Efficiency analysis of Oriental hospitals according to characteristics (한방병원 특성별 경영효율성 분석)

  • This study analyzes the efficiency of oriental hospitals using DEA(Data Envelopment Analysis). The input variables include the numbers of doctors, nurses, medical technicians, and beds. The output variable iscomprised of the sales account. The analysis tools used are EnPas and IBM SPSS Statistics 19. As a result of efficiency analysis, the private hospitals(establishment), less than 10 years in operation(operating period), containing less than 50 beds (number of the beds), located in the metropolitan area(location) showed high efficiency in the BCC(Banker, Charnes & Cooper) model, but indicated relatively low efficiency in CCR(Charnes, Cooper & Rhodes) model. This contradictory result is caused by inefficiencies in hospital size. The logistic regression analysis conducted to analyze the variables that affect the efficiency of oriental hospitals found that the efficiency decreased by 0.955 with each increase of 1 bed in the hospital.

An indicator for managing the regional variations in approval rates of long-term care (LTC) service (지역별 장기요양 인정의 차이 관리지표 개발)

  • This study develops an indicator to manage regional variations of approval rates for long-term care (LTC) service. We used LTC insurance data for grade assessment that include 433,155 applicants from 227 LTC centers across Korea in 2015. The approval rate for each center was defined as the proportion of the numbers of approved applicants out of all applicants. We assumed that the approval rates depended on the characteristics of applicants. We estimated the 'standard' approval rates from a multiple linear regression analysis using the characteristics of applicants as independent variables. The difference between the observed and the standard rates was then defined as an indicator for deviation. A center having a large difference could be considered as a center with a potential error in grade assessment. We also examined if the characteristics of investigators affected the approval rates. We found that the socio-demographic characteristics of applicants and reapplication rate for LTC grade were independent factors affecting the approval rates. Centers having the management indicator values falling outside the middle 95% of the distribution were identified as centers with an error in grading. We expect that this study will contribute to enhancing reliability and equity in LTC grading.

A Statistical Study of the Patients Admitted to the Department of Acupuncture and Moxibustion Medicine (침구의학과에 입원한 환자들에 대한 통계적 고찰)

  • Objectives : The aim of this study was to investigate clinical features of the patients admitted to the department of acupuncture and moxibustion in an oriental medicine hospital in order to provide clinical preliminary data for advancement of acupuncture and moxibustion Methods : Total 526 patients were admitted to the department of Acupuncture and Moxibustion in Dunsan Oriental Hospital of Daejeon University from September 1st, 2011 to August 31st, 2012. Of these patients 11 were discharged on the day that they were admitted and the remaining 526 patients who were hospitalized for over 2 days were investigated this study. We analyzed inpatients by discharge progress notes and medical records, using PASW statistics 18.0 for statistical analysis. Results : 1. There were 361 females(62.9%) and 213 males(37.1%). The age distribution was skewed to 40s and 50s and the average age of the patients was 42.91 years. 2. The prevalence of the diseases groups was in the order of musculoskeletal disease, neuromuscular disease, neuro-psychological disease, and EENT disease. The musculoskeletal disease and neuromuscular disease together made up 92% of all diseases. 3. The prevalence of individual diagnosis was in the order of Bell's palsy, cervical sprain, lumbar sprain, herniated lumbar disc, herniated cervical disc, and lumbar spinal stenosis. 4. There were 261 patients with single diagnosis and 193 with two and 120 with three or more diagnoses. The days of hospitalization significantly increased with the number of diseases the patients were diagnosed with. Conclusions : With high concentration of patients seen in certain disease groups, continuous efforts needed to expand the usage of acupuncture and moxibustion treatments to other disease groups and to explore and develop other diverse treatment methods for better outcome of the disease treated.

