  • PURPOSE: Bridge exercises are broadly used to develop trunk co-activation patterns that promote spine stability. This study was to analyze the trunk muscle activity during bridge exercise with various shoulder support surface(stable, sling, Swiss ball). METHODS: The subjects were 20 healthy subjects in their twenties. Subjects were performed bridge exercise on 4 different shoulder support surfaces using stable and labile instruments. 1) Bridge exercise on a stable surface. 2) Bridge exercise with their shoulder on a stable bench. 3) Bridge exercise with their shoulder on a sling. 4) Bridge exercise with their shoulder on a Swiss ball. Rectus abdominis, erector spinae, internal oblique, external oblique muscle activities were measured using electromyography. RESULTS: There were significant differences in RA, EO muscles between performing each of the 4 exercises(p<.05). RA and EO was recorded the highest activity during the bridge exercise with their shoulder on a sling. The lowest activity was recorded during conventional supine bridge on a stable surface. There were no differences found for the EO/RA and IO/RA ratio. The EO/RA and IO/RA ratio was the highest in the bridge exercise with their shoulders resting on a stable bench. CONCLUSION: These findings suggest that change of shoulder support surface during bridge exercise may be useful for enhancing the trunk stability.

  • An urinoma(uriniferous pseudocyst, pararenal pseudocyst) denotes an encapsulated collection of urine in the perirenal or paraureteral space. It was usually reported in relation to trauma and acquired obstructive uropathy but rarely reported in renal infection including renal candidiasis. The mechanism is believed due to rupture of fornix through weakened portion of suppurated kidney and pyelosinus backflow by increased intrapelvic pressure in obstructive uropathy and fungus ball obstruction. We report a case of urinoma in a premature as the first case in Korea which developed as a complication of renal candidiasis.

  • A 47-years-old woman presented with a 2-month history of a dry mouth and dry cough. The patient had been taking medication for Sj$\ddot{o}$gren's syndrome for approximately 7 years. The chest radiography showed multiple cystic lesions and a hazy density in both lower lung fields. The HRCT showed a diffuse ground glass like appearance and multiple variable sized cystic lesions in both lung fields. After medication, the symptoms were aggravated. Bronchoscopy was preformed with a transbronchial lung biopsy. The biopsies showed an infiltration of lymphocytes, neutrophils, monocytes and histiocytes through the interstitial space of the alveola and a widening of the alveolar septa. However, the histological findings of the cysts were not obtained. Sj$\ddot{o}$gren's syndrome is a slowly progressive inflammatory autoimmune disease, which is characterized by lymphocyte mediated destruction of the exocrine glands, with pulmonary involvement in approximately 19-65%, High-resolution CT is a sensitive technique for assessing the pulmonary involvement in patients with Sj$\ddot{o}$gren's syndrome. Although a lung biopsy is not always necessary for establishing a diagnosis of an interstitial lung disease in Sj$\ddot{o}$gren's syndrome. A lung biopsy may reveal a wide spectrum of changes ranging from a mild inflammatory response to end stage fibrosis with honeycombing. Because of the predominantly peribronchiolar inflammatory infiltration and inspissated secretions the cysts were suspected to have been formed by the ball-valve phenomen. However, no definite evidence was obtained.

  • A 25-year-old Uzbek male presented with right upper abdominal pain for 20 days. On radiologic studies, a huge cystic mass was noticed in the right liver which was suspected as parasitic. The patient received right hepatic segmentectomy (segment 7), and the surgically resected mass was confirmed as cystic echinococcosis (CE), measuring 10.5 cm in its diameter. The inner surface of the cyst was bile-stained. The patient was discharged on the 8th hospital day, and was rechecked 6 months after the surgical intervention without any evidence of recurrence. The present report describes findings of an imported case of CE which represented ultrasound images of the 'ball of wool'.

  • Objective: The incidence rate of elbow ulnar collateral ligament injuries is dependent on the throwing speed or pitching type, especially in adolescent baseball players. However, mixed results have been reported due to a lack of controlled biomechanical analysis. Thus, the purpose of this study was to investigate the biomechanical analysis of the elbow in relation to throwing speed and pitching type. Method: Four overhead type high-school baseball players were recruited for this study. The participants were asked to throw balls with different types of pitch and speed. While the throwing speeds were measured, each pitching moment of the elbow was recorded. Descriptive statistics, frequency analysis, mean comparison analysis, and Pearson's correlation analysis were performed in order to examine differences in peak varus and valgus moment during pitching motion in the elbow in all throwing speed and pitching types. Results: There was no significant difference in physical characteristics, throwing speed, and momentum variability among all players. The mean varus moments were 44.38±1.55 Nm, 48.83±1.66 Nm, and 48.94±0.95 Nm, and the moment gaps between varus and valgus were 7.36±3.25 Nm, 7.44±2.02 Nm, and 7.36±2.62 Nm in fastball, curveball, and slider ball, respectively. The varus moment was higher in the curved and slider balls than in the fastballs, and there was no significant differences between the varus moments regarding the pitching type. However, the increase in valgus moment and decrease in moment gap according to throwing speed was significantly increased in the slider ball (r=0.718 and -0.591, respectively). Conclusion: The possibility of elbow injury caused by the valgus moment or moment gapincreases more rapidly in slider balls as the speed increases. Based on our results, appropriate pitching guidelines should be suggested to prevent ulnarligament injuries, especially in adolescent baseball players.

  • In this study, numerical simulations were conducted for conjugate heat transfer around ice balls in an encapsulated ice thermal storage system. Four shapes of ice balls were modeled; the default one was a sphere, and the other three shapes were designed to enhance convective heat transfer through the ball surface. The flow around the ball was laminar, for which the Reynolds number was 300, and both forced and natural convections inside and outside the balls were considered. The simulations revealed that the magnitude of convective heat transfer for the different shapes decreased in the following order: bone, dimple, hole, and sphere. For the entire simulation, the maximum difference in the average temperatures of water inside the capsules was found to be $0.9^{\circ}C$. Therefore, it can be said that the effect of ice-ball shape on the performance of the ice thermal storage system is significant, considering that more than 0.3 million balls are used in this system.

  • Cytopathologic findings of two cases of adenoid cystic carcinoma of the trachea are reported. The carcinomas grew as an intratracheal mass. By bronchial washing, brushing and/or post-bronchoscopic sputum cytology, large cohesive sheets, lobulated clusters, or three dimensional ball-like structures were obtained. They had numerous cyst-like spaces containing characteristic globular basophilic material. The tumor cells were uniform and had a small amount of cytoplasm. Nuclei were small and hyperchromatic, Nucleoli were occasionally observed. The cytological diagnosis was confirmed by bronchoscopic biopsies. Since the cytomorphology of adenoid cystic carcinoma is characteristic, review of these cytologic features will enhance the diagnostic accuracy in exfoliative cytology of the respiratory tract.
