Cognitive Discrepancies of the Mutual Development Strategies of Western and Oriental Medicine in Korea - A Comparison between Western and Oriental Medical Practitioners and Health Professionals - (양.한방 상호보완 방안에 관한 인식도 연구 - 의사, 한의사 및 보건의료전문가간 비교를 중심으로 -)

  • This study is conducted to compare how the Western.Oriental medical practitioners and health professionals perceive reciprocal development strategies respectively of Western.Oriental Medicine in Korea. A total of 3,273 persons were questioned by direct distribution, e-mail, and mail with a self-developed questionnaire. Of those questioned, 362 responded (the response rate of 11.1%), and of them n persons (Western medical practitioners 206, Oriental medical practitioners 90, and health professionals 64) were analyzed with SPSS for Windows. The results were as follows: The need for legislation on the mutual development system for Western.Oriental Medicine was recognized by 66.9% (218 persons) of respondents. Among them, largest group who agreed with this need was the health professionals. Western medical practitioners chose the "the difference of approach methods on the diseases between Western.Oriental Medicine" as their response, and Oriental medical practitioners & health professionals selected "the indifference or bias of Western medical practitioners"as the reasons for the inactivity in developing a mutual system of Western.Oriental Medicine. Therefore, Western medical practitioners and health professionals selected the category of "the reformation of educational system", while Oriental medical practitioners selected the category of "the activation of joint research on Western.Oriental medical care" as the most important condition for setting a precedent. Also, Western medical practitioners preferred "cooperative health care system for Western medical care supported by Oriental medical care", but Oriental medical practitioners and health professionals preferred “cooperative health care system of Western.Oriental medicine on equal terms" In conclusion, Western '||'&'||' Oriental medical practitioners have to make every effort to close the gap between differing views through mutual understanding and respect if joint research of Western.Oriental medical care is to become a reality. The government should continuously enforce the health policy on development of a legal and systematic infrastructure for mutual development strategy of Western.Oriental Medicine in Korea.strategy of Western.Oriental Medicine in Korea.

Physical Functions of Industrial Workers with Chronic Low Back Pain and Changes in Health-related Quality of Life according to Virtual Reality Exercise Program (가상현실 운동프로그램에 따른 산업체 만성요통환자의 신체기능과 건강관련 삶의 질의 변화)

  • This study performed a virtual reality exercise program for self-back pain control of industrial workers with chronic back pain and analysed their physical function and health-related quality of life relating to industrial chronic back pain. The subjects of the study were thirty chronic back pain patients working for K company and were divided into Nintendo Wii Sports Group made of 15 subjects and Physical Therapy Group made of 15. As variables for physical functions, back strength, flexibility, balance and pain were measured and for the health-related quality of life, SF-36 of each group was measured before and after the exercise program. To compare differences before and after the program, a paired t-test was used and to compare differences between both groups, an independent samples t-test was used. As a result of the test, it was discovered that balance and pain of the both groups were improved statistically significantly. Physical functions of the Nintendo Wii Sports Group were improved statistically significantly in comparison with those of the Physical Therapy Group. The health-related quality of life was improved statistically significantly only for the Nintendo Wii Sports Group. In conclusion, it is suggested that as the virtual reality exercise program had a positive influence on chronic back pain of industrial workers, it should be considered as a home exercise program for back pain control.

Awareness of Reality and Tradition in Oh Yun's Theory of Arts during His Final Period(1984~86) - Review on the Text of "Expansion of Artistic Imagination and World" (오윤의 말기(1984~86) 예술론에서의 현실과 전통 인식 - "미술적 상상력과 세계의 확대"에 대한 텍스트 검토)

  • An artist, Oh Yun(1946~86)'s theory of people's art during his final period is summed up in his essay 'Expansion of Artistic Imagination and World' (1985). Emphasizing the mystic and traditional characteristics of Oh Yun's artistic oeuvre during his final period, some critics focus on Oh Yun's experience of medical treatment and shamanistic custom at Jin Do island, and his belief in Jeung San Do, the dao of Jeung-san, the Ruler of the Universe. However, they forget the practical intention and implication of his theory of art during his final period, which aimed to overcome the contradiction of revelation itself. Oh Yun's essay criticized the loss of artistic imagination and the ignorance of traditional culture that resulted from the elevation of science to a religion, and insisted that the stereotyped idealism, scientism and elitism in art should be overcome in order to recover the full reality in realism and to continue traditional cultures. The essay is comprised of 18 paragraphs. Oh Yun criticized monochromatic art, conceptual art, hyper-realistic art, objet d'art, and neo-dadaist art, saying that they were simply mechanical forms of modern art derived from scientism and a fetishistic lens culture. In addition, he criticized naturalism in art, which had continued as a tendency in the development of western art, for the same reason. He pointed out that even the world of realism had been diminished by elite stereotypes and diagrams. He declared the need to overcome the imitation of shells or stereotyped propaganda, and recover full realism, which seems to have started with a reflective examination of current problems in 'Reality and Utterance', in which he participated. Especially, he thought that universality and the extension of full realism could be achieved by building on the views of traditional cultures, which is meaningful. This logic is same as the theory of epic theatre that Bertolt Brecht(1898~1956) has developed under the ancient Greek masque and Pieter Bruegel the Elder(1525~69)'s story-like picture style. The universality of realism and the extension of acquisition to include incantation art, rather than move toward incantation art, is what Oh Yun intended to propose in 'Artistic Imagination'. This attitude is same as Bertoh Brecht's aesthetic viewpoint in the 1930s. But regrettably, Oh Yun's style wording, which seems covert and far-sighted, is often misunderstood as 'mysticism'. In the flow of people's art in the 1980s, Oh Yun was a traditionalist in a narrow sense, and an realist in a broad sense. However, his critical mind, which comprehends tradition and reality, was attempting to expand universality and extend full realism, and this attempt found many sympathizers and had an influence on the next generation of people's artists, such as "Levee" which is field-centered, to which we should pay attention. This means that while their works thought about 'tradition', we should be careful not to connect them with 'aesthetic conservatism' or 'classical art'. This is the why the meaning of Oh Yun's theory of art during his final period should be closely examined again.

