A Proposal on the Addition of Wǔjī(五積) in KCD(Korean standard classification of disease and cause of death) (한국표준질병사인분류(KCD)에 오적(五積) 추가에 대한 제안)

  • Objective : The purpose of this study is to propose adding Wǔjī(五積) to KCD. Methods : The pathological details of Wǔjī(五積) mentioned in the classic literatures were extracted and compiled to provide the basis that Wǔjī(五積) should be registered in the KCD. Conclusions : 1. Jī(積) is a very important pathological and clinically element of Korean Medicine. So not only Shíjī(食積) but also Wǔjī(五積) should be registered in the KCD. 2. We suggest adding Wǔjī((五積) to the U-Code 64~71 of the KCD due to its pathological nature. 3. If Wǔjī((五積) is registered in the KCD, it will provide a more systematic and objective approach to Jī(積) in the future, and it is expected to provide an opportunity to drastically enhance the performance of Korean Medicine treatments for cancer or tumors by forming various and extensive big data. 4. Regardless of whether or not Wǔjī((五積) is added to the KCD, the clinical and pathological re-establishment of Wǔjī(五積) should be performed in accordance with the present period and social situation through continuous and extensive clinical research about Jī(積).

Machine learning-based Predictive Model of Suicidal Thoughts among Korean Adolescents. (머신러닝 기반 한국 청소년의 자살 생각 예측 모델)

  • This study developed models using decision forest, support vector machine, and logistic regression methods to predict and prevent suicidal ideation among Korean adolescents. The study sample consisted of 51,407 individuals after removing missing data from the raw data of the 18th (2022) Youth Health Behavior Survey conducted by the Korea Centers for Disease Control and Prevention. Analysis was performed using the MS Azure program with Two-Class Decision Forest, Two-Class Support Vector Machine, and Two-Class Logistic Regression. The results of the study showed that the decision forest model achieved an accuracy of 84.8% and an F1-score of 36.7%. The support vector machine model achieved an accuracy of 86.3% and an F1-score of 24.5%. The logistic regression model achieved an accuracy of 87.2% and an F1-score of 40.1%. Applying the logistic regression model with SMOTE to address data imbalance resulted in an accuracy of 81.7% and an F1-score of 57.7%. Although the accuracy slightly decreased, the recall, precision, and F1-score improved, demonstrating excellent performance. These findings have significant implications for the development of prediction models for suicidal ideation among Korean adolescents and can contribute to the prevention and improvement of youth suicide.

Analysis of Factors Affecting on Satisfaction of Pharmacy Service (약국서비스 만족에 영향을 미치는 요인 분석 - 환자체감시간과 실 조제시간 비교를 중심으로 -)

  • Purpose : To shorten processing time for variety of medical affairs of the patient at the outpatient clinic of a big hospital is very important to qualify medical care of the patient. Therefore, patient's waiting time for drug delivery after doctor's prescription is often utilized as a strong tool to evaluate patient satisfaction with a medical care provided. We performed this study to investigate factors influencing patient satisfaction related with waiting time for drug delivery. Methods : The data were collected from July 21 to August 12, 1998. A total 535 patients or their families who visited outpatient clinics of Inha University Hospital were subjected to evaluate the drug delivery time and the level of their satisfaction related, which were compared with those objectively evaluated by Quality Improvement Team. The reliability of the scale was tested with Cronbach's alpha, and the data were analyzed using frequency, t-test, ANOVA, correlation analysis and multiple regression. Results : The mean drug delivery time subjectively evaluated by the patient (16.1 13.0 min) was longer than that objectively evaluated (10.9 7.6 min) by 5.2 min. Drug delivery time objectively evaluated was influenced by the prescription contents, total amount or type of drug dispensed, etc, as expected. The time discrepancy between two evaluations was influenced by several causative factors. One of those proved to be a patient's late response to the information from the pharmacy which the drug is ready to deliver. Interestingly, this discrepancy was found to be more prominent especially when waiting place for drug delivery was not less crowded. Other factors, pharmaceutical counseling at the pharmacy, emotional status or behavior of a patient while he waits for the medicine, were also found to influence the time subjectively evaluated. Regarding the degree of patient satisfaction with the drug delivery, majority of patients accepted drug delivery time with less than 10 min. It was also found to be influenced by emotional status of the patient as well as kindness or activity of pharmaceutical counselor. Conclusion : The results show that, besides prescription contents, behavior pattern or emotional status of a patient, environment of the waiting place, and quality of pharmaceutical counseling at the pharmacy, may influence the patient's subjective evaluation of waiting time for drug delivery and his satisfaction related with the service in the big hospital. In order to improve patient satisfaction related with waiting time for drug delivery, it will be cost effective to qualify pharmaceutical counseling and information system at the drug delivery site or waiting place rather than to shorten the real processing time within the pharmacy.

