Study on Application of Skin Care Cosmetic and Stabilization of Idebenone by Forming Niosome Vesicle Technology

  • This study is to stabilize insoluble and unstable active ingredient which is Idebenone (INCI name: hydroxydecyl ubiquinone) in a multi-lamellar vesicle (MLV) and to stabilize it in the skin care cosmetics. Idebenone is good effective raw material in the treatment of Alzheimer's disease in the medical field and a powerful antioxidant in dermatology. It is well known as a substance that inhibits the formation of melanin and cleans the skin pigment. However, it did not dissolve in any solvent and it was difficult to apply in cosmetic applications. Niosome vesicle was able to develop a nano-particle by making a multi-layer of idebenone encapsulated with a nonionic surfactant, hydrogenated lecithin and glycine soja (soybean) sterols and passing it through a high pressure microfluidizer. Idebenone niosome vesicle (INV) has been developed to have the ability to dissolve transparently in water and to promote transdermal penetration. The appearance of the INV was a yellowish liquid having specific odor, and the particle size distribution of INV was about 10~80 nm. The pH was 5~8 (mean=6.8). This capsulation with idebenone was stored in a $45^{\circ}C$ incubator for 3 months and its stability was observed and quantitatively measured by HPLC. As a result, the stability of the sample encapsulated in the niosome vesicle (97.5%) was about 66.3% higher than that of the non-capsule sample of 32.5%. Idebenone 1% INV was used for the efficacy test and clinical trial evaluation as follows. The anti-oxidative activity of INV was 38.2%, which was superior to that of 12.8% tocopherol (control). The melanin-reducing effect of B16 melanoma cells was better than INV (17.4%) and Albutin (control) (9.6%). Pro-collagen synthesis rate was 128.2% for INV and 89.3% for tocopherol (control). The skin moisturizing effect was 15.5% better than the placebo sample. The elasticity effect was 9.7% better than the placebo sample. As an application field, INV containing 1% of idebenone is expected to be able to develop various functional cosmetic formulations such as skin toner, ampoule essence, cream, eye cream and sunblock cream. In addition, it is expected that this encapsulated material will be widely applicable to emulsifying agents for skin use in the pharmaceutical industry as well as the cosmetics industry.

  • Purpose: The biggest problem of wound healing is a possible occurrence of lesion. Especially, in the case of patients who have a skin injury around exposed body parts, if their treatment period drag on for long time, they can suffer from aftereffects and the costs can be passed on to a society. Therefore, in this research, we investigated the need to develop the effective medicine and appliances for the patients by examining which therapy methods are being applying to the skin damage and what is the advantage and limit by evaluating the patient's satisfaction level. Methods: We carried out an online and offline survey targeting medical teams in order to analyze device for wound care. A total of 125 medical teams applied to the research, and investigate the level of customer satisfaction. Results: The moist dressings are the most used method for wound healing. When it comes to the level of customer satisfaction, biological dressing product also has a high satisfaction level. However its high cost tends to limit the use. Conclusion: This research reached a conclusion that it is need to develop a low cost and high efficiency wound care product considering the fact that its high cost and low efficiency induced economic problems. Generally, it is needed to develop a product for skin regeneration based on biological technologies, not a product just for damage cure.

Analysis of Characteristics of Dermatology Outpatients in Korean Medicine Ophthalmology, Otolaryngology & Dermatology at Dunsan Korean Medicine Hospital - From March, 2016 to February, 2019 - (둔산한방병원 한방안이비인후피부과 내원 피부질환 환자 특성 분석 - 2016년 3월부터 2019년 2월까지 -)