Study on Special Points in Chimgugyeongheombang (『침구경험방(鍼灸經驗方)』 별혈(別穴)에 대한 고찰)

  • Objectives : Chimgugyeongheombang's Byulhyul, or special points, is a list of extra points, and its acupoint types and positions are different from other books. Its source, however, is kept behind the vale. This paper aims to reveal Byulhyul's contents and source. Methods : Qixiaoliangfang, Jinjiudacheng, and Donguibogam's Gihyul were studied to determine the source of Chimgugyeongheombang's Byulhyul. Results & Conclusions : Chimgugyeongheombang lists a total of 59 special points. It cites 22 meridian points from Qixiaoliangfang, 26 points from Jinjiudacheng, and 36 points from Donguibogam. The Dictionary of Acupuncture records the source of 6 meridian points that aren't found in conventional books, namely the Soongol, Backro, Yeose, Eumdo, Naetaechoong, and Gabgeun points, as Zhenjiujicheng. However, Zhenjiujicheng, in reality, is a book put together with excerpts from three books: Donguibogam, Chimgugyeongheombang, and Leijingtuyi. This means that it's Chimgugyeongheombang that is the source of Zhenjiujicheng. Moreover, all of the points save for Soongol and Eumdo were also used in the acupunctural treatment of 9 diseases including epilepsy. In other words, it can be safely assumed that Heo Im came up with and utilized new special points based on his personal clinical experiences, and this is significant in that he expanded the boundary of the conventional study of acupuncture through his independent experiences and studies.

Effects of Various Sensory Stimulation on Surface Area and Velocity of Center of Pressure During One Leg Standing in Healthy Adults (건강인의 한발 서기 시 다양한 감각 자극이 압력 중심점 이동 면적과 속도에 미치는 영향)

  • This study aimed to evaluate the surface area and velocity of center of pressure (COP) during one leg standing by stimulating the sensory system in normal adults. Thirty subjects were enrolled in this study. Subjects were asked to stand on one leg during testing conditions. Testing conditions included 6 different sensory stimulations as follows: eyes opened, eyes closed, eyes opened with vibrator, eyes opened with head-mounted display (HMD), eyes opened with vibrator and HMD, and eyes closed with vibrator. During each testing condition, the surface area and velocity of center of pressure were measured. There were significant differences in the mean surface area and the mean velocity of COP between the "eyes opened" condition and the other five testing conditions (p<.05). However, in the comparison between the "eyes closed" and "eyes opened with HMD" conditions, there were no significant differences in the tested parameters. This study shows that closing eyes or keeping eyes opened while using HMD to experience virtual reality has the same effect on one leg standing balance. This finding should be considered in the evaluation or intervention of balance, especially one leg standing balance and balance while standing with a small base of support.

Hereditary Testimonies and Medico-historical Facts of the Eminent Brother Doctors Young Hur and Eon Hur (허영(許郢), 허언(許鄢) 형제명의(兄弟名醫)의 가전행적(家傳行蹟)과 의약사실(醫藥事實))

