  • The article provides an examination of those temporary states of disturbances in the psychophysiological and socio-psychological spheres of a person, which develop against the background of normally formed mechanisms of mental activity, that is, in mentally healthy people. A healthy person is a person adapted to the conditions of the external environment, and mental health is inseparable from the physical and social well-being of a person. The cause of the disorder is the impact on a person of any environmental factors that weaken the mechanisms of social and psychological adaptation of the individual.

  • Coronavirus disease 2019 (COVID-19), which was a global pandemic caused by severe acute respiratory syndrome corona virus 2 (SARS-CoV-2), is still a serious public health problem. COVID-19 causes various symptoms not only in the respiratory system but also in various parts of the body and has a significant effect on sleep. Insomnia and poor sleep quality were observed at high rates in patients with COVID-19 as well as in the uninfected general population. Obstructive sleep apnea is also considered a risk factor in patients with severe COVID-19. Virus-induced central nervous system damage is likely to be the cause of many sleep disorders in COVID-19, but psychosocial influences also seem to have played a significant role. Sleep problems persisted at high rates for a considerable period after the infection phase was over. More attention and research on the effect of COVID-19 on sleep is needed in the future.

3차원 물체의 심상회전이 만성 편마비 뇌졸중 환자의 반응시간에 미치는 영향 (Changes in Reaction Time during Mental Rotation of Three-Dimensional Objects for Chronic Hemiparetic Stroke)

  • 심상회전은 어떤 대상이나 형태를 심상으로 하나의 각도에서 다른 각도로 회전시키는 활동으로, 일상생활동작을 수행할 때 환경과 자신에 대한 위치 및 움직임을 계획하는 단계에서 필요한 기능 중 하나이다. 본 연구는 정상인과 만성 편마비환자를 대상으로 반응시간(reaction time)과 정답율(precision)에 심상회전과제가 미치는 영향을 알아보고, 임상에서 편마비 환자를 치료할 때 심상회전능력의 변화가 과제수행에 장애요인이 될 수 있는지를 알아보기 위함이다. 본 연구는 만성 편마비 환자 31명과 정상성인 20명이 참여하였다. 연구대상자는 거울상(mirror image)을 이용한 대조과제(comparison task)와 각도불일치상(angular disparity image)을 이용한 회전과제(rotation task) 등의 2 가지 유형의 심상회전과제를 수행하였다. 검사자는 대상자에게 한 쌍의 상에 대하여 동일한 상인지 아닌지를 '예'와 '아니오'로 가능한 한 빠르게 대답하라고 요구하였고, 상을 노출한 시간부터 대상자가 대답한 시간까지의 반응시간과 정답률을 측정하였다. 대조과제 및 회전과제에 대한 반응시간은 편마비 환자에서 정상인에 비하여 유의하게 느리게 나타났다. 그러나 편마비 환자와 정상인은 심상회전과제에 대한 정답율에서 유의한 차이를 보이지 않았다. 본 연구에서 편마비 환자군과 정상군 모두에서 쌍을 이룬 상의 각이 불일치할 때 반응시간이 느리게 나타났지만, 정상군보다 편마비 환자군에서 심상회전에 오랜 시간이 걸렸다. 따라서 재활과정에서 편마비 환자를 대상으로 치료계획을 수립하고 치료를 할 때, 회전각도를 이용한 난이도 조절이 가능할 것으로 사료된다. 또한 본 연구결과는 정상인에 비하여 편마비 환자의 심상회전에 차이가 있다는 것을 재활과정에서 치료사는 염두 해 두어야 한다고 제안한다.

