사상의학(四象醫學)으로 본 여성 기미의 원인과 치료 (Causes and treatment of Melasma in Sasang constitutional medicine)

  • Melasma is a common disorder that causes dark colored patches. It generally causes brown spots on the face, especially on the forehead, cheeks, and upper lips. The pattern of patches has bilateral symmetry in yellowish-brown to gray-brown colors. It is much more common in women than in men. Melasma is considered to be caused by environmental and physical constitutional factors and often occurs when a woman's hormone changes by pregnancy or the use of oral contraceptive pills. It is important to combine oriental medicine and Sasang constitutional medicine for treatment of Melasma. Normal treatment of Melasma includes warming oneself, removal of the emotional stress factors, and good nutrition.

기미에 關한 文獻的 考察 (A literatual study on the causes and treatments of the melasma.)

  • In the literatual studies on the causes and treatments of the melasma, the results were as follows. 1. Melasma is the blackish patch on the face, is belong to the cartegory of the 'Myunjin(面塵)', 'Myunganzung(면간증)', 'Jakban(雀斑)' etc. in Oriental medicine 2. Melasma is deeply associated with Yangming channel in Meridian, with the spleen, stomach, heart and kidney in Viscera. 3. The pathogenic factors of Melasma is divided five parts. One is insufficient of Yangming's energy in Neijing(內經). Two is incoordination between vital energy and blood caused by wind-evil and phlegm-retention syndrome. Three is anxiety impairing the spleen. Four is kidney-asthenia and fire-hyperactivity. Five is heat-evil. 4. In the treatments of Melasma, Sthenia-syndrome was used cooling blood and activating blood circulation, or dispelling wind-evil and promoting meridian, or expelling fire-evil and removing toxic material etc. Asthenia-syndrome was used invigorating the liver and kidney, or nourishing yin and keeping fire downwards etc. 5. Melasma is concerned with sun-light, is mostly seen in female. 6. In the prescription of Melasma, it was used Jujesamultang(酒製四物湯加減), Okyong -san(玉容散), Chunghwasungitang(沖和順氣湯), Okyongseosisan(玉容西施散), Yukmiji-hwanghwan(六味地黃丸) etc.

Efficacy of 694-nm Fractional Toning Ruby Laser in the Treatment of Malar Melasma

  • Background and Objectives Among the numerous available therapies, the usefulness of laser treatment has been proven in melasma, especially for refractory to topical agents or chemical peels. This study evaluated the effectiveness of 694-nm wavelength fractional toning ruby laser in melasma presenting on the malar area. Materials and Methods This was a retrospective study of 40 melasma patients treated with 694-nm wavelength ruby fractional toning laser for a total of 8 cycles at intervals of 2 to 3 weeks. Two independent investigators assessed the photographic findings and evaluated the severity of melasma by applying the Melasma Severity Scale (MSS). Subject satisfaction score was also surveyed at every visit. Results After the 4th treatment, subjects were continuously satisfied with their results. At the 8th visit and 1-month after the last treatment, MSS was determined to be significantly improved as compared to the first visit. One case of minor hypopigmentation was reported, but was resolved after one month. Conclusion Application of 694-nm wavelength fractional toning ruby laser for treating melasma on the malar area showed significant improvement in the MSS. Moreover, the treatment could be repeatedly applied, with low complication rate and significant patient satisfaction.

Treatment of Refractory Melasma with Microwave-generated, Atmospheric-pressure, Non-thermal Nitrogen Plasma

  • Periorbital melasma is often refractory to treatment and highly associated with rebound hyperpigmentation or mottled hypopigmentation after laser treatment in Asian patients. In this report, we describe 2 patients with cluster-1 periorbital melasma and 1 patient with cluster-2 periorbital melasma who experienced remarkable clinical improvements after microwave-generated, atmospheric-pressure, non-thermal nitrogen plasma treatments. All patients exhibited limited clinical responses after combination treatments with topical bleaching agents, systemic oral tranexamic acid, and low-fluenced Q-switched neodymium (Nd):yttrium-aluminum-garnet (YAG) lasers. Low-energy nitrogen plasma treatment at 0.75 J elicited remarkable clinical improvement in the periorbital melasma lesions without post-laser therapy rebound hyperpigmentation and mottled hypopigmentation. We deemed that a single pass of nitrogen plasma treatment at 0.75 J induces mild microscopic thermal tissue coagulation and modification within the epidermis while preserving the integrity of the basement membrane in patients with periorbital melasma. Accordingly, nitrogen plasma-induced dermal tissue regeneration could play a role in the treatment of melasma lesions.

