중환자실 환자의 욕창 관련 경계압력 예측요인 (Factors Predicting the Interface Pressure Related to Pressure Injury in Intensive Care Unit Patients)

  • Purpose: Interface pressure is a factor that contributes to the occurrence of pressure injuries. This study aimed to investigate interface pressure at common sites of pressure injury (occipital, gluteal and peritrochanteric areas), to explore the relationships among risk factors, skin condition and interface pressure, and to identify risk factors influencing interface pressure. Methods: A total of 100 patients admitted to the intensive care unit were enrolled at a tertiary teaching hospital in Korea. Interface pressure was recorded by a scanning aid device (PalmQ). Patient data regarding age, pulmonary disease, Braden Scale score, body mass index, serum albumin, hemoglobin, mean blood pressure, body temperature, and oxygen saturation were included as risk factors. Data collected from July to September 2016 were analyzed using binary logistic regression. Results: The mean interface pressure of the occipital, gluteal, and right and left peritrochanteric areas were 37.96 (${\pm}14.90$), 41.15 (${\pm}16.04$), 53.44(${\pm}24.67$), and 54.33 (${\pm}22.80$) mmHg, respectively. Predictive factors for pressure injuries in the occipital area were age ${\geq}70$ years (OR 3.45, 95% confidence interval [CI]: 1.19~9.98), serum albumin deficit (OR 2.88, 95% CI: 1.00~8.26) and body temperature ${\geq}36.5^{\circ}C$ (OR 3.12, 95% CI: 1.17~8.17); age ${\geq}70$ years (OR 2.81, 95% CI: 1.10~7.15) in the right peritrochanteric area; and body temperature ${\geq}36.5^{\circ}C$ (OR 2.86, 95% CI: 1.17~6.98) in the left peritrochanteric area. Conclusion: Our findings suggest that old age, hypoalbuminemia, and high body temperature may be contributory factors to increasing interface pressure; therefore, careful assessment and nursing care of these patients are needed to prevent pressure injury. Further studies are needed to establish cutoff values of interface pressure for patients with pressure ulcers.

  • This study presents a new hybrid fNIRS-EEG system to meet the demand for a lightweight and low-cost sleep pattern monitoring device. For multiple-channel configuration, a six-channel electroencephalogram (EEG) and a functional near-infrared spectroscopy (fNIRS) system with eight photodiodes (PD) and four dual-wavelength LEDs are designed. To enhance the convenience of signal measurement, the device is miniaturized into a patch-like form, enabling simultaneous measurement on the forehead. Due to its fully integrated functionality, the developed system is advantageous for performing sleep stage classification with high-temporal and spatial resolution data. This can be realized by utilizing a two-dimensional (2D) brain activation map based on the concentration changes in oxyhemoglobin and deoxyhemoglobin during sleep stage transitions. For the system verification, the phantom model with known optical properties was tested at first, and then the sleep experiment for a human subject was conducted. The experimental results show that the developed system qualifies as a portable hybrid fNIRS-EEG sleep pattern monitoring device.

공공설치용 자동제세동기의 사용성 연구 (Usability Study on Public Access Defibrillator)

  • Usability study of the CU Medical "i-PAD" AED was conducted to validate the usability of the device, primarily focused on the trained user as well as the utility of the training material. Comparing these results of the trained participants to those obtained by Callejas et al. (2004), who studied video-trained users of two commercially available Philips AEDs (FR2 and HS1) using a male manikin only, the participants in this study were more successful (100% vs. 85.7%-FR2 vs. 88.9%-HS1) and faster to shock (58.5s-i-PAD vs. 91s-FR2 vs. 90s-HS1). While the focus of this study was on validating ease of use for trained bystanders, the results of the untrained condition showed that, from a usability perspective, these devices perform as well as any other AEDs already approved and available for purchase for untrained bystander use, as evidenced by the comparison to various published research results.

