우리나라 성인의 허혈성 심장질환, 뇌졸중으로 인한 총 진료비 중 과체중 및 비만의 기여분 (Medical Expenditure Attributable to Overweight and Obesity in Adults with Ischemic Heart Disease and Stroke in Korea)

  • objectives: This study was conducted to estimate medical expenditure attributable to overweight and obesity in adults with ischemic heart disease and stroke using Korea National Health and Nutrition Examination survey data and Korea National Health Corporation data. methods : The medical expenditure of ischemic heart disease and stroke related to overweight and obesity were composed of inpatient care costs, outpatient care costs and medication costs. The population attributable risk (PAR) of overweight and obesity was calculated from national representative data of Korea such as the National Health Insurance Corporation cohort data and 2005 Korea National Health and Nutrition Examination survey data. results: The medical expenditure attributable to overweight and obesity of ischemic heart disease were 97.4 billion won(74.1-122 billion won). and stroke were 64.6 billion won(33.1-98.1 billion won). Consequently, these costs corresponded to 11.4% of total medical expenditure due to ischemic heart disease and stroke. conclusion: We conclude that overweight and obesity have increased medical expenditure from ischemic heart disease and stroke in Korea. These findings provide important support for implementing overweight and obesity management strategies in Korea.

산업장 근로자의 치과의료이용양상 및 관련요인 (Dental care utilization patterns and its related factors of industrial workers)

  • Objectives : This purpose of this study was to examine worker's Dental care utilization patterns and its related factors, to prepare basic data for oral health promotion of workers. Methods : The study subject were 1016 industrial workers from7 working places in Gyung-Sang-Buk Do and Daegu-City. Data for this study was obtained by self-administrated questionnaire. The questionnaire consists of general characteristics, yes or no of dental care utilization before six month, purpose of visit, yes or no of dental disease form the results of dental examination screening before one years, yes or no of and patterns dental care utilization of workers with dental disease, etc. The data was utilized using frequency test and $x^2-test$. Results : Rate of dental care utilization was 40% within six month last, among the general characteristics, it were statistically significant that higher age, married state, bad oral health state, high Work period. Rate of dental disease as a result of their dental examination last was 53.1% among the general characteristics, it were statistically significant that enough economic state, bad oral health state, blue collar work type. Dental care utilization's rate of dental disease as a result of their dental examination last was 48.6% among the general characteristics, it were statistically significant that Enough economic state, higher age, married state, high Work period. Conclusions : In order to promote workers of abnormal opinions to take the medical treatment on their dental diseases later dental examination, it is necessary to educate them consistently for the importance of early medical treatment.

건강검진 수검 및 검진유형 선택의 결정요인 (Determinants of the Use and Type of Comprehensive Medical Examination Services)

  • Objectives : This study analyzed the factors that determine the use and type of medical examination services (MES) to develop a model explaining the use behavior of MES which could consequently contribute to policy implications for medical examinations. Methods : Based on Anderson's healthcare utilization model, the MES utilization model was developed by including the perceived needs for MES. The data were collected from an online survey of a population aged 20-39 years and from a telephone survey of a population aged 40 years or older, respectively. Chi-Square tests and hierarchical logistic regression analyses were done with SAS version 9.3. Results : Generally, as health status became lower, the use of MES increased. However, patients with two or more chronic diseases were less likely to use private MES compared to patients with one chronic disease. The perceived needs for MES were only related to the use of service and not to the choice of the MES type. Conclusions : There were different results for the significant determinants between the use of the MES and the choice of the MES type. The healthcare industry needs to aware of consumer needs to provide MES based on empirical findings.

의료기관 종별에 따른 전산화단층촬영장비 보유현황과 검사료간의 관계 (Relationship Between Type of Medical Institutions According to the Equipment List and Inspection Fee Computed Tomography)

  • 본 연구에서는 CT 장비의 국내에서의 보급률과 이용률이 증가되고 있는 흐름과 증가의 변화와 장치의 변화가 검사부위에 나타나는 빈도 등을 정량적으로 파악하기 위하여 현재 우리나라의 의료기관 종별 및 시기별의 의료영상 진단 장비인 CT장치의 보유현황, 검사료 등을 조사하였다. 최근 10년간의 우리나라 의료기관 종별(상급종합병원, 종합병원, 병원, 의원, 치과병원, 보건의료원, 한방병원) 및 연도별(2003~2012)에 따른 CT장치 보유현황, 연도별 CT검사료를 의료기관 종별의 분포를 조사하였다. 우리나라 CT장치는 상급종합병원과 종합병원급 의료기관에서 보유한 총 비율은 2003년에 25.6%, 2011년에 32.5%정도인 반면, 병원과 의원급 의료기관에서 보유한 비율은 2003년에 73.5%와 2009년에 67.5%로 약 70%를 차지하고 있는 것으로 나타났다. 2011년도를 기준으로 하였을 때 상급종합병원과 종합병원에서 총 검사료의 82.4% 정도인 반면, 병원과 의원에서는 총 검사료의 17.6%로 나타났다. CT장비는 병원급은 증가하고 있는 추세이나 의원급은 감소하고 있는 것으로 나타났다. CT검사료 대부분은 80% 이상이 상급종합병원과 종합병원에서 청구하는 것으로 나타났다. 의료기관 종별 CT장비 보유현황과 검사료간의 상관관계가 유의미함을 알 수 있다. 그 중에서도 상급종합병원의 CT보유대수와 검사료간의 상관관계가 높았으며(p<.001), CT보유대수가 많아질수록 검사료 청구액도 높아진 것으로 나타났다.