  • The eminent brother doctors Cho-Gaek Hur (Young Hur) and Cho-Sam Hur (Eon Hur) represent the medical tradition of the southeastern region of Mt. Jirisan incluiding Sancheong. This study investigated and examined their herediatary testimonies and medico-historical facts described in the literature tale, genealogy and family collection of works. At an early age after their family run away to the southern region from the harm of political struggle, they gave up taking the highest-level state examination to recruit ranking officials and set their heart on learning medicine. Their choice of medicine was the second best plan for the survival of their family and descendents from the influence of their ancestors who suffered from the party strife during the late Joseon Dynasty. The case is a good example of the deliberate action of the nobility intellectuals since the late Goryeo and early Joseon dynasties to who made attempts to have the minimum elegance and cause while securing livelihood after they have been collapsed politically. The tale of Cho-Gaek and Cho-Sam is a typical story of eminent doctors which combined reality and fiction about the persons who had outstanding medical skills. In the tale, the elder brother was good at herbal prescriptions and the younger brother was good at acupuncture, contrasting the treatment of administrative internal medicine and insertive acupuncture treatment. It shows that the prepositions for the choice of the treatment method for the public during that period were safety and fast efficacy. In their old age, they wrote a book titled Jinyangsinbang. Another Jinwoosinbang is estimated as a different kind of copy, which was a medical writing from experience that derived from the same original book as Jinyangsinbang. written by Cho-Gaek Hur and it is a valuable medical material that possesses the distinct features of the local medicine during the late Joseon Dynasty. Most of the contents are variations in application using the basis of Dongeuibogam, forming the roots of the traditional Korean medicine in Sancheong region together with the medical activities of the Hur brothers.

  • With the recent advance of Web and its associated technologies, information sharing on distribute computing environments has gained a great amount of attention from many researchers in many application areas, such as medicine, engineering, and business. One basic requirement of distributed medical consultation systems is that geographically dispersed, disparate participants are allowed to exchange information readily with each other. Such software also needs to be supported on a broad range of computer platforms to increase the software's accessibility. In this paper, the development of world-wide-web based medical consultation system or radiology imaging is addressed to provide the platform independence and great accessibility. The system supports sharing of 3-dimensional objects. We use VRML (Virtual Reality Modeling Language), which is the de-facto standard in 3-D modeling on the Web. 3-D objects are reconstructed from CT or MRI volume data using a VRML format, which can be viewed and manipulated easily in Web-browsers with a VRML plug-in. A Marching cubes method is used in the transformation of scanned volume data set to polygonal surfaces of VRML. A decimation algorithm is adopted to reduce the number of meshes in the resulting VRML file. 3-D volume data are often very large-sized, and hence loading the data on PC level computers requires a significant reduction of the size of the data, while minimizing the loss of the original shape information. This is also important to decrease network delays. A prototype system has been implemented (http://netopia.snu.ac.kr/-cyber/). and several sessions of experiments are carried out.

  • Haptics and virtual reality are rapidly growing technologies in medical fields. Physicians and nurses can benefit from medical simulation via training and acquire surgical and clinical techniques. In this paper, the research on needle insertion force of biological tissue for haptic based intravenous injection simulator was carried out. We built the setup for needle insertion (intravenous injection) experiments and performed the experiments on live pigs. The force responses against needle insertion were measured using the experimental setup. In addition, the modeling of needle insertion force was carried out with the experimental results and numerical models via nonlinear least-squares method. The results presented in this paper indicate that the developed models can be applied not only to estimate the force feedback during intravenous injection procedure but also to improve the overall training quality of the medical simulator.

  • Objectives : The purpose of this paper is to examine Zhang Zihe's reinforcement method based on the theory of keeping balance(以平爲期). Methods : Based on his arguments in the Rumenshiqin and formula compositions, Zhang Zihe's reinforcement method was examined. Results & Conclusions : 1. Despite preconceptions that Zhang Zihe must have been critical of reinforcement methods due to his alliance to the offensive purgative group, in reality his criticism was not aimed at reinforcement methods entirely, but rather at warming and tonifying. 2. Zhang Zihe's criticism of warming and tonifying was based on his research of the Neijing, in which he realized the pathogenic effects of fire-heat and the side effects of warming and tonifying in clinical practice. This is a common conception among the Four Great Doctors of the Jin-Yuan period. 3. Zhang Zihe focused on eliminating pathogenic qi so that the body's qi would circulate smoothly to slowly restore the source qi. The theory of keeping balance was emphasized to achieve this goal. 4. Based on the theory of keeping balance, Zhang Zihe used various medicinals with different properties and flavors to allow for the medicinals in each of the sovereign, minister, assistant and courier positions to co-operate with each other, while using food reinforcement to restore the source qi.

Experience of Patients with Mucopolysaccharide Receiving Enzyme Replacement Therapy (효소보충치료(ERT)를 받고 있는 뮤코다당증 환자의 경험)

  • The purpose of this study was to explore the meaning of the experience of Mucopolysaccharidosis patients receiving Enzyme Replacement Therapy(ERT). A qualitative research design was adopted. The participants were 7 patients diagnosed with MPS who is receiving ERT for several years. Data were collected through in-depth interviews from 1 July 2018 to 22 July 2018 for seven adult patients with MPS receiving ERT. Individual interviews were recorded, and transcribed data were analyzed using the inductive method of content analysis. The final 4 categories and 15 subcategories were identified. The 4 categories of the experience of Mucopolysaccharidosis patients receiving ERT were "Physical and psychological changes through ERT", "Continuous tunnel called ERT", "Accepting the reality" and "Cautious hope". This study provides deep insight into the experience of Mucopolysaccharidosis patients receiving ERT. Medical staff including nurses and related organizations should concern their distress during ERT as well as physical symptoms.
