The association factors of infection control practice based on health belief model in the dental hygienists (건강신념모형을 적용한 치과의원급 치과위생사의 감염관리 수행도 관련요인)

  • Objectives : The purpose of this study was to investigate the association factors of infection control practice based on health belief model in the dental hygienists in dental clinics. Methods : A self-reported questionnaire was filled out by 278 dental hygienists in 160 dental clinics in Gwangju by a proportional stratified sampling method from September 13 to October 7, 2013. Data were analyzed by t-test, ANOVA, correlation analysis, and multiple regression analysis using SPSS version 12.0. Results : In multiple regression analysis, practice scores were significantly higher in aged dental hygienists and those who took infectious disease history from the patients before treatment. With regard to health belief model, perceived barrier was negatively associated with the practice(${\beta}$=-.16, p<.001), importance of infection control in hand hygiene(${\beta}$=.14, p=.026), and use of personal protective equipment(${\beta}$=.17, p=.043). The intention of action was positively associated with the practice(${\beta}$=.13, p=.002). Conclusions : This study will provide the basic evidence for the quality improvement of infection control and prevention. So the dental hygienists will be able to put into practice in infection control management.

Comparison of Attention-Deficit/Hyperactivity Disorder Practice in Adults According to a Training Background in Child Psychiatry

  • Objectives: Awareness of attention-deficit/hyperactivity disorder (ADHD) in adults has significantly increased; however, clinical data specific to the Korean population are insufficient. Clinical experience of ADHD may differ based on whether psychiatrists have received pediatric psychiatry-specific training. In order to prepare a practice parameter for adult ADHD patients in Korea, we examined questionnaire data to observe how pediatric psychiatry training could affect clinical practice for adults with ADHD. Methods: A questionnaire about the diagnosis and treatment process was distributed to both general psychiatrists (GPsy) and child and adolescent psychiatrists (CAP) at the summer and winter workshop meetings of Korean Academy of Child and Adolescent Psychiatry. Results: In total, 142 psychiatrists participated in the survey (86 GPsy, 56 CAP). GPsy and CAP preferred pharmacotherapy (GPsy 82.40%, CAP 64.30%) as the primary treatment option and answered that the clinical psychiatric interview is the most necessary step in diagnostic assessment (GPsy 22.16%, CAP 19.00%). The GPsy responded with an optimal and average treatment duration that was shorter than that reported by CAP. Conclusion: Identification of the initial presenting symptom as the correct diagnosis and the optimal duration of pharmacotherapy differed between GPsy and CAP in practice, whereas concepts in diagnosis and treatment of ADHD in adults were similar for both groups. These results suggest the urgent need for the Continuing Medical Education program for psychiatrists treating adults with ADHD.

Analysis on the contents and satisfaction level of the dental hygiene students' Clinical Practice (치위생과 학생의 임상실습 내용과 만족도 분석)

  • Objectives : This research identified the frequency and satisfaction level of the observation and performance practice during the clinical practice process conducted by dental hygiene students. With the goal of providing base data required for the development and operation of increasingly effective clinical practice program and for the improvement of the existing clinical practice training. Methods : The subjects in this study were 278 students of a university located in Masan. Survey comprised of 49 questions in 10 clinical practice domains was conducted. Results : 1. As for the observation practice by each grade, there was significant difference in oral radiology, pediatric dentistry, periodontology, oral surgery(p<0.05). As for the performance practice executed for each grade, there was significant difference in the oral radiology, preventive dentistry and oral surgery(p<0.05). As for the satisfaction level for each grade, there was significant difference in basic medical service(p<0.05). 2. As for the observation practice by each clinical practice institution, there was significant difference in oral medicine, oral radiology, preventive dentistry, prosthodontics, pediatric dentistry, orthodontics and oral surgery(p<0.05). As for the performance practice executed by each practice institution, there was significant difference in basic medical service, oral medicine, oral radiology, operative dentistry, prosthodontics, orthodontics, oral surgery(p<0.05). As for the satisfaction level by each practice institution, there was significant difference in the oral radiology, preventive dentistry, operative dentistry, prosthodontics, orthodontics, oral surgery(p<0.05). Conclusions : As for the satisfaction level for the clinical practice, basic medical service is the crimary care clinical practice which is most basic and that is executed the most in a dental clinic. Satisfaction level was the highest in the domains where there were extensive observation and practice frequency. By conducting observation and practice frequency in the clinical practice process by each clinical practice domain in a broad and diverse manner, it would be possible to improve the ability of the clinical practice trainees who improve their clinical work execution capability and satisfaction level on the clinical practice.

