Medication Error Management Climate and Perception for System Use according to Construction of Medication Error Prevention System (환자안전 관리자가 인식한 투약오류예방 시스템 구축실태에 따른 투약오류관리풍토 및 활용인식)

  • Purpose: The purpose of this cross-sectional study was to examine current status of IT-based medication error prevention system construction and the relationships among system construction, medication error management climate and perception for system use. Methods: The participants were 124 patient safety chief managers working for 124 hospitals with over 300 beds in Korea. The characteristics of the participants, construction status and perception of systems (electric pharmacopoeia, electric drug dosage calculation system, computer-based patient safety reporting and bar-code system) and medication error management climate were measured in this study. The data were collected between June and August 2011. Descriptive statistics, partial Pearson correlation and MANCOVA were used for data analysis. Results: Electric pharmacopoeia were constructed in 67.7% of participating hospitals, computer-based patient safety reporting systems were constructed in 50.8%, electric drug dosage calculation systems were in use in 32.3%. Bar-code systems showed up the lowest construction rate at 16.1% of Korean hospitals. Higher rates of construction of IT-based medication error prevention systems resulted in greater safety and a more positive error management climate prevailed. Conclusion: The supportive strategies for improving perception for use of IT-based systems would add to system construction, and positive error management climate would be more easily promoted.

A Smart Drug/Medicine Pack for u-Medication Monitoring (유비쿼터스 투약 감시를 위한 스마트 투약 팩)

  • As a pervasive healthcare application to help medication of elderly patients, the smart compliance monitors have been recently proposed. In this paper, we present a new smart drug/medicine pack (SDP) for ubiquitous medication monitoring, for which the packaging is either paper packet or blister pack. This ubiquitous compliance monitor is composed of several SDPs and a Bluetooth equipped PDA phone. The SDP is a wireless module that monitors the medication consumption unobtrusively by transmitting the sensed data of 'drug removal' events to the medical center and by reminding patients when they take a dose on their mobile phone. The communication between SDP modules and PDA is realized via Bluetooth. The PDA is basically embedded to indicate the medication record and to alert every prescribed medication time during treatment. Experimental results show that the proposed system works exactly when the SDPs are far within about 2 [m] from PDA.

Design of a Diabetic Patients Medication Adherence Help System Supporting both Smart Phone Apps and Feature Phone SMS (스마트폰 앱과 피처폰 SMS를 지원하는 당뇨병 환자의 복약 이행 지원 시스템 설계)

  • Medication adherence is a basic and important element in diabetes management, and it has been known the adherence rate should be keep over 80% to get positive result in diabetes management. In order to increase medication adherence, there have been smart phone apps that record medication, exercise, and diet. However, diabetes patients are generally over 50s, and most of them do not use smart phones. Therefore, in this paper, we propose a medication adherence help system that support both smart phone apps and feature phone SMS. Furthermore, we introduce system architecture for the system. Our work will help ICT-based diabetic management system developers to consider some issues for mobile based diabetic management systems..

Development of Textile Proximity Sensor for Medication Adherence Management System

  • In this study, we developed textile proximity sensor using conductive textile to develop a Medication adherence management system. The textile proximity sensor utilizes the principle of the capacitor, and the ring type sensor used to reduce the fringe-effect. When take medicines, we made a custom PCB that converts the change of the data measured by the sensor into a digital value so that transmitted the PC. In order to compare the performance of the system, we evaluated the correlation between the data variation according to the quantity of pills in the electronic compact scale and the data in this system. As a result, Pearson's correlation coefficient was 0.956(p<0.01), confirmed a good correlation between the scale and our system. Therefore, we concluded that our system evaluated able to whether or not to take medication.

Development and evaluation of a high-risk intravenous medication safety program applying the Six Sigma technique: Empirical Research Quantitative

  • This was a one-group pretest-posttest study that developed a high-risk intravenous medication safety program by applying the Six Sigma technique to hospital nurses and measuring the pre- and post-effects. Participants were nurses with more than three years of experience in administering chemotherapy and highly concentrated intravenous electrolytes working at a university hospital in City D. Data analysis was performed using the Minitab 16 PC+ program; frequency, percentage, and 4-block matrix analyses were performed. In the definition stage, an improvement team was formed to proceed with the task, which was defined as a high-risk intravenous medication safety issue in ward nursing. In the analysis stage, a lack of high-risk intravenous medication safety education was identified. In the improvement stage, nurses were provided with high-risk intravenous medication safety education. Post-study knowledge scores increased significantly from 11.24 to 21.32, while performance scores improved from 36.61 to 68.45 (p < 0.05). This study identified and improved the problems in hospital organizational systems and presented a strategy for establishing a safety system in medical institutions and improving human factors. This ultimately enhanced the safety of high-risk intravenous administration.

