Climate Change and Health - A Systemic Review of Low and High Temperature Effects on Mortality (기후변화와 건강 - 저온과 고온이 사망에 미치는 영향에 관한 체계적 고찰)

  • Objectives: The impact of climate change on the health has been of increasing concern due to a recent temperature increase and weather abnormality, and the research results of the impact varied depending on regions. We synthesized risk estimates of the overall health effects of low and high temperature taking account of the heterogeneity. Methods: A comprehensive literature search was conducted using PUBMED to identify journal articles of low and/or high temperature effects on mortality. The search was limited to the English language and epidemiological studies using time-series analysis and/or case-crossover design. Random-effect models in meta analysis were used to estimate the percent increase in mortality with $1^{\circ}C$ temperature decrease or increase with 95% confidence intervals (CI) in cold or hot days. Results: Twenty three studies were presented in two tables: 1) low temperature effects; 2) high temperature effects on mortality. The combined effects of low and high temperatures on total mortality were 2% (95% CI, 1-4%) per $1^{\circ}C$ decrease and 4% (95% CI, 2-5%) per $1^{\circ}C$ increase of temperature, respectively. Conclusions: This meta analysis found that both low and high temperatures affected mortality, and the magnitude of high temperature appeared to be stronger than that of low temperature.

The Effects of Robot-Assisted Gait Training for the Patient With Post Stroke: A Meta-Analysis (뇌졸중 환자에게 적용한 로봇보행 재활훈련의 효과: 메타분석)

  • Robot-assisted rehabilitation therapy has been used to increase physical function in post-stroke patients. The aim of this meta-analysis was to identify whether robot-assisted gait training can improve patients' functional abilities. A comprehensive search was performed of PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), Physiotherapy Evidence Database (PEDro), Academic Search Premier (ASP), ScienceDirect, Korean Studies Information Service System (KISS), Research Information Sharing Service (RISS), Korea National Library, and the Korean Medical Database up to April, 2014. Fifteen eligible studies researched the effects of robot-assisted gait training to a control group. All outcome measures were classified by International Classification of Functioning, Disability, and Health (ICF) domains (body function and structures, activity, and participation) and were pooled for calculating the effect size. The overall effect size of the robot-assisted gait training was .356 [95% confidence interval (CI): .186~.526]. When the effect was compared by the type of electromechanical robot, Gait Trainer (GT) (.471, 95% CI: .320~.621) showed more effective than Lokomat (.169, 95% CI: .063~.275). In addition, acute stroke patients showed more improvement than others. Although robot-assisted gait training may improve function, but there is no scientific evidence about the appropriate treatment time for one session or the appropriate duration of treatment. Additional researchers are needed to include more well-designed trials in order to resolve these uncertainties.

A Meta-Analysis of the Variables related to Agitation in Patients with Dementia (치매 환자의 초조행동 관련변인에 관한 메타분석)

  • Purpose: The purpose of this study was to provide basic data about agitation in patients with dementia by surveying the literature. Method: Key words used for search through hand-search and electronic database (CINHAL, Pubmed, Google scholar, Riss, Kiss, DBpia) included 'dementia', 'Alzheimer disease', 'agitation', 'aggression or aggressive behavior', 'problem or disruptive behavior', and 'abnormal behavior.' Seventeen studies met the inclusion criteria for the Meta-analysis and 'R' version 3.2.2 was used to analyze the correlated effect size. Results: Study results showed that variables related to agitation were identified as the demographic (age, gender), dementia-related (cognition, medication uses), physical (Activity of Daily Living [ADL], pain), psychological (depression, psychotic symptom, caregiver burden) and environmental (psychosocial environment) factors. The effect size between the correlated variables and agitation were low to moderate (caregiver burden .36; ADL -.24; psychotic symptom and depression .21; pain .19; cognition -.15; medication uses .12; and psychosocial environment -.12). Conclusion: Based on the findings of this study, strategies to improve patients' depressive and psychotic symptoms and ADL and to reduce caregivers' burden are needed for prevention and management of agitation in patients with dementia.

Responsiveness of Gait Speed to Physical Exercise Interventions in At-risk Older Adults: A Systematic Review and Meta-Analysis

  • Background: In at-risk older adults, gait speed is an important factor associated with quality of life and falling risk. In this study, we assessed whether therapeutic exercise could improve gait speed. Methods: We conducted a meta-analysis to evaluate the 'best' therapeutic exercise method by analyzing each exercise in terms of intensity, type, and several gait speed indices. For the analysis, we gathered 122 papers through a database search and selected 9 (n=627) that were appropriate for the meta-analysis. Results: In 8 of the 9 included papers, gait speed improved with therapeutic exercise. Usual gait speed (n=246) improved more than maximal gait speed (n=574). A resistance program was more effective than a nonresistance program for improving maximal, but not usual, gait speed. We also found that the effects of therapeutic exercise were greater in noncommunity than in community-dwelling elderly people. Conclusion: In conclusion, therapeutic exercise was effective in improving gait speed.

