뇌졸중 환자의 말, 언어장애 선별에 대한 검사자간 신뢰도 및 훈련효과 (Inter-rater Reliability and Training Effect of the Differential Diagnosis of Speech and Language Disorder for Stroke Patients)

  • 뇌졸중 환자의 실어증 유무를 정확히 판별하고 환자의 미세한 언어적 변화를 적절히 관찰하기 위해서는 일차적으로 신뢰로운 검사도구를 사용하여야 하고, 또한 검사자가 해당 도구의 사용에 있어 충분히 숙지하고 있어야 한다. 본 연구에서는 뇌졸중 환자의 실어증 및 말장애 유무의 진단에서 검사자의 전공영역에 따른 관찰자간 신뢰도를 살펴보고 훈련 전과 후의 차이를 비교해보고자 하였다. 이를 위해, 침상에 있는 뇌졸중 환자 46명을 대상으로 언어치료사, 신경과 전공의, 그리고 간호사 각각 4명씩 총 12명이 동시에 실어증 및 말장애 유무를 평가하였다. 연구 결과, 서로 다른 전공 영역의 전문가들 간에 말명료도 과제와/아/모음연장발성 과제의 '음질' 영역에서만 'acceptable'로 나타났고, 나머지 하부검사 영역에서는 'good-excellent'로 나타났다. 관찰자간 신뢰도가 'acceptable'로 나타난 과제들에 대해 3주간의 비디오 훈련 전후의 점수 차이를 비교하였다. 그 결과, 훈련 후 말명료도 과제에서 검사자들 간의 평정 점수의 차이는 유의하게 줄어들었으며, '음질' 평정의 정확성도 유의하게 증가하였다. 임상 경험 정도와 각 하부검사에 대한 평정 정확성 간의 상관관계를 알아본 결과, 언어치료사들은 임상 경험 정도가 증가할수록 그림설명하기 과제와 말명료도 과제에서, 그리고 의사 및 간호사들은 그림설명하기 과제에서 판정 정확도가 높아지는 것으로 나타났다. 결론적으로, 이 연구 결과는 뇌졸중 환자의 의사소통장애 진단에 있어 신경언어장애 환자 중, 특히 말장애환자에 대한 꾸준한 경험과 훈련이 반드시 필요하며, 훈련을 통해 평정 신뢰도를 확보할 수 있음을 시사한다.

A follow-up study of 51 cases treated by endodontic microsurgery

  • The aim of this study was to investigate the success rate of endodontic microsurgery. Three calibrated examiners evaluated a total of 51 teeth that were treated with endodontic microsurgery in Samsung Medical Center, Seoul, Korea, during the period of 1996-2000. The periapical radiographs were evaluated and compared with the preoperative and follow-up radiographs. In addition, a standardized questionable was used to assess the clinical status of each tooth. The data was integrated and each case was categorized as being a clinical success, questionable success or a failure.(omitted)

  • In Vol. 31, No. 2, our journal prepared a review article, two original papers, and three case reports. First, autonomous systems are increasingly being introduced in aircraft systems. Therefore, it is crucial that flight crews are trained to adapt and handle these autonomous systems better. Consequently, we discussed crew resource management for handling autonomous systems. The fatigue of aviation workers is one of the biggest threats to aviation safety. Therefore, we analyzed the degree of fatigue in commercial aircraft pilots and evaluated the relationship between fatigue and their workload and the effect of fatigue on sleep. Since obesity has a significant impact on the health of aviation workers, we also comparatively analyzed the prevalence of obesity in Korea and Japan based on the data from the National Health and Nutrition Examination Survey. We defined cases that are difficult for aviation medical examiners to evaluate. This issue included cases of peripheral dizziness and tinnitus, cases of atopic dermatitis treated with biological agents, and cases of malignant tumors.

  • Aviation medical examiners should always keep two principles in mind when evaluating patients with vertigo: Does this disease render him or her incapacitated? And does this disease recur? Meniere's disease accompanies vertigo and hearing symptoms such as hearing loss and tinnitus and is particularly important because it can render the patient incapacitated and has recurrent consequences. In this case, we intended to investigate the course of a patient who was evaluated with symptoms of dizziness and tinnitus and to review the patient's judgment result according to the principles of aeromedical evaluation.

  • In Vol. 31, No. 1, our journal prepared four review articles and two original papers. First, in commemoration of the 30th anniversary of our journal, the first review article described the history, development, and recent efforts for the qualitative improvement of the journal. For aviation workers, dizziness is an important disease that can interfere with their work. So, in the second review, we introduced guidelines that aviation medical examiners should be aware of in evaluating aviation workers who complain of dizziness. In the era of coronavirus disease 19 (COVID-19) pandemic, it is very important for aviation workers to get the vaccination safely and return to their work. So, Kwon introduced the latest updated guidelines for vaccination for aviation workers. In addition, Kim described the relationship between COVID-19 infection and respiratory allergic disease, and appropriate treatment guidelines. Lim investigated the recent trend of human papillomavirus vaccination for foreigners during the COVID-19 period, and investigated its implications. Finally, Ra developed various programs to strengthen students' global competencies and analyzed their satisfaction.

