The purpose of this study is to develop the order communication system for Traditional Korean Medicine(TKM) which can support prescribing decisions and provide the toxicological information. The relative vulnerability of the infrastructure of TKM has made us start the study. We carried out the benchmarking for TKM charting solution firstly, and then designed the intelligent search and supporting method for prescription decisions. We developed of the medical herbs database and the web-based order communication program which can be used in medical field actually. This system supplies a various functions to oriental medical doctors such as management for prescription history, search for herb's effects, generating prescriptions, inventory management, alerting of toxicity and taboo, guideline for taking medicine, and so on. The design and implementation process has been described in this research. We expect that this system will play an important role in electronic medical record(EMR) or electronic health record(EHR) binding diagnosis and management functions.

의료정보 보안에 대한 대안은 무엇보다 의료정보의 안전한 보존과 각종의 보안 위협으로부터 안전성을 강구하는 소프트웨어 설계로부터 시작되어야 한다. 의료정보시스템은 산재된 의료정보를 실시간으로 통합할 수 있어야 하고 의료정보의 교환은 신뢰할 수 있는 데이터 통신이어야 한다. 의료정보시스템의 소프트웨어 아키텍처 설계는 의료정보 공유상 보안 문제를 파악하고 의사소통을 통해 사용자 요구사항을 파악하여 소프트웨어 설계에 반영한다. 소프트웨어 프레임워크 설계, 메시지 표준 설계, 웹기반 프로세스간 통신절차 설계, 접근제어 알고리즘 설계, 아키텍처 기술서 작성, 아키텍처 평가의 제반 절차를 정립한다. 소프트웨어 아키텍처 초기 결정은 설계, 개발, 테스트, 유지보수에 지속적인 영향을 미친다. 또한 프로젝트상의 세부적 의사결정 근거가 된다. 의료정보보안 기반 소프트웨어 아키텍처 설계방법은 오늘날 중요한 과제가 되고 있는 의료정보 보안의 기본틀을 제공할 수 있을 것이다.

농어촌 지역은 산업화 도시화로 도시보다 건강관리가 미흡하며 이에 따른 지역 간 건강불균형 문제가 중요한 사회적 아젠다가 되었다. 이 연구는 사회적 자본과 건강정보, 의료 커뮤니케이션 요인들이 지역사회의 건강상태에 어떠한 영향을 미치는지를 구조방정식 모델을 통해서 검증하고자 하였다. 먼저 사회적 자본은 의료진과의 의사소통성에 영향을 미치는 것으로 나타났고, 의료진과의 의사소통성은 건강상태에 영향을 미치는 것으로 나타났다. 또한 건강정보추구는 건강행위, 인터넷 건강정보이용에 영향을 미치는 것으로 확인되었으며, 인터넷 건강정보이용을 매개로 건강상태에 영향을 미치는 것으로 나타났다. 마지막으로 인터넷 건강정보이용과 건강행위는 건강상태에 영향을 미치는 것으로 확인되었다. 이 연구는 사회적 요인과 커뮤니케이션 요인이 농어촌 건강상태에 영향을 미치는 선행 경로구조를 형성하고 있다는 점을 검증함으로써 지역사회 건강상태의 결정요인에 대한 설명력을 한층 풍부하게 하는 데 기여하고 있다.

This study attempted to identify the service satisfaction with the 119 emergency service and the factors that affect it based on the responses of subjects who used the service. Data collection was conducted from February 11 to March 11, 2021, using convenience sampling. A total of 1842 subjects who used emergency medical services using 119 ambulance in 2020 participated. For the collected data, frequency, percentage, mean, and standard deviation were calculated, and t-test, analysis of variance, and multiple regression analysis were performed using SPSS 23.0. Based on these results, to further enhance the satisfaction of users of the 119 emergency service and the quality of emergency medical services, it is important to improve the quality of paramedics through education and systems. In addition, to increase user satisfaction, efficient and systematic communication education is required. The quality of emergency medical services will increase only when communication skills required for explanations and promoting understanding are improved.

In the medical fields, many efforts have been made to develop and improve Hospital Information System (HIS) including Electronic Medical Record (EMR), Order Communication System (OCS), and Picture Archiving and Communication System (PACS). However, materials generated and used in medical fields have various types and forms. The current HISs separately store and manage them by different systems, even though they relate to each other and contain redundant data. These systems are not helpful particularly in emergency where medical experts cannot check all of clinical materials in the golden time. Therefore, in this paper, we propose a process to build an integrated data model for medical information currently stored in various HISs. The proposed data model integrates vast information by focusing on medical images since they are most important materials for the diagnosis and treatment. Moreover, the model is disease-specific to consider that medical information and clinical materials including images are different by diseases. Two case studies show the feasibility and the usefulness of our proposed data model by building models about two diseases, acute myocardial infarction (AMI) and ischemic stroke.