  • Objectives : This study is designed to analyze outpatients who received medical treatment for skin diseases in ophthalmology & otolaryngology & dermatology clinic of Korean Medical Hospital from March 1st, 2016 to February 28th, 2019. To discover the tendencies of dermatology outpatients and provide the base data for future researches in dermatology patients' characteristics, korean medical treatments on dermatology field, more statistical analysis and establishment of korean medical health care policy. Methods : We classified dermatology outpatients who received treatments in ophthalmology & otolaryngology & dermatology clinic of Korean Medical Hospital from March 1st, 2016 to February 28th, 2019 according to gender, year, age, seasonal, main diagnosis, visit days. The statistical analysis was performed using IBM SPSS 23.0 Results : The results of the analysis on 651 dermatology patients treated in ophthalmology & otolaryngology & dermatology clinic of Dunsan Korean Medical Hospital for the past 3 years are as follows. 1. The number of outpatients who visited ophthalmology & otolaryngology & dermatology clinic of Dunsan Korean Medical Hospital for skin diseases were 651; 286 male(43.9%) and 365 female(56.1%). The number of female patients were approximately 1.27 times higher. 2. The average age of the dermatology outpatients were 37.6. The most frequent visitors turned out to be 20s, and the number of 20s, 30s patients were 267, consisting 41.0% of the whole patients. 3. By examining the change on yearly basis, we noticed that the number of outpatients treated with skin diseases steadily increased with the years. 1) In the analysis of the number of patients classified by gender and year, the number of male and female patients both increased with the years. 2) In the analysis of the number of patients classified by age and year, there were statistically significant difference between two groups. 3) In the analysis of the number of patients classified by season and year, there were statistically significant difference between two groups. 4) In the analysis of the number of patients classified by main diagnosis and year, there were statistically significant difference between two groups. 4. In the analysis of the number of outpatients by seasonal basis, the number of the outpatients visited in summer was the largest by 185(28.4%), followed by winter 181(27.8%), spring 148(22.7%), and the autumn 137(21.0%). 1) In the analysis of the number of patients classified by age and season, there were no statistically significant difference. 2) In the analysis of the number of patients classified by age and season, there were no statistically significant difference. 5. By classifying the outpatients according to main diagnosis, it turned out that there were 40 diagnosis, with eczema the largest followed by urticaria, allergic contact dermatitis, seborrheic dermatitis, and folliculitis. 6. The average number of visit days of dermatology outpatients were 10.24 days, and the number declined steadily with the years. 1) In the analysis of the number of patients classified by gender and visit days, there were no statistically significant difference. 2) In the analysis of the number of patients classified by age and visit days, the visit days of 70s were the shortest and the patients below 20s were longer than the other age groups. Conclusions : By analyzing the outpatients who received medical treatment for skin diseases in ophthalmology & otolaryngology & dermatology clinic of Korean Medical Hospital from March 1st, 2016 to February 28th, 2019 according to gender, year, age, seasonal, main diagnosis, visit days, We drew various meaningful results. The outpatients with skin diseases increased steadily by the years, and patients visited in winter especially shown the outstanding increase. And though the number of female patients were larger than male, the increase was bigger in male patients. We hope that this results would provide useful resources for future researches in dermatology patients'characteristics, korean medical treatment and establishment of korean medical health care policy.

Dual-Toehold-Probe-Mediated Exonuclease-III-Assisted Signal Recycles Integrated with CHA for Detection of mecA Gene Using a Personal Glucose Meter in Skin and Soft Tissue Infection

  • Staphylococcus aureus integrated with mecA gene, which codes for penicillin-binding protein 2a, is resistant to all penicillins and other beta-lactam antibiotics, resulting in poor treatment expectations in skin and soft tissue infections. The development of a simple, sensitive and portable biosensor for mecA gene analysis in S. aureus is urgently needed. Herein, we propose a dual-toehold-probe (sensing probe)-mediated exonuclease-III (Exo-III)-assisted signal recycling for portable detection of the mecA gene in S. aureus. When the target mecA gene is present, it hybridizes with the sensing probe, initiating Exo III-assisted dual signal recycles, which in turn release numerous "3" sequences. The released "3" sequences initiate catalytic hairpin amplification, resulting in the fixation of a sucrase-labeled H2 probe on the surface of magnetic beads (MBs). After magnet-based enrichment of an MB-H1-H2-sucrase complex and removal of a liquid supernatant containing free sucrase, the complex is then used to catalyze sucrose to glucose, which can be quantitatively detected by a personal glucose meter. With a limit of detection of 4.36 fM for mecA gene, the developed strategy exhibits high sensitivity. In addition, good selectivity and anti-interference capability were also attained with this method, making it promising for antibiotic tolerance analysis at the point-of-care.

