Purpose : A survey was conducted to investigate the intake pattern of health food by using questionnaire. Methods : This study was carried out to patients visiting OB&GY clinic in Jaseng and Dongseo hospital of oriental medicine during 2005.3.4-2005.5.13. Results : 1. Among the 114 respondents, most respondents(66.6%) were located between 3rd and 4th decade. 2. Among the respondents, 107(93.9%) persons had the experience of intake of health foods and 7(6.1%) persons not. 3. Among the health foods, the natural foods(48.0%) were most frequently taken. 4. Two major reasons for the intake of the health food were 'for relieving easy fatigability'(24.8%) and 'for the prevention of disease'(23.0%). 5. Most respondents felt that the effects of health foods were 'intermideate'(65.8%). Positive responses to the effect were 24.6%, and negative ones were 3.5%. 6. The discussion with medical personnel about taking health foods was present in fractionary population (13.2% ). 7. Among 107 respondents who had taken health food, 7 respondents(6.1%) experienced side effect of health food. Side effects were gastrointestinal disturbance in 4 cases, dermatologic problems in 2 cases. Conclusion : A study on analysis of health food in Oriental Medicine is necessary to make a guide for efficient use of that.
Purpose: This study aims to determine the infection rate of Helicobacter pylori (H. pylori) in gastric cancer patients who received gastrectomies, and to compare the rates of H.pylori infection detected by serological test and that of histopathological test, and to evaluate its clinical meaning. Materials and Methods: Fifty two patients were selected from those who underwent gastrectomies at the Department of Surgery, Samsung Medical Center, from March 1997 to May 1997. The control group consisted of healthy 103 persons visited the center for health promotion in Samsung Medical Center. In both groups, we quantitatively checked serum level of IgG anti H. pylori antibody titer by ELISA using GAP IgG test kit (BioRad, USA) for the serological test, and we microscopically examined the surgical specimen stained by Warthin-Starry silver staining method for the histopathological test. Results: The seropositive rate of H. pylori in the patients' group was $71.2\%$ (37/52), and the control group was $65.0\%$ (67/103). The difference between two groups was statistically significant. However the histopathological study showed that the overall detection rate of H. pylori was $61.5\%$ (32/52) in the patients' group and $61.2\%$ (63/103) in the control group; nd this difference was not statistically significant Conclusion: We could confirm that H.pylori infection rate in the gastric cancer resected patients was statistically higher than in the normal healthy persons even in small population. And the detection method for the H. pylori infection by serological test was presumed to be better than that of histopathological test using surgical specimen. Further study for the larger population by well-organized multicenters will be needed.
Background: To evaluate the efficacy of autologous platelet-rich plasma (PRP) injections in the treatment of common shoulder diseases. Methods: The PubMed, Medline, and Central databases and trial registries were searched from their inception to October 2020 for randomized controlled trials of autologous PRP injections for shoulder diseases versus placebo or any control intervention. Preferred reporting items for systematic reviews and meta-analyses (PRISMA) guidelines were followed in the selection, analysis, and reporting of findings. The primary outcome was pain intensity (visual analog scale), and secondary outcomes were changes in function and quality of life (QoL). Results: A total of 17 randomized controlled trials of PRP versus control were analyzed. From 8-12 weeks to ≥1 year, PRP injections were associated with better pain relief and functional outcomes than control interventions. PRP injections were also associated with greater QoL, with an effect size of 2.61 (95% confidence interval, 2.01-14.17) at medium-term follow-up. Compared with placebo and corticosteroid injections, PRP injections provided better pain relief and functional improvement. In subgroup analyses, trials in which PRP was prepared by the double centrifugation technique, the platelet concentration in the PRP was enriched ≥5 times, leucocyte-rich PRP was used, or an activating agent was used before application reported the most effective pain relief at 6-7 months. Conclusions: PRP injections could provide better pain relief and functional outcomes than other treatments for persons presenting with common shoulder diseases. PRP injections have a greater capacity to improve shoulder-related QoL than other interventions.
Purpose: The purpose of this study was to provide basic data for developing strategic programs based on 119 emergency medical technicians leadership perception types. Methods: The subjects were thirty six 119 emergency medical technicians working for out-of hospital in Jeollanam-Do, Korea. Data were collected from July 20, 2017 to October 30, 2017. Q sample of 40 statements and P sample of 36 persons using PC QUANL software. Results: Three types of leadership styles were identified from the explanation (71.5% of the variables). The relationship oriented progressive type (the most common), the performance and value oriented type (the second most common), and the experience oriented filed participation type (the third most common). Conclusion: The fire department will need to develop and implemented leadership improvement programs based on the leadership types in order to meet the organization's goals.
