BDSS: Blockchain-based Data Sharing Scheme With Fine-grained Access Control And Permission Revocation In Medical Environment

  • Due to the increasing need for data sharing in the age of big data, how to achieve data access control and implement user permission revocation in the blockchain environment becomes an urgent problem. To solve the above problems, we propose a novel blockchain-based data sharing scheme (BDSS) with fine-grained access control and permission revocation in this paper, which regards the medical environment as the application scenario. In this scheme, we separate the public part and private part of the electronic medical record (EMR). Then, we use symmetric searchable encryption (SSE) technology to encrypt these two parts separately, and use attribute-based encryption (ABE) technology to encrypt symmetric keys which used in SSE technology separately. This guarantees better fine-grained access control and makes patients to share data at ease. In addition, we design a mechanism for EMR permission grant and revocation so that hospital can verify attribute set to determine whether to grant and revoke access permission through blockchain, so it is no longer necessary for ciphertext re-encryption and key update. Finally, security analysis, security proof and performance evaluation demonstrate that the proposed scheme is safe and effective in practical applications.

Development of Advanced TB Case Classification Model Using NHI Claims Data (국민건강보험 청구자료 기반의 결핵환자 분류 고도화 모형 개발)

  • The aim of this study was to enhance the NHI claims data-based tuberculosis classification rule of KCDC(Korea centers for disease control & prevention) for an effective TB surveillance system. 8,118 cases, 10% samples of 81,199 TB cases from NHI claims data during 2009, were subject to the Medical Record Survey about whether they are real TB patients. The final study population was 7,132 cases whose medical records were surveyed. The decision tree model was evaluated as the most superior TB patients detection model. This model required the main independent variables of age, the number of anti-tuberculosis drugs, types of medical institution, tuberculosis tests, prescription days, types of TB. This model had sensitivity of 90.6%, PPV of 96.1%, and correct classification rate of 93.8%, which was better than KCDC's TB detection model with two or more NHI claims for TB and TB drugs(sensitivity of 82.6%, PPV of 95%, and correct classification rate of 80%).

Design for a Medical Devices Safety Information Reporting System for the Hospitals (의료기기 안전성 정보 원내 보고 시스템 설계)

  • In this study, medical devices safety information reporting system was designed to manage medical devices for the efficient management of in-hospital adverse events. The current management of medical device adverse event reporting regulations and the legal status of the system and procedures for identifying the system were reviewed. MSF/CD(Microsoft Solution Framework/Component Design) was applied to the system design. Through this study, we can understand medical devices management including the notice provisions of the Ministry of Food and Drug Safety for medical devices safety information reporting. We also expect this study will help to improve patient safety and the effective management of medical equipment, and contribute to activating medical devices safety information reporting.

  • The strength of our country is based on high-speed networks to build a nationwide health information network to improve the quality of health care is needed. However, nature of the medical services in the hospital medical record form, so that all departments characterize each medical department, a common format that can be used for all disease is a very difficult challenge. This study is one of the ways to improve this priority issue on health information sharing medical information system(PACS/HIS) information linkage between the problem of information, compatibility and security issues derived. The problem is derived from the requirements of theorem 3 to 6 items. This is effective in order to improve sharing of medical information for the purpose of designing the software system. Sharing of medical information software system design are derived from stakeholders, requirements analysis, architecture design, software framework, configuration, architecture evaluation process. This study is based on the medical information standardization environmental diagnosis of a medical information system design. Software design philosophy is based on a new framework for deriving the function and mechanism made up.

Waveform Biosignal Interface based on International Standard MEER (MFER 표준을 적용한 생체신호정보 공유시스템 개발)

  • Recently, many of hospitals have hurried to computerize the resulting data from medical devices, in order to introduce Electric Medical Record(EMR). In terms of the linkage between medical devices and hospital information systems, however, many difficulties have arisen due to some reasons such as the variety of prescription input, the format difference of the resulting data sheet, and the interface difference between medical devices from different companies. To solve these problems, many researches on standardization of the resulting data of medical devices have been performed. In this study, the linkage between hospital information systems and resulting datum in Electrocardiogram(ECG) generating biosignal waveform was tested by applying Medical waveform Format Encoding Rules(MFER) Version 1.02, which has more advantages than existing global standard. MFER viewer, in addition, was made to display the resulting data on a screen. The MFER viewer was tested and compared to the existing Scalable Vector Graphics (SVG) Viewer. The results showed that this method is more effective in the interface the data storage and application, because of simplicity and easiness in data applications. And the results show that the MFER is convenience and effective for physician. It is considered that the role of MFER as the interface in biosignal waveform including Electrocardiogram medical devices would expand in the near future.

Exploring the Ways to Apply Korean Traditional Medical Practices to the International Classification of Health Interventions through Focus Group Discussion (초점집단토론을 통한 국제의료행위분류의 한의의료행위 적용 방안 연구)

  • Objectives: This study was aimed to present the experts' opinions for the successful application of Korean traditional medical practices (KTMPs) to the International Classification of Health Intervention (ICHI). Methods: Two doctors of Korean Medicine and two health information managers who had tried coding 131 KTMPs using ICHI participated in the focus group discussion. The remarks from the discussion were summarized according to the thematic analysis method. Results: The participants expected ICHI to be mainly used for statistics when applied to TKMPs. It can be used for payment systems as well, but it was expected that additional work would be required. They thought the current version of the ICHI did not sufficiently reflect the KMTPs of the real world, and even the interventions already included in the ICHI were not explained enough in the system. They thought it would not be easy to explain more KTMPs within the current structure of the ICHI, but they also said it seemed possible. In the process, rather than adding entirely new stem codes, it would be better to generate new combinations of the existing codes, to suppose subdivided codes, and to utilize the include terms or extension codes. Conclusions: For the successful introduction of ICHI, clarifying the definition of each intervention of KTMPs is a top priority. In addition, it is necessary to continue the matching work of ICHI - KMPTs and also required to make this effort together with the field of traditional medicine and complementary medicine worldwide.