복부외상환자의 예후에 영향을 미치는 인자들에 대한 분석 (Analysis of the Prognostic Factors for Abdominal Trauma)

  • Purpose: Recently, trauma is more frequent due to the increases in the population, the number of traffic accident, and the incidence of violence. Especially, abdominal trauma is a leading cause of morbidity and mortality. We analyzed the clinical features and the factors associated with morbidity and mortality. Methods: We analyzed 136 patients of abdominal trauma who were admitted at the Department of Surgery, Chonnam National University Hospital, from January 2003 to June 2005. We analyzed the cause of trauma, the injured organ, combined injuries, mental status, blood pressure, laboratory findings, morbidity, and mortality. The relationships between by variable were assesed by using the independent samples test and the Kruskal?Wallis test. Results: The causes of trauma were traffic accidents (98 cases, 72%), falling accidents (9 cases, 6.6%), violence (6 cases, 4.4%), and stab injuries (6 cases, 4.4%). The injured organs were the small intestines (47 cases, 34.6%), the liver (35 cases, 25.7%), the spleen (26 cases, 19.1%), the mesentery (17 cases, 12.5%), the large intestines (15 cases, 11.0%), the pancreas (14 cases, 10.3%), etc. The most common combined injury was chest injury (53 cases, 39%). Comatose or semicomatose mental status and shock on admission (<60 mmHg in systolic) were related to high mortality (85.7%). In laboratory findings, decreased hemoglobin (<8 g/dL), and platelet count (<$50,000/mm^3$), and increased creatinine level (>1.6 mg/dL) were significant prognostic factors. The incidence of postoperative complications was 40.4%, and frequent complications were wound infection (8.1%) and re-bleeding (8.1%). The overall mortality rate was 18.4%, and most common cause was hypovolemic shock (18 cases, 13.2%), however, there was no statistical difference according to injurd organ. Conclusion: In the multivariate analysis, mental status, hemoglobin, and serum creatinine level were the most significant prognostic factors. When an abdominal trauma patient arrives at the emergency room, a rapid and accurate evaluation of the patient's status and risk factors, and resuscitation, if necessary, have to be performed to lower the morbidity and mortality.

칠정상(七情傷)의 기전에 대한 고찰 - 감정적 요인을 중심으로 - (Emotional factors in the mechanism of Chiljeongsang(七情傷))

  • This paper is about the emotional factors in Chiljeongsang(七情傷). To specify the collective term into specific categories, an examination of the meaning of emotion of the East and West was primarily undertaken. The importance of emotion lies in the fact that it provides a link between the individual and the outside world. Emotion was perceived as reflections of the human nature and mental activity by philosophers and psychiatrists throughout history. In the views of psychologists, the focus was on not emotion itself, but rather the emotional experience in human, and its psychological and physiological mechanism. In Traditional Medicine, problematic emotion is called Chiljeong(七情). The term Chiljeongsang(七情傷), which is a compound word of 'Chiljeong(七情)' and 'Sang(傷)' meaning 'damage', is used in referring to illness related to unhealthy emotions. It is used to describe illness caused by Chiljeong(七情) and mental disorders that result in emotional symptoms. It also refers to illness where Chiljeong(七情) plays a role in creating a more direct cause of illness in the larger pathogenic process. In the contents of Chiljeongsang(七情傷) in Traditional Medicine, emotion as causation of Gi(氣) disorder could be found the most, while explanation on the psychological process related to the creation of Chiljeong(七情) was insufficient. This tendency is related to the philosophical basis of Traditional Medicine. Out of the two patterns suggested, the mind-based Chiljeongsang(七情傷) pattern includes the psychological aspects of damage, in which key focus is on the cause that arouses problematic emotion, its process and the meaning this emotion entails. To explain this, concepts from psychiatry, especially Jung's theory on neurosis was adopted. In treating a Chiljeongsang(七情傷) patient, mere knowledge of the physiological changes of the body in terms of Gi(氣) is insufficient. This is because mental factors play a large role in Chiljeongsang(七情傷), and the more a condition is 'mental', the more complex it becomes and the more it influences life as whole. Therefore, the doctor must approach the patient with not only medical knowledge, but also overall knowledge on all aspects of human life.