Effects of Sling Exercise on Balance and Gait in Patients with Stroke: Meta-Analysis of Studies in Korea

  • Background: Stroke patients have multiple disorders, but most have problems with balance and gait. Post-stroke rehabilitation exercise has been shown to be very important for functional recovery. Objectives: To systematically review and meta-analyze the effects of sling exercise (SE) on patients with stroke in studies published in Korea. Design: Meta-analysis. Methods: Five databases, namely, RISS, KISS, NDSL, DBpia and Earticle, were used to collect articles on vibration. Keywords such as "Stroke," "Hemiplegia," "Sling," and "Sling Exercise" were used in the search for published articles. Interventions and comparisons were SE and other exercise (OE). Outcome measures were berg balance scale (BBS) and timed up and go (TUG). Consequently, eight studies were selected in the second screening using meta-analyses. Results: Based on the results of the meta-analysis, SE was effective for BBS in the comparison of SE and OE [2.71, 95% CI (1.42, 4.01)]. It was effective for TUG in the comparison of SE and OE [-1.89, 95% CI (-3.01, -0.77)]. Conclusion: Based on eight limited studies, SE improved BBS and TUG over OE, suggesting improved stroke balance and gait. Therefore, more studies and large-scale sample randomized controlled trials are needed to confirm clinical application.

Meta-Heuristic Algorithms for a Multi-Product Dynamic Lot-Sizing Problem with a Freight Container Cost

  • Lot sizing and shipment scheduling are two interrelated decisions made by a manufacturing plant and a third-party logistics distribution center. This paper analyzes a dynamic inbound ordering problem and shipment problem with a freight container cost, in which the order size of multiple products and single container type are simultaneously considered. In the problem, each ordered product placed in a period is immediately shipped by some freight containers in the period, and the total freight cost is proportional to the number of containers employed. It is assumed that the load size of each product is equal and backlogging is not allowed. The objective of this study is to simultaneously determine the lot-sizes and the shipment schedule that minimize the total costs, which consist of production cost, inventory holding cost, and freight cost. Because the problem is NP-hard, we propose three meta-heuristic algorithms: a simulated annealing algorithm, a genetic algorithm, and a new population-based evolutionary meta-heuristic called self-evolution algorithm. The performance of the meta-heuristic algorithms is compared with a local search heuristic proposed by the previous paper in terms of the average deviation from the optimal solution in small size problems and the average deviation from the best one among the replications of the meta-heuristic algorithms in large size problems.

Effect of Breastfeeding Individual Visits Program: A Systematic Review and a Meta-Analysis (모유 수유 개별 방문 프로그램의 효과: 체계적 문헌고찰과 메타분석)

  • Purpose: This study was a systematic review and meta-analysis designed to evaluate the effects of breastfeeding individual visit program. Methods: Based on the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA), a systematic search was conducted using four core electronic databases from October 28, 2018 to November 14, 2018. The topics of breastfeeding individual visit program were analyzed using descriptive analysis and the effects of intervention were meta-analyzed using the Review Manager 5.3 software. Results: A total of 13 studies were included in the review and 12 were included for meta-analysis. Breastfeeding individual visit programs were found to be more effective in breastfeeding rates, attitude, skill and self-efficacy compared to group education or non-education. Conclusion: Effective breastfeeding education can help mothers to start and sustain breastfeeding. Breastfeeding individual visits programs that are more effective than group education to maintain breastfeeding for pregnant women should be developed and applied.

타부탐색(Tabu Search)의 확장모델을 이용한 '외판원 문제(Traveling Salesman Problem)' 풀기

  • In solving the Travel Salesman Problem(TSP), we easily reach local optimal solutions with the existing methods such as TWO-OPT, THREE-OPT, and Lin-Kernighen. Tabu search, as a meta heuristic, is a good mechanism to get an optimal or a near optimal solution escaping from the local optimal. By utilizing AI concepts, tabu search continues to search for improved solutions. In this study, we focus on developing a new neighborhood structure that maintains the feasibility of the tours created by exchange operations in TSP. Intelligent methods are discussed, which keeps feasible tour routes even after exchanging several edges continuously. An extended tabu search model, performing cycle detection and diversification with memory structure, is applied to TSP. The model uses effectively the information gathered during the search process. Finally, the results of tabu search and simulated annealing are compared based on the TSP problems in the prior literatures.

Harmony search algorithm for optimum design of steel frame structures: A comparative study with other optimization methods

  • In this article, a harmony search algorithm is presented for optimum design of steel frame structures. Harmony search is a meta-heuristic search method which has been developed recently. It is based on the analogy between the performance process of natural music and searching for solutions of optimization problems. The design algorithms obtain minimum weight frames by selecting suitable sections from a standard set of steel sections such as American Institute of Steel Construction (AISC) wide-flange (W) shapes. Stress constraints of AISC Load and Resistance Factor Design (LRFD) and AISC Allowable Stress Design (ASD) specifications, maximum (lateral displacement) and interstorey drift constraints, and also size constraint for columns were imposed on frames. The results of harmony search algorithm were compared to those of the other optimization algorithms such as genetic algorithm, optimality criterion and simulated annealing for two planar and two space frame structures taken from the literature. The comparisons showed that the harmony search algorithm yielded lighter designs for the design examples presented.
