Oriental Medical Ontology for Personalized Diagnostic Services (맞춤형 진단 서비스를 위한 한의학 온톨로지)

  • With the advancement of information technology and increasing diversity in medical field, there are ongoing researches on ontology based intelligent medical system in Oriental medicine field. Intelligent diagnostic support system uses ontology to give a structure to complex medical knowledge and personal medical history so that we can make diagnosis more scientific, and provide better medical services. In this paper, we suggest an ontology that structuralize three knowledge types basic medical data, clinical trial data, and personal health information, which can be used as important information for individually tailored diagnosis. Especially in Oriental medicine diagnosis, both patient's symptoms of illness and physical constitution play a great role; it can lead to distinct diagnosis depending on their combination. Thus, it is much needed to have a diagnostic support system that uses personal health history and physical constitution along with basic medical data and clinical trial data in the field. In this paper, we implemented an Oriental medicine diagnostic support system that provides individualized diagnosis service to each patient by building an ontology on Oriental medicine focused on individual physical constitution and disease information.

  • The paper's suggestion is about hereditary facts between family members. Diagnosing patients from the point of patients temporary conditions, and so performing primitive examinations and treatments, can lead not only to frequent wrong diagnoses, and to huge medical expenses and times to the patients, but even to critical situation of patients or taking lives away. As a means to cut these cases down to a minimum, sharing medical treatment information between family members is suggested. This approach makes possible understanding physical constitution and environment between family members, and can result in bringing a faster treatment effect if some family member suffers from a similar disease. This approach, since a participation in a family membership effectuates all of family members, can minimize the membership fees, thus enabling inter-family health care on a home doctor basis.

  • 1) Objective This research is a process of examining the Traditional Acupunctural Methods left in our documents. The objectives of this paper are explaining the process of researching documents and reporting several points revealed in this process. 2) Process This study was done through the following steps: (1) We located the Acupunctural Methods in the original text database on the web. (2) We sorted them according to criterion. (3) We set up some standard clauses in order to organize the selected information. (4) We re-organized the information following the standards within the categories. (5) As for clauses that were not fully explained in the documents, we tried to restore them through other documents. 3) Results (1) 11 standard clauses were set up in order to organize the Traditional Acupunctural Methods. (2) Traditional Acupunctural Methods were extracted from 22 documents, (3) 24 Traditional Acupunctural Methods were extracted and organized by standards. (4) We learned of similar methods with resembling ways of drawing blood and methods with deep interest in the depth of needle insertion. Also, we were able to gain a clue to the exchange of Acupunctural Methods among the three Asian countries and the possibility of research using the Korean Medicine Database.

  • PACS is one of the most used medical information system and share information from other hospitals through the PACS. Data integrity means zero defects data and this is a prerequisite of information system performance. but I wonder if I can trust these informations that Incorrect information from radiotechnologist's mistakes, anonymous in emergency department, Newborn baby department, modified informations at later. And Modified informations causes defect in integrity of the data. When we import, we use DICOM header not DB data. so error occurs that DB data is deferent with DICOM Header information. This paper discusses to resolve as above problem using DICOM object such as DICOM PR, SR. And propose quality management system that can guarantee the patient information and can manage exam history.

The Need for a Table of Descent in Studying Medical Persons (의학인물 연구에 있어서 족보의 필요성)

  • Although a table of descent is a mandatory primary source for studying medical persons, but most of those who practiced medicine were not but of step families. Because members of step families are not usually recorded accurately, preceeding studies did not put much emphasis on them. But as more researchers study biography and the number of studies done on historical figures are increasing tremendously, its value as a source has increased as well. In order to research a person, obtaining background information is a priority, which includes to what family they belong to, to what specific branch of the family they belong to, etc. Whether or not the person has ever worked with a public institution is important as well.

