뎅기열 발병은 전 세계 인구의 약 1/3이 거주하고 있는 열대, 아열대 기후에 집중되며, 우리나라도 아열대 기후로 바꾸고 있어 뎅기열 발병에 취약해지고 있다. 뎅기열은 감염병 관리 차원에서 진단 이력 관리가 중요하다. 감염병 이력에 따라서 지역별, 연령별, 남녀비율 등에 따라서 개개인의 치료 방법과 전략을 수립할 수 있는 체계가 필요하다. 본 논문에서는 뎅기열 관제시스템을 제안하며, 이러한 시스템은 뎅기열의 발병에 대한 체외진단기기를 이용한 실시간 집계방식으로 발병률과 사망률을 감소시킬 수 있는 전략을 수립하는 데 유용하게 활용될 수 있다. 뎅기열 관리를 위한 관제시스템 구성으로 형광면역진단 키트를 이용한 뎅기열 체외진단기기와 실시간 뎅기열 관제시스템으로 구성되어 있다. 본 논문으로 개발된 뎅기열 관제시스템은 향후 정부의 감염병 통합정보와 결합되어 다양한 감염병 관리 및 정책 활용을 위해서 활용될 수 있을 것이다.
Objectives: Based on the results from the Korean Total Diet Study (KTDS), the sodium (Na) and potassium (K) intake of Koreans were estimated and compared with intake estimates from the Food & Nutrient Database (FNDB), as in the Korea National Health and Nutrition Examination Survey (KNHANES) to verify the validity of these estimates. Methods: One hundred and thirty-four representative foods (RFs) covering 92.5% of the total food intake of Koreans were selected, and 228 pairs of corresponding 'RF x representative cooking method' were derived by reflecting the methods used mainly in terms of frequency and quantity in their cooking. RF samples were collected from three cities with a larger population size in three regions (nine cities) nationwide, and six composite samples were made for each RF, considering its regional and/or seasonal characteristics. One thousand three hundred and sixty-eight 'RF x representative cooking method' pair samples were prepared, and the Na and K contents were assessed using inductively coupled plasma atomic emission spectrometry (ICP-MS). The Na and K intake of the Korean population was estimated by linking the content with the food intake data from the 7th KNHANES. Results: The mean Na and K intake of Koreans were 2,807.4 mg and 2,335.0 mg per person per day, respectively. A comparison with the Na and K intake from KNHANES, including only RFs of KTDS, showed comparable results with less than 5% variation. While the contribution and ranking of food items to Na intake were similar between KNHANES and KTDS, there were differences in K intake. This was attributed to the large discrepancies in the K content of rice and coffee between KTDS results and the values in the 9th Revision of the National Food Composition Table used in KNHANES. Conclusions: The Na and K intake of Koreans estimated based on the KTDS, which performed nutrient analysis on samples prepared to a 'table-ready' state using foods of the representative collection, was similar and comparable with that of KNHANES. This supports the validity and usefulness of FNDB-based nutrient intake estimation at the population level. The list of nutrients studied in KTDS is expected to be expanded, allowing for intake estimation of nutrients with currently insufficient or absent information in the FNDBs in use.