  • There is a system in Korea named "Advanced Practice Nurse System" qualified by the Minister of Health, Welfare and Family Affairs for Advanced Practice Nurse besides nurse licence. Medical practice is, in today's medical law, understood as a general concept colligating medical practice, nursing practice and midwife practice and so on, for it is defined as a deed of medical technique practiced by medical personnel. Referring to the fact that the Supreme Court recognizes medical personnel as people who have medical expert knowledge, nursing practice can be recognized as a region of medical business and therefore it is not necessary to prescribe nursing practice separately from the definition of medical practice on a precedent, because nurse belongs to medical personnel. According to the precedent regarding 'Unlicensed Medical Practice of Advanced Practice Nurse for Anesthesia' recently sentenced by the Supreme Court, the medical practice is only allowed a doctor because it is 'in need of special knowledge and experience because of high danger on human body' and it is judged to be an unlicensed medical practice prohibited in medical law if it is to be done by a nurse. When considering the actual situation that System for Advanced Practice Nurse for Anesthesia is established under the circumstance that an anesthetist is in want and therefore the operation has not been performed on time, and that it is being expected an anesthetist to be in need, it is necessary to legislate for the range of medical practice of Advanced Practice Nurse so that Advanced Practice Nurse System can be practically legalized, for the role of Advanced Practice Nurse has the great possibility of shrinking because the precedent has considered Advanced Practice Nurse for Anesthesia doing anesthetic operation in clinic today as a potential wrongdoer.

A Study on the Architectural Planning of Clinical Practice Spaces at the College of Medicine (의과대학의 임상실습공간에 관한 건축계획 연구)

  • Purpose: This study summarized architectural planning data for clinical practice spaces for CPX/OSCE and Simulation spaces, through analysis of domestic and foreign literature and a survey of domestic colleges of medicine. Methods: This study analyzed the architectural planning elements of the clinical practice spaces of the college of medicine, through a literature review on the clinical practice space and the survey of four recently established colleges of medicine. Results: ① It is desirable to plan the floor plan type for Simulation and CPX/OSCE spaces as a corridor-separated type that separates the circulations between students and staff (faculty). ② The simulation area arranges their practice rooms around the control space where the circulation of the staff is made. Conversely, it is desirable to arrange the CPX/OSCE rooms around the test waiting room where students enter. ③ It is desirable to separate access to the staff and students to allow student control, and to install various support spaces facing the staff corridor to secure the movement of equipment and the flow of evaluators. ④ In the simulation area, the equipment stored in the storage must be set in each room before the practice according to the setting of delivery or surgery like, and the equipment that has been removed must be back, so the circulation is short and does not intersect with the student circulations. ⑤ Debriefing rooms, seminar rooms, etc. are arranged in the staff area for review and discussion of clinical situations where practice and evaluation have been completed. Implications: It is important to configure an appropriate movement system and spatial zoning for each space for simulation and CPX/OSCE.

A Study on the KAP for Prevention of Hypertension and Diabetes in a Rural Area, Korea (농촌지역주민의 고혈압 및 당뇨병 예방을 위한 KAP 조사연구)

  • The desirable change of KAP aimed at the prevention and early diagnosis of the disease. In Korea, Hypertension and Diabetes have been the major chronic disease. Especially, Hypertension and Diabetes are related to over-weight and diet behavior, which can be prevented through weight control and dietary treatment. Therefore this study the KAP for Hypertension and Diabetes in a rural area. The survey of the KAP were performed to a rural population of 288 in Namwon, Cheonbuk, Korea. The self-questionnaire was consisted of 15 questions of knowledge, 10 questions of attitude, and 15 questions of practice. To analyze the data, the score of knowledge was taken 1 when they were right. The scores of attitude and practice were taken from 4 to 1 by 4 scale. The results were as follows. 1. The mean of knowledge for Hypertension was 10.4(sd=3.28) and that of attitude was 31.5(sd=4.05), that of practice was 42.3(sd=6.14). In Diabetes, knowledge mean was 9.1(sd=3.51) and that of attitude was 31.2(sd=3.81), that of practice was 41.6(sd=6.21). The knowledge for Diabetes was lower than that for Hypertension. 2. To compare the scores by sex, the score of male's knowledge was higher than female's. However, in case of practice score, female's was higher than male's. And the scores of attitude were not different between male and female. This results were shown that the practice did not always derived from the knowledge. Although they have a little knowledge for disease, they are able to conduct the desirable practice when the importance for disease are increased. Therefore we thought that desirable practice derived from the combination of significance and knowledge for Hypertension and Diabetes.

Study on Laws related to the Scope of Both Medical Doctors' Practice in Korea (의료인 업무범위 관련 법률 고찰)

  • Objective : This study was aimed to reassess the scope of practice for medical personnel based on laws. Method : The law specifying the scope of practice for medical personnel has been selected searching Korean Law Information Center(https://www.law.gov.kr). The result was categorized as 'examination, diagnosis, treatment, procedure, prescription, and others'. Results : The laws related to medical procedures were divided into three categories: diagnosis, treatments, and public health and others. In the field of diagnosis, traditional Korean medicine practitioners are generally allowed to play a role. However, some laws specify that only medical doctors can be the primary authorities for diagnosing infectious diseases. In the area of treatments, particularly in emergency medical situations, only medical doctors or nurses are typically mentioned. There are debates in the field of public health and other areas concerning issues such as vaccination, disability diagnosis, and the qualifications for health center directors. A reevaluation is also needed for the Occupational Safety and Health Act, where only medical doctors are set as the personnel standard for workers' health examinations. Conclusion : To safeguard and promote the health of the citizens, there is a need for a clear definition of the licensure and scope of practice for healthcare professionals. Consistent interpretation of conflicting provisions among various laws and clear criteria for the term 'physician' in legal contexts are essential.

