A Study on the Actual Case of Medical Examination in the Joseon Dynasty (조선시대 의과시험의 실제 사례에 대한 고찰)

  • The appointment of medical officials during the Joseon Dynasty was conducted through medical testing. To date, most of the research done on these tests has focused on the testing system and its formal qualities without any study of actual test cases. This paper considers the significance of the medical examination format and contents through a test paper contained in the book "Joseonuihaksageupjilbyeongsa (朝鮮醫學史及疾病史)" by MikiSakae (三木榮). The literature confirms that the medical exam paper is the same format as "Gangji (講紙)," and that the test format called "Gangseo (講書)" is a very high-level test. Therefore, analysis of the case suggests that the format of the medical examination reflects the demand for high-quality medical learning for applicants. This consideration is meaningful because existing medical test papers are very rare.

A Study on the Way of Organizing Contents of State Sponsored Medical Text in Ancient China (중국 주요 국가간행의학서의 편제구성과 질병분류인식에 대한 소고)

  • This paper is focused on the 'contents' of database level medical texts sponsored by the Chinese government. The premise of the study is that the contents of state-sponsored medical texts would show how medical policy makers and practitioners approached the body and diseases of the time, and by association the medical text would reveal the policy associated with state medical education and distribution of medical resources associated with the practitioners' approaches. This paper analyzes the contents of four representative state-sponsored medical texts: Cao's Treatise on the Origins and Symptoms of Various Diseases (巢氏諸病源候論, 610, Sui China); Great Peace and Sagely Benevolence Formulas (太平聖惠方, 996, Song China); Complete Record of Sagely Benevolence (聖濟總錄, 1117, Song China); Formulas for Universal Relief (普濟方, 1406, Ming China).

Comparative study on the Satisfaction difference of Korean and Western Medical Services (한양방 의료서비스 만족도 비교연구)

  • Objective : In South Korea, there are two kinds of medical systems that have Western medical institution and Korean medical institution. Patients who use medical services have different satisfaction in this environment. This study explores the satisfaction difference of korean/western medical services. Method : The data for this study were collected through a self-administered survey to 680 subject from patients who use medical services. frequency analysis, factor analysis, t-test, ANOVA were used for the statistical analysis. Results : The result of factor analysis, three factors were extracted. That was Information, Environment, Human services. Overall, the satisfaction scores of Korean medical service was higher than western medical service. Especially the human service satisfaction of the korean clinic was higher than western clinic. Conclusion : The satisfaction score of korean/western medical services is different. It is necessary to develop the medical management strategy and medical policy including korean medical system.

  • Signet ring cell carcinoma is a rare type of mucinous adenocarcinoma of the uterine cervix. To the best of our knowledge, there is no report on cytologlc findings of primary signet ring cell carcinoma of the uterine cervix in the literature. Recently, we experienced two cases of signet ring cell carcinoma of the uterine cervix. The finding of characteristic signet ring cells on cervicovaginal smear led to the diagnosis of signet ring cell carcinoma. However, primary signet ring cell carcinoma could not be cytologically distinguished from more common metastatic tumor. Therefore, diagnosis rests upon the recognition of signet ring cells and the absence of signet ring cell carcinoma elsewhere.

  • The intradural lumbar disc herniation which was first described by Dandy in 1942 is unusual. The pathogenesis is obscure and preoperative diagnosis is not easy. The patient usually have more severe neurologic deficits than those found in the much more common extradural disc herniations. We experienced two cases of the intradural lumbar disc herniation who had previous disc operations and the symptoms and neurologic signs were improved after removal of the intradural sequestration of disc fragments.

Analysis of the curriculum of medical humanities and social medicine in Colleges of Korean Medicine based on KAS2021

  • Objectives: The purpose of this study was to examine the curriculum of the medical humanities and social medicine at 11 Colleges of Korean Medicine (CKM) based on the Korean Medicine Education Accreditation Standards 2021 (KAS2021) and suggest ways to improve the quality of Korean Medical education in the future. Methods: The curricula for each grade were collected from the websites of 11 CKM. Based on KAS2021, medical humanities and social medicine courses were extracted and compiled. Courses offered, frequency of course offerings, time of offerings, credits, and course hours were investigated. Results: Courses in languages and literature were most frequently offered in CKM. Most medical humanities and social medicine courses were offered in the pre-medical program and the fourth year of the medical program and were conducted as individual courses. Developing a curriculum that integrates conceptual definitions of the medical humanities and social medicine with basic and clinical studies is necessary. Conclusion: Eleven CKM should reorganize and operate their medical humanities and social medicine curricula based on each college's circumstances. This will allow each college to improve the quality of its educational offerings, creating a foundation for fostering excellent korean medicine doctors with professional medical skills and communication skills.