  • 이 연구는 고혈압, 당뇨병, 이상지질혈증으로 지출된 진료비 중 과체중 및 비만의 기여분을 추계하기 위한 질병비용연구로서 국민건강보험공단의 건강검진 및 건강보험급여 자료를 활용한 연구이다. 연구결과 2005년 현재 고혈압, 당뇨병, 이상지질혈증 때문에 지출된 총 진료비 중 과체중 및 비만의 기여분은 각각 4,561억원(4,328억원-4,805억원), 2,823억원(2,487억원-3,176억원), 165억원(147억원-183억원)이었고, 이를 모두 합하면 7,549억원(6,961억원-8,164억원)이었다. 이는 해당 연도의 고혈압 총 진료비의 34.6%(32.9%-36.5%), 당뇨병 총 진료비의 32.5%(28.6%-36.6%), 이상지질혈증 총 진료비의 19.4%(17.3%-21.6%)를 차지한다. 또한 이 세 가지 질병의 총 진료비를 기준으로 했을 때 과체중 및 비만이 기여한 금액은 33.3%(30.7%-36.0%)에 해당하였다. 이는 과체중과 비만을 예방한다면 고혈압, 당뇨병, 이상지질혈증으로 지출되는 총 진료비의 33.3%를 감소시킬 수 있다는 의미이며, 향후 만성질환 예방과 관련하여 과체중 및 비만의 예방관리사업이 얼마나 중요한가를 시사해 주는 것이다.

Data Mining Model Analysis for The Risk Factor of Hypertension - By Medical Examination of Health Data -

  • The data mining is a new approach to extract useful information through effective analysis of huge data in numerous fields. We utilized this data mining technique to analyze medical record of 39,900 people. Whole data were separated by gender first and divided into three groups, including normal, stage 1 hypertension, and stage 2 hypertension. The data from each group were analyzed with data mining technique. Based on the result that we have extracted with this data mining technique, major risk factors for the hypertension are age, BMI score, family history.

디지털 흉부 후·전 방향 방사선영상을 이용한 정상 한국인 폐 크기의 영상의학적 계측 (Radiological Measurements of Lung Field Size in Normal Korean using Digital Chest Posteroanterior Radiography)

  • The purpose of this study is to provide baseline data on lung field size measured radiological method by chest PA image in normal Korean. The subject of this study is 496 normal persons who performed chest PA examination using x-ray digital radiography system. The measurement method is from the apex of right and left lung to the costophrenic angle of both lung, from the top of the image to the lowest costophrenic angle of both lung and transverse line of the largest lung area. As a result of this study, the following conclusions were obtained. A lung field size of male is larger than the female(p<0.05). The younger the age, the longer both lung length and total lung height statistically significant. As a increase height and length, A lung field size was increased(p<0.05). But, BMI is not associated with a lung field size. This study will be data of reference data when radiological technologists perform chest PA examination.

  • The data mining is a new approach to extract useful information through effective analysis of huge data in numerous fields. We utilized this data mining technique to analyze medical record of 35,671 people. Whole data were assorted by BMI score and divided into two groups. We tried to find out BMI risk factor from overweight group by analyzing the raw data with data mining approach. The result extracted by C5.0 decision tree method showed that important risk factors for BMI score are triglyceride, gender, age and HDL cholesterol. Odds ratio of major risk factors were calculated to show individual effect of each factors.

Cytoplasmatic Localization of Six1 in Male Testis and Spermatogonial Stem Cells

  • Sine oculis homeobox 1 (Six1) is an important factor for embryonic development and carcinoma malignancy. However, the localization of Six1 varies due to protein size and cell types in different organs. In this study, we focus on the expression and localization of Six1 in male reproductive organ via bioinformatics analysis and immunofluorescent detection. The potential interacted proteins with Six1 were also predicted by protein-protein interactions (PPIs) and Enrichr analysis. Bioinformatic data from The Cancer Genome Atlas and Genotype-Tissue Expression project databases showed that SIX1 was highly expressed in normal human testis, but low expressed in the testicular germ cell tumor sample. Human Protein Atlas examination verified that SIX1 level was higher in normal than that in cancer samples. The sub-localization of SIX1 in different reproductive tissues varies but specifically in the cytoplasm and membrane in testicular cells. In mouse cells, single cell RNA-sequencing data analysis indicated that Six1 expression level was higher in mouse spermatogonial stem cells (mSSCs) and differentiating spermatogonial than in other somatic cells. Immunofluorescence staining showed the cytoplasmic localization of Six1 in mouse testis and mSSCs. Further PPIs and Enrichr examination showed the potential interaction of Six1 with bone morphogenetic protein 4 (Bmp4) and catenin Beta-1 (CtnnB1) and stem cell signal pathways. Cytoplasmic localization of Six1 in male testis and mSSCs was probably associated with stem cell related proteins Bmp4 and CtnnB1 for stem cell development.

  • Purpose: This study measures the influence of facilitating conditions on employees' attitudes towards the adoption of big data analytics by selected medical aid organizations in Durban. In the health care sector, there are various sources of big data such as patients' medical records, medical examination results, and pharmacy prescriptions. Several organizations take the benefits of big data to improve their performance and productivity. Research design, data, and methodology: A survey research strategy was conducted on some selected medical aid organizations. A non-probability sampling and the purposive sampling technique were adopted in this study. The collected data was analysed using version 23 of Statistical Package for Social Science (SPSS) Results: the results show that the "facilitating conditions" have a positive influence on employees' attitudes in the adoption of big data analytics Conclusions: The findings of this study provide empirical and scientific contributions of the facilitating conditions issues regarding employee attitudes toward big data analytics adoption. The findings of this study will add to the body of knowledge in this field and raise awareness, which will spur further research, particularly in developing countries.
