Design and Implementation of a Personal Health Record Platform Based on Patient-consent Blockchain Technology

  • In the 4th Industrial Revolution, the healthcare industry is undergoing a paradigm shift from post-care and management systems based on diagnosis and treatment to disease prevention and management based on personal precision medicine. To optimize medical services for individual patients, an open ecosystem for the healthcare industry that allows the exchange and utilization of personal health records (PHRs) is required. However, under the current system of hospital-centered data management, it is difficult to implement the linking and sharing of PHRs in practice. To address this problem, in this study, we present the design and implementation of a patient-centered PHR platform using blockchain technology. This platform achieved transparency and reliability in information management by eliminating the risk of leakage and tampering/altering personal information, which could occur when using a PHR. In addition, the patient-consent system was applied to a PHR; thus, the patient acted as the user with ownership. The proposed blockchain-based PHR platform enables the integration of personal medical information with scattered distribution across multiple hospitals, and allows patients to freely use their health records in their daily lives and emergencies. The proposed platform is expected to serve as a stepping stone for patient-centered healthcare data management and utilization.

  • In recent years, the developments of medical technology and emergency medical services have been changed to home from the hospital. In this regard, the researches for the prevention or early diagnosis have become actively. In particular, bio-signal monitoring is applied to a variety of u-healthcare application services. The proposed system in this paper is to provide a security technology to protect the medical information measured from the various sensors. Especially, bio-signal information is privacy-sensitive personal information that must be protected. We applied a two-dimensional code technology, QR code, for the protection and management. In the client side, it can analyze the QR code and confirm the results on devices. Finally, with this proposed platform, we show the results of application service to verify the creation and distribution of integrated image file between the bio-signal and medical image information.

  • As the ARRA enacted recently in the United States, the interest in EHR systems have been increased in the field of medical industry. The passage of the ARRA presents a program that provides incentives to office-based physicians and hospitals adapting the EHR systems to guarantee interoperability with various medical standards. Thanks to the incentive program, a great number of EHR systems have been developed and lots of office-based physicians and hospitals have adapted the EHR systems certified by CCHIT. Keeping pace with the rapid changes in the market of healthcare, some enterprises try to push in to the United States healthcare market based on the experience acquired by developing EHR systems for hospitals in Korea. However, the developed system must be customized because of the different medical environment between Korea and the United States. In this paper, therefore, we design and develop an integrated EHR platform to guarantee the interoperability between different medical information systems based on medical standard technologies. In the developed platform, an integrated system has been composed by integrating various basic techniques such as data transmission standards and its methods, medical standard terminologies and its usage, and knowledge management for medical decision-making support. Moreover, medical data can be processed electronically by adapting an HL7 interface engine and the terminologies for exchanging medical information and the standardization of medical information. We develop SeniCare, an EHR system for supporting ambulatory care of the office-based physicians, based on the platform, and we verify the usability of the platform by confirming whether SeniCare satisfies the criteria of "meaningful use" issued by CMS or not.

  • Recently, the healthcare field has undergone rapid changes owing to the accumulation of health big data and the development of machine learning. Data mining research in the field of healthcare has different characteristics from those of other data analyses, such as the structural complexity of the medical data, requirement for medical expertise, and security of personal medical information. Various methods have been implemented to address these issues, including the machine learning model and cloud platform. However, the machine learning model presents the problem of opaque result interpretation, and the cloud platform requires more in-depth research on security and efficiency. To address these issues, this paper presents a recent technology for Internet-of-Things-based (IoT-based) health big data processing. We present a cloud-based IoT health platform and health big data processing technology that reduces the medical data management costs and enhances safety. We also present a data mining technology for health-risk prediction, which is the core of healthcare. Finally, we propose a study using explainable artificial intelligence that enhances the reliability and transparency of the decision-making system, which is called the black box model owing to its lack of transparency.

  • COVID-19 pandemic outbreak increased the use of Internet of Medical Things (IoMT), but the existing IoMT solutions are not free from attacks. This paper proposes a secure and resilient framework for IoMT, it computes the risk using Risk Impact Parameters (RIP) and Risk is also calculated based upon the Threat Events in the Internet of Medical Things (IoMT). UICC (Universal Integrated Circuit Card) and TPM (Trusted Platform Module) are used to ensure security in IoMT. PILAR Risk Management Tool is used to perform qualitative and quantitative risk analysis. It is designed to support the risk management process along long periods, providing incremental analysis as the safeguards improve.