본 연구에서는 병원정보시스템에서 분야별로 발생하는 의료 빅데이터 자료를 활용하여 가치있는 의료정보를 생성하고 활용할 수 있는 방안을 마련하고자 한다. 본 연구의 결과는 첫 번째, 의료정보시스템의 진료정보와 각종 검사장비 및 의료영상장비와 연동된 PACS의 발생자료를 통합하고 의료 빅데이터를 분석하여 새로운 의료정보를 생성한다. 이렇게 생성된 의료정보는 감염병 및 질병 예방과 질병의 치료를 위한 다양한 건강정보를 생성하게 된다. 두 번째, 환자의 접수내역과 수납내역 그리고 청구내역들을 통합하여 축적해온 의료 빅데이터를 분석하여 다양한 수익통계정보를 생성한다. 이렇게 생성된 수익통계정보는 의료기관의 운영과 수익분석에 활용하기 위한 다양한 경영정보를 생성하게 된다. 이와 같이 병원정보시스템에서 발생하는 의료정보와 공공기관의 의료정보 그리고 개인건강기록의 자료들이 통합이 되면 의료자료를 활용한 가치있는 보건의료정보를 창출하게 된다.

급증하는 정확하고 빠른 정보 제공의 요구 및 다양한 기술적 발달에 부응하여 본 논문은 무선 인터넷 서비스를 기반으로 하는 의료 영상 처리 게이트웨이 서버 및 서비스를 구축하고 이를 통한 의료 서비스의 질적 향상을 도모하였다. 무선 단말기인 스마트 폰의 한계를 고려하여 영상 처리가 0.5초 이상 걸리는 경우는 의료 영상 게이트웨이 서버(Medical Image Gateway Server: MIGS)에서 처리하여 전송하고, 나머지 경우에는 직접 연산하여 적절한 영상 처리 서비스를 제공한다.

원격의료란 의사와 환자가 멀리 떨어져 있는 장소에서 행하는 의료행위로, 통신 수단에 의해 환자의 상태를 파악하여 적절한 진료를 하는 것이다. 일반적으로 상호작용하는 정보통신 기술을 이용하여 원거리에 의료정보와 의료서비스를 전달하는 모든 활동으로 정의된다. 즉 텔레비전, 통신, 컴퓨터, 공학들의 정보통신의 다양한 기술과 의료서비스가 융합된 응용 분야라고 할 수 있다. 환자 및 정보가 먼 거리로 떨어져 있거나 시간적으로 많은 차이가 발생하는 등 여러 가지 문제로 인해 도달할 수 없는 경우 의료정보 및 전문적 조언을 원격으로 제공하는 시스템으로, 환자 진료뿐만 아니라 의료행정, 의학교육, 자문과 의뢰 등을 포함하는 포괄적인 개념으로 쓰인다. 이에 본 논문에서는 원격의료의 법적인 규제를 살펴보고, 현행법에서 규제개선의 쟁점사항을 분석한다.

본 논문은 IoT환경에서 의료장비를 연결하는 eHealth구현에 관련한 연구이다. 본 연구의 구현환경은 센싱노드, 게이트웨이 그리고 서버가 연결된 구조이다. 센서노드인 의료장비에서 발생한 의료정보들을 게이트웨이에 전달하고 게이트웨이에서 서버로 의료정보를 전달하여 대량데이타를 서버에 저장할 수 있게 한다. 센서노드로부터 발생하여 서버에 축적된 의료정보는 데이터베이스 형태로 저장되어 의료정보 사용에 대한 준비가 된다. 본 연구에서는 일반 상업용으로 판매하는 센서노드들을 게이트웨이에 연결하고 게이트웨이에서 센서노드를 관리하는 소프트웨어 구조를 연구하고 서버에 데이터를 어떻게 전달할 것인가를 구현한다.

Current medical tourism is focused on the services of large hospitals and it is hard to find ways to attract the users. Users collect information for medical tourism through various paths in order to receive the medical consultations and customized tour services. To expand medical tourism to small and medium sized hospitals, it is necessary to have the customized medical consultations, tours and interpreter services, which are the key elements of medical tourism. This study suggests ways to provide the services based on information on medical consultations, tours and interpreter services that users had experienced directly, and also based on the platform for the essential items integrated from users, hospitals and guides' viewpoints. With information on hospitals that provide medical consultations and guides who are able to provide professional services in translation, interpretation and customized tour, users may accumulate and share the information about hospitals and customized tours verified by other users from the integrated platform. To match the contents provided by hospitals and guides with information experienced by users into a system, this study suggests the construction plan for the service model that can match the experience information between users and hospitals, between users and guides and between hospitals and guides systematically by operating the data in the universal container.