  • In this study, the cartridge part of the focused ultrasound stimulation system was used as a sample to conduct a skin irritation test and a skin sensitization test through animal experiments among the tests related to GLP (Good Laboratory Practice), a medical device safety evaluation standard. The test was conducted after IACUC approval using 6 female New Zeland White Rabbits. The polar and non-polar stimulation indices were all '0.0'. In addition, in the case of skin sensitization evaluation, 30 guinea pigs approved by IACUC were divided into control and experimental groups, and all induction and induction steps were applied using the eluate itself without separate dilution of the test sample and blank test solution. As a result of the experiment, the skin reaction grade of the control animal was 0 grade, and there was no animal showing more than 1 grade in the skin reaction of the test animal. As a result of both tests, no abnormal skin symptoms were observed, and when applied to the human body to treat patients, the test materials used in the tests will investigate the stability of whether any diseases that cause skin abnormal symptoms will occur.

Adaptation and Evaluation of the Incontinence Care Protocol (실금간호 프로토콜 수용개작 및 효과 검증)

  • Purpose: This study was done to develop an evidence-based incontinence care protocol through an adaptation process and to evaluate the effects of the protocol. Methods: The protocol was developed according to the guideline of adaptation. A non-randomized controlled trial was used for testing the effects of the new Incontinence Care Protocol. A total of 120 patients having bowel incontinence with Bristol stool type 5, 6, and 7 and admitted to intensive care units were recruited to this study. The newly developed incontinence care protocol was used with patients in the experimental group and conventional skin care was given to patients in the control group. Outcome variables were incontinence-associated dermatitis (IAD) severity, pressure ulcer occurrence and severity. Results: The experimental group had significantly less severe IAD (t=6.69, p <.001), lower occurrence of pressure ulcers (${\chi}^2=7.35$, p =.007), and less severity of pressure ulcers (Mann-Whitney=86.00, p =.009) than the control group. Conclusion: Use of this incontinence care protocol has the effects of preventing pressure ulcers and inhibiting worsening of IAD and pressure ulcers. Therefore, this incontinence care protocol is expected to contribute to managing IAD and pressure ulcers.

Health Problems and Health Services in Child Day Care Centers (서울시 1개 구 구립 보육시설의 영유아 건강관리 실태)

  • Purpose: To investigate the health problems and health services in child day care centers. Methods: Data were collected from 115 teachers at 16 child day care centers in Seoul city. A questionnaire was used to collect data, which were analyzed with the SPSS 12.0 program. Results: Most teachers had experienced various child health problems such as colds, hand․foot․mouth diseases, chicken pox, skin injuries, nasal bleeding, vomiting and diarrhea. Furthermore, they even experienced some serious ones including dysentery, measles, asthma and seizures, which demand professional skill. However, there were no registered nurses and most teachers requested that parents take a child home when these health problems happened. Only 31.3% of the child care centers had a teacher with CPR training. Approximately half of the centers kept child health records which included reports on allergic substances, and medical history but only 18.7% of the child care centers offered regular immunizations for the children. Conclusion: Various health problems were found in child day care centers. To maintain the children's health, there is a need to develop and make provisions for health services and programs in child day care centers.

