A clinical study on the patient of Cervical radiculopathy by Bee-venom threapy (경추 신경근증 환자에 있어서 봉약침 치료의 효과에 대한 임상적 고찰)

  • Objective : The aim of this study is to investigate the effectiveness of Bee-venom therapy for Cervical radiculopathy patients. Methods : To evaluate the effectiveness of Bee-venom therapy, 14 patients were treated by Bee-venom therapy. To estimate the efficacy of treatment, we used Quardruple Visual Analog Scale (QVAS), JOA score and Odom's degree. Results & Conclusions : 1. As a objectivity treatment record, they test treatment record excellent(7 case) 50%, good(4 case) 28.57%, fair(,3 case) 21.43%. 2. After Bee-venom therapy, pain rate changed from 8.82 to 3.25.(p=0.000) 3. After Bee-venom therapy, JOA score changed from 11.00 to 12.79.(p=0.000) 4. By the results which puts out the statistics in sex, age, existence of finger numbness and disc type, the pain rate is not significantly difference as a therapy. (p<0.05) 5. By the results which puts out the statistics in sex, age and disc type, the JOA score is not significantly difference as a therapy.(p<0.05) But by the results which puts out the statistics in existence of finger numbness the JOA score is significantly difference as a therapy.(p=.025) There was reports about Bee-venom therapy of the Patient with Cervical radiculopathy. It is very effective to reduce the pain and increase the JOA score.

Prognostic Value of Coronary CT Angiography for Predicting Poor Cardiac Outcome in Stroke Patients without Known Cardiac Disease or Chest Pain: The Assessment of Coronary Artery Disease in Stroke Patients Study

  • Objective: To assess the incremental prognostic value of coronary computed tomography angiography (CCTA) in comparison to a clinical risk model (Framingham risk score, FRS) and coronary artery calcium score (CACS) for future cardiac events in ischemic stroke patients without chest pain. Materials and Methods: This retrospective study included 1418 patients with acute stroke who had no previous cardiac disease and underwent CCTA, including CACS. Stenosis degree and plaque types (high-risk, non-calcified, mixed, or calcified plaques) were assessed as CCTA variables. High-risk plaque was defined when at least two of the following characteristics were observed: low-density plaque, positive remodeling, spotty calcification, or napkin-ring sign. We compared the incremental prognostic value of CCTA for major adverse cardiovascular events (MACE) over CACS and FRS. Results: The prevalence of any plaque and obstructive coronary artery disease (CAD) (stenosis ≥ 50%) were 70.7% and 30.2%, respectively. During the median follow-up period of 48 months, 108 patients (7.6%) experienced MACE. Increasing FRS, CACS, and stenosis degree were positively associated with MACE (all p < 0.05). Patients with high-risk plaque type showed the highest incidence of MACE, followed by non-calcified, mixed, and calcified plaque, respectively (log-rank p < 0.001). Among the prediction models for MACE, adding stenosis degree to FRS showed better discrimination and risk reclassification compared to FRS or the FRS + CACS model (all p < 0.05). Furthermore, incorporating plaque type in the prediction model significantly improved reclassification (integrated discrimination improvement, 0.08; p = 0.023) and showed the highest discrimination index (C-statistics, 0.85). However, the addition of CACS on CCTA with FRS did not add to the prediction ability for MACE (p > 0.05). Conclusion: Assessment of stenosis degree and plaque type using CCTA provided additional prognostic value over CACS and FRS to risk stratify stroke patients without prior history of CAD better.

  • The purpose of this study is to examine the statistical errors, which may occur in the process of applying statistics, by major stage of research, for greatly contributing to promoting research power through rightly utilizing statistics at the point of time that the application of statistics is increased in the research of clothing and textiles. As a result, it confirmed that there are many errors by each stage. First, it is a point that there was not any research that mentioned on the impact of analysis in the researches, which test the appearance of significance based on the collected data through experiment or survey. Second, the existence of mechanical description which is monotonous as well as not applying right analytical method in factor analysis. Also, even in case of cross tabulation, there was no hypothesis establishment.