  • As COVID-19, which occurred at the end of 2019, has become a global pandemic, it has emerged as an unprecedented event that quickly destroys a nation's medical and healthcare system in both developed and developing countries. In the 21st century, most of the civil society that aimed for hyperconnected society is facing a new crisis that has not been experienced so far. Indeed, lack of personal protective equipment, isolation of clustered communities, disruption of medical systems necessary for diagnosis and treatment, and disruption of educational and economic activities due to social isolation are emerging. Since the COVID-19 has occurred, many of the difficulties that have occurred in the past six months indicate the basic infrastructure a society should have particularly in a pandemic. These include personal protective equipment (PPE), decontamination and quarantine tools essential for effective response, rapid and precise large-scale diagnosis, medical devices required for patient care, and identification and fast and wide on-line networks that can be used in social isolation. In this first part, we would like to introduce some representative examples of 1) personal protective equipment, 2) prevention of personal and community health, 3) social response through big data and networks within the framework of appropriate technology.

  • This study is a review study attempted to analyze the current situation of surgical robots based on previous research on surgical robots in the era of the 4th revolution, and to forecast the future direction of surgical robots. Surgical robots have made full progress since the launch of the da Vinci and the surgical robot is playing a role of supporting the surgeries of the surgeons or the master-slave method reflecting the intention of the surgeons. Recently, technologies are being developed to combine artificial intelligence and big data with surgical robots, and to commercialize a universal platform rather than a platform dedicated to surgery. Moreover, technologies for automating surgical robots are being developed by generating 3D image data based on diagnostic image data, providing real-time images, and integrating image data into one system. For the development of surgical robots, cooperation with clinicians and engineers, safety management of surgical robot, and institutional support for the use of surgical robots will be required.

  • Biometric technology is a technology for authenticating a user using the physical or behavioral features of the inherent characteristics of the individual. With the necessity and efficiency of the technology in the fields of finance, security, access control, medical welfare, inspection, and entertainment, the service range has been expanding. Biometrics using biometric information such as fingerprints and faces have been exposed to counterfeit and disguised threats and become a social problem. Recent studies using a bio-signal from the inside of the body other than the bio-information of the external body are being developed. This paper analyzes the recent research and technology of biometric systems using bio-signals, ECG, heart sounds, EEG, and EMG to present the skills needed for the development direction. In the future, utilizing the deep learning to build and analyze database to manage bio-signal based big data for the complex condition of individuals, biometrics technologies suitable for real time environment are expected to be researched.

Improvement of Accessibility to Dental Care due to Expansion of National Health Insurance Coverage for Scaling in South Korea (치석제거 요양급여 확대 정책으로 인한 치과의료 접근성 향상)

  • Since 2013, adults aged over 20 can receive national health insurance scaling once a year in South Korea. In this study, we analyzed the usage status of national health insurance care service for periodontal disease in 2010-2018 by using Healthcare big data of the Health Insurance Review and Assessment Service. The increase rate of the dental care users was very high at 7.8 and 11.2% in 2013 and 2014, respectively. These are higher than the increase rate of all medical institution users, which is between -1.7 and 3.7%. In 2017, the rate of dental use was 44.4%, which has increased more than 10% compared to 2012. Percent receiver of national health insurance scaling was 19.5% in 2017. The 20s had the highest rate of 23.2%. The rate decreased with age. Based on these results, it can be evaluated that the expansion of national health insurance coverage for scaling improves accessibility to dental care. A more long-term assessment of the effect of periodic dental examination and scaling on reducing the prevalence of periodontal disease is needed. National health insurance coverage should be extended to oral hygiene education and supportive periodontal therapy in order to prevent periodontal disease.