오늘날 의료영상매체의 획기적인 발전으로 각 병원에서 최신 의료장비를 도입함으로써 첨단화, 디지털화로 급변하는 추세이다. 이러한 움직임에 발맞추어 방사선 종양학과에서도 CR system을 도입하여 film system의 단점을 보완하고 병원에서 사용하고 있는 Picture Archiving and Communication System(PACS)과 Electronic Medical Record (EMR) , 그리고 Radiation therapy Treatment Planning system(RTP)의 network를 원활히 하여 업무효율 증대 및 환자에 대한 의료의 질 개선과 서비스 향상을 이루고자 하고 있는데, 방사선 종양학과의 Computed Radiography system(CR system)을 이용하여 PACS에 통합한 사례를 소개하고 그 유용성을 평가하고자 한다. 의료용 선형가속기인 MEVATRON-MX를 이용하여 현재 시행하고 있는 정도관리 중 Gantry, Collimator Star Shot, Light vs. Radiation Field Accuracy, HDR QA(Dwell position accuracy)를 시행하여 PACS 상에 구현하였고 모니터 상에서 디지털 영상을 통한 QA가 가능한지 확인하였다. 또한, 현재 S병원에서 사용 중인 Operation Control System(OCS)과 연동하여 치료에 필요한 코드를 각각의 치료에 부과하여 네트워크로 연결, CR상에 입력한 order가 나타나도록 하였으며, Planning System인 Pinacle과 PACS상의 지원 data 오류를 해결하여 PACS 상에서도 Planning 영상을 볼 수 있도록 하였다. CR system을 이용하여 L-gram, simulation image, planning image를 병원 내 어느 곳에서나 영상을 조회하고 볼 수 있게 PACS에 통합 구축되어있다. Filmless 환경에서 Dosimetry용 IP를 이용하여 Light/Radition field size 일치, gantry rotation axis의 정확성, collimator rotation axis의 정확성, brachy therapy의 Dwell position check등 QA의 시행이 가능하였다. CR system을 이용하여 방사선 종양학과에서 얻어지는 영상을 PACS에 통합함으로써 작업시간 단축과 그에 따른 불필요한 인력소모의 감소 등으로 인하여 업무효율이 증대되었지만 향후 환자정보에 대한 보안을 필요로 한다.
Hiatal hernia is a very rare disease in the pediatric population. However information from our esophageal atresia postoperative follow-up program has hypotheses; "Hiatal hernia may more frequently occur in postoperative esophageal atresia patients (EA group) than in the general pediatric population (GP group)" and "The tension on the esophagus after esophageal anastomosis may be an important etiologic factor of hiatal hernia in EA group". To prove the first hypotheses, we compared the incidence of hiatal hernia in the GP group with the incidence in the EA group. The Incidence in the GP group was obtained from national statistic data from Statistics Korea and Health Insurance Review and Assessment Service of Korea. The incidence in the EA group was obtained from the medical record and the imaging studies of our esophageal atresia postoperative follow-up program. To prove the second hypothesis, the presumptive risk factors for the development of hiatal hernia in EA group, such as the type of esophageal atresia, degree of esophageal gap, the stage operation and the redo-operation with resection and re-anastomosis of esophagus were analyzed statistically. The total number of patients in the EA group was ninety-nine and there were 5 hiatus hernias. The incidence of EA group (5 %) is significantly higher than incidence of GP group (0.024 %). (p=0.0001) The statistical analysis of the presumptive risk factors for hiatal hernia development in EA group failed to show any evidence of correlation between postoperative esophageal tension and the hiatal hernia. This study shows that the postoperative patients with esophageal atresia have high occurrence of hiatal hernia and should be followed up carefully to detect hiatal hernia.
본 연구는 환자가 느끼는 자신의 주관적 구강보건영향지수(OHIP-14; Oral Health Impact Profile-14)와 주관적 전신건강지수(THI; Todai Health Index)를 사용하여, 구강보건이 전신건강과 삶의 질(QOL; Quality of Life)에 어떤 영향을 미치는지 알아보고 구강질환 예방과 구강보건 향상을 위한 프로그램을 개발에 필요한 기초 자료를 제공하여 지역사회주민의 건강과 삶의 질을 증진하고자 한다. 주관적 구강건강상태에 따른 OHIP를 분석한 결과 자가 인식 구강 건강상태는 전 영역에서 유의한 차이가 있었고, 총 점수가 건강한 편이 4.33으로 유의하게 높았다. 주관적 구강건강상태에 따른 THI를 분석한 결과 자가 인식 구강건강상태는 전 영역에 걸쳐 유의한 차이가 있었으며, 총 점수에서는 매우건강 한 자가 3.83으로 점수가 높았다. 주관적 구강건강상태에 따른 QOL을 분석한 결과 자가 인식 구강건강상태에서는 사회적 영역을 제외한 전 영역에서 유의한 차이가 있었으며, 총 점수는 건강한 편이 3.39로 가장 높았다.