Purpose: The aim of this study is to present the basic data for qualitative improvement of emergency care for emergency patient by paramedic in fire station by understanding the level of awareness and practice in prehospital and transfer step, and understanding the level of emergency care and improvement of clinical knowledge through hospital clinical training. Methods: The researchers explained the objective for 143 persons who completed hospital clinical training from June 2nd, 2006 to October 23rd, 2009 among paramedic in fire station. The questionnaire in this research consisted of 80 questions. In the reliability for the awareness of emergency patient assessment, cronbach's $\alpha$ was 0.95, and in the reliability for emergency care fulfillment, cronbach's $\alpha$ was 0.93. reliability for clinical knowledge improvement is cronbach's $\alpha=.95$, and reliability for emergency care fulfillment is cronbach's $\alpha=.82$. Collected data was analyzed through SPSS 18.0 statistics program for frequency, percentage, average, standard deviation, Paired t-test, t-test, Correlation Coefficient, and internal consistency reliability was analyzed by cronbach's $\alpha$. Results: 1) The paramedic awareness and practice difference for emergency patient is statistically signification for general patient assessment(t=14.159, p=.000), trauma patient assessment(t=11.288, p=.000), internal medicine patient assessment(t=10.898, p=.000), and it shows the level of practice is lower than the level of awareness. 2) The paramedic difference between the level of awareness and practice according to whether or not they have clinical career is not signification on awareness(t=3.119, p=.125), and is high on practice(t=3.119, p=.002). 3) The correlation between paramedic awareness and the level of practice shows positive correlation(r=.61, p=.000). The higher the awareness of emergency patient assessment is, the higher the level of practice is. 4) The difference between paramedic clinical knowledge improvement and the level of emergency care practice is statistically significant(t=3.351, p=.001). 5) 89.6%(128 persons) of paramedic replied hospital clinical training experiences are helpful for field activity. 92.3%(133 persons) replied they apply well for clinical knowledge learned during hospital clinical training and emergency care skills in the field. Conclusion: Paramedic in fire station must evaluate the patient's initial assessment and activate the transfer system to the emergency department. It is necessary to develop and implement the effective education program continuously. The education program should systemize currently operated hospital clinical training. emergency disease and symptoms emergency care method, and practice mainly skill education should be progressed. In the prehospital and transfer management, high quality of medical assessment is required to the emergency medical service system. Medical direction from the doctors can feedback the paramedic continuously and continuing education must be provided to the paramedic in fire station.
Currently, we have seen sudden increase of demand for emergency medical services by reason for high-speed economic development increase of traffics, etc. in this society. Consequently the government enforced & operated emergency medical system in 1991 as a link of more positive countermeasure against it, but many problem; still remain in reality. In particular, the regional emergency medical center designated by the Ministry of Health and Welfare falls short of our expectations for its essential role by the reason of such as insufficiency in professional manpower and institutions concerned in small-scale hospital, matters with transportation system, preference of large hospital, etc. Therefore, this study was conducted grasp for the actual conditions of emergency medical system based upon literature research & the preceding studies and interview research the motive of coming hospital, satisfaction & understand to the subject of 150 persons of patients and their guardians who used regional emergency center of Pusan National University Hospital, thereby examining & analysing the cause of emergency room overcrowding by non-emergency cases, one of the problem; enumerated from preceding studies. The main result of this study is as follows. First the actual condition of non-emergency patients coming hospital for examining overcrowding of emergency medical center showed that, of the patients who used the emergency medical center, non-emergency patients accounted for 49.3%, which acted as the main cause of delaying medical care for emergency cases, cases of which medical person or first-aid man decided to come hospital accounted for 36.1 %, thereby suggesting essential need for re-education & wide public information to even the professional manpower besides patients & guardians for their using emergency medical system. Second, as the result of researching patient acknowledgement with reference to their using emergency medical center, the rate of their giving right answer is no more than 60%, which means that non-emergency cases' using as such is due to the shortage of their knowledge of the said emergency medical center, which suggests us that wider P.R for emergency medical system to common people who may be one of the patients of it at any time is still in need. The purpose of this study was to investigate the association of obesity with medical care use and costs according to overall diseases, cerebrovascular diseases (CVD), ischemic heart disease (IHD), hypertension (HTN) and diabetes mellitus (DM). The final sample was a group of persons who were free of diseases mentioned above and were not underweight. Their baseline screening program data and health insurance contribution data were connected with a 7-year medical claim database. The participants were classified according to their baseline BMI into normal, overweight, obese, and severely obese groups. Given the disease type, the total costs of DM showed the largest difference in each obesity group in both males and females. Also, the pharmacy costs for DM were more relevant than any other type of service to the obesity level. Considering the high prevalence of obesity and the relevantly increased medical care use and costs, there is a need for reduction in medical costs through obesity prevention efforts.
