외과의사 관점에서 외상전문의의 필요성과 과제 (The Necessity for a Trauma Surgeon and the Trauma Surgeon's Role in the Trauma Care System)

  • When man first walked on this planet, injury must have been a close encounter of the first kind. The outbreak of World War I, during a period of rapid scientific growth in the basic sciences, demonstrated the need to develop better methods of care for the wounded, methods that were later applicable to the civilian population. Trauma is a multisystem disease and, as such, benefits from almost any advance in medical science. As we learn more about the physiology and the biochemistry of various organ systems, we can provide better management for trauma victims. Improved imaging techniques, better appreciation of physiologic tolerance, and increased understanding of the side effects of specific surgical procedures have combined to reduce operative intervention as a component of trauma patient care. On the other hand, because of this rapid development of medical science, only a few doctors still have the ability to treat multisystem injuries because almost doctor has his or her specialty, which means a doctor tends to see only patients with diagnoses in the doctor's specialty. Trauma Surgeons are physicians who have completed the typical general surgery residency and who usually continue with a one to two year fellowship leading to additional board certification in Surgical Critical Care. It is important to note that trauma surgeons do not need to do all kinds of operations, such as neurosurgery and orthopedic surgery. Trauma surgeons are not only a surgeon but also general medical practitioners who are very good at critical care and coordination of patient. In order to achieve the best patient outcomes, trauma surgeons should be involved in prehospital Emergency Medical Services, the Trauma Resuscitation Room, the Operating Room, the Surgical Intensive Care and Trauma Unit, the Trauma Ward, the Rehabilitation Department, and the Trauma Outpatient Clinic. In conclusion, according to worldwide experience and research, the trauma surgeon is the key factor in the trauma care system, so the trauma surgeon should receive strong support to accomplish his or her role successfully.

Household Overcrowding in Iran, a Low-middle-income Country: How Major of a Public Health Concern Is It?

  • Objectives: Household overcrowding (HC) can contribute to both physical and mental disorders among the members of overcrowded households. This study aimed to measure the status of HC and its main determinants across the provinces of Iran. Methods: Data from 39 864 households from the 2016 Iranian Household Income and Expenditures Survey were used in this study. The Equivalized Crowding Index (ECI) and HC index were applied to measure the overcrowding of households. Regression models were estimated to show the relationships between different variables and the ECI. Results: The overall, urban, and rural prevalence of HC was 8.2%, 6.3%, and 10.1%, respectively. The highest prevalence of HC was found in Sistan and Baluchestan Province (28.7%), while the lowest was found in Guilan Province (1.8%). The number of men in the household, rural residency, the average age of household members, yearly income, and the household wealth index were identified as the main determinants of the ECI and HC. Conclusions: The study demonstrated that the ECI and HC were higher in regions near the borders of Iran than in other regions. Therefore, health promotion and empowerment strategies are required to avoid the negative consequences of HC, and screening programs are needed to identify at-risk families.

의무기록정리율의 문제점과 개선책 (COMPLETION RATE OF THE MEDICAL RECORDING: PROBLEMS AND MEASURES FOR IMPROVEMENT)

  • As the first step to improve the medical recording for the hospitalized patients, we tried to identify problems of completion rate of the medical recording and made the completion rate of the medical recording within due date known to the public in the hospital and commended the best Department and resident officially. The results were as follows: 1. The average number of the medical recordings per a recorder by years of residency(average number to quorum) was 293.3(398.8) in 1st year, 82.5(68.1) in 2nd year, 21.2(8.3) in 3rd year, and 20.5(1.9) in 4th year-residents. There was only 2 residents who prepared the medical recordings more than the average of 125.6 recordings per a resident. 2. Among 13 Departments, the medical recording was wholly put in charge of the 1st year-resident in 6 Departments. The duty was shared with the 2nd year-resident in 5 Departments and the 3rd year-resident in 1 and the 4th year-resident in Only 1 Department. 3. The more the cases requiring the medical recording, the lower the completion rate(80% less than 100 recordings, 70% in 100-299, 60% in 300-399 and 33.3% in more than 400). 4. There was no difference in the completion rate of the medical recording before(1991) and after(1993) a public nitice(1992). However, 4 Departments showed improvement of 21-45% in the completion rate, and no case was found where billing for medical assurance was postponed due to delayed completion of the medical recording. 5. The completion rate was relatively low(72-78%) from January to March. The main reasons were shortage of men power due to preparation of board examination, attendance to military duty, and lack of training in the medical recording for the new 1st year-residents. 6. The official commendation of the best Department and resident by letters did not improve the completion rate of the medical recording. In conclusion, The main reason of the low completion rate of the medical recording was the fact that the 1st year-residents were almost exclusively responsible for the medical recording. Hence, it is mandotory that this practice gets staffs' attention to improve the completion rate. Public notice of the completion rate of the medical recording shows prompt improvement of the completion rate. Prize money rather than commendation by a letter for the best Department would also be more effective to improve the quality of medical recording.