Exposure to Tobacco Advertising and Promotion among School Children Aged 13-15 in Vietnam - an Overview from GYTS 2014

  • Evidence shows that tobacco advertising and promotion activities may increase tobacco consumption and usage, especially in youth. Despite the regulation on prohibiting advertisement of any tobacco product, tobacco advertisement and promotion activities are still common in Vietnam. This article presents current exposure to tobacco advertising and promotion (TAP) among school children aged 13 to 15 years in Vietnam in 2014 and potential influencing factors. Data from the Global Youth Tobacco Survey 2014 in Vietnam covering 3,430 school aged children were used. Both descriptive and analytical statistics were carried out with Stata 13 statistical software. Binary logistic regression was applied to explain the exposure to TAP among youth and examine relationships with individual factors. A significance level of p<0.05 and sampling weights were used in all of the computations. In the past 30 days, 48.6% of the students experienced exposure to at least 1 type of tobacco advertising or promotion. Wearing or otherwise using products related to tobacco was the most exposure TAP type reported by students (22.3%). The internet (22.1), points of sales (19.2) and social events (11.5) were three places that students aged 13-15 frequently were exposed to TAP. Binary logistic results showed that gender (female vs male) (OR = 0.61, 95%CI: 0.52 - 0.71), susceptibility to smoking (OR = 2.12, 95%CI: 1.53 - 2.92), closest friends' smoked (OR = 1.43, 95%CI: 1.2 - 1.7) and parents smoking status (OR = 2.83, 95%CI: 1.6 - 5.01) were significantly associated with TAP exposure among school-aged children. The research findings should contribute to effective implementation of measures for preventing and controlling tobacco use among students aged 13-15 in Viet Nam.

TREATMENT OF DENTOFACIAL DEFORMITY PATIENT WITH CEREBRAL PALSY (뇌성 마비를 동반한 악안면 기형 환자의 치험례)

  • Cerebral palsy(CP) is one of the most common motor disease, due to brain injury during fetal and neonatal development which results in neuromotor paralysis and associated neuromuscular symptoms. Features of CP include motor disability due to the lack of muscle control, often accompanied by sensory disorders, mental retardation, speech disorders, hearing loss, epilepsy, behavior disorders, etc. There are increasing chances of treatment of dental patients with cerebral palsy, as the occurrence of CP is increasing with the decrease in infant mortality and an increase in immature birth and premature birth and also, there is a trend to pursue of higher quality of life. Reports on the relationship between CP and maxillofacial deformity are uncommon, but it is well known that the unbalance and discontrol of the facial muscles, lip, tongue and the jaws leads to malocclusion and temporomandibular joint disorders, and statistics show that class 2 relationship of the jaws and open bite is frequently reported. However, it is difficult to perform maxillofacial deformity treatment, which consists of orthodontic treatment, maxillofacial surgery and muscle adaptation training, due to difficulties in communication and problems of muscle adaptation caused by difficulties in motor control which leads to a high recurrence rate. This case report is to trearment of maxillofacial deformity in CP patient. A 26 year old female patient came to the department with the chief complaint of prognathism of the mandible and facial asymmetry. According to the past medical history, she was diagnosed as cerebral palsy 1 week after birth, classified as GMFC, classII accompanied with left side torticollis. The patient's intelligence was moderate, and there were no serious problems in communication. For two years time, the patient underwent lingual frenectomy, pre-operation orthodontic treatment and then bimaxillary orthognathic surgery to treat mandibular prognathism and facial asymmetry followed by rehabilitatory exercise of facial muscle. After 6 months of follow up, there was a good result. This is to report to the typical signs and symptoms of DFD in CP patient and the limitation of the usual method of the treatment of DFD in CP patient with literature review.

