Surgical Treatment of the Supravalvular Aortic Stenosis (판막상부 대동맥 협착증의 수술요법)

  • Supravavular aortic stenosis is a congenital narrowing of the ascending aorta just distal to the level of the origins of the coronary arteries, that may be localized or diffuse. Five patients with supravalvular aortic stenosis were operated upon between July, 1986 arid June, 1990. Four of these patients were William`s syndrome [mental retardation, elfin face], and one was isolated supravalvular aortic stenosis. Preoperative diagnosis of the supravalvular aortic stenosis was made by left side cardiac catheterization and angiocardiography. There are three types of supravalvular aortic stenosis such as membranous, hourglass and hypoplastic. Four of our patients were of hourglass type, and one was hypoplastic type. Patch aortoplasty was performed in all cases. Preoperative systolic gradients ranged from 45 to 1SO mmHg [average 102.6 mmHg]: postoperative gradients ranged from 0 to 75 mmHg [average 39 mmHg]. The patient of hypoplastic type has been suffered from mild exercise intolerance even after the operation, and the postoperative echocardiography revealed the systolic gradient of 100 mmHg [preoperative 180 mmHg]. The results of surgery for hourglass type were excellent. But the patient with hypoplastic form would be benefited from some modifications of the operation.

Intracardiac Thrombosis Involving All Four Cardiac Chambers after Extracardiac Membranous Oxygenation Associated with MTHFR Mutations

  • A 4-month-old boy diagnosed with acute myocarditis was treated with extracorporeal membrane oxygenation (ECMO). Follow-up echocardiography eight hours after ECMO revealed intracardiac thrombosis involving all four heart chambers. Because of the high risk of systemic embolization due to a pedunculated thrombus of the aortic valve, we performed an emergency thrombectomy. After the operation, the patient had a minor neurologic sequela of left upper arm hypertonia, which had almost disappeared at the last outpatient clinic two months later. He was diagnosed with a major mutation in MTHFR (methylenetetrahydrofolate reductase), which is related to thrombosis.

  • The case of a patient with a transfusion-related acute lung injury (TRALI) to whom extracorporeal membrane oxygenation (ECMO) had been applied is reported. A 55-year-old male injured with liver laceration (grade 3) without chest injury after car accident. He received lots of blood transfusion and underwent damage control abdominal surgery. In the immediate postoperative period, he suffered from severe hypoxia and respiratory acidosis despite of vigorous management such as 100% oxygen with mechanical ventilation, high PEEP and muscle relaxant. Finally, ECMO was applied to the patients as a last resort. Aggressive treatment with ECMO improved the oxygenation and reduced the acidosis. Unfortunately, the patient died of liver failure and infection. TRALI is a part of acute respiratory distress syndrome (ARDS). The use of ECMO for TRALI induced severe hypoxemia might be a useful option for providing time to allow the injured lung to recover.

  • A stargazer fish, Ichthyscopus lebeck sannio, of family Uranoscopidae was collected for the first time in Cheju-shi, Cheju-do. Ichthyscopus lebeck sannio has several taxonomic characteristics : large white spots on the surface of body ; without cleithrum spine ; developed and branched labial fimbriae on the both lips ; triangular pectoral fin ; having fimbriated membranous projection on upper part of pectoral fin and having single dorsal fin.

Cryptonemia rotunda (Halymeniales) and Schizymenia apoda (Nemastomatales), two new records of red algae from Korea

  • Here we report the first finding of Cryptonemia rotunda and Schizymenia apoda in Korea based on plastid rbcL sequence analysis and morphological observations. C. rotunda occurred in the subtidal in Hongdo and Namyeodo on the south coast. Thalli have short stipe with foliose, membranous blades with dentate margin, a length of 2-8 cm. S. apoda occurred in many locations along the coast of Korea. Thalli are foliose, simple or broadly lanceolate blades with irregular lobes and proliferations from the margins, sometimes undulate, soft fleshy, and slippery, up to 32 cm tall, and have cystocarps with ostioles on the blade. Plastid rbcL sequences reveal that each of these species is clearly separated from other species of their respective genera. S. apoda was well resolved within the genus. However, C. rotunda was distantly related to other members of the genus, and this result needs an urgent revision of the genus.

