Effect of Sex and Menopausal Age on Thickness and Density of Membranous Bone : Focused on Computed Tomography in Squamous Portion of Temporal Bone (성별과 폐경기 나이가 막성골의 두께와 밀도에 미치는 영향 : 측두골 비늘부위 전산화단층영상을 중심으로)

  • The purpose of this study was to investigate the effect of sex and menopausal age on the thickness and density of squamous portion of the temporal bone as the membranous bone. Patients who visited a general hospital in Chungnam and had a computed tomography (CT) examination of the head. A retrospective study was conducted with 120 subjects (30 men under 55 years old, 30 men over 56 years old, 30 women under 55 years old, and 30 women over 56 years old). Axial images of the squamous portion in the temporal bone were obtained from CT of the head. For this image, a slice sensitive profile (SSP) was acquired with an image analysis program and these were evaluated. The thickness was measured by using a digital ruler to measure the full width at half maximum (FWHM) of the SSP, and the density was measured in hounsfield unit (HU). These by gender were measured to be about 0.5 mm thinner in the temporal bone in men than in women, and there was a significant difference. The density was measured to be about 200 HU higher in women than in men of it, and there was a significant difference. As a result, it in women was thicker and had a lower density. The thickness of it in men and women over 56 years of age was 0.8 mm or more thicker in women and less than 400 HU in density. As a result, the women group over the age of 56 showed a distinct increase in thickness and decrease in density, different from other target groups. It is expected that the results of this study could be used as basic data for a new bone density measurement site study.

  • The bone formation accompanied with other diseases in brain has been rarely reported. Furthermore, it has not been reported without any specific disease. We report a case of a 27 year old female who was referred to our hospital because of the incidentally found calcified lesion in plain X-ray of the skull. The CT and MRI of the brain showed a calcification with minimal enhancement at left parietal area. The calcified lesion was removed and biopsy was performed with stereotactic guided craniotomy. Pathologically, the lesion was confirmed as the membranous bone which was composed of bony trabeculations with osteocytes and the biopsy from adjacent area to the bone revealed a gliosis without any other disease.

  • In the reconstruction of the hard tissue defect of the oral cavity, the usefulness of the chin bone graft, one of the intraoral donor site, is gradually increased. The advantages include reduced resorption rate after graft due to its membranous bone nature, relatively ease to harvest under local anesthesia, reduced operative time because of the same operative field, decreased morbidity, and relatively large amount of bone can be harvested compared to other intraoral donor site. It has also postoperative complications including paresthesia of the lip or chin area, discomfort of lower anterior teeth, and facial swelling around chin area. Of these complications, facial swelling occurs more frequently, is more severe as a early postoperative discomfort, and prevents fast recover of patient's social activity since this procedure is generally accomplished in the outpatient base under local anesthesia. So we applied a modified " gull-wing" type incision to minimize this complication, and now we report this simple but effective surgical technique with clinically favorable result.

The Ultrastructure of Osteogenesis in Distal Extremity of the Distal Phalanges of Human Fetus (인태아 수지말절골의 골화에 관한 전자현미경적 연구)

  • Fine structure of the processes of intramembranous ossification and endochondral ossification at the tip of the distal phalanx of human fetuses was studied by electron microscopy. In 50 mm fetus, intramembranous ossification of the tip of cartilaginous phalanx was first noted. The osteoblasts of the perichondral zone of tip of cartilaginous phalanx started to lay down a thick membranous bony lamella. Most of the hypertrophied chondrocytes in the marginal parts of tip of the distal phalanx remained viable after being embeded in mineralized cartilaginous septa. The tuberosity of the distal phalanx was formed by membranous bony trabeculae on the exterior of the subperiosteal cap at 80 mm fetus. At this stage endochondral ossification was first observed in distal extremity of the distal phalanx. The maority of hypertrophied chondrocytes in the center of distal extremity appeared to be disintegrating. Resorption of calcified matrix was undertaken by perivascular cells and chondroclasts. From the periosteum, zone of calcification, vascular sprouts expanded within a recently opened lacunae, and the invading osteoblasts laid down osteoid and bone. After 120 mm fetus, endochondral and subperiosteal ossification proceeded in only one direction, just proximally. These findings demonstrate that intramembranous ossification, calcification, and endochondral ossification start at tip of the distal phalanx instead of at the center of the shaft, as was the case in other long bones.

  • Vscularized calvarial bone flaps have been useful tools in the reconstuction of different type of bone defects in craniofacial surgery Vascularized bone transfer are often preferred to nonvascularized bone grafts because nonvascularized bone grafts are less resistant to infection, not as mechanically strong, and do not survive well in a poorly vascularized bed. Potential advantages of vascularized outer table calvarial bone flap include proximity to the operative field, membranous origin similar to that of the facial skeleton, maleability which allows for a variety of shapes and sizes, inconspicous donor sit scar. The following are the case reports of two patients who underwent craniofacial reconstruction suing vascularized outer table calvarial bone flap.

