Working Conditions in Home Health Care Nursing: A Survey of Home Care Nurses in Hospitals in Korea (전국 의료기관 가정간호사 근무 실태)

  • Purpose: The purpose of this research was to address the working conditions of home health nurses through a nationwide home health agency survey conducted at hospitals. Method: The mail surveys were sent to 303 home health nurses nation wide and returned with a response rate of $71.8\%$. Result: (a) Seventy-five percent of home health agencies were established within the past5 years and half of home health nurses are over 40 years old. (b) Working conditions were considered as follows: Seventy-one percent of respondents were full-time employees, sixty-sixpercent of home health nurses had unscheduled visits on a regular day of duty and forty-eight percent were on vacation. Fifty-one percent of home health nurses have experienced traffic accidents and paid. penalties ($65.9\%$). Self-reported monthly income level per year was an average of 28,364,000 won. (c) Rates were significantly higher for shoulder pain ($61.5\%$), lower back pain ($54.1\%$), knee pain ($39.4\%$), and gastrointestinal problems ($33.0\%$). Conclusion: These baseline results show the importance of improving home health nursing working conditions, a comprehensive prevention system and safeguards from physical discomfort.

  • Big Data is gathering all the attention from every business community. Pervasive use of machine-to-machine (M2M) applications and mobile devices bring an explosion of data. By analyzing this data, the private and public sectors can benefit in the areas of cost reduction and productivity. The Korean government is actively pursuing Big Data initiatives to promote its usage. This paper aims to select industries which fit for the development of Big Data with a verification of the experts. The analytic hierarchy process (AHP) is applied to systematically derive the opinion of more than 50 professionals. Medical / welfare, transportation / warehousing, information and communications / information security, energy, the financial sector have been identified as promising industries. The results can be utilized in developing Big Data best practices thus contributing industrial development.

  • Although Korea's e-government is at the level of developed countries, it has been pointed out that the introduction of SOA is too slow. It also points out that the government's low SOA level has a negative impact on the UN's e-government rankings. In 2002, Korea introduced the first web service in the domestic public sector. At that time, there were few cases in the world where Web services were introduced in the public sector. However, the advanced e-government advanced countries already have SOA ideology based on e-government. Denmark and so on have SOA ideologies on the basis of e-government, and many countries make and use service repositories. In this environment, we analyze the application status of web service centered on the case of e - government in Japan.

Nursing research trends for the consciousness of bioethics published in korea (국내학술지에 게재된 생명윤리의식에 관한 간호연구의 동향)

  • The purpose of this study was to identify nursing research trends for the consciousness of bioethics published in korea. A total of 30 studies were analyzed using descriptive statistics. 70% of the studies was published for the last five years, and 76.7% of studies were conducted in a descriptive study. Nursing students accounted for 83.1% in terms of the participants, and a questionnaire was used for the data collections in the entire studies. The studies considered ethical aspect accounted for 90.0%. In the experimental studies, the nursing intervention was a education program, and dependent variables of the studies included consciousness of bioethics, moral sensitivity, critical thinking and moral judgment. Among the top 10 priorities in the key words, the first priorities was the bio-medical ethics. In conclusion, it is suggested to develop an expanding-iterative studies, qualitative studies, and development of bioethics programs for nurses.

Enhancement of 4 Bar Parallelogram Linkage for a Medical Bed (의료용 침대를 위한 평행 4절 링크의 개선)

  • The design and actual implementation of the four-bar parallel link was studied in the paper. The parallel four-section link is widely used as a basic kinematic mechanism for transmitting the rotation of one axis to the rotational motion of the other axis. However, the parallel 4 link has a problem that phase reversal occurs at the turning point during the movement. In order to prevent the link reversal, it is known that a double parallelogram-type link is formed by attaching an additional phase reversal suppression link with an offset. However, as a result of the actual fabrication experiment, the movement is not smooth at the transition point. In order to solve this problem, in this study, a link for smooth movement is added in addition to a link that provides an offset to prevent phase reversal, so that the phase reversal does not occur at a specific point when the driven shaft rotates along the drive shaft. The test result confirms the validity of our suggestion.

Creating and Validating Scale of Resilience to Burnout and Scale of Burnout Risk with Mixed Methods (질적-양적 연구방법론의 혼합에 의한 의료사회복지사의 소진탄력성 및 소진위험성 척도개발 연구)

  • The purpose of this study was to create and validate Scale of Resilience to Burnout and Scale of Burnout Risk for social workers in medical settings. For the purpose, instrument development model, a kind of mixed methods research was used. In the first phase, six dimensions of resilience(: professional competency, accomplishment and worthwhileness, firm belief and value about their profession, good teamwork, support by their agency, and individual resources) and six dimensions of risk to burnout(: dissatisfaction with organizational condition, interpersonal stress among team members, organizational conflict, work related stress, deficiency of professionalism, and individual stress) were suggested thorough the preceding papers with qualitative approaches. The second phase involved analysis of a survey of 185 participants that appeared to validate the dimensions of the measures. The construct validity and reliability of each measure were tested. And it was founded that there were its own factors in each concept, although resilience to burnout related negatively to burnout risk. The results of this study suggest mixed methods research is useful to develop measures reflecting voices in the social work field.