A Literatural Study on the Traditional Korean Medical Physiotherapy (한방물리요법에 대한 문헌적 고찰)

  • Objectives : The purpose of this study is to establish literatural evidence about traditional Korean medical physiotherapy through literatural review. Methods : Applicable paragraphs which were related to the traditional Korean medical physiotherapy were phrased from in "Hwang-je-nae-gyung(黃帝內經)", "Yumun-sa-chin(儒門事親)", "Geum-guei-yo-ryack(金匱要略)", "Eui-hack-ip-mun(醫學入門)", "Gyung-ack-jeon-seo(景岳全書)", "Eui-hack-gang-mock(醫學綱目)", "Dong-eui-bo-gam(東醫寶鑑)" where were archiving of Oriental or Korean medicine literatures. Searched paragraphs were analysed for establishing historical and theoretical bases of Korean medical physiotherapy. Results : 1. Theromtherapy is originated from yu(熨), hot hand, warm, water bath, warm earth, fumigation, lamp, torchlight and brazier. Matching indications are various musculoskeletal pain and diseases, skin diseases such as chronic inflammation, frostbite, gynecological and urogenital diseases such as uterine myoma, lower abdominal pain, metrorrhagia, breast diseases, digestive tract diseases such as vomiting, diarrhea, stomachache and also it treats cardiovascular disease. Other matching indications are sunstroke, rhinorrhea, psycosis and anal pain. And it used helping acupuncture therapy. 2. Cryotherapy is originated from lengfu(冷敷), lengtie(冷貼), cold stone and cold water. Matching indications are various fever disease, skin diseases such as acute inflammation, wound, fever skin disease, otorhinolaryngological diseases such as acute sore throat, otitis, stomatitis, nasal bleeding and contusions, abrasions, burn and dysuria. In addition, it treats extremity coldness, chillness, stomachache caused to accumulate fever. 3. Physiotherapy by circulating meridian system is originated from manipulation such as pushing, massage and beating to therapy point. Matching indications are musculoskeletal diseases such as various muscular pain, heavy sensation, convulsion, urogenital diseases such as dysuria, lower abdomen pain, digestive tract diseases such as stomachache, and paralysis due to blocked meridian system. And other matching indications are psycosis, leprosy, hypoesthesia, acute stroke. In addition it helps other therapy methods. In addition, the power of manipulation classify to strengthen and reduce. 4. Physiotherapy by balancing meridian system is originated from sunshine, magnet, horn, bamboo and tube. Sunshine helps improving self's care and cure inflammatory skin disease. Magnet's matching indications are paralysis, various injuries, abscess, fever and eye disease. Cupping therapy treats to skin diseases and internal diseases such as tuberculosis. Conclusions : Traditional Korean medical physiotherapy was widely used in traditional Korean medicine since ancient time based on the traditional Korean medical principle and anatomical knowledge. With modern physiotherapy and traditional Korean medicine together, effects of treatment are enhanced. And traditional Korean medical physiotherapy is appropriated to world physiotherapy's expectation.

Study for Enhanced Skin Penetration of Cosmetics by Plasma-Sono Stimuli (플라즈마-초음파 자극에 의한 화장품의 피부 침투 증진 연구)

  • The demand for skincare has increased due to the end of the COVID-19 pandemic, leading to a focus on skincare devices and technologies designed to improve the delivery of cosmetics. Among these technologies, skincare medical devices that utilize plasma therapy (Plasma) and sonophoresis (Sono) are commonly used in dermatology clinics. However, there is still a lack of quantitative analysis for transdermal absorption effects of Plasma and Sono skincare medical devices. In this study, we quantified enhanced transdermal absorption effects of Plasma and Sono devices through in-silico and ex-vivo studies. The Sono treatment demonstrated an increased transdermal absorption effect, showing a 10~13% difference in penetration compared to the control group in the in-silico experiment, and 159% and 184% increase in the ex-vivo experiment. The Plasma treatment revealed increased transdermal absorption effects, with a 1.0~2.5% penetration difference in the in-silico experiment, and a 124% increase in the ex-vivo experiment compared to the control group. We also observed a synergistic effect from the combined treatment of Plasma and Sono, as indicated by the highest increases of 197% and 242% in penetration. Furthermore, we have determined the optimal device settings and treatment conditions for Plasma-Sono skincare medical devices. Notably, higher on/off durations (Intensity levels) and longer Sono treatments resulted in greater transdermal absorption effects.
