A Study on the Utilization of health Subcenter for Primary Medical Care in a Korean Rural Community -Su-Dong Myun, NamYang-Ju City, Kyung-Gi Do (일개 농촌 면단위 지역 주민의 보건소 의료이용실태에 관한 조사)

  • This study was analyzed through the reports which published on the subject matter of Su Dong-Myun from 1994 to 1997 and the medical record of patient in Su-Dong Myun health subcenter. The result are as follow: 1. The number of population in Su-Dong Myun(study area) was 5,475 in 1994, 5,707 in 1995, 6,079 in 1996 and 6,253 in 1997. In composition rate of population, "65 and over" of age group only showed markedly increasing tendancy from 9.8% in l995 to 10.2% in 1997. However, the rest of all age group showed decreasing tendancy. 2. Annual utilization rate showed decreasing tendancy, such as 247 in 1994. 203 in 1995, 146 in 1996, and 140 in 1997 per 1000 population. But visiting time is increasing tendency, such as 3.1 in 1994, 2.8 in 1995. 2.4 in 1996 and 3.4 in 1997 per disease case. 3. Age specific annual utilization rate, all age showed decreasing tendancy in the age group of "0-14", "15-44" and "45-64", however showed increasing tendency in the age group of "65 and over" from 1994 to 1997. 4. The major disease were disease of Respiratory system. Gastrointestinal system. Musculoskeletal system and Connective tissue. Skin and Subcutaneous tissue and Circulatory system The disease of Musculoskeletal system and Connective tissue and Circulatory system are increasing.

Analysis of Core Interventions of Operating Room using Nursing Intervention Classification (NIC간호중재분류체계를 이용한 수술실 핵심간호중재분석)

  • Purpose : The purpose of study was to identify to analysis of core nursing interventions performed by Operating Room nurses. Method : The subjects of the study were arbitrarily selected nurses(n=104) working in Operating Room. The period for data collection was 15 days from July, 15, 2002 to July, 30. 2002. The instrument for study was 486 Nursing Interventions Classification developed by McClosky & Bulechek(2000) and was translated into Korean. In 486 nursing interventions, 57 nursing interventions were selected by more than half of 47 professional nurses group of Operating Room. 57 nursing interventions were used as a secondary questionnaire. In the secondary questionnaire, labels and definitions of all 57 interventions were listed. The collected data were self reported by Operating Room nurses. The data were analysed with SPSS program. Result : In 57 nursing interventions, the 'Behavior' domain was the most frequently used. Core interventions of Operating Room were performed several times a day by more than 50% of Operating Room nurses. Core interventions of Operating Room were 16 Core interventions, 7 classes, 5 domains. In the core interventions, the 'Physiological:Complex' domain was the most frequently used. Core interventions of Operating Room were Surgical Preperation, Infection Control:Intraoperative, Surgical Precautions, Fall Prevention, Documentation, Surgical Assistance, Environmental Management:Safety, Skin Surveillance, Physical Restraint, Pressure Ulcer Prevention, Environmental Management:Comfort, Infection Protection, Presence, Emotional Support, Specimen Management, Shift Report. Conclusion : Core interventions of Operating Room have implications for nursing care practice, nursing education, nursing research, and nursing information system in Operating Room.

Classification Model of Facial Acne Using Deep Learning (딥 러닝을 이용한 안면 여드름 분류 모델)

  • The limitations of applying a variety of artificial intelligence to the medical community are, first, subjective views, extensive interpreters and physical fatigue in interpreting the image of an interpreter's illness. And there are questions about how long it takes to collect annotated data sets for each illness and whether to get sufficient training data without compromising the performance of the developed deep learning algorithm. In this paper, when collecting basic images based on acne data sets, the selection criteria and collection procedures are described, and a model is proposed to classify data into small loss rates (5.46%) and high accuracy (96.26%) in the sequential structure. The performance of the proposed model is compared and verified through a comparative experiment with the model provided by Keras. Similar phenomena are expected to be applied to the field of medical and skin care by applying them to the acne classification model proposed in this paper in the future.

Effects of 630-nm Organic Light-emitting Diodes on Antioxidant Regulation and Aging-related Gene Expression Compared to Light-emitting Diodes of the Same Wavelength

