4 Cases Report of Menopause Syndrome with Ondam-tang Gagambang (온담탕(溫膽湯) 가감방(加減方)으로 증상 호전된 갱년기 증후군 환자 치험 4례)

  • Objectives: The Purpose of this study is to report the effects of Ondam-tang gagambang on menopause syndrome. Methods: Four patients who had menopause syndrome as chief complaints were treated by Gamiondam-tang or Kuibiondam-tang for two weeks. We evaluated the results of treatment by change of symptoms, Kupperman's Index and Menopause Rating Scale (MRS). Results: After treatment, menopause syndrome cases were improved. Almost scores of Kupperman's Index & Menopause Rating Scale (MRS) were reduced. Conclusions: This study suggests that Ondam-tang gagambang is effective on overall Menopause syndrome cases.

Psychosocial Aspects of Menopause Syndrome (폐경기 증후군의 사회심리적 측면)

  • Regarding menopause as a turning point in woman's lift, the author reviewed the psychosocial aspects of menopause. Seeing menstruation as the symbolic meaning of female sexuality, psychoanalysts argue that menopause, the loss of symbolic meaning of female sexuality, may bring a mourning reaction to woman. It is reported that the high ratio of depression does not have a serious relationship with the menopause syndrome, and that depression cannot be viewed as a different symptoms. It is also proved that most women successfully recover themselves from temporary menopause symptom. Research data shows that some women have a negative thought on the meaning of menopause. Others, however, have a positive concept on the meaning of menopause because of the feeling of release and freedom. In sum, the author suggests that psychosocial consideration is very important as much as bilogical consideration in considering the meaning of menopausal syndrome.

  • The purpose of this study is to examine the medicalizing process of menopanse with literature review, and then is to explore the knowledge of 'menopausal syndrome' critically, focused on four critics on the biomedical model which were suggested by Mishler. Although menopause is a natural biological phenomenon, the view of many medical researchers and practitioners is that menopause is a disease. After synthetic estrogen was developel in 1938, physicians did agree on two basic assumptions : menopausal women should be managed by physicians, and medical intervention should be given. Menopouse was defined as a deficiency disease (estrogen difficiency) by Wilson in 60's and is redefined as a cause of disease(for example, osteoporosis, heart disease) at the present. But the other view of non-medical researcher is that 'menopausal syndrome' as a disease is constructed medically. It was reported that Only hot flush and sweating of physical symptoms experienced by menopausal women, were associated with menopause. Symptoms of menopausal syndrome are also related with symptons of aging. So, it cann't conclude that menopausal syndrome is resulted from menopause, and it cann't be only explained that menopausal syndrome is related causally to estrogen deficiency, and only treatment by ERT or HRT is best relevant. It cann't assume that menopausal syndrome is a common phenomenon to all menopause women, because culture affected on women's experience of menopause.

  • Purpose: This study was conducted to determine the impact of premature menopause (PM) on metabolic syndrome and its components to determine its contribution to the health of Korean women undergoing natural menopause. Methods: This study used data from the 4th to 8th National Health and Nutrition Examination Survey (2007~2021). A total of 11,989 women aged 30 to 65 who responded to questions about their current menstrual status, natural menopause status and its age. Complex sample multivariate Cox proportional hazards regression analysis was used. Results: Compared with women with late menopause, women with PM had a 1.34 times higher risk of developing metabolic syndrome. Excluding the risk of high blood pressure, women with PM had a significantly higher risks of increased waist circumference, decreased high-density lipoprotein, increased risk of high triglycerides, and increased fasting blood glucose. Conclusions: The results of this study provided data that can be applied to policies or interventions to prevent or manage metabolic syndrome in women with PM in Korea.

The Comparison of Kupperman's Index and MENQOL between Women with Surgical Menopause and Women with Natural Menopause (수술적 폐경을 경험한 여성과 자연 폐경 된 여성의 Kupperman's Index, MENQOL 비교)

  • Purpose: To investigate the change of the climacteric syndrome with women with induced menopause by hysterectomy or ovariectomy compared with women with natural menopause. Methods: Menopausal women(aged 45-55) were selected at two oriental medicine hospitals, Sam-se Oriental Medicine Hospital and Daegu Hannny Oriental Medicine Hospital. They were asked about Kupperman's Index, MENQOL and a few additional questions. They were divided into two groups: women with induced menopause by hysterectomy or ovariectomy and women with natural menopause. To analyse the Kupperman's index and MENQOL, SPSS for Windows 12.0 and Wilcoxon Ranked Sum Test were used. Results: The total sums of Kupperman's index and MENQOL were significantly correlated with women with induced menopause and women with natural menopause and women with induced menopause had higher total sums. Both the renal and urinary tract disorder and the sexual disorder were significantly correlated with women with induced menopause and women with natural menopause. The gastrointestinal and psychosocial disorder of Kupperman's index and MENQOL were not significantly correlated with women with induced menopause and women with natural menopause. Conclusions: Women with induced menopause by hysterectomy or ovariectomy experienced climacteric syndrome more severely compared with women with natural menopause, specifically the renal and urinary tract disorder and sexual disorder.