DEA를 이용한 종합병원의 효율성 평가 (Efficiency Evaluation of General Hospitals using DEA)

  • 병원은 이윤추구를 목적으로 하는 기업과 달리 공익성을 위하여 서비스를 제공하는 측면이 강하다. 하지만, 변화하는 의료환경 속에서 지속적인 성장을 위해서는 경영혁신이 필요하다. 병원의 경영혁신에서 핵심적인 내용은 효율성을 제고하는 것이다. 이를 위해 우선 병원 내부적으로 비효율성이 어떻게 나타나고 있으며 이에 영향을 미치는 요인이 무엇인지 파악하는 것이 필요하다. 그러나 병원의 효율성 평가는 민간부문에서 활용되고 있는 평가방식을 적용하기가 어렵다. 따라서 이 연구에서는 DEA 기법을 이용하여 종합병원의 효율성을 평가하였다. 국내 500병상이상 수련병원 74개를 대상으로 2006년 자료를 이용하여 분석하였다. 분석에 사용된 투입변수는 병상수, 의사수, 간호사수, 의료기사수, 인건비, 관리비, 재료비를 산출변수는 연외래환자수, 연입원환자수, 수술건수, 입원 및 외래 수익을 사용하였다. 또한 병원의 설립형태, 소재지, 설립년, 병원유형에 관해서 효율성에 어떤 영향이 미치는가에 대해서도 검토하였다.

Validity of Self-reported Hypertension and Factors Related to Discordance Between Self-reported and Objectively Measured Hypertension: Evidence From a Cohort Study in Iran

  • Objectives: Self-reporting can be used to determine the incidence and prevalence of hypertension (HTN). The present study was conducted to determine the validity of self-reported HTN and to identify factors affecting discordance between self-reported and objectively measured HTN in participants in the Ravansar Non-Communicable Diseases (RaNCD) cohort. Methods: The RaNCD cohort included permanent residents of Ravansar, Iran aged 35-65 years. Self-reported data were collected before clinical examinations were conducted by well-trained staff members. The gold standard for HTN was anti-hypertensive medication use and blood pressure measurements. The sensitivity, specificity, positive and negative predictive values, and overall accuracy of self-reporting were calculated. Univariate and multivariate logistic regression were used to examine the discordance between self-reported HTN and the gold standard. Results: Of the 10 065 participants in the RaNCD, 4755 (47.4%) were male. The prevalence of HTN was 16.8% based on self-reporting and 15.7% based on medical history and HTN measurements. Of the participants with HTN, 297 (18.8%) had no knowledge of their disease, and 313 (19.9%) had not properly controlled their HTN despite receiving treatment. The sensitivity, specificity, and kappa for self-reported HTN were 75.5%, 96.4%, and 73.4%, respectively. False positives became more likely with age, body mass index (BMI), low socioeconomic status, and female sex, whereas false negatives became more likely with age, BMI, high socioeconomic status, smoking, and urban residency. Conclusions: The sensitivity and specificity of self-reported HTN were acceptable, suggesting that this method can be used for public health initiatives in the absence of countrywide HTN control and detection programs.

농어촌지역 공동생활시설 이용실태 고찰 (A Study on the Utilization Status of Community Living Facilities in Rural Area)

  • The housing environment of the weak class in rural area is inferior, the elderly ratio is high, and senior citizens whose living by themselves are increasing. This study was to analyze the actual conditions of utilization status, characterization of residency, management method of the federation of construction associate's love house and local government's case, and usage of the village hall. For better study, we reviewed the literatures and conducted field surveys. The purpose of this study is to suggest a development plan of rural community living homes. The results of this study were as follows; First, it is important for proper site of rural community living home plans to select adjacent location in the surrounding medical and welfare facilities(including community facilities). Second, the living space for one person does not need such a space; about $33m^2$ is suitable. Third, it is important to use surplus area to make kitchen garden in the site planning. Also the architectural plans must be considered to minimize the maintenance costs for the sustainability of the house and winter heating expense problem.