자하거 약침을 이용한 기미 환자 치료에 대한 임상연구 (Efficacy of Hominis Placenta Aqua-acupuncture Solution in the Treatment of Melasma)

  • Objectives: The purpose of this study is to evaluate the efficacy of Hominis Placenta Aqua-acupuncture Solution in patients of melasma. Methods: The treatment was performed twice weekly in 15 volunteer patients for 8 weeks by Hominis Placenta Aqua-acupuncture Solution. We used Melasma Area and Severity Index(MASI) to evaluate the severity of melasma. Results: MASI score was decreased from A to B and the significant clinical improvement was proved compared to before treatment. Conclusions: On the basis of this study, Hominis Placenta Aqua-acupuncture Solution is an effective treatment of melasma.

  • We had treated one melasma patient occurred by pregnancy with oriental medicine's method and got a good result. Melasma is often occured in pregnancy and dysmenorrhea. Most common pathology of melasma in oriental medicine is Yin deficiency with Fire and Blood Stasis. The Fire and Blood Stasis are made by Qi stagnation which is occurred by depression and stress. So we used the methods of promoting Qi and Blood, increasing Yin with clearing heat, and the result is very successful. We found out that Oriental Gynecologic therapy is most appropriate for treating woman's melasma, and could provide the treatment model.

피부 미백 평가 시 기미 병변 부위에서의 적색도 평가의 효용 (Use of Redness Assessment in Melasma Lesions in Skin Whitening Evaluation)

  • 기미는 몇몇의 병인학적 요인을 갖고 있지만, 많은 연구가 멜라닌 형성에 집중되어 있다. 본 연구에서는 멜라닌 형성과 구별되는 혈관의 감소에 의한 기미의 개선을 보고자 하였다. 피부과 전문의의 육안평가를 통해 기미와 일광 흑자를 가진 한국 여성 20명을 대상으로 실험을 진행하였다. 연구 대상자는 8주 간 기능성 화장품을 사용하였다. Chromameter 기기로 측정한 연구 대상자의 기미 병변, 일광 흑자 병변 및 정상 피부 부위에서의 각 피부색 측정 결과를 분석하였다. 시료 사용 8주 경과 후, 기미 병변 부위의 피부색 밝기와 적색도가 정상 피부 부위에 비해 통계적으로 유의하게 개선되었다. 또한, 기미 병변 및 일광 흑자 병변 부위의 피부색 밝기의 개선 정도는 유사하였다. 그러나, 시료 사용 8주 경과 후 기미 병변 부위의 적색도가 일광 흑자 병변 부위에 비해 통계적으로 유의하게 개선되었다. 본 연구에서는 기능성 제품을 사용함으로써 기미 병변 부위에서 피부색 밝기 뿐만 아니라 적색도 또한 개선됨을 확인하였다. 이에 적색도가 기미 병변 부위의 평가를 위한 부가적이고 적합한 요인이 될 수 있음을 제시하고자 한다.

  • 스쿠바다이빙에 의한 제주도 남부해역에 위치한 문섬 주변해역의 어류상을 조사하던 중 수심 약 13 m에서 망둑어과의 풀비늘망둑속 Eviota에 속하는 한국미기록종 어류 3개체를 채집하였다. 본 종은 후두부에 1쌍의 큰 흑점이 있고, 등쪽 정중선을 따라 흑점이 없는 점에서 E. melasma로 동정되었다. 제주도 문섬 주변해역에서의 출현은 지금까지 알려진 본 종의 최고 북방기록이며, 본 종의 국명은 '흑점풀비늘망둑'이라고 명명하였다.

Korean Red Ginseng Powder in the Treatment of Melasma: An Uncontrolled Observational Study

  • Exposure to the sun, ultraviolet radiation, and oxidative stress are the chief etiologic factors responsible for melasma. The ingredients of Korean red ginseng powder, which include ginsenoside and phenolic compounds, have antioxidative effects and reduce ultraviolet B-induced pigmentation. This study was designed to evaluate the effectiveness and safety of Korean red ginseng powder in patients with melasma. In 25 female patients, 3 g of Korean red ginseng powder was orally administered for a 24 week period. The level of pigmentation and erythema were determined and clinical improvement was evaluated by the melasma area and severity index (MASI), melasma quality of life scale (MELASQoL), and patient- and investigator-rated global improvement scale. After 24 weeks, the MASI score decreased from 8.8 to 5.6, and MELASQoL showed improvement in 91% of patients (p<0.05). The mean level of pigmentation decreased from 184.3 to 159.7 and erythema levels decreased from 253.6 to 216.4 (p<0.05). Additionally, 74% of the patients showed some improvement in both patient- and investigator-rated global improvement scales at week 24. Korean red ginseng powder was well tolerated by most of the patients. In conclusion, Korean red ginseng powder showed good tolerability and beneficial effects in patients with melasma. The use of Korean red ginseng would be counted as a useful adjunctive therapy for patients with melasma.