BACKGROUND/OBJECTIVES: Type 2 Diabetes mellitus (T2DM) is a hereditary disease that is also strongly dependent on environmental factors, lifestyles, and dietary habits. This study explored the relationship between lifestyle habits and glycosylated hemoglobin management in T2DM patients to provide empirical outcomes to improve T2DM management and patient health literacy. SUBJECTS/METHODS: This study enrolled 349 diabetic patients with more than 5 care visits to a Diabetes Mellitus care network under the Health Management Plan led by Taiwan Department of Health (DOH). Based on relevant literature, an Outpatient Record Form of Diabetes Mellitus Care was designed and lipid profile tests were conducted for data collection and analysis. RESULTS: When modeling the data, the results showed that the odds for HbA1c > 7.5% in T2DM patients duration over 10 years was 3.785 (P = 0.002) times that in patients with disease duration of fewer than 3 years. The odds of HbA1c > 7.5% in illiterate patients was 3.128 (P = 0.039) times that in patients with senior high school education or above. The odds of HbA1c > 7.5% in patients with other chronic illness was 2.207 (P = 0.019) times that in participants without chronic illness. Among 5 beneficial lifestyle habits, the odds of HbA1c > 7.5% in patients with 2 or 3 good habits were 3.243 (P = 0.003) and 3.424 (P = 0.001) times that in patients with more than 3 good habits, respectively. CONCLUSION: This empirical outcome shows that maintaining a good lifestyle improves T2DM management and patients' knowledge, motivation, and ability to use health information. Patients with longer disease duration, education, or good lifestyle habits had optimal HbA1c management than those in patients who did not. Thus, effective selfmanagement and precaution in daily life and improved health literacy of diabetic patients are necessary to increase the quality of T2DM care.
이 연구의 목적은 고강도로 실시되는 12주간의 웨이트 트레이닝을 통하여 노인 여성들의 신체조성 및 기능적 체력에 어떠한 변화가 있는지를 규명하는 것이다. 연구대상자들은 U경로당에 정기적으로 출입하는 70세 이상 노인 여성 중에 실험의 취지를 알고 자발적 참여의사를 가진 24명을 선정하였다. 선정된 24명을 무선할당 방법을 통해 두 개의 운동집단인 고강도 저항성 운동 집단 (High intensity resistance group, n=8), 저강도 저항성 운동 집단(Low intensity resistance group, n=8) 그리고 비교군 (Control group, n=8)으로 나누었다. 운동집단은 12주간의 고강도 및 저강도 저항성 운동을 실시하였으며, 비교군은 12주간 어떠한 운동에도 참가하지 못하도록 제한하였다. 연구결과, 12주간 실시된 고강도 웨이트 트레이닝은 근력과 민첩성을 유의하게 향상시켰으며, 체지방률과 체지방량은 유의하게 감소시켰다(p<0.05). 평형성 및 심폐능력은 향상되었으나 통계적 유의성은 없었다. 종합해보면 정기적인 고강도 웨이트 트레이닝은 노인 여성들의 체력 개선에 도움을 줄 수 있는 운동이라 사료된다.