New data on Limoniinae and Limnophilinae crane flies (Diptera: Limoniidae) of Korea

  • This study is based on crane fly specimens collected from 1936-2019 and are in collections maintained at the United States National Museum, Smithsonian Institution, Washington DC, USA; the Snow Entomological Museum, University of Kansas, Lawrence, KS, USA; the Hungarian Natural History Museum in Budapest, Hungary, and the National Institute of Biological Resources, Incheon, South Korea. The genus Dicranophragma Osten Sacken, 1860 with two species D. (Brachylimnophila) transitorium (Alexander, 1941) and D. (Dicranophragma) melaleucum melaleucum (Alexander, 1933), is a new record for the Korean Peninsula. New findings of Dicranomyia (Erostrata) submelas Kato et al., 2018, Dicranoptycha venosa Alexander, 1924a, Austrolimnophila (Archilimnophila) subunicoides(Alexander, 1950b), A. (A.) unica (Osten Sacken, 1869), A. (Austrolimnophila) asiatica (Alexander, 1925), Conosia irrorata (Wiedemann, 1828), Eloeophila persalsa (Alexander, 1940), E. serenensis (Alexander, 1940), E. subaprilina (Alexander, 1919), E. ussuriana ussuriana (Alexander, 1933), E. yezoensis (Alexander, 1924b), Paradelphomyia chosenica Alexander, 1950b, and P. macracantha Alexander, 1957 are discussed. General information on genera and subgenera morphological characters, redescriptions of species based on Korean specimens, illustrations of both sexes, elevation range, period of activity, habitat information, general distribution, and a distribution map for the Korean Peninsula (including North Korea) are presented for each species.

A Development Study of Common Clinical Document Forms for Traditional Korean Medicine Information Standardization (한의 정보 표준화를 위한 공통 임상 기록 서식 개발 연구)

  • Objectives: The clinical document forms, a format for collecting clinical data, is the most fundamental object of standardization. Doctors must have a mutual understanding of the clinical chart. Methods: Clinical document forms were developed by investigating existing conditions in hospitals and conducting demand surveys, doing literature research, and seeking expert advice for the improvement of version 1.0. In addition, an organization of a network of 19 Oriental medical doctors and nurses, 190 patients, and users of collected and assessed data was formed to come up with version 2.0. Results: The overall format was divided into different portions that the patient, nurse, and doctor must fill out, respectively. The patient's section consists of demographic data, lifestyle details, history, and symptoms. The data to be supplied by the nurse include the patient's vital signs and anthropometric parameters. As for the doctors, they are to supply data regarding the patient's palpitation, the detailed symptoms of the patient's head, ophthalmological and otorhinolaryngological symptoms (mouth), respiration, circulatory organ and chest conditions, digestive-organ conditions (thirst), neuropsychiatric conditions, reproductive system, musculoskeletal system, skin (depilation), etc. Conclusions: Common clinical chart development is the prior question to Traditional Korean Medicine standardization. A web-based clinical document format should be developed to support diagnosis and treatment, and furthermore EMR (electronic medical record system) and EHR (electronic health record) developed. Clinical information could be shared through a network of medical institutions and be useful Traditional Korean Medicine for evidence-based medicine.

Development of a Management Tool of CCD/CCR-centric Standard Clinical Document (CCD/CCR 중심의 표준진료문서 관리 도구의 개발)

  • XML-based standards such as CCD(Continuity of Care Document) and CCR(Continuity of Care Record) have been developed for representation, integration, and exchange of personal health record(PHR), and various of researches on PHR based on the standards have been conducted. These researches have developed and used CCD/CCR parsers each with their own different ways, but it can be hard to develop and update the parsers because of the structural complexity of the standards. Moreover, inter-exchange between CCD and CCR documents in the PHR-related medical information systems should be possible for the interoperability of the systems. Therefore, we proposed a designing method to develop the tools treating XML-based CCD/CCR documents. And we implemented CCD/CCR parser based on the proposed method and developed a converter from CCD to CCR using the parsers. To confirm the usefulness of the developed tool, we performed an experiment of creating CCD documents using the personal health data gathered from chronically ill patients in Kyungpook National University Hospital and of converting from the CCD documents to CCR documents.

  • Through the convergence of medical services and the IT technique, the patient's personal health information computerization has been rapidly spread with propagation of electronic medical record(EHR). In addition, by entering u-health, the demand of the secondary use for public health, medical research, and medical service using electronic patient health care records are increasing. The personal health information secondary uses for the development of academic medical area and service, are very good thing. But, carelessly to use personal health information, the patient privacy would be damaged. However, there are not yet systematic studies about secondary use of personal health information. Therefore, in this paper, we analyze the difference of the internal and external bill for personal medical data secondary use and propose the direction of the medical service development and preservation of the individual's privacy.