  • To investigate the aging-related physiological functions of organic light-emitting diodes (OLEDs), we examined mRNA expression changes in aging-related genes due to oxidative stress inhibition by 630-nm red light OLEDs. As a result of irradiating 630-nm OLED with an intensity of 5 mW/cm2 for 15 min, the viability of dermal fibroblasts significantly increased by 1.3-fold. In addition, reactive oxygen species generated by H2O2 were significantly reduced about 4.9-fold by irradiation with 630-nm OLED. Quantitative reverse-transcription polymerase chain reaction results showed that 630-nm OLEDs altered aging-related gene mRNA expression levels through antioxidant activity. The mRNA expression levels of matrix metalloproteinase1 (MMP1) and MMP9 decreased significantly, by about 2.2- and 2.5-fold, compared to the control group, whereas those of collagen, type I, and alpha 1 increased significantly, by 4.9-fold. The mRNA expression levels of cancer suppression genes p16 and p53 in dermal fibroblasts were also significantly reduced by 630-nm OLED irradiation, by about 1.4- and three-fold, respectively, compared to the control. Overall, it was confirmed that 630-nm OLED irradiation lowered the level of ROS formation induced by H2O2 in dermal fibroblasts, and that this antioxidant effect could regulate the mRNA expression levels of aging- and tumor suppression-related genes. This study shows a link between 630-nm OLED irradiation and anti-aging physiological functions such as antioxidant function, and suggests the potential of OLEDs as a useful light source for skin care.

Mucormycosis Management in COVID-19 Era: Is Immediate Surgical Debridement and Reconstruction the Answer?

  • Background Excessive use of corticosteroids therapy along with gross immunocompromised conditions in the novel coronavirus disease 2019 (COVID-19) pandemic has raised the risks of contracting opportunistic fungal infections. Here, we describe our experience with the implementation of a surgical protocol to treat and reconstruct rhino-orbital-cerebral mucormycosis. Methods A retrospective review of our prospectively maintained database was conducted on consecutive patients diagnosed with mucormycosis undergoing immediate reconstruction utilizing our "Mucormycosis Management Protocol." All patients included in this study underwent reconstruction after recovering from COVID-19. Wide local excision was performed in all cases removing all suspected and edematous tissue. Reconstruction was done primarily after clear margins were achieved on clinical assessment under a cover of injectable liposomal amphotericin B. Results Fourteen patients were included. The average age was 43.6 years and follow-up was 24.3 days. Thirteen patients had been admitted for inpatient care of COVID-19. Steroid therapy was implemented for 2 weeks in 11 patients and for 3 weeks in 3 patients. Eight patients (57.1%) had a maxillectomy and mucosal lining resection with/without skin excision, and six patients (42.8%) underwent maxillectomy and wide tissue excision (maxillectomy and partial zygomatic resection, orbital exenteration, orbital floor resection, nose debridement, or skull base debridement). Anterolateral thigh (ALT) flaps were used to cover defects in all patients. All flaps survived. No major or minor complications occurred. No recurrence of mucormycosis was noted. Conclusion The approach presented in this study indicates that immediate reconstruction is safe and reliable in cases when appropriate tissue resection is accomplished. Further studies are required to verify the external validity of these findings.

  • This study was conducted for the purpose of developing a curriculum to educate the practical skills required in the industry by convergence with the 4th industry in the beauty field, which has been accelerated since the Corona era. As a result of an exploratory investigation of several literatures and collecting expert opinions, it was analyzed that the field of beauty industry will expand to a personalized service providing industry that combines medical, bio, ICT, and artificial intelligence technologies, rather than providing a single item or service. Based on the analysis contents, the curriculum was composed and subjects were derived by adding digital application skills to have in addition to the basic job skills required in the traditional beauty industry. The post-Corona era will bring changes in various industries based on the Fourth industrial revolution, and in response to these changes, universities should always pay attention to changes in the industry to develop talent for the development and sustainability of the beauty industry.

Changes in the visual display terminal syndrome of college students in online classes during the COVID-19 pandemic (COVID-19 팬데믹 상황에서 온라인 수업 중인 대학생의 영상표시단말기 증후군의 변화)

  • Purpose: The purpose of this study was to identify changes in the experience rate and level of symptoms of visual display terminal syndrome in college students attending online classes during the COVID-19 pandemic. Methods: Data were collected from February 22 to June 8, 2021 at three measurement points. A total of 117 college students were administered a visual display terminal syndrome survey just before online classes (T1), one month after the start of online classes (T2), and three months after the start of online classes (T3). The collected data were analyzed by frequency and percentage, paired t-test, McNemar test, and repeated measures analysis of variance using the IBM SPSS 25.0 program. Results: The intensity of college students' visual display terminal syndrome during online classes increased at T2 and T3 compared to T1. The rate of experiencing back discomfort or pain increased abruptly at T2 compared to T1. The intensity of college students' eye related symptoms and skin related symptoms increased at T2 and T3 compared to T1, while the intensity of college students' psychological symptoms, general body discomfort, and musculoskeletal symptoms increased at T3 compared to T1. Conclusion: The results of this study suggest that self-care programs are needed to prevent visual display terminal syndrome in college students who are in long-term online classes.