  • Medical robotics and computer aided surgery in general, and robotic telesurgery in particular, are promising applications of robotics. In this paper, we shows a novel single-master (PHANTOM based single-master multi-slave telerobotic system) multi-slave system using two parallel mechanism micromanipulators as a slave device. After a general introduction to the systems structure and configuration of telerobotic system, a manipulation control strategy to build the system that human and both manipulators perform the cooperative manipulation, is introduced, followed by its kinematic analysis, mapping method, and experimental results.

  • As biosensors are widely used in the medical field, flexible devices compatible with live animals have aroused great interest. Especially, significant research has been carried out to develop implantable or skin-attachable devices for real-time bio-signal sensing. From the device point of view, various biosensor types such as field-effect transistors (FETs) and multi-electrode arrays (MEAs) have been reported as diverse sensing strategies. In particular, the flexible FETs and MEAs allow semiconductor engineering to expand its application, which had been impossible with stiff devices and materials. This review summarizes the state-of-the-art research on flexible FET and MEA biosensors focusing on their materials, structures, sensing targets, and methods.

  • In this paper, we analyzed the spectroscopic properties of strip to analyzer urine qualitatively & quantitatively and make urine analyzer system stable by spectroscopy, and research the property of preamplifier unit. The analysis of spectroscopic properties of 10 pads in the strip is used for determine the wave length of light source of optic detector unit and used for basic materials which are necessary that we develop the algorithm analyzing the density grade of pad accurately. We make preamplifier unit by using the current to frequency method to measure the distribution of pad color. We implemented urine analyzer system. This system's hardware is composed of measuring unit for detect of distribution density of strip pad, main processing unit, communication unit, interface device, thermal printer, and indicator. The software consists of the program which manage the argument of test, proportion initial value of urine analyzer and calibrate analyzed result.

  • 의학 기술의 발달과 저 출산 현상으로 인구의 고령화가 진행되어감에 따라, 실버세대를 위한 다양한 문화와 산업이 대두되고 있다. 실버 세대는 상대적으로 많은 여가시간을 보내기 위해 다양한 사회 활동과 문화 콘텐츠를 경험하길 원하지만, 노령화에 따른 신체적 심리적 정신적 기능 저하로 인해 제약이 따르게 된다. 본 논문에서는 실버 세대들이 사회활동에서 가장 필요로 하는 이동성 확보에 도움이 되는 하체 단련과 체력 증진 기능을 갖춘 체감형 자전거 게임 콘텐츠를 제안한다. 제안된 게임 콘텐츠는 실버세대에게 익숙한 자전거라는 기구를 이용하여 노인들에게 적합한 게임 구성과 디자인을 통해 게임에 대한 흥미를 유발시킬 수 있다. 노인들은 게임의 몰입감을 증대시키기 위해 개발된 체감형 인터페이스와 실시간 3D 그래픽스 디스플레이를 통해 놀이와 운동을 동시에 즐기며 여가시간을 활용할 수 있다.