Assessing the Suitability of Interruption Intervention Strategies in Nursing Medication Administration: A Delphi Study (간호사의 투약업무흐름 중단 중재전략 적합성 연구: 전문가 델파이 조사를 중심으로)

  • Purpose: This study explored the suitability of interventions for medication interruption and intervention preferences. Methods: Two rounds of Delphi surveys were conducted with 18 expert panels comprising staff (or charge) nurses, nursing managers, and Quality Improvement (QI) team nurses working in a tertiary general hospital. For 47 situations involving the location of interruption, medication step, and source of interruption, the suitability of three interventions (no-interruption zone, medication safety vest, and education) was evaluated using a 5-point scale. Results: A total of 51 interventions for each situation were found appropriate by satisfying the degree of convergence and consensus. Patients or caregivers, peer nurses, doctors, telephones, and call bells were sources of interruption and were identified as appropriate for the application of interventions. 'Responding to requests and inquiries' by patients or caregivers showed high overall suitability. The nurses' preferred color for the intervention design (no-interruption zone, medication safety vest) is blue text on a yellow background. The priority groups for education related to medication interruptions were patients or caregivers, nurses, and non-nursing staff, in that order. Conclusion: Effective implementation of tailored intervention strategies that consider the specific characteristics of medication interruptions is crucial for mitigating interruptions and enhancing patient safety. Comprehensive educational programs aimed at reducing medication interruptions by improving awareness are necessary. Moreover, future research should evaluate these strategies in clinical settings to ensure their effectiveness in enhancing patient safety.

Improving Medication Adherence in Isolated Patients With Cognitive Impairment Using Automated Telephone Reminders

  • Background and Purpose: Medication adherence is essential for effective medical treatment. However, it is challenging for cognitively impaired patients. We investigated whether an automated telephone reminder service improves medication adherence and reduces the decline of cognitive function in isolated patients with cognitive impairment. Methods: This was a single-center, randomized clinical trial. We enrolled mild cognitive impairment (MCI) or Alzheimer's disease (AD) patients who lived alone or with a cognitively impaired spouse. We provided an automated telephone reminder service for taking medication to the intervention group for 6 months. The control group was provided with general guidelines for taking the medication every month. The participants underwent neuropsychological assessment at the beginning and end of the study. Statistical significance was tested using nonparametric Wilcoxon rank sum and Wilcoxon matched-pairs signed-rank tests. Results: Thirty participants were allocated randomly to groups, and data for 29 participants were analyzed. The mean age was 79.6 (standard deviation, 6.0) years and 79.3% of the participants were female. There was no significant difference in medication adherence between the 2 groups. However, a subgroup analysis among participants with more than 70% response rates showed better medication adherence compared to the control group (intervention: 94.6%; control: 90.2%, p=0.0478). There was no significant difference in the change in cognitive function between the 2 groups. Conclusions: If a patient's compliance is good, telephone reminders might be effective in improving medication adherence. It is necessary to develop reminder tools that can improve compliance for cognitively impaired patients.

  • This paper presents the Vessel Medicine Management System (VMMS), designed to enhance the management of medications on ships through the application of advanced image processing technologies. The system incorporates Optical Character Recognition (OCR) for the extraction of text from medication packaging, barcode recognition for standardized drug coding, and an intuitive search interface to facilitate efficient inventory management. By automating these processes, the VMMS alleviates the workload of navigational officers and ensures precise tracking of medication usage in accordance with international medical guidelines. A comprehensive database provides essential information regarding dosage, efficacy, and side effects, thereby promoting safety in medical care at sea. Additionally, the system enhances communication with onshore medical professionals, resulting in improved emergency response times and crew welfare. This integration not only addresses the complexities associated with onboard medication management but also supports prospective advancements in maritime remote medical systems. Overall, the VMMS signifies a substantial advancement in the enhancement of healthcare delivery within isolated maritime environments.

  • Purpose: The purpose of this study was to develop an e-Learning program about medication for nurses to enhance nurses' medication performance ability and to analyze learners' responses after studying with this program. Method: For the development of the e-Learning program, the NBISD(Network Based Instructional Systems Design) model, suggested by Jung(I999) was applied as a basic model and the instruction design theory of Gagne & Briggs(1979) and ARCS theory of Keller(I983) were applied. After the operation of this program for one month to 34 new nurses, learners' responses were analyzed. Result: Learners' knowledge of medication was greatly improved after this program. In addition learners' satisfaction with the overall education program, help in field applicability, ease of screen shift and exploration, and tutor activities were high and the contents were regarded suitable for e-Learning. Many things were advantageous such as easy accessibility, easy understandability with pictures and flash animation, practical cases and feedback from a tutor. Provision of a supplementary handout and improvement of a tight time schedule were pointed out as things to be improved. Conclusion: This e-Learning program can be used effectively for medication education for registered nurses, student nurses, and new nurses.