  • Tuberculosis causes high morbidity and mortality. However, Korea still has the highest tuberculosis (TB) incidence and mortality among OECD countries despite decreasing incidence and mortality due to the development of modern medicine. Korea has now implemented various policy projects to prevent and control tuberculosis. This study analyzes the effects of public-private mix (PPM) tuberculosis control program on treatment outcomes and identifies the factors that affecting the success of TB treatment. We analyzed 130,000 new tuberculosis patient cohort from 2012 to 2015 using data of tuberculosis patient reports managed by the Disease Control Headquarters. A cumulative incidence function (CIF) compared the cumulative treatment success rates for each factor. We compared the results of the analysis using two popular types of competition risk models (cause-specific Cox's proportional hazards model and subdistribution hazard model) that account for the main event of interest (treatment success) and competing events (death).

Nomogram building to predict dyslipidemia using a naïve Bayesian classifier model (순수 베이지안 분류기 모델을 사용하여 이상지질혈증을 예측하는 노모 그램 구축)

  • Dyslipidemia is a representative chronic disease affecting Koreans that requires continuous management. It is also a known risk factor for cardiovascular disease such as hypertension and diabetes. However, it is difficult to diagnose vascular disease without a medical examination. This study identifies risk factors for the recognition and prevention of dyslipidemia. By integrating them, we construct a statistical instrumental nomogram that can predict the incidence rate while visualizing. Data were from the Korean National Health and Nutrition Examination Survey (KNHANES) for 2013-2016. First, a chi-squared test identified twelve risk factors of dyslipidemia. We used a naïve Bayesian classifier model to construct a nomogram for the dyslipidemia. The constructed nomogram was verified using a receiver operating characteristics curve and calibration plot. Finally, we compared the logistic nomogram previously presented with the Bayesian nomogram proposed in this study.

  • Multi-view learning considers data from various viewpoints as well as attempts to integrate various information from data. Multi-view learning has been studied recently and has showed superior performance to a model learned from only a single view. With the introduction of deep learning techniques to a multi-view learning approach, it has showed good results in various fields such as image, text, voice, and video. In this study, we introduce how multi-view learning methods solve various problems faced in human behavior recognition, medical areas, information retrieval and facial expression recognition. In addition, we review data integration principles of multi-view learning methods by classifying traditional multi-view learning methods into data integration, classifiers integration, and representation integration. Finally, we examine how CNN, RNN, RBM, Autoencoder, and GAN, which are commonly used among various deep learning methods, are applied to multi-view learning algorithms. We categorize CNN and RNN-based learning methods as supervised learning, and RBM, Autoencoder, and GAN-based learning methods as unsupervised learning.

Analysis and Improving ways of Factors affecting the Ill-defined Causes of Death of the Aged in Korea (노인의 불명확한 사망원인진단 관련요인 분석과 개선방안)

  • This research analyzed 168,010 cases of death of the aged over 65 from 244,867cases of death excluding 7 unknown age cases from 244,874 all age cases of death by using the death data of the National Statistical Office for 2007 to figure out factors affecting the quality of causes of death statistics of the aged and to suggest the ways of improving the quality of death statistics of the aged in korea. This research tried to derive factors affecting ill-defined cause of death category in acordance with WHO's guidelines and to find causes of lowering the accuracy of causes of death statistics of the aged. This research identified the problems of causes of death statistics of the aged by using both demographic characteristics such as sex, age, marital status, educational attainment, residential region, region size and factors of death items as independent variable to find causes of ill-defined cause of death of the aged. Logistic regression analysis was executed to calculate the hazard ratio about the ill-defined causes of death of the aged and multiple regression analysis was conducted to derive factors affecting the ill-defined cause of death by regional groups through using these independent variables such as the component ratio of over age 65, female death rates, doctors insitutions rate, medical institutions rate, attaching rates of death certificate by neighborhood. As a results of this research, R-code was the highest of ill-defined causes of death, accounting for 82.1%, and senility death(R54) of R-code was the highest, accounting for 91.2%. through subdivided order distribution of the ill-defined causes of death of the aged. As ill-defined causes of death by regional groups, attaching rates of death certificate by neighborhood was the most important factor(p<0.05) and also showed regression model's description with 83.8% ($R^2$=83.8%). Furthermore, Jeon-nam was the highest in the regional groups and these regions such as Je-ju, Jeon-buk, Chung-nam were not only attaching the death certificate by neighborhood but also were high at the rate of ill-defined causes of death. Therefore, this research found that both reconsideration about death certificate by neighborhood and education for doctors who write death certificate were needed the most.