A Cross-sectional Study of Nutrient Intake for Korean Women with Menopause and Metabolic Syndrome (한국여성의 폐경에 따른 대사증후군과 영양섭취에 대한 단면 연구)

  • Menopausal status and hormonal changes are important factors related to the prevalence of metabolic syndrome in women. Nutrient intake is also a risk factor for metabolic syndrome. Although, postmenopausal status and hormonal changes result in a 60% increased risk for metabolic syndrome, there has been no method to elucidate the effects of nutrient intake on metabolic syndrome following menopause. This study was conducted to evaluate the effects of nutrient intake by menopausal women on the prevalence of metabolic syndrome. All data were obtained from the Korean National Health and Nutrition Examination Survey 2007-2009. Menopausal women showed a 1.8-fold increase in the prevalence of metabolic syndrome. Metabolic syndrome group showed significantly lower values in calcium, iron, vitamin A, carotin, retinol and riboflavin intake than those of normal group in premenopausal women. In postmenopausal women, there are significant differences in crude fiber, calcium, iron, potassium, riboflavin and niacin. Indeed, different patterns of nutrient intake were observed by menopausal status and metabolic syndrome. As menopause cannot be controlled, a diet with adequate nutrient intake may be useful to control the rapid increase in the prevalence of metabolic syndrome due to menopause.

A convergence of association between menopause, breastfeeding and first birth age and metabolic syndrome in Women (여성의 폐경과 모유수유 기간 및 첫출산 연령과 대사증후군의 연관성에 관한 융합적 고찰)

  • The purpose of this study was investigate the association between birth age related health factors and metabolic syndrome in women. The data of 6743 women from Korean National Health and Nutrition Examination Survey(2012-2014 KNHANES) were analyzed. Dependent variable was metabolic syndrom related variables were collected. Menopause status was associated with metabolic syndrome [odds ratio of with post-menopause 1.44(95% CI: 1.09-1.90)]. First birth age was associated with metabolic syndrome [odds ratio of women with 26-30 year : 0.81 (95% CI: 0.68-0.98); odds ratio of women 31 year over: 0.62(95% CI: 0.45-0.87)] after adjusted for age, region, household income, smoking, occupation. In conclusion, the metabolic syndrome is closely related to menopause and first birth age, and will be used as a basic data in developing future educational programs for women.

Effect of Non-Alcoholic Fatty Liver Disease on Components of Metabolic Syndrome in Post-menopausal Women

  • Non-alcoholic fatty liver disease (NAFLD) is associated with various metabolic abnormalities, including central obesity, type 2 diabetes, dyslipidemia, and high blood pressure. This suggests that NAFLD may represent the hepatic manifestation of the metabolic syndrome. In this study, we investigated unfavorable effects NAFLD on components of metabolic syndrome in post-menopause women. Eight hundred sixty-nine postmenopausal women were recruited for this study. The diagnosis of fatty liver was based on the results of abdominal ultrasonography. Serum levels of fasting glucose, total cholesterol, triglyceride, and HDL-cholesterol were measured. The prevalence of component of metabolic syndrome such as hypertension, hyperglycemia, hypertriglyceridemia, and low-HDL-cholesterol was significantly higher in subjects with NAFLD as compared with those without NAFLD. The moderate to severe grade of NAFLD presented higher levels of serum fasting glucose, fasting insulin, HOMA-IR, total cholesterol, and triglycerides than the mild NAFLD and the normal group. In conclusion, metabolic syndrome risk was increased in post-menopause women with NAFLD as compared with those without NAFLD. The severity of NAFLD affected metabolic syndrome risk factors. The optimal strategy for the treatment of NAFLD is likely to include lifestyle modifications and therapy to improve insulin resistance.

  • Objectives : This study was carried out to investigate the effect of Sosiho-tang on Burning mouth syndrome of female after menopause. Methods : We treated 3 Burning mouth syndrome patients with Sosiho-tang-Based Korean Medicine. We evaluated the severity of pain and heat by using the Visual Analogue Scale(VAS). Results : After the treatments, all patient's objective score of Burning mouth syndrome was remarkably decreased. Conclusions : This study suggests that Sosiho-tang is effective against Burning mouth syndrome of female after menopause.

Impact of Menopausal Status, Metabolic Syndrome and its Risk Factors on Impaired Quality of Life above Middle-aged Women (중년 이후 여성에서 삶의 질 저하에 영향을 미치는 요인: 폐경 형태와 대사증후군 위험요인을 중심으로)

  • Purpose: This study explored influencing factors on quality of life (QoL) above middle-aged women in relation to demographic factors, health-related factors, menopausal status, metabolic syndrome (MS) and its risk factors. Methods: This study was secondary data analysis from the Sixth Korea National Health and Nutrition Examination Survey 2013~2015 that utilized a complex, multi-stage probability sample design. Study sample of 2,310 was inclusive of (28.8%) of women who were over 40. To evaluate the factors that would influence an impaired quality of life, $x^2$ test, GLM, and logistic regression analysis were done. Results: Level of quality of life was lower in women with late post-menopause(over 10 years since menopause) than women with pre-menopause. Factors influencing impaired QoL were as follows: graduated middle school and elementary school or less (OR=2.43, 4.42, respectively, p<.05), no job (OR=1.92, p<.001), stress (OR=1.92, p=.001), depression (OR=1.93, p=.001), insufficient sleep (OR=1.64, p=.003), late post-menopause (OR=2.61, p=.044) and over 85cm of waist circumference (OR=1.76, p=.01). Conclusion: These results suggest that late post-menopause may be an independent factor influencing an impaired QoL. To promote post-menopausal womens' health, a nursing strategy is required to teach women how to manage levels of stress, depression, insufficient sleep, and abdominal obesity through health education, nutritional counselling, and physical activity program.