Korea is to open its medical markets to foreign hospitals starting in the year 2006 regardless of our will(DDA, Doha Development Agenda). To accurately understand the characteristics of Korean medical users, their detailed and various needs, their attitudes toward the opening of Korean medical markets, and factors affecting these users in choosing foreign medical service providers would be first step needs to be taken by the Korean medical facilities that need to survive and develope through the fiercely competitive era coming with the opening of Korean medical markets to foreign medical service providers and would be very important in hospital management. The subjects of this study were 500 patients randomly selected from the outpatients who visited one of university hospitals in Seoul on the 14th-16th days of April 2003, and conducted a self-completion questionnaire. The answers of 463 respondents among the selected patients(93% of a responding rate)were analyzed through the Excel and statistics programs. The attitudes on the opening of the medical markets were shown in agreement 56.5%(247 persons), disagreement 6.9%(30 persons), and no idea 36.6%(160 persons). In consideration of only the answers as agreement and disagreement exclusive of the answer as no idea, 89.2% of the respondents agreed to the opening of the medical markets while 10.8% objected to the opening. The approval rate was higher with the higher education and income levels. Moreover, The approval rate for the opening of the medical markets was relatively high regardless of the satisfaction in the medical service, and the most important reason of the agreement was the guarantee of the patients(national)option. The main reason of the disagreement was high medical fee(50.5%), and the other reasons showing low rates were outflow of the domestic fund to the foreign countries(13.6%), damage of medical influences on the public(11.4%), lack of competition of the domestic medical industry(9.1%)and so on. As for the factors of selecting the foreign hospitals in the opening of the medical markets, the patients considered the authority(competency)of doctors firstly, and the other principal factors were worldwide fame and reliance, specific explanation of doctors, modernized medical instruments, convenient consultation procedure, etc. The patients agreed to the opening of the medical markets at a high rate regardless of the satisfaction in the medical service, and the most principal reason of the agreement was the guarantee of the patients(national)option for the medical care. Connected with the factors to select the hospitals, the approval reasons for the opening of the medical markets were the authority(competency)of the doctors as the first one, and then fame and tradition, reliance, overall diagnosis and modernized medical instruments, doctors specific explanation, and so on. This study was conducted as a survey to find out middle-aged persons's housing preferences for a future elderly housing and to grasp background characteristics of persons influencing the preferences and the effect of elderly's aging situations on the changes of their preferences. A sample of 300 middle-aged people was selected purposely from parents of university students living in Gwang-ju city. Questionnaires were administered to parents by students and analysed with SPSS Windows 10 program. Middle-aged persons's housing preferences were differed according to elderly's aging situations, in the case of healthy situation, they tended to prefer to stay present house without their children in irrespective to couple or single, whereas in the case of unhealthy situation, they tended to prefer to live with their children. and couples showed preferences for planned housing for the elderly and singles showed preferences for elderly housing facilities with care services. The most important characteristics of elderly housing among middle-aged people was a places where they can enjoy leisure and provide home-help services, located in the suburbs. Also middle-aged persons were likely to prefer to a green spaces such as garden, path for walking, and madang for the elderly housing. Needs for community care services (NCCS) were required above the average. Among the items of NCCS, a visiting medical examination was the most demanding, and a visiting nursing services, a emergency calling bell, and home repairs were sequently followed in the level of needs. The characteristics of the elderly housing which preferred to and needs for community care services were influenced by middle-aged persons's age, gender, subjective evaluation of economic abilities, tenure status, the cost of living, and the size of housing. This findings suggests that it must to be considered to develop various types of the elderly housing depending on socioeconomic status.

The proposals for the advanced death investigation system were as follows. The unnatural deaths including the obvious accidental deaths and homicides have to be defined by the rules. The human resources to perform the autopsy can be supported by the forensic pathologists from medical school. The special training and quality assurance programs are needed for the crime scene investigator. To regain the impaired reputation from the suicide and to support the bereaved family has to be discussed by the government.