Bicluster and Pathway Enrichment Analysis of HCV-induced Cirrhosis and Hepatocellular Carcinoma

  • Background: Hepatocellular carcinoma (HCC) is the sixth most common cancer worldwide and the most common form of liver cancer. However, while it is associated frequently with hepatitis C virus (HCV) there is only an elementary understanding of its molecular pathogenesis. Methods: To gain insight into the molecular mechanisms of HCV-induced hepatocarcinogenesis, we performed microarray analysis on 75 surgical liver samples from 48 HCV-infected patients. Results: There were 395 differentially expressed geness between cirrhotic samples and HCC samples. Of these, 125 genes were up-regulated and 270 genes were down-regulated. We performed pathway enrichment analysis and screened as described previously. Conclusions: The differentially expressed genes might be involved in hepatocarcinogenesis through upregulating the pathways of ECM-receptor interaction, focal adhesion, cell adhesion molecules and other cancer-related pathways, and downregulating the pathways of "complement and coagulation cascades". We hope our results could aid in seeking of therapeutic targets for HCV-induced hepatocellular carcinoma.

Comparison of Inhibitory Effects of 17-AAG Nanoparticles and Free 17-AAG on HSP90 Gene Expression in Breast Cancer

  • Background: HSP90 may be overexpressed in cancer cells which are greatly dependent on Hsp90 function. Geldanamycin derivative 17 allylamino-17-demethoxygeldanamycin (17-AAG) inhibits the function and expression of HSP90. 17-AAG has poor water-solubility which is a potential problem for clinical practice. In this study for improving the stability and solubility of molecules in drug delivery systems we used a ${\beta}$-cyclodextrin-17AAG complex. Materials and Methods: To assess cytotoxic effects of ${\beta}$-cyclodextrin-17AAG complexes and free 17AAG, colorimetric cell viability (MTT) assays were performed. Cells were treated with equal concentrations of ${\beta}$-cyclodextrin- 17AAG complex and free 17AAG and Hsp90 gene expression levels in the two groups was compared by real-time PCR. Results: MTT assay confirmed that ${\beta}$-cyclodextrin- 17AAG complex enhanced 17AAG cytotoxicity and drug delivery in T47D breast cancer cells. The level of Hsp90 gene expression in cells treated with ${\beta}$-cyclodextrin- 17AAG complex was lower than that of cells treated with free 17AAG (P=0.001). Conclusions: The results demonstrated that ${\beta}$-cyclodextrin- 17AAG complexes are more effective than free 17AAG in down-regulating HSP90 expression due to enhanced ${\beta}$-cyclodextrin-17AAG uptake by cells. Therefore, ${\beta}$-cyclodextrin could be superior carrier for this kind of hydrophobic agent.

Comparative Study of the National Policies for Korean Oriental Medicine and Traditional Chinese Medicine (한의학과 중의학에 대한 국가정책 비교연구)

  • The structure of medical profession is composed of multiple relations among state, patients, and medical professions. There are conflicts between the nation and medical professions because the nation controls the monopoly of medical professions through medical policies. Patients make relations with medical profession as medical consumers. And medical professions compete each other in order to gain the control of the medical market. This paper attempts to review the dynamic relations between the nation and medical professions. The medical professions and the nation are in conflict about the control of the autonomy of medical professions. The medical professions want to exercise the monopoly rights in their own area and, on the other had, the nation wants to prevent problems that might result from the monopoly by regulations and to have the control over the national operation. Given this, the common view of medical sociology is that the nation and the medical professions are in constant conflict. The arguments that the present medical sociology has on the relationship between the nation and medical professions can be summarized like these: first, the nation is the authenticator of medical system; second, the nation is a medical provider and consumer; and, third, the nation is a mediator of regulations and conflicts. Based on the above mentioned relations between the nation and medical professions, this paper attempts to see how the nation, which is one component of the medical structure, make influences on Korean Oriental doctors and Traditional Chinese doctors. So as to do this, the changes in medical policies and promoting policies for Korean Oriental medicine and Traditional Chinese medicine are analyzed. Finally, the differences in national policies of Korean Oriental medicine and Traditional Chinese medicine are compared.

An Open Medical Platform to Share Source Code and Various Pre-Trained Weights for Models to Use in Deep Learning Research

  • Deep learning-based applications have great potential to enhance the quality of medical services. The power of deep learning depends on open databases and innovation. Radiologists can act as important mediators between deep learning and medicine by simultaneously playing pioneering and gatekeeping roles. The application of deep learning technology in medicine is sometimes restricted by ethical or legal issues, including patient privacy and confidentiality, data ownership, and limitations in patient agreement. In this paper, we present an open platform, MI2RLNet, for sharing source code and various pre-trained weights for models to use in downstream tasks, including education, application, and transfer learning, to encourage deep learning research in radiology. In addition, we describe how to use this open platform in the GitHub environment. Our source code and models may contribute to further deep learning research in radiology, which may facilitate applications in medicine and healthcare, especially in medical imaging, in the near future.