A Study of Disease Prescription Using Rehmannia glutinosa, Cnidium officinale, Angelica gigas, Paeonia lactiflora in Dongeuibogam (『동의보감(東醫寶鑑)』에서 숙지황(熟地黃), 천궁(川芎), 당귀(當歸), 백작약(白芍藥)이 군약으로 사용된 방제의 주치병증 비교)

  • This research aims to find a way to utilize the literature to select the herbs to be added or subtracted according to the condition of the patient after determining the basic prescription to be used for the patient. The study is based on data concerning the herbal composition and application of prescriptions described in the Donguibogam. The composition herbs was expressed as a weight ratio, the standard deviation of the weight ratio was calculated for each prescription, and the case where the weight ratio was greater than the base value (highest weight ratio - standard deviation) was designated as a principal herb. The various expressions of applications were changed to representative terms by the researcher and used to calculate the frequency. Of the 4,373 prescriptions for Donguibogam, 3,307 were able to be expressed in terms of weight ratios with 640 prescriptions using Rehmannia glutinosa, Cnidium officinale, Angelica gigas and Paeonia lactiflora as the principle herb. Of the 640 prescriptions, 508 prescriptions included applied information. Rehmannia glutinosa can also be used for symptoms such as fetal leakage(胎漏), short ingestion(食少), fever(發熱), dazed expression(眼花). Cnidium officinale can also be used for symptoms such as migraine(偏頭痛), stroke(中風癱瘓), tetanus(破傷風). Angelica gigas can also be used for symptoms such as forgetfulness(健忘), incontinence of urination(小便不通), palpitations(驚悸). Paeonia lactiflora can also be used for symptoms such as heart pain and abdominal pain(心腹痛), cough(咳嗽).

Sparganosis in the Lumbar Spine : Report of Two Cases and Review of the Literature

  • Sparganosis is a rare parasitic infection affecting various organs, including the central nervous system, especially the lumbar epidural space. This report describes the identification of disease and different strategies of treatments with preoperative information. A 42-year-old man presented with a 2-year history of urinary incontinence and impotence. He had a history of ingesting raw frogs 40 years ago. Magnetic resonance (MR) imaging showed an intramedullary nodular mass at conus medullaris and severe inflammation in the cauda equina. A 51-year-old woman was admitted with acute pain in the left inguinal area. We observed a lesion which seemed to be a tumor of the lumbar epidural space on MR imaging. She also had a history of ingesting inadequately cooked snakes 10 years ago. In the first patient, mass removal was attempted through laminectomy and parasite infection was identified during intra-operative frozen biopsy. Total removal could not be performed because of severe arachnoiditis and adhesion. We therefore decided to terminate the operation and final histology confirmed dead sparganum infection. We also concluded further surgical trial for total removal of the dead worm and inflammatory grannulation totally. However, after seeing another physician at different hospital, he was operated again which resulted in worsening of pain and neurological deficit. In the second patient, we totally removed dorsal epidural mass. Final histology and enzyme-linked immunosorbent assay (ELISA) confirmed living sparganum infection and her pain disappeared. Although the treatment of choice is surgical resection of living sparganum with inflammation, the attempt to remove dead worm and adhesive granulation tissue may cause unwanted complications to the patients. Therefore, the result of preoperative ELISA, as well as the information from image and history, must be considered as important factors to decide whether a surgery is necessary or not.

A Research of the Ancient Book Named 'Yeogwa-daeyo' (여과대요(女科大要)에 관한 연구)

  • Purpose: Actually until now, there is few historic records concern on oriental OB&GY from late Joseon Dynasty colonized by Japan. therefore, this research was studied in priority with historic background and medical-history for this book and writer. Methods: the related data were collected through books on history, medicine books, essays, and family councils, contents of $\ll$女科大要$\gg$ were made a comparative study with $\ll$東醫寶鑑$\gg$. Results : As it seems to be accomplished writing and publication of this book, $\ll$女科大要$\gg$ at that times, and there is no former days data about the author, 李載建, this research was progressed by dividing into background research and comparison study. Conclusion: there is insufficient report on information of this book and the author, but it is considered that enough value is by existence of a book and person's discovery. Besides, it is thought to need more study on comparison with books same age and exchange with historic persons of Korean traditional medicine.

  • The present study examined subject classification and the characteristics of old Oriental medicine literature focused on Web services of Oriental medicine knowledge and information resources. For this, we reviewed how subject classification is applied to Oriental medicine in the codified literature classification table and, based on the results, examined how the classification system is used in libraries. Second, subject classification and the characteristics of old Oriental medicine literature were studied focused on Web services of Oriental medicine knowledge and information resources, and related problems and solutions were suggested.

  • As we enter the 21st century, the Information Era, we are making a national effort to digitalize the information resources of Korean Studies, based on our leading digital technology. However, there is much difficulty of computerizing Chinese characters used in Korea, China, and Japan, with the of technologies developed by the West. This paper gives an example of how to register and process the Newly found ideographs(新出漢字) put forth by Digitalization of Knowledge information resource on Korean oriental medicine.
