07 times in male and 4.55 times in female intermediate risk group and 3.60 times in male and 6.15 times in female high risk group compared to hs-CRP low risk group. As a result, there was a proportional relation between hs-CRP level and the occurrence of metabolic syndrome, and it occurs more frequently among women than men.

  • Objectives: The prevalence of metabolic syndrome has recently increased, Payments from the Korea Labor Welfare Corporation for compensation for mortality in workers caused by cardiovascular and cerebrovascular diseases have also increased in Korea in recent years. The association of metabolic syndrome and cardiocerebrovascular disease has been investigated by several researchers in recent studies, This study was conducted in an attempt to characterize the relationship between metabolic syndrome and Korean cardiocerebrovascular disease risk assessment, and to provide basic data to group health practices for the prevention of cardiocerebrovascular disease. Methods: Health examinations were previously conducted for 1526 male researchers at a private laboratory. The prevalence by age and the odds ratio of metabolic syndrome scores into the "cardiocerebrovascular risk group" (sum of low, intermediate, and high risk groups) of the Korean cardiocerebrovascular disease risk assessment were assessed, in an effort to elucidate the associations between metabolic syndrome and cardiocere brovascular disease risk assessment. Results: The prevalence of metabolic syndrome and inclusion in the cardiocerebrovascular risk group was 11,7% and 22.1% respectively. The severity of metabolic syndrome and cardiocerebrovascular risk assessment showed that individuals in their 40's and 50's were at higher risk than those in their 30's (p<0,001). The age-adjusted odds ratio of metabolic syndrome to cardiocere brovascular risk group inclusion was 5.6. Conclusions: An active prevention program for cardiocerebrovascular disease needs to begin in the 40's, as the prevalence of metabolic syndrome and the risk group of cardiocerebrovascular risk assessment peak in the 40's age group. The odds ratio between metabolic syndrome and the cardiocerebrovascular risk group was high, which indicates that metabolic syndrome scores should be utilized as guidelines during the consultation and behavioral modification program for the workplace prevention of cardiocerebrovascular diseases in group health practices.

급성기 뇌경색 환자의 사상체질에 따른 대사증후군 분포 연구 (A Cross-sectional Study on the Distribution of Metabolic Syndrome according to Sasang Constitution in acute stroke patients)

  • Objectives : This study was aimed to assess the distribution of metabolic syndrome according to Sasang constitution(SC) in acute stroke patients. Methods : We assessed the type of SC of acute stroke patients in the oriental medical hospitals of 3 universities located in metropolitan region from October 2005 to June 2007 by Ouestionnaire for the Sasang Constitution Classification II(QSCC II). Then, we investigated the general characteristics, the prevalence of each factor of the metabolic syndrome and the prevalence of the metabolic syndrome according to SC. Results : 338 subjects were included into the final analysis. 1. This study showed that the proportion of So-yang was highest of all constitutions(45.9%). 2. The prevalence of high TG, low HDL-chol, high BP, high FBS of Tae-um were higher than those of the other constitutions, but the differencecs were not statistically significant. The prevalence of high WC of Tae-um was significantly higher than other constitutions(p<0.001). 3. The prevalence of metabolic syndrome of Tae-um was significantly higher than other constitutions (p=0.002). Conclusion : There were significant differences in the prevalence of metabolic syndrome according to SC in acute stroke patients. For the further research, it seems to be necessary to construct fundamental databases for stroke by increasing the number of patients and by diagnosing SC more delicately.