Utilizing the Application of High-Intensity Yttrium Aluminum Garnet (YAG) Lasers Focused on Acupoint Irradiation (경혈 조사를 중심으로 본 고출력 Yttrium Aluminum Garnet (YAG) 레이저의 활용)

  • Objectives : The purpose of this study is to investigate on the application of the yttrium aluminum garnet (YAG) lasers for acupoints irradiation. Methods : We conducted a systematic search for peer-reviewed studies published from inception to November 2023, in the following electronic databases: PubMed, Scopus, and Web of Science in English, Science ON, Oriental Medicine Advanced Searching Integrated System (OASIS) and Research Information Sharing Service (RISS) in Korean, and China National Knowledge Infrastructure (CNKI) and Wanfang in Chinese, and Japan Science Technology Information Aggregator, Electronic (J-STAGE) and Citation Information by NII (CiNii) in Japanese. Inclusion criteria were original articles including clinical and experimental studies related with YAG lasers for acupoints including Ashi or meridian sinews. Results : Among the 8 selected studies, there were 7 studies on human subjects and 1 study on animals, 7 studies on Nd:YAG (1,064 nm) laser, and 1 study on Er:YAG (2,940 nm) laser. A total of 16 acupoints were used, 15 of which were in the face and 1 of which was located in the foot. In addition, there were two studies using Ashi. 4 studies looked at the effect of pain relief, 2 studies looked at safety, 1 study looked at changes in blood flow, and 1 study looked at the effect of skin care. There were no reported adverse events, and the YAG laser was confirmed to be safe and effective in pain relief, beautifying the skin, and increasing blood flow. Conclusions : We suggest that high intensity YAG lasers can be applied to laser acupuncture or laser moxibustion. YAG lasers are considered to be worth using for various clinical indications of Korean medicine because of photobiomodulation effects, analgesic action, and deep penetration depth. Further scientific research and clinical evidences should be warranted.

A Study on the Contamination of Saline Used in the Operation (수술에 사용하는 생리식염수의 오염수준 변화)

  • Post-operative wound infections have been the serious problems in nursing care in the operating room and appear to be strongly related to the infection occurring during the operation. The purpose of this study is to identify the level of contamination in saline used in the operation and also examine the correlation between the contaminated saline and the length of the operation, and unclean atmospheric factor. Subjects for this study include 13 cases of operation performed at the operative theatre of a hospital in Seoul area. Test samples and related data were collected from this medical facility between Oct. 6 through Dec. 10, 1994 by the author and anurse who worked in the operating room. For the study, multiple batches of saline sample were collected at the various time intervals duringthe operation and filtered through the membrane filters. Viable microorganisms retained on the filters were cultured on the appropriate culture media and the levels of existing cells in saline were enumerated according to Koch's method. In the analyses of the data, Pearson's correlation coefficient was obtained for the examination of relationship between the length of operation and numbers of microorganisms existing in saline and for the comparison of the differences in numbers if microrganisms in saline sample collected at the various operative stages, e. g. pre-incision, excision and skin suturing stages, ANOVA and Scheff Tests were performed. The results of this study are summarized as follows. 1) The lenth of the operation and numbers of microorganisms in the saline used in the operation appeared to be significantly correlated (r=0.5467, P<0,001). 2) In case of saline exposed to air, but not used in the operation, the length of exposure to the air and the numbers of microorganisms present in saline also showed an apparent correlation(r=0.5087, P<0. 001). 3) The frequencies of occurrence of microorganisms in saline used in the operation and in saline exposed only to the air in the given time showed significant differences(t=3.73, p=.0000). 4) In case of saline used in the operation, there is significant differences in its numbers of contained microorganisms between the operative stages ; pre-incision, excision, and skin suture (F=17.7500, p=.0000). 5) In case of saline exposed only to the air in the given time, there is significant differences in its numbers of contained microorganisms between the operative stages . pre-incision, excision, and skin suture(F=6.3807, p=.00031).

Interferon-${\gamma}$ Release Assay among Tuberculin Skin Test Positive Students in Korean High Schools

  • Background: There are several active tuberculosis (TB) cases in Korean high schools each school year. The risk of transmission in schools is extremely high due to the considerable time spent in closed classrooms. We evaluated the control of latent tuberculosis infection in Korean high schools. Methods: When a student was identified with active TB, tuberculin skin testing was performed on their classmates and on students in their same school grade. When a student had a positive tuberculin skin tests (TST), they underwent follow-up testing with QuantiFERON-TB Gold In-Tube (QFT). The manufacturer recommended a cut-off of 0.35 IU/mL to determine QFT positivity was applied. Results: A total of 131 pulmonary tuberculosis (TB) patients were included based on the criteria for screening TB contacts in the National Tuberculosis Control Program. Seventy-five (57.2%) students tested smear positive. TST were performed on 7,109 students who were classmates of, or in the same grade as, a TB patient. Of the contacts, 1,231 students (17.3%) were TST positive and they were screened with QFT. Six hundred-sixty-six (55.0%) of the tested students returned a positive QFT result and the rate of positivity was significantly associated with the increasing size of TST indurations (p<0.0001). Conclusion: The use of QFT resulted in approximately 45% of TST positive students not being given chemoprophylaxis.