  • Hospitals today use independent systems for each department and job such as Hospital Information Sytem(HIS), Picture Archiving Communications System(PACS), Ordering Communication System(OCS), Electronic Medical Record(EMR), Enterprise Resource Planning(ERP), etc and each system employs its own DB. So, it is impossible to integrate information within the institution and difficult to keep transparency and consistency of data. I in this study offered a data integration environment through flexible management linked with other systems, and by doing that, designed a medical CRM frame which offers the optimum service the customer wants at the optimum time. I designed 4 of medical CRM frame: customer relationship management, public relations/marketing, service management, and statistics/analysis by the customer relationship management process standardization and aimed to offer tailored mobile contents according to customer's characters and health situation on the basis of customer's data by securing mobile medical contents for personalized medical information service.

A Research of Trend on Japanese Medical Tourism in Oriental Hospital of Daejeon University (대전대학교 한방병원에 내원한 일본인 관광객의 의료관광 현황)

  • Objectives : This study is to evaluate the current situation of Japanese tourists for medical tour of Daejeon University hospital and to draw up a plan for better policies. Methods : 59 Japanese tourists visiting oriental medicines hospital of Daejeon University from January 2012 to September 2012, were analysed in the statistics. And 8 of them answer a questionnaire about reasons for selection of Daejeon University hospital, satisfaction for thread embedding therapy and side effects of thread embedding therapy. Results : A total of 59 foreigners visited oriental medicines hospital of Daejeon University for medical service, consisting of 54 females(92 %), the thirties to fifties 71 % by age. The 87.5 % of patients answered that the reason for choosing this hospital was the subsidization of the medical expenses, and 50 % for appropriate payment, 37.5 % for safety, 12.5 % for recommendation of the people who had good experience at this hospital. The 71% of patients selected thread-embedding therapy for treating wrinkles. The degree of satisfaction was evaluated as quite satisfaction of 87.5 %, full satisfaction of 12.5 % showing that a whole number of the patients treated with thread-embedding therapy showed relative satisfaction. This evaluation is, however, a short term survey which should lead to a further term study. For the evaluation of side effects of thread-embedding therapy, 75 % of patients answered as pain occurred during the therapy, 62.5 % as edema, 50 % as hypodermal bleeding. Conclusion : First of all, there should be more constructive promotion and support for medical tour of oriental medicines, ultimately leading to promoting better clusters of oriental medicines. Secondly, support in terms of a medical law should be established for medical disputes, and the best follow-up service should be considered.

Awareness and Attitude of Nurses in Regard to Providing Hospice Care

  • Background: Awareness and attitudes of nurses regarding end of life care are important factors in providing hospice care. In an extensive literature review, we found no related articles investigating Iranian nurses awareness and attitudes about providing such care. Objectives: The aims of this study were to investigate the awareness and attitudes of Iranian nurses in providing hospice care. Materials and Methods: In this descriptive-correlational study, 240 nurses employed in six educational centers were selected by non-randomized stratified sampling. The data collection instruments included an awareness test and attitudes regarding providing end of life care in hospice questionnaire. The data were analyzed using descriptive statistics and independent sample t-tests, one-way ANOVA, and Pearson correlation tests. Results: The nurses' awareness score was 14.3 out of 29 and 55.7% of them stated that they had not received any education in providing end of life care. Also, by obtaining the score of 91.7 out of 120 the attitudes of participants in providing end of life care in hospices were positive. In addition, the highest attitudes score of nurses were in the dimensions of benefits of implementation and health care team. Conclusions: Considering low awareness of nurses about end of life care in hospices, continuing education should be provided for them in this regard. Especially, by considering the positive attitude of nurses, providing such programs could help develop hospice care in Iran.