  • 본 논문에서는 Funology(퍼놀로지)와 몸의 체험 통해 형성된 언어개념 및 사고를 바탕으로 새로운 어플리케이션 분류 체계를 연구하고, 이를 Funology Body로 명명하였다. Funology Body는 크게 몸, 세계(환경), 디바이스 툴의 구성을 가진 분류 및 검색 체계이다. 몸은 신체의 부위에 따라 Brain, Eyes, Ears, Nose, Mouth, Hand, Torso, Feet, Heat로 구분할 수 있는데, 몸 부분의 개념과 관련된 앱을 연결시켜 분류체계를 구성함으로써 쉽고 직관적이며, 체험적인 검색을 가능하게 하였다. 봄의 구성 중 Brain은 사고와 관련된 어플리케이션을 포함할 수 있도록 Book, Account, Business, Memory, Education, Search, Aphorism로 하위분류되어 있으며, Eyes는 시각과 관련된 Video, Photography, Broadcast로, Ears는 청각과 관련된 Music, Instrument, Audio, Radio로, Nose는 후각과 관련된 Perfume, Smell로, Mouth는 먹을 것 및 의사소통과 관련된 Food, SNS, Chatting, Email, Blog로 Hand는 조작하고 만들고 장난치는 것과 관련된 Games, Kits, Editing으로 Torso는 내장을 보호하고 몸의 중심을 이루는 개념과 관련된 Health, Medical, Dance, Sport, Fashion, Testyuorself로 Feet는 이동하여 영토를 넓히는 개념과 관련된 Travel, Transportation, Map, Outdoor로, Heat는 감정과 관련된 Fear, Anger, Joy, Sadness, Acceptance, Disgust, Expectation, Surprise로 이루어져 있다. 그밖에 세계(환경)는 News, Time, Weather, Map, Fortune, Shop으로 디바이스 툴은 Interface, Utilities로 구성되어 있다. 직관적이고, 감각적인 재미를 줄 수 있다는 것뿐만 아니라 변화하는 어플리케이션 사용자의 태도와 취향을 유연성 있게 반영할 수 있다는 점에서 기존 분류체계와는 다른 Funology Body의 특정을 찾을 수 있다.

  • 컴팩트 플래쉬방식의 휴대용산소포화도와 ECG감시 시스템을 구현하는데 목적이 있다. 먼저 휴대용 산소포화도의 측정은 산소포화도와 맥박을 동시에 측정해서 2채널을 기록하도록 설계했다. 다른 장치없이 환자의 상태를 감지할 수 있고 소형화되고 휴대 가능하게 했다. 환자모니터링에 의해 발생하는 문제점을 해결하기 위해 기저선변동을 없애기 위해 2D 섹터 알고리즘을 적용시킨 아날로그 보정회로를 추가했다. 현재 SpO2모듈은 완성됐지만 컴팩트 플래쉬 방식을 사용하는데는 아직까지 많은 개선의 여지가 있다. ECG감시장치는 3극자 ECG시스템을 적용했다. 필터로는 2가지가 혼합된 방식이다. 하나는 전력선을 없애는 것이고 다른 하나는 기저선 변동을 없애는 것이다. 이것은 DC간섭을 줄이고 전력선의 주파수 간섭을 없애는 효과가 있다. 컴팩트플래쉬 방식으로 환자의 정보를 적은 비용으로 쉽게 기록하고 의료기관에 알릴수 있다.

Cutaneous Patches to Monitor Myoelectric Activity of the Gastrointestinal Tract in Postoperative Pediatric Patients

  • Purpose: Limited means exist to assess gastrointestinal activity in pediatric patients postoperatively. Recently, myoelectric gastrointestinal activity recorded by cutaneous patches has been shown in adult patients to be predictive of clinical return of gastrointestinal function postoperatively. The aim of this case series is to demonstrate the feasibility of this system in pediatric patients and to correlate myoelectric signals with return of bowel function clinically. Methods: Pediatric patients undergoing abdominal surgery were recruited to have wireless patches placed on the abdomen within two hours postoperatively. Myoelectric data were transmitted wirelessly to a mobile device with a user-interface and forwarded to a cloud server where processing algorithms identified episodes of motor activity, quantified their parameters and nominally assigned them to specific gastrointestinal organs based on their frequencies. Results: Three patients (ages 5 months, 4 year, 16 year) were recruited for this study. Multiple patches were placed on the older subjects, while the youngest had a single patch due to space limitations. Rhythmic signals of the stomach, small intestine, and colon could be identified in all three subjects. Patients showed gradual increase in myoelectric intestinal and colonic activity leading up to the first recorded bowel movement. Conclusion: Measuring myoelectric intestinal activity continuously using a wireless patch system is feasible in a wide age range of pediatric patients. The increase in activity over time correlated well with the patients' return of bowel function. More studies are planned to determine if this technology can predict return of bowel function or differentiate between physiologic ileus and pathologic conditions.