Network Analysis using Cross-citation Frequency of Clothing & Textiles -Related Journals (의류 관련 학술지의 상호인용 빈도를 이용한 네트워크 분석)

  • The purpose of this study was twofold: 1) to analyze the impact factor of clothing and textile-related journals by utilizing the average number of cross-citation that evaluates the relative importance of an academic journal and 2) to provide a list of journals with higher impact factor by analyzing closeness and betweenness among individual journals through graph networks. To fulfill this, a total of 10 clothing and textile-related journals, which are accredited by National Research Foundation of Korea, were analyzed. For analysis of the average number of cross-citation, the targeted research papers were limited to those published between 2008 and 2011 and they were derived from the Korea Citation Index. The software used for network analysis was R ver. 2.15. The results of the study were as follow: First, 'The Korean Society of Knit Design' was indicated as the highest rate of self-citation, followed by 'Journal of the Korean Fashion & Costume Design Association.' Secondly, the average impact power of clothing & textile-related journals was relatively lower (0.681) compared to that (1.00) of 23 journals under the human ecology discipline. 'Korean Journal of Human Ecology' was found to have impact factor of 1.24, which was higher than the average impact factor of human ecology-related journals. Lastly, together with 'Journal of the Korean Society of Clothing and Textiles.'

Health Promoting Behaviors, Health Problems and Self-Rated Health status in Female marriage immigrant in Korea (여성결혼이민자의 건강증진행위, 건강문제 및 주관적 건강상태)

  • This study is to identify health promoting behaviors and health problems of female marriage immigrant in Korea and is an descriptive study for searching the factors influencing health problems of them. And to provide basic data for developing health promoting interventions for them. Methods: The tools for this study were HPLP II developed by Walker et al. (1995) and modified and translated by Park (2005), CMI developed by Weider et al. (1945) and translated by Go & Park(1980), and modified by Seo and Park(2010). Data were analyzed with descriptive statistics, t-test, ANOVA, and correlation and stepwise multiple regression with the SPSS WIN 18.0 program. Results Conclusion: Female marriage immigrants have done low level of health promoting behavior(2.01) and they had more mental health problems than physical health problems. Factors influencing health problems of the participants were found to be self-rated health and general character which includes age-gap with partner, partner's education and age of female marriage immigrant. Factors influencing self-rated health of the participants were found to be exercise of health promoting behaviors and monthly income. In consideration of this result, we need to provide health education and develop health programs for female marriage immigrant in Korea.

A Study for Convergence Curriculum of Creative Practical Use of the Direction of Characterization of Healthcare - Centred on a University - (보건의료 특성화 방향에 대한 창의실용 융복합 교육과정을 위한 연구 - 일개 대학을 중심으로 -)

  • This study is to establish the direction of special education curriculum by linking the demands of the health industry and the university curriculum. For this, we conducted a questionnaire survey for 4 months from 18 January to 9 May, 2017. Cronbach alpha coefficient was used for the reliability of the study, frequency analysis was used mainly for the contents of the study, and chi-square test, variance analysis and T test were applied to the study. Cronbach's alpha coefficient was 0.891. Variance analysis, T test and chi-square test show some significant differences. The results of this study are as follows: 35% of practitioners and 22% of convergent intellectuals are desirable in institutions. Practical needs of practitioners are creative practicality, medical policy (58%). It will be necessary to conduct a nationwide survey in the future.

An Association of Periodontitis and Diabetes (치주질환과 당뇨병의 관련성)