예방의학의 발전을 위한 학술활동 (Future of Scientific Research on Preventive Medicine in Korea)

  • The Korean Society of Preventive Medicine has undergone continuous development, after overcoming the difficult early years, in the 59 years since its establishment in 1947. It has repeatedly upgraded its quality and quantity of research with the first journal edition in 1968 and the continuous increase in publication numbers, scientific articles, joint research projects, intra-field exchanges and participation in various international scientific activities. In the future, we should gather a more extensive collection of opinions regarding the introduction of clinical preventive medicine specialists and prepare for the establishment of a training program for clinical specialists into a preventive medicine residency course. Moreover, we should raise interest in the importance of protecting individual information and maintaining medicine ethics. It's impossible to develop academic activities without cooperation. We need such cooperation with basic medical approaches across a wide range of fields. Furthermore, we should strengthen our cooperation with aspects of clinical and drug epidemiology in many fields including public health, statistics, and dietetics. Finally, we should raise the level of international cooperation with many countries, including North Korea, to prevent diseases and promote health. Preventive medicine is a science in which practice is as important as theory. We must aim to nurture preventive medicine specialists who practice in many areas of society with the goal of preventing diseases, promoting health, improving fertility, and securing healthy elderly life for individuals and the entire population. To this end, we will endeavor to promote both theoretical and practical components of academic development.

Health Care Optimization by Maximizing the Air-Ambulance Operation Time

  • Employing the available technologies and utilizing the advanced means to improve the level of health care provided to citizens in their various locations. Citizens have the right to get a proper health care services despite the location of their residency or the distance from the health care delivery centers, a goal that can be achieved by utilizing air ambulance systems. In such systems, aircrafts and their life spans are the essential component, the flight duration of the aircraft during its life span is determined by the maintenance schedule. This research, enhances the air ambulance systems by presenting a proposal that maximizes the aircraft flight duration during its life span. The enhancement will be reached by developing a set of algorithms that handles the aircraft maintenance problem. The objective of these algorithms is to minimize the maximum completion time of all maintenance tasks, thus increasing the aircraft operation time. Practical experiments performed to these algorithms showed the ability of these algorithms to achieve the desired goal. The developed algorithms will manage the maintenance scheduling problem to maximize the uptime of the air ambulance which can be achieved by maximizing the minimum life of spare parts. The developed algorithms showed good performance measures during experimental tests. The 3LSL algorithm showed a higher performance compared to other algorithms during all performed experiments.

IL-15 in T-Cell Responses and Immunopathogenesis

  • IL-15 belongs to the common gamma chain cytokine family and has pleiotropic immunological functions. IL-15 is a homeostatic cytokine essential for the development and maintenance of NK cells and memory CD8+ T cells. In addition, IL-15 plays a critical role in the activation, effector functions, tissue residency, and senescence of CD8+ T cells. IL-15 also activates virtual memory T cells, mucosal-associated invariant T cells and γδ T cells. Recently, IL-15 has been highlighted as a major trigger of TCR-independent activation of T cells. This mechanism is involved in T cell-mediated immunopathogenesis in diverse diseases, including viral infections and chronic inflammatory diseases. Deeper understanding of IL-15-mediated T-cell responses and their underlying mechanisms could optimize therapeutic strategies to ameliorate host injury by T cell-mediated immunopathogenesis. This review highlights recent advancements in comprehending the role of IL-15 in relation to T cell responses and immunopathogenesis under various host conditions.

Ensuring Patient Safety in Pediatric Dental Care

  • This review aims to examine safety concerns in pediatric dental care and underscore the need for comprehensive patient safety initiatives within the Korean Academy of Pediatric Dentistry. Drawing insights from the prevailing patient safety policies of the American Academy of Pediatric Dentistry, case reports, and systematic reviews, this review elucidates issues such as dental fires during sedation, ocular complications from local anesthesia, and surgical emphysema. This review highlights the significance of safety toolkits encompassing infection control, medical error reduction, dental unit waterline infection, and nitrous oxide safety in pediatric dental settings, underscoring the need to foster a safety culture. Furthermore, this study explores the curriculum for pediatric dentistry residency programs, emphasizing concepts such as high-reliability organizations and mortality and morbidity conferences. The study suggests the need for initiatives to enhance patient safety, including establishing safety committees, expanding reporting systems, policy development, and supporting research related to patient safety. In conclusion, this study underlines key messages, emphasizing the utmost priority of patient safety, acknowledging the inevitability of human error, promoting effective communication, and cultivating a patient safety culture. These principles are vital for advancing patient safety in pediatric dental care and improving outcomes among pediatric patients.
