Research of Database Model of Kim-YoungHun's Medical Chart (청강 김영훈 진료기록 데이터베이스모형 개발연구)

  • Cheong-Gang Medical Chart is 60 years worth of diagnosis records kept by Oriental Medicine Doctor Kim Young Hoon [金永勳, 號 晴崗 1882-1974], who held practice in Seoul's Jong-ro from 1915 till 1974. Kim Young Hoon's eldest son, Kim Ki Su (金琦洙) donated the medical records exceeding a thousand volumes to KyungHee University, and researches are being made presently. The author of Cheong-Gang Medical Chart, Kim Young Hoon, was a medical scholar who studied the essence of the traditional medicine of his time. He was handed down the quintessence of traditional medicine by keeping in touch with the prominent oriental doctors in Seoul at that time, and he constantly applied it to his practice and made records of it. Consequently, his diagnosis charts contain a whole new form of prescriptions, treatment skills, and processes of clinical application that have never been seen before in the texts of Korean Medicine. The writer has written a paper on the present condition of Cheong-Gang Medical Chart, which was published in the Journal of Korean Oriental Medicine in 2004. This manuscript reports the results of the test studies made to develop an efficient database model as a prior step to organizing the medical records into a data bank.

  • Treatment chart(醫案) is a document that doctors have described about their own practice. Western doctor's chart has a prescribed form, but otherwise chart of oriental medicine is a free form. As It has no prescribed form, doctors of oriental medicine have recorded important facts that they think especially. For example, details are patient's name, age, sex and address, the name of disease, state of pulse and tongue, state of secretions(stools, urine, perspiration, etc.), color of face, state of sleep and thirst, diagnosis, prescription, improvement of herb, teaching, and so forth. If we study on chart of oriental medicine, we draw a lesson about practices of a famous doctors. Through that we can increase ability of diagnosis and adaptation to circumstance, make ourselves familiar with use of past prescriptions and way of changing prescription, and learn narrative story of past doctors' personal experience, their though and teaching. As chart of oriental medicine had many methods to take a measure to meet the false situation, we can learn that. Through chart of oriental medicine, we can improve our practice of oriental medicine.

  • As the need of enhanced medical treatment grows up, Purchasing step which include medical engineer's opinion has to be required. On this procedure, Medical Engineer Departments has used $\ulcorner$comparision chart$\lrcorner$. In this paper, How to compose and utilize comparison chart for purchasing of medical equipment by medical engineer in the hospital. It is made through the technical and objective investigation. From these points, We think that it can be available for proper installation and improved maintenance.

  • As the radiotherapy technique development, the needs for using of medical electronic chart in the department of radiation oncology is growing. However, the complexity of affairs of radiation oncology make it difficult to develop a electronic medical chart. In this study, we introduce the electronic medical chart developed by domestic hospital. The function and example of electronic medical chart designed as radiation treatment progress was showed and the future study was presented.

Survey for Standardization of Medical Examination Items in Oriental Medicine (한의 진찰항목 표준화를 위한 실태 및 수요조사)

  • Objective : To poll oriental medical doctors on their opinions about the necessity of standard medical charts and of detailed items in their development. Currently, oriental medical institutions use their own medical charts, but a standard medical chart is necessary for medical information sharing. Methods : Report by 912 of the 10,490 oriental medical doctors surveyed on their general aspects, actual conditions, and requirements. Results : The oriental medical doctors surveyed who worked in oriental medicine clinics, oriental medicine hospitals, and public health centers said medical examination programs should consist of chief complaints (said by 814 respondents or 89.25%), history (792 or 86.84%), ordinal symptoms (753 or 82.57%), diet (727 or 79.71%), emotions and overstrain (654 or 71.71%), side effects and allergies (622 or 68.20%), improvement of symptoms (605 or 66.34%), a questionnaire on particular diseases (558 or 61.18%), social aspects (523 or 57.35%), a physical examination (520 or 57.02%), a questionnaire on syndrome differentiation (514 or 56.36%), diagnosis using medical devices and laboratory tests (471 or 51.64%), and Sasang constitution (357 or 39.14%). Ninety-one percent of the respondents said they intended to use a standard chart, and 82.19% agreed to share patient information with medical institutions. Conclusions : Over 90 percent of the oriental medical doctors surveyed said they need a standard medical examination program. Oriental medical examination items that correspond with the opinions of the oriental medical doctors surveyed and of experts will thus be developed, and the draft chart will be distributed to oriental medical institutions with the developed medical forms and electronic medical chart.

  • This study aims at proposing the basic architectural data of the medical recording department for the general hospital planning and design. This research investigated the size of working space and the chart custody space through actual survey and the present conditions of space were analyzed. In result, this study proposes the formula for the calulation of the chart custody space according to computerization level and the chart custody planning year.

  • There are various kinds of methods to estimate the image quality of x-ray film. The Hawlet-chart is one of the methods to estimate the depiction power of x-ray film image. Authors made a test using the Hawlet-chart in clinical field and demonstrated the superiority of utility value.

  • High kilovoltage technique as compared with low kilovoltage in taking chest radiogram has much advantages. Authors performed an experiment by using acryl phantom to make the technical chart at 120KV and obtained the results as follows; 1. Increase and decrease of tube voltage in thickness change was 3.2KV per cm at variable technic chart. 2. At fixed kilovoltage technic chart, increase and decrease was $0.12{\sim}0.2$ mAs per cm in chest thickness. 3. Increase and decrease of distance was 1.3inches per cm in thicknese change

  • The standardization of the dental chart proposed to computerization of the dental hospital information. To objective standardization, we gathered the dental chart from four university hospital(seoul, yonsei, kyunghee, pusan) and held a conference with dental physicians. From gathered charts, we classified the common and non-common items. We proposed the standardization to choose the most of the common items and necessary non-common things. The proposed chart include those items and the new things which the dental physicians want. After this paper, we will hold a conference with more dental physicians about more data to make more useful proposal.

  • The paper chart of medical record had been used as an important medium of the medical information in the medical recording department. This chart has not dealt with the development of information industry and the change of use of medical record in several decades. This study is to show the data which is helpful for the current spacial situation of medical recording in Korea and understand problems to reconsider the medical recording department of hospital architectural plan. In addition, this study is to look for the spacial changes by computerizing of medical recording and its special confrontation and the prospect for the future medical recording department which is going to work as a medical information center.