계절 변화의 원인에 관한 초등학생의 멘탈 모델 변화 과정 분석 (Analysis of Changes in Elementary Students' Mental Models about the Causes of the Seasonal Change)

  • 이 연구의 목적은 계절 변화의 원인에 대한 초등학생의 멘탈 모델의 변화 과정을 파악하는 것이다. 미시 발생적 연구 방법을 사용하여 총 8회기 동안 초등학교 6학년생 8명에게 계절 변화의 원인을 그림과 글, 사고 발성을 통해 설명하게 함으로써 멘탈 모델의 변화를 파악하였다. 연구 진행 시 연구 참여자의 언어적, 행동적 요소 및 면담 내용을 모두 비디오 녹화하였으며, 연구자의 현장 관찰 기록지와 학생이 작성한 멘탈 모델 기록지 등 다각적 자료를 함께 수집하였다. 수집한 결과를 통합하여 프로토콜을 작성하고, 이를 반복적으로 읽으며 귀납적으로 범주화하였다. 연구 결과는 다음과 같다. 첫째, 계절 변화의 원인에 대한 학습자의 멘탈 모델은 회기 내 및 회기 간에 걸쳐 개인마다 다양한 경로로 변화하였으며, 다양한 변화를 일으킨 학습자의 멘탈 모델이 과학적 모델에 더 근접하였다. 또한 자전이나 공전처럼, 계절의 변화와 관련된 선행 지식이 바르게 정착된 학생들은 새로운 정보에 기초하여 과학적 개념과 일치하는 멘탈 모델을 형성하였다. 반면에, 선행 지식이 바르게 정착되지 않은 경우에는 변형된 멘탈 모델에서 벗어나지 못하였다. 둘째, 멘탈 모델이 변화하는 데에는 학습자의 선행지식과 경험 및 정보, 선행 지식의 정확성, 새로운 지식과 기존 멘탈 모델 사이의 불일치 해결, 모형 조작을 통한 멘탈 모델 활성화, 그림으로 그려보기와 같은 요인들이 영향을 미쳤다. 교사는 학습자의 과학적 개념 형성을 위하여 그들에게 다양한 멘탈 모델을 구성할 수 있는 경험을 충분히 제공하고, 기존 멘탈 모델과 불일치하는 새로운 수업자료 제시를 통해 학습자가 의문을 느끼게 하고, 이를 해결하도록 할 필요가 있다.

  • A field-sequential color projection system can display color images using panel. However, it suffers from a characteristic trichromatic separation known as "color breakup". The viewing of images exhibiting color breakup may cause visual fatigue and mental stress. In this study, the authors examine, from the standpoint of human factors, the objective and subjective symptoms that can result from the viewing of images with color breakup.

The most common prodrome appears to be postauricular pain. including headache and dizziness. 3. As the more serious the paralysis becomes. the prodrome gets worse, and the treatment term extends. It also gets harder to perform prognosis 4. We confirm that either physical or mental overwork and stress can be the most likely cause of facial palsy.

Negative myoclonus associated with pregabalin

  • Negative myoclonus (NM) is a jerky, shock-like involuntary movement caused by a sudden, brief interruption of muscle contraction. An 80-year-old man presented with multifocal NM and confusion. Two days before the onset of NM, he commenced the intake of pregabalin at a dose of 150 mg/day for neuropathic pain. His NM resolved completely and mental status improved gradually after the administration of lorazepam intravenously and the discontinuation of pregabalin. Our study suggests that pregabalin can cause NM even in patients without a history of seizures.

  • Traumatic brain injury (TBI) is the leading cause of death and disability in children. Survivors of severe TBI are more susceptible to functional deficits, resulting in disability, poor quality of life, cognitive decline, and mental health problems. Despite this, little is known about the pathophysiology of TBI in children and how to manage it most effectively. Internationally, efforts are being made to expand knowledge of pathophysiology and develop practical clinical treatment recommendations to improve outcomes. Here we discuss recently updated evidence and management of severe pediatric TBI.