  • This study was conducted to evaluate the association between periodontal disease and diabetes mellitus. The final analysis population of this study was composed of 4,830 adults with diabetes mellitus or periodontal disease and aged 19 years or older, based on the third-edition data of the 4th Korea National Health and Nutrition Examination Survey (KNHANES) (in 2009). Diabetic status and potential confounders were used in questionnaire materials and physical examination materials, and the presence of periodontal disease was used in the materials for oral health examination by a dentist. For diabetic status, fasting plasma glucose (FPG) levels <100 mg/dl were subcategorized into normal group and FPG levels ${\geq}100mg/dl$ into impaired fasting glucose group; glycosylated hemoglobin (HbA1c) levels <7% into normal group and HbA1c ${\geq}7%$ into diabetes group, on the basis of the American Diabetes Association. According to the 2009 Korea Health Statistics, the case where the pocket depth is 3 mm or more was defined as periodontal disease. The association between the two diseases was evaluated through $x^2$-test and logistic regression analysis using R-commander 2.14. In impaired fasting glucose group, community periodontal index (CPI) groups 3 to 4 had higher risks for periodontal disease 1.23 times (95% confidence interval, 1.07~1.42) than those of CPI groups CPI 0~2, even after adjustment for several confounders. In addition, periodontal disease and diabetes mellitus showed statistically significant differences depending on age, sex, income level, educational background, smoking status, alcohol consumption, and snack intake. The analyses of the third-edition data of the 4th KNHANES showed that there was a statistically significant association between periodontal disease and diabetes mellitus as examined by means of CPI in this study.

The Relationship between Health Belief.Self-efficacy and Exercise.Diet Compliance in Coronary Heart Disease Patients (관상동맥질환자의 건강신념 및 자기효능감과 운동 및 식이요법 이행과의 관계)

  • This study evaluates the relationship between health belief. self-efficacy and exercise and diet compliance in coronary heart disease patients. The study subjects were 96 CHD patients who visited the outpatient clinic at 3 general hospitals in Taegu city from April 3, 1997 to May 3, 1997. Data was collected by the researcher and two registered nurses who work at a cardiac outpatient clinic. Face-to-face interview was conducted. Moon's health belief scale(l990) and Hicky and others' Cardiac Diet Self-Efficacy Instrument (CDSEI, 1992), Cardiac Exercise Self-Efficacy Instrument (CESEI, 1992) were used. The instrument developed by the researcher based on the reference review was used to measure exercise and diet compliance. The data were analyzed by using descriptive statistics, Pearson correlation coefficient, t-test, ANOVA, Tukey verification and Stepwise multiple regression with the SAS program. The results of this study were as follows; 1. The degree of health belief(score range: 1-4) perceived benefit: 3.06 barrier: 2.04 severity: 2.93 2. The degree of self-efficacy(score range: 1-5) exercise self-efficacy: 2.91 diet self-efficacy: 3.32 3. The degree of compliance (score range: 1-4) exercise compliance: 2.34 diet compliance: 2.95 4. The exercise compliance had a positive correlation with perceived benefit(r=0.5327, p=0. 0001), severity(r=0.2780, p=0.0061), exercise self-efficacy(r=0.6675, p=0.0001), and a negative correlation with barrier{r= -0.4236, p=0.0001). The diet compliance had a positive correlation with perceived benefit (r=0.6439, p=0.0001), severity(r=0.4244, p=0.0001), diet self-efficacy(r=0.6629, p=0.0001), and a negative correlation with barrier{r= -0.5098, p=0.0001). 5. According to pt's education level, (F=3.02, p=0.0336), received massage from mass media on exercise and diet(t=3.81, p=0.0002), presence of cardiac patients in the family members or friends(t=2.00, p=0.0478), created significant differences in exercise compliance. According to occuption(F=3.03, p=0.0215), hospitalized experience(t=4.59, p=0.0000), presence of chest pain(t=3.63, p=0.0005), there was also a significant difference in diet compliance. 6. The combination of exercise self-efficacy, perceived benefit and pt's education level explained 50.18% of the variance in exercise compliance. The combination of diet self-efficacy, perceived benefit and barrier explained 56.76% of the variance in diet compliance. On the basis of the above findings, the follow ing recommendations are suggested: 1. To promote the exercise. diet compliance for CHD patients, a well organized health teaching and nursing intervention program should be developed. 2. More research is needed to investigate other variables affecting exercise and diet compliance of CHD patients. 3. To promote self-efficacy and a positive health belief in CHD patients, a well organized and an approachable nursing intervention program should be developed. 4. Factors other than diet. exercise compliance should be evaluated to discover the impact on CHD patients.