A retrospective computed tomography analysis of maxillary fractures and the clinical outcomes of their unreduced parts

  • Background: Some parts of a maxillary fracture-for example, the medial and posterior walls-may remain unreduced because they are unapproachable or hard to deal with. This study aimed to investigate the self-healing process of unreduced maxillary membranous parts of fractures through a longitudinal computed tomography (CT) analysis of cases of unilateral facial bone injuries involving the maxillary sinus walls. Methods: Thirty-two patients who had undergone unilateral facial bone reduction surgery involving the maxillary sinus walls without reduction of the medial and posterior walls were analyzed in this retrospective chart review. Preoperative, immediate postoperative, and 3-month postoperative CT scans were analyzed. The maxillary sinus volume was calculated and improvements in bone continuity and alignment were evaluated. Results: The volume of the traumatized maxillary sinuses increased after surgery, and expanded significantly by 3 months postoperatively (p< 0.05). The significant preoperative volume difference between the normal and traumatized sides (p= 0.024) resolved after surgery (p> 0.05), and this resolution was maintained at 3 months postoperatively (p > 0.05). The unreduced parts of the maxillary bone showed improved alignment and continuity (in 75.0% and 90.6% of cases, respectively), and improvements in bone alignment and bone continuity were found to be correlated using the Pearson chi-square test (p= 0.002). Conclusion: Maxillary wall remodeling through self-healing occurred concomitantly with an increase in sinus volume and simultaneous improvements in bone alignment and continuity. Midfacial surgeons should be aware of the natural course of unreduced fractured medial and posterior maxillary walls in complex maxillary fractures.

An unusual cause for trismus caused by mandibular coronoid osteoma: a case report

  • Osteoma is a dense bony proliferation of otherwise normal membranous bone. This tumor is essentially restricted to the craniofacial skeleton and is rarely diagnosed in other bones. The mandible and the paranasal sinuses are the most common sites, while the involvement of the coronoid process has been reported in only 6 cases so far. This report demonstrated a case of osteoma occurring at the mandibular coronoid process in a 26-year-old female patient. The patient was managed with surgical resection of the osteoma followed by physiotherapy.

  • This study investigated the distribution of scuticociliates in the intestine, spleen, kidney, testis, brain, pericondrial bone and muscle layer of the tiger puffer, Takifugu rubripes, infected with scuticociliates. Scuticociliates were infiltrated in the connective tissues of the outer layer of the intestine, spleen, kidney, testis, brain, pericondrial bone and muscle layer. In brain, membranous tissue and optic lobe cortex separation were accompanied by the infection of scuticociliates. Other internal tissues and organs did not show any lesions expect for heavy deposition of hemosiderin in spleen.

The Role of Cartilage Canals in Osteogenesis and Growth of the Vertebrae in the Human Fetuses (인태아 척추 골화과정에서 연골관의 역할)

  • To investigate a role of cartilage canals in osteogenesis and growth of the vertebrae, in human fetuses ranging from 50 mm to 260 mm crown rump length were studied by electron microscopy. The initial appearance of cartilage canals of the vertebral body was observed at 60 mm fetus. In 80 mm fetus, primary ossification center in the vertebral body was first noted. The vertebral body showed calcified chondrocytes surrounded by a tone of hypertrophied chondrocytes and deep canals which terminated in calcified matrix. Most hypertrophied chondrocytes in the centrum showed in various stage of degeneration in disorderly arrangement. At the blind end of deep canal, osteogenic cells, osteoblasts and chondroclasts were observed. Resorption of unmineralized cartilage septa was undertaken by perivascular cells within cartilage canals. The ruffled border of the chondroclast was restricted to resorption site of calcified cartilagenous matrix. The periosteal bone formation was followed by the appearance of primary center of the centrum at 120 mm fetus. The osteoblasts of the perichondrium started to lay down a thin membranous bony lamella on the outer surface of the osseous trabeculae of the centrum. The processes of bone formation in the vertebral bodies were found to possess morphological similarities to that occurring at secondary center of the epiphysis of a long bone. These results indicate that the connective tissue cells within the cartilage canals proliferate and differentiate into osteoblasts at the site of endochondral ossification of the vertebrae.

Study on the Pathogenesis of Hantaan Virus with Monoclonal Antibodies (단일항체를 이용한 한국형출혈열의 병인성 연구)

  • Hantaan virus(HV) 76-118 strain was inoculated into suckling ICR mice by intra-nasal route with an inoculum of $10LD_{50}$. Mortality was 65% at the 3rd week after inoculation, but declined to 35% at the 4th week. Infectivity was determined by the measuring immuno-fluorescent antibody in sera. The peak of infectivity was 80% at the 4'th week after inoculation. Viremia was reached peak level of $1.7{\times}10^4\;PFU/ml$ by day 10. Immunofluorescent antibody and neutralizing antibody appeared by 2 weeks and 15-17 days respectively, but achieved similar titer by 35 days. By using a monoclonal antibody to HV 76-118, viral antigens were initially detected in inguinal and axillary lymph node by 2 days. Viral antigens in bone marrow and lung were delayed much more than in those of lymph node. These were similar with those of intra-peritoneal and intra-muscular route. Immune complex against IgG, IgM and C3 appeared by 16 days, 14 days, and 18 days respectively. The pattern of immunofluorescence in the basement membrane of glomeruli was diffuse membranous. Spotted pattern was also observed in the tissue stained with anti-mouse C3 antibody. By 20 days, control tissue was also shown immune complex in the glomeruli.