Evaluation of the rat tissue reaction to experimental new resin cement and mineral trioxide aggregate cement

  • Objectives: New resin cement (NRC) has been developed as a root repairing material and the material is composed of organic resin matrix and inorganic powders. The aim of this study was to compare the rat subcutaneous tissue response to NRC and mineral trioxide aggregate (MTA) cement and to investigate the tissue toxicity of both materials. Materials and Methods: Sixty rats received two polyethylene tube-implants in dorsal subcutaneous regions, MTA and NRC specimens. Twenty rats were sacrificed respectively at 1, 4 and 8 wk after implantation and sectioned to 5 ${\mu}m$ thickness and stained with Hematoxylin-Eosin (H-E) or von-Kossa staining. The condition of tissue adjacent to the implanted materials and the extent of inflammation to each implant were evaluated by two examiners who were unaware of the type of implanted materials in the tissues. Data were statistically analyzed with paired t-test (p < 0.05). Results: In specimens implanted with both NRC and MTA, severe inflammatory reactions were present at one wk, which decreased with time. At eighth wk, MTA implanted tissue showed mild inflammatory reaction, while there were moderate inflammatory reactions in NRC implanted tissue, respectively. In NRC group, von-Kossa staining showed more calcification materials than MTA group at eighth wk. Conclusions: It was concluded that the calcium reservoir capability of NRC may contribute to mineralization of the tissues.

부산·경남지역 작업환경측정기관의 산업안전보건법 준수 실태 및 준수율 제고를 통한 측정기관 종사자 건강보호 방안 고찰 (A Study on the Compliance of the Occupational Safety and Health Act by Busan and Gyungsangnam-do Province Working Environment Measurement Institutions)

  • Objectives: This study aims to investigate whether working environment measurement institutions(WEMIs) are conducting safety and health education, risk assessment, and oversight of special management materials, and whether working environment measurement and special medical examinations are being conducted as prescribed in the Occupational Safety and Health Act(OSHA). Methods: In of February 2021, a questionnaire was prepared and distributed to 33 WMEIs registered with the Ministry of Employment and Labor(MoEL) in Busan and Gyeongsangnam-do Province. The responses were collected and then analyzed. Results: The findings show that 5 WEMIs(15%) complied fully with OSHA. Risk assessment was conducted by 13 WMEIs(39%) and safety education by 11 WMEIs(33%). Eighteen WMEIs(55%) conducted working environment measurement, and 29 WMEIs(88%) conducted special medical examinations. The implementation rate of the risk assessment in the health industry(85%) was higher than the one in the special technology industry(11%)(p<0.05). The implementation rate of the special medical examination in the examiners(54%) was not as high as the one in the analysts(91%)(p<0.05). Conclusions: The MoEL needs to check whether basic OSHA requirements are being observed during regular inspections by WEMIs. These findings indicate that it is necessary to prepare a plan to improve the rate of compliance with OSHA regulations.

The reliability of tablet computers in depicting maxillofacial radiographic landmarks

  • Purpose: This study was performed to evaluate the reliability of the identification of anatomical landmarks in panoramic and lateral cephalometric radiographs on a standard medical grade picture archiving communication system (PACS) monitor and a tablet computer (iPad 5). Materials and Methods: A total of 1000 radiographs, including 500 panoramic and 500 lateral cephalometric radiographs, were retrieved from the de-identified dataset of the archive of the Section of Oral and Maxillofacial Radiology of the University Of Connecticut School Of Dental Medicine. Major radiographic anatomical landmarks were independently reviewed by two examiners on both displays. The examiners initially reviewed ten panoramic and ten lateral cephalometric radiographs using each imaging system, in order to verify interoperator agreement in landmark identification. The images were scored on a four-point scale reflecting the diagnostic image quality and exposure level of the images. Results: Statistical analysis showed no significant difference between the two displays regarding the visibility and clarity of the landmarks in either the panoramic or cephalometric radiographs. Conclusion: Tablet computers can reliably show anatomical landmarks in panoramic and lateral cephalometric radiographs.

인위적 기원 가돌리늄(gadolinium)의 해양 유입 연구 동향 및 향후 연구 방향 (Anthropogenic Gadolinium (Gd) Inputs into the Ocean: Review and Future Direction)

  • Gadolinium (Gd), one of a rare earth element (REE), has been widely used worldwide since the 1980s, as a resource material for contrast agents injected into examiners of magnetic resonance imaging (MRI) test. The organic complexed form of Gd shows an extremely stable behavior in natural environment (water), so is known that the artificial Gd from medical uses is not removed from the waste water treatment plant (WWTP) and eventually introduced into the ocean through the estuary. Since the 1990s, some previous studies have often been conducted on Gd anomalies in natural water and their effects an artificial origin from land or metropolitan areas, but little research has been potential impacts on the ocean water. In this paper, we review and introduce recent studies related to Gd anomaly in natural water and related marine effects, and also propose the future research directions.

기능성 화장품의 특허청구범위 기재 (A Study of Patent Examination Practice for the Use Claims of Cosmeceuticals)

  • 의약품의 치료 효과와 유사한 치료활성 기능을 갖는 화장품(코스메슈티컬)의 개발이 증가함에 따라 화장품법에서 규정한 기능성 화장품의 용도인 피부 미백, 주름개선, 자외선 차단 또는 피부를 곱게 태워주는 기능이외의 항염증, 탈모 방지, 비만 개선 등과 같은 의약품의 치료 효과와 차별화되지 않는 다양한 기능을 화장품의 용도로서 특허청구범위에 기재하여 특허출원하는 경우가 증가하고 있다. 그러나 상기와 같은 의약품의 치료 효과와 차별화되지 않는 기능을 화장품의 용도로서 화장품 발명에 기재한 경우 특허요건 판단에 있어서 심사과 및 심사관에 따라 차이를 보이고 있어 심사의 일관성에 대한 요구가 제기되어 왔다. 위와 같은 요구에 부응하기 위하여, 본 논문에서는 국내 기능성 화장품의 특허등록 현황, 심사관들의 견해 및 미국, 유럽, 일본의 화장품 관련 발명의 심사 실무를 파악하여 일관성 있는 특허 심사를 위한 대응방안을 제시하고자 하였다.