In order to increase the clinical value of an identification of patterns according to the eight principles (IPEP) in Korean medicine practice, The research on the Clinical Practice Guideline (CPG) of IPEP should comprehend the situation of clinical usage of IPEP practiced by Korean medicine doctors at first. Google survey form were emailed to Korean Medicine doctors registered in the Association of Korean Medicine on 04/15/2021 and the survey was closed at 04/22/2021. Data of 505 answered cases were analyzed by Frequency analysis, Chi-Square analysis, correlation analysis for understanding differences by groups. Out of 505 respondents, 57.6% have answered that they are using IPEP. It means that 42.4% of KM doctors don't use in the medical practice reversely in spite of fundamental diagnostic theory. The 64.7% respondents of no using IPEP presented their opinion about the theoretical problem that it is difficult to use because the concept of IPEP is ambiguous. And next, the 52.1% of the respondents expressed that there is no objective tools to measure and record the IPEP evidences in actual implementation. And 49.6% of the respondents also suggested that it is hard to trust and use IPEP similar to the previous comment. Even about 50% of the respondents are carrying out diagnosis and treatment using IPEP, it showed that there were several unsolved problems such as lack of understanding and practical tools or objective indicators for diagnosis of IPEP. Through the above results, the concept, usage, measurement requirements with indices and discriminant logic of IPEP were manifested as the main hopes of attending members of Korean medicine in the survey, so the IPEP CPG should make clear about these difficult but necessary assignment in the near future.
본 연구의 목적은 입원환자가 낙상사고 전후에 따라 건강상태 관련 특성의 차이를 확인하기 위함이며, 2016년 1월 1일부터 2020년 12월 31일까지 입원 후 낙상사고가 발생하여 환자안전보고시스템에 보고된 환자 328명의 데이터를 활용한 후향적 연구이다. 연구대상자의 연령은 평균 68.57±14.13세로서 70대가 30.49%로 가장 많았고, 입원 이후 평균 13.86±25.03일이 경과 시에 낙상사건이 발생하였고, 발생 시간대는 야간(22:30- 06:59)이 42.99%로 가장 많았다. 입원 후 낙상 전후에 배변문제(x2=314.0, p<.001), 배뇨문제(x2=284.0, p<.001), 정맥수액요법(x2=85.16, p<.001), 걸음걸이의 허약감(x2=69.77. p<.001), 와상(x2=51.60, p<.001), 의식이 불명하거나 자신의 기능을 과대평가한 경우(x2=17.52, p<.001) 비협조적 태도(x2=220.17, p<.001)에서 통계적으로 유의한 차이가 있었다. 낙상 전후 환자의 건강상태 관련 특성의 차이를 파악하여 환자의 특성에 맞는 적절한 낙상예방 및 개별적인 교육중재활동을 모색할 필요가 있다.

Background: People with special needs tend to require diverse behavioral management in dentistry. They may feel anxious or uncomfortable or may not respond to any communication with the dentists. Patients with medical, physical, or psychological disorders may not cooperate and therefore require sedation (SED) or general anesthesia (GA) to receive dental treatment. Using the healthcare big data in Korea, this study aimed to analyze the trends of SED and GA in special needs patients undergoing dental treatment. It is believed that these data can be used as reference material for hospitals and for preparation of guidelines and related policy decisions of associations or governments for special needs patients in dentistry. Methods: The study used selected health information data provided by the Korean National Health Insurance Service. Patients with a record of use of one of the eight selected drugs used in dental SED between January 2007 and September 2019, those with International Classification of Diseases-10 codes for attention deficit hyperactivity disorder (ADHD), phobia, brain disease, cerebral palsy, epilepsy, genetic disease, autism, mental disorder, mental retardation, and dementia were selected. The insurance claims data were analyzed for age, sex, sedative use, GA, year, and institution. Results: The number of special needs patients who received dental treatment under SED or GA from January 2007 to September 2019 was 116,623. Number of SED cases was 136,018, performed on 69,265 patients, and the number of GA cases was 56,308, implemented on 47,257 patients. In 2007, 3100 special needs patients received dental treatment under SED while in 2018 the number of cases increased 6 times to 18,528 SED cases. In dentistry, ADHD was the most common disability for SED cases while phobia was the most common cause of disability for GA. The male-to-female ratio with respect to SED cases was higher for males (M : F = 64.36% : 35.64%). Conclusion: The application of the SED method and GA for patients with special needs in dentistry is increasing rapidly; thus, preparing guidelines and reinforcing the education and system are necessary.