A Secure and Efficient E-Medical Record System via Searchable Encryption in Public Platform

  • This paper mainly presents a secure and efficient e-Medical Record System via searchable encryption scheme from asymmetric pairings, which could provide privacy data search and encrypt function for patients and doctors in public platform. The core technique of this system is an extension public key encryption system with keyword search, which the server could test whether or not the files stored in platform contain the keyword without leaking the information about the encrypted file. Compared with former e-medical record systems, the system proposed here has several superior features: (1)Users could search the data stored in cloud server contains some keywords without leaking anything about the origin data. (2) We apply asymmetric pairings to achieve shorter key size scheme in the standard model, and adopt the dual system encryption technique to reduce the scheme's secure problem to the hard Symmetric External Diffie-Hellman assumption, which could against the variety of attacks in the future complex network environment. (3) In the last of paper, we analyze the scheme's efficiency and point out that our scheme is more efficient and secure than some other classical searchable encryption models.

  • At ubiquitous environment of the future, the platform that is mobile and suitable in such environment transmit and receive many data freely. In medical treatment field, medical device with advancement of the sensor and multimedia technique will develop as the household and hand-held device. So It will be developed as the health care device easily exchanging data to other devices and transmitting information of medical device to server. In this paper, currently the mobile platform based on the PXA255 such as a powerful microprocessor which connected the WLAN for guaranteeing a mobility and a speed transmits medical information to hospital server. It put an importance in providing the system for an efficient service.

  • Medical tourism is recognized as a high value-added industry due to its longer stay and more useful costs than general tourism, and is growing rapidly around the world, especially in Asia. In addition, medical tourism is a new form of tourism that combines medical services with tourism activities such as recreation, leisure, and cultural activities, nowadays, medical tourism is emphasizing the combination of medical and tourism. Although Korea medical system and clinic have excellent medical equipment and infrastructure, it is not consistent with policy support and legislation. That have make it difficult for consumers to choose clinics for medical tourism. Therefore, it is necessary to provide customized information to the clinics that consumers want to match each other's services. In particular, it is aimed at consumers with simple procedures and cosmetic purposes, especially at small hospitals. In this paper, we propose a customized service platform that matches users seeking clinics and also medical tourism. Also we can easy to reservation clinic for medical tour.

  • This paper proposes an ontology-based context-aware home healthcare platform employing environmental factors obtained from home. The proposed platform manages the health of home residents, and notifies relatives or a medical team of critical condition through context-awareness based on home ontology by using information sensed from various sensors. The ontology definition of context-awareness from the sensed information provides technically more precise decision for us. Therefore the platform can be aware of the health state of residents and environment by reasoning exactly from data gathered from various sensors and heterogeneous devices. The platform also can individually provide the customized service for users by setting priority for critical status that can be occurred in the health state of residents.

  • The O2O services in the healthcare sector have only been in full swing for about three years, and unlike existing O2O consumer goods, the scale and scope of the dispute are more complicated due to restrictions on medical treatment. In this study, O2O service platform operators and medical institutions' roles and responsibilities were redefined as a countermeasure for resolving disputes in healthcare O2O services and the laws for changing the transaction environment. A change in institutional mechanisms was proposed. This study looked at the types of consumer disputes related to healthcare O2O services as insufficient information problems, problems in the course of medical service implementation, problems with immunity provisions for platform operators, cancellations, and non-compliance with refunds. All the information generated during transactions in the healthcare sector was extensive in scale and included the most sensitive information among personal information, stressing the importance of ensuring security. The area that started in the O2O range before the medical institution visit also proposed a plan to establish a system for the delivery of proven information as a pre-medical person. The scale and growth will grow faster, given that consumers can experience the information they want anytime, anywhere they want. However, the platform broker's role, a link player, will become more important because consumers who use the service will have their first meeting with non-face-to-face product providers. On the other hand, service providers may have side effects of misleading consumers by providing false information or misleading consumers through exaggerated advertisements. The O2O service market is expected to expand beyond distribution and dining out to the entire industry. However, since it is challenging to check accurate statistics on the detailed market, various disputes and consumer protection measures will be required for each detailed market, and comprehensive leading solutions will be essential in the future.