Preconditioning technique for a simultaneous solution to wind-membrane interaction

  • A preconditioning technique is presented for a simultaneous solution to wind-membrane interaction. In the simultaneous equations, a linear elastic model was employed to deal with the fluid-structure data transfer at the interface. A Lagrange multiplier was introduced to impose the specified boundary conditions at the interface and strongly coupled simultaneous equations are derived after space and time discretization. An initial linear elastic model preconditioner and modified one were derived by treating the linearized elastic model equation as a saddle point problem, respectively. Accordingly, initial and modified fluid-structure interaction (FSI) preconditioner for the simultaneous equations were derived based on the initial and modified linear elastic model preconditioners, respectively. Wind-membrane interaction analysis by the proposed preconditioners, for two and three dimensional membranous structures respectively, was performed. Comparison was made between the performance of initial and modified preconditioners by comparing parameters such as iteration numbers, relative residuals and convergence in FSI computation. The results show that the proposed preconditioning technique greatly improves calculation accuracy and efficiency. The priority of the modified FSI preconditioner is verified. The proposed preconditioning technique provides an efficient solution procedure and paves the way for practical application of simultaneous solution for wind-structure interaction computation.

  • Female genital morphology of Cryptocercus Kyebangensis is described, focusing on the structural features of ovipositors at each developmental stage. Ovipositor valves were the first genital structures to appear in female nymphs. The caudal margin of the 9th sternum was nearly straight in the 1st instars of both sexes, with a slight median notch, but was deeply concave in the female nymphs of the End instar. The 1st valve of ovipositor budded off from membranous fold between the 8th 8nd 9th sternum and a pair of the 2nd valve came from the 9th sternum. Separation of styli occurred in the female nymphs of the 8th or 9th instar. Some parts of the female genitalia were sclerotized after the final molt. Spermathecae were the fork type and leaned a little to the right in the middle line of the genital chamber from ventro-posterior view. The trace sealed with sclerite materials existed on the spermathecal opening. The genital segments (segments Ⅷ, IX, and X) were concealed completely by the 7th tergum and sternum posteriorly expanded. Terminal lobes which exist in the Blattidae were not observed in C. Kyebangensis. Instead, the apicolateral emargination existed on the terminal abdominal segment of C. Kyebangensis females.

Double-Outlet of Left Ventricle in Corrected Transposition of Great Arteries -One case report- (좌심실 이중유출로를 동반한 교정형 대혈관전위증 -1예보고-)

  • This is one case report of the extremely rare congenital cardiac malformation, Double-outlet of left ventricle in corrected transposition of great arteries. 11-year-old boy complained acrocyanosis and exertional dyspnea, the parents noticed cyanosis since birth. Physical examination revealed acrocyanosis, clubbed fingers and toes, G-III pansystolic murmur on 2nd and 3rd ICS, LSB. Right heart catheterization revealed significant $O_2$ jump in ventricular level. Right and left ventriculography showed the both catheters arriving in the same ventricle i.e. anterior chamber, morphological left ventricle was in right and anterior position, simultaneous visualization of aorta and pulmonary artery and aorta locating anterior and right side of pulmonary artery. Echo cardiogram surely disclosed interventricular septum. Conclusively it was clarified that the patient has Double-outlet of left ventricle and corrected transposition of great arteries [S.L.D.]. Operation was performed to correct the anomalies under extracorporeal circulation with intermittent moderate hypothermia. Right-sided ventriculotomy disclosed the following findings. 1. Right-sided ventricle was morphological left ventricle. 2. Left-sided ventricle was morphological right ventricle. 3. Right side atrioventricular valve was bicuspid. 4. Left side atrioventricular valve was tricuspid. 5. Aortic valve was superior, anterior and right side of pulmonary valve. 6. Subpulmonary membranous stenosis. 7. Non-committed ventricular septal defect. We made a tunnel between VSD and aorta with Teflon patch so that arterial blood comes through VSD and the tunnel into aorta. After correction the patient needed assisted circulation for 135 min. to have adequate blood pressure. Postoperatively by any means, adequate blood pressure could not be maintained and expired in the evening of operation day.

  • Tracheopathia osteoplastica(TO) is a rare, clinical and pathologic benign condition of unknown cause and characterized by submucosal cartilaginous or bony projections into tracheobroncheal lumen, usually not involved posterior membranous portion of tracheobroncheal tree. We report two cases of tracheopathia osteoplastica that involved trachea and both main bronchus, diagnosed by chest CT, fiberoptic bronchoscopic biopsy.

  • Clostridium difficile-associated disease (CDAD) is an important nosocomial infectious diarrhea and is associated with antibiotic use. Recently, incidence of C. difficile has been increasing in hospitals. A total of 1,329 stool specimens were examined from January, 2005 to December, 2008. This study analyzed the incidence and clinical characteristics of C. difficile infections on them. Out of 1,329 stool specimens, 283 specimens showed toxin A/B positive, using EIA. The positive rate was 21.2%; with the highest incidence among and above the 70years old. On endoscopy, psedo membranous colitis was found in 57.7%, and 19.5% of patients were normal. Pathologic finding showed PMC in 26.8% of patients, AAC in 52.2%. C. difficile was associated with PMC, however, endoscopic and pathologic findings tests showed normal to PMC.