Biopsychosocial Factors and Perceived Disability in Saleswomen with Concurrent Low Back Pain

  • Objectives: To quantify disability level in salespeople with concurrent low back pain (LBP) and to determine the relative associations between demographic, occupational, psychosocial and clinical factors and back disability. LBP is the most common cause of work-related disability in people under 45 years of age and the most expensive cause of work-related disability, in terms of workers' compensation and medical expenses. Evidence suggests high prevalence of LBP in salespeople. Methods: A cross-sectional survey was conducted in which 184 saleswomen with a current episode of self-reported LBP working in a large up-scale department store filled out a battery of 6 self-administered questionnaires and received a standardised physical examination. Results: Saleswomen with concurrent LBP had low disability levels. Factors significantly associated with disability were pain intensity, measured by a visual analogue scale, in the past week (p < 0.001), physical and mental health status (p < 0.001, p = 0.003, respectively), fear avoidance scores for both work and physical activities (p = 0.031, p = 0.014, respectively), past history of LBP (p = 0.019), and self-reported frequency of pushing or pulling objects placed in high positions during work (p = 0.047). A significant level (45%) of the variance in disability status was explained by these variables. Conclusion: In clinical management of LBP workers who required prolonged standing, such as salespeople, clinicians should look for modifiable risk factors associated with disability. Specific measures need to be taken to prevent disability due to LBP among salespeople.

Human Body Mechanics of Pushing and Pulling: Analyzing the Factors of Task-related Strain on the Musculoskeletal System

  • The purpose of this review is to name and describe the important factors of musculoskeletal strain originating from pushing and pulling tasks such as cart handling that are commonly found in industrial contexts. A literature database search was performed using the research platform Web of Science. For a study to be included in this review differences in measured or calculated strain had to be investigated with regard to: (1) cart weight/ load; (2) handle position and design; (3) exerted forces; (4) handling task (push and pull); or (5) task experience. Thirteen studies met the inclusion criteria and proved to be of adequate methodological quality by the standards of the Alberta Heritage Foundation for Medical Research. External load or cart weight proved to be the most influential factor of strain. The ideal handle positions ranged from hip to shoulder height and were dependent on the strain factor that was focused on as well as the handling task. Furthermore, task experience and subsequently handling technique were also key to reducing strain. Workplace settings that regularly involve pushing and pulling should be checked for potential improvements with regards to lower weight of the loaded handling device, handle design, and good practice guidelines to further reduce musculoskeletal disease prevalence.

Post Occupancy Evaluation of the Central Hospital in Quang Nam Province of Vietnam Funded by Korean ODA (ODA 지원 베트남 중부 꽝남성 종합병원의 거주후 평가)

  • Purpose: This is the ex-post evaluation on "the Central General Hospital in Quang Nam Province in Vietnam" funded by Korean ODA. Through this evaluation, this study tries to explore useful ways to improve the effectiveness of Korean ODA projects related to hospital establishment. Methods: Field survey and analysis of questionaire to the staffs and patients of Quang Nam Province General Hospital were conducted twice in 2012 and 2014 after the completion of the Hospital. Two sets of surveys and questionaire outcomes have been comparatively analysed by statistics in order to seek the trend of satisfaction level of hospital users and find out the improvement way in hospital design and construction. Results: The satisfaction level on the hospital was relatively high and that of 2014 is higher than that of 2012. The satisfaction level of the users was recorded in Likert 5 scales. Continuous facility complement and maintenance is considered as the main contributor in increasing the satisfaction level of users. As far as the satisfaction level of outpatient is concerned, that of 2012 was recorded as 3.65 and that of 2014 was 4.05. In case of inpatient, the satisfaction level on the hospital in 2012 was 3.76, and 4.06 in 2014. Implications: In overseas hospital projects, proper hospital project management system including medical service plan, strategic plan, brief, design & construction and maintenance have to be put together effectively and efficiently for the success of the hospital project. At the same time local climate, culture, building material and local code have to be considered in order to build sustainable hospital.

Mesothelioma in Sweden: Dose-Response Analysis for Exposure to 29 Potential Occupational Carcinogenic Agents

  • Background: There is little information on the dose-response relationship between exposure to occupational carcinogenic agents and mesothelioma. This study aimed to investigate this association as well as the existence of agents other than asbestos that might cause mesothelioma. Methods: The Swedish component of the Nordic Occupational Cancer (NOCCA) study consists of 6.78 million individuals with detailed information on occupation. Mesothelioma diagnoses recorded in 1961-2009 were identified through linkage to the Swedish Cancer Registry. We determined cumulative exposure, time of first exposure, and maximum exposure intensity by linking data on occupation to the Swedish NOCCA job-exposure matrix, which includes 29 carcinogenic agents and corresponding exposure for 283 occupations. To assess the risk of mesothelioma, we used conditional logistic regression models to estimate hazard ratios and 95% confidence intervals. Results: 2,757 mesothelioma cases were identified in males, including 1,416 who were exposed to asbestos. Univariate analyses showed not only a significant excess risk for maximum exposure intensity, with a hazard ratio of 4.81 at exposure levels 1.25-2.0 fb/ml but also a clear dose-response effect for cumulative exposure with a 30-, 40-, and 50-year latency time. No convincing excess risk was revealed for any of the other carcinogenic agents included in the Swedish NOCCA job-exposure matrix. Conclusion: When considering asbestos exposure, past exposure, even for short periods, might be enough to cause mesothelioma of the pleura later in life.