Study on the Proposals for Clinical Research in Korean Medicine Worldwide - Future Clinical Research Strategy II - (한의 임상 연구에 대한 국내외 제언 고찰 - 미래 임상 연구 전략 II -)

  • The purpose of this paper is to explore the strategy of future Korean medicine(KM) clinical research through the study on the proposals for KM clinical research worldwide. In this study, the papers published in English through Pubmed were investigated mainly. Among them, we analyzed the methodological proposals from the clinical research papers that were published in the KM related fields. Various proposals for improving the problems in KM clinical studies are as follows. First, KM clinical research should be designed based on understanding for the theory, backgrounds, paradigms and worldviews of KM. In addition, considering the model validity, KM clinical research model should include the diagnosis, interventions and outcomes measurement methods reflecting the characteristics and real practice in KM. The internal validity and external validity should be also taken into account. One of the most important thing is to identify the contents about various and complex 'real practice' in KM. A prospective observational study was suitable for the purpose of this study. Finally, we suggested a few improvement directions for RCTs studies in KM. First, we would be able to improve the quality and the internal validity in KM clinical research using the checklists of CONSORT(Consolidated Standards for Reporting Trials) Statement and STRICTA(Standards for Reporting Interventions in Clinical Trials of Acupuncture). Second, we could use various clinical research methods and the modified research of RCTs such as PCT(pragmatic clinical trial) to reflect the characteristics of actual KM practice. Consequently, we would be able to improve the external validity. Third, the KM diagnosis and outcomes measurement methods should be developed based on an actual KM practice and it should reflect a real practice. The 'pattern identification(辨證)' is the core to KM diagnosis. But in order to be applied to the clinical research, the pattern identification(辨證) should be objectified and standardized. Future KM clinical research model should reflect the characteristics and a real practice in KM. In addition, it should include the advantage of rigorous RCTs research.Specially, the diagnosis, interventions and outcomes measurement methods in KM clinical research should reflect this view.

Effects of Dioscoreae Rhizoma on Excercise Practice Ability and Blood Fatigue Variable Factor (산약의 운동수행능력 증진 및 항피로 효과)

  • Objectives In this study, we analyzed the effect of Dioscoreae Rhizoma on exercise practice ability and blood fatigue variable factor in the forced swimming test. Methods Male mice were divided into four group. Mice were administrated with saline, Discorae Rhizoma extracted by water (DRW), Discorae Rhizoma extracted by 30% ethanol (DRE), red ginseng (RG), orally once per day, for 1 week. Exercise performance was evaluated by exhaustive swimming time and weights. Glycogen contents of liver and soleus muscle were measured at the end of the experiment. In addition, the biochemical parameters such as asparate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen, creatine, lactate dehydrogenase (LDH), ammonia, triglycerides (TG), total cholesterol (TC) and free fatty acid (FFA) were analyzed. Results Both DRW and DRE showed an improvement of swimming endurance compared with control group. Levels of ALT, ammonia, TG, TC and FFA were significantly decreased in both DRW and DRE compared with control group. Levels of AST, Creatine were significantly reduced in DRE group compared with control group. Moreover LDH showed only a tendency to increase in the DRW and DRE groups. Conclusions In summary, the present study showed that Dioscoreae Rhizoma improved in physical fatigue and exercise practice ability and DRE improved the superior than those of DRW.

Poor Knowledge and Practice Towards Breast Cancer among Women in Baghdad City, Iraq

  • Background: Breast cancer is one of the most common cancers among females worldwide. The aim of this study was to assess the knowledge and practice about breast cancer and its related factors among women in Baghdad city, Iraq. Materials and Methods: A cross-sectional study was conducted among 508 women aged 18 to 55 years from four non-governmental organizations (NGO) in Baghdad city, Iraq. A self-administered questionnaire on breast cancer knowledge and practice was distributed to participants during weekly activity of the NGO. Results: A total of 61.2% of the respondents had poor knowledge, only 30.3% performed breast self-examination (BSE) and 41.8% said that they did not know the technique to perform BSE. Associations between knowledge and marital status and age were significant. For practice, working status, education, age and family income were significant. After controlling for cofounders, the most important contributing factors for poor knowledge among respondents were marital status and not performing BSE, with adjusted odds ratio of 1.6 and 1.8 respectively. Conclusions: Breast cancer knowledge and practice of BSE are poor among women in Baghdad city, Iraq. More promotion regarding breast cancer signs and symptoms and also how to perform BSE should be conducted using media such as television and internet as these constituted the main sources of information for most women in our study.