Updating Korean Disability Weights for Causes of Disease: Adopting an Add-on Study Method

  • Objectives: Disability weights require regular updates, as they are influenced by both diseases and societal perceptions. Consequently, it is necessary to develop an up-to-date list of the causes of diseases and establish a survey panel for estimating disability weights. Accordingly, this study was conducted to calculate, assess, modify, and validate disability weights suitable for Korea, accounting for its cultural and social characteristics. Methods: The 380 causes of disease used in the survey were derived from the 2019 Global Burden of Disease Collaborative Network and from 2019 and 2020 Korean studies on disability weights for causes of disease. Disability weights were reanalyzed by integrating the findings of an earlier survey on disability weights in Korea with those of the additional survey conducted in this study. The responses were transformed into paired comparisons and analyzed using probit regression analysis. Coefficients for the causes of disease were converted into predicted probabilities, and disability weights in 2 models (model 1 and 2) were rescaled using a normal distribution and the natural logarithm, respectively. Results: The mean values for the 380 causes of disease in models 1 and 2 were 0.488 and 0.369, respectively. Both models exhibited the same order of disability weights. The disability weights for the 300 causes of disease present in both the current and 2019 studies demonstrated a Pearson correlation coefficient of 0.994 (p=0.001 for both models). This study presents a detailed add-on approach for calculating disability weights. Conclusions: This method can be employed in other countries to obtain timely disability weight estimations.

  • Despite the fact that Korea has supported national cancer screening worldwide for a long time, the cancer screening screening rate is low compared to other health screenings and infant screening. In particular, women with disabilities show a lower screening rate for cervical cancer screening compared to women without disabilities. Therefore, from 2011 to 2019, this study investigated the trends in cervical cancer screening rates of women with disabilities and non-disabled women aged 30 and older, and analyzed the factors affecting cervical cancer screening of women with disabilities at the individual and regional level through the cervical cancer screening rates among regions. The purpose of this study is to identify the factors that affect regional variation by dividing into regions. From 2011~2019, the cervical cancer screening rate of people with disabilities was the highest in Daejeon and the lowest in Sejong and Jeju, and the gap in cervical cancer screening rate of people with disabilities by region was analyzed to be up to 13.1%.

The Effects of Adherence on Hypertension Control among Newly Diagnosed Hypertension Patients (신규 고혈압 환자에서 치료지속성이 고혈압 조절에 미치는 영향)

  • Background: This study is to research on how hypertension control is associated with adherence in newly diagnosed hypertension patients. Methods: The study is based on 255,916 patients who were diagnosed with hypertension in 2009 and didn't have any previous medical history of hypertension or associated complication for the past year using data collected by National Health Insurance Corporation. Newly diagnosed hypertension patients are divided into two group by visiting medical center numbers (more than 300 days was adherence group, if not non-adherence group). Patients are considered to have successfully controlled their hypertension based on blood pressure measured by health examination. Chi-square test and logistic regression, repeated measured analysis of variance was used to analyze. Results: The relations between adherence and hypertension control show that 1.12 times of patients in adherence group was able to control their hypertension. The additional analysis proves that adherence group are more decreased level of blood pressure than non-adherence group except for patients who are over 70. Comparison of the average of systolic blood pressure and diastolic blood pressure between adherence and non-adherence groups shows that the blood pressure has been significantly among the adherence group. Conclusion: The study proves that constant treatment for hypertension could control the blood pressure and encourages patients to put more effort for persistent treatment. It also shows that hypertension treatment are more effective in younger patients than the elderly and strategies of approaching are different depending on age.
