A feasibility study of audio-video communication application using mobile telecommunication in inter-hospital transfer situations (병원 간 전원 상황에서 이동통신망을 이용한 음성화상정보통신 애플리케이션의 유용성에 관한 연구)

  • Purpose: The aim of this study was to evaluate the usefulness of video communication with medical staff located at a remote location, through a communication application connected to a long term evolution (LTE) mobile communication network in a moving ambulance. Methods: In this study, we recruited patients who were transferred by mobile intensive care unit ambulance from one hospital to another. In the moving ambulance, the information of the patient was transferred to a physician using the application in real time. Recorded video files were evaluated by emergency physicians with experience in video direct medical control. Results: A total of 18 patients were evaluated, and the average score was 5.9 out of a possible 9. It was expected that applying the use of the technology to actual clinical sites would enable the sites to provide assistance. Conclusion: In this study, we confirmed the possibility of benefiting from the clinical field when using the video-audio communication application which is connected to the remote location in real time through the current LTE mobile communication network in the ambulance.

  • 본 논문에서는 스마트케어 서비스를 위해, Android 운영체제를 기반으로 하는 Device에서 ADK를 이용해 혈압계를 제어하는 방법을 제안한다. 이 방법은 소프트웨어와 하드웨어를 결합한 kit인 ADK를 사용하여 Hardware를 잘 모르는 사람들도 Android에서 Hardware의 GPIO에 좀 더 쉽게 접근하고 제어할 수 있어 보다 다양한 기능을 구현할 수 있다. 또한 이 방법으로 얻은 혈압계 데이터를 Database에 저장하여 데이터통신망을 통해 전문의에게 Feedback을 받음으로써 굳이 병원을 방문하지 않더라도 개인의 건강을 보다 편리하게 관리할 수 있어 한 단계 수준 높은 진단을 받을 수 있다.

The Study on Service Design Development for Pharmacy Users: Using Smart Phone Application

  • Objective: This study is to review the current state of services offered to pharmacy users, and develop a service design converging a prescription service and a smart phone application service in order to enhance patients' experience at pharmacies and home. Background: Under the new medical system; separation of prescribing and dispensing drugs, a doctor writes a prescription to their patients and patients have their prescription filled at the pharmacy. As the number of flue, allergy and atopy patients has increased, waiting lines have been longer at pharmacies. Besides, the current medical service system lacks of providing proper information on prescribed pills to patients. There are already services offered during waiting times such as offering free drinks, magazines or suggesting general drugs which you can buy without prescription, however they neither cover the current medical service's shorts nor shorten the waiting time. Method: I researched objective and perceived waiting times reduction methods, the current service status at pharmacies, and the government's policy direction in a medical service. Also, I observed a patient's journey from the hospital to pharmacy and then home. I examined the circumstance at pharmacies, patients' behaviors and their thoughts during their journey, and extracted three main goals to design a service in order to help patients have positive perception during the waiting time; (1) to reduce the perceived time by the way of visualizing time and offering readings about what patients consider necessary, (2) to educate patients what they are into and how to get through, (3) to establish trust among patients, doctors and pharmacists. Based on three goals, I designed a structure and a wireframe for a new service application of smart phones. Results: With a new service design for pharmacy users, users can track their medical record and visit the information about their current medical treatments anytime. Also the service helps patients build reliable relationships with doctors and pharmacists. Conclusion: Experience is not just an activity but series of multiple activities. The serving range of a medical service should not be determined by stakeholders but user's holistic experience. By approaching a service design with a holistic vision, it can enrich not only a temporary experience but also a whole life well being. Application: Since there are already many service applications advising patients about their illness and finding right doctors, this service design is focused on the experience from getting a prescription till feeling better. The next move is to combine those two parts medical services and design an integrated service application. As a prescription is going to be coded in numbers, we might consider to design an un-attended pharmacy which can shorten huge amount of time for filling prescriptions.

Oxaliplatin Sensitizes OS Cells to TRAIL-induced Apoptosis Via Down-regulation of Mcl1

  • Purpose: To investigate the killing effect on OS cells of a combination of oxaliplatin and TRAIL and related molecular mechanisms. Methods: TRAIL and oxaliplatin were applied to OS732 cells singly or jointly and survival inhibition rates were measured by MTT assay, changes of cellular shape being assessed with inverted phase contrast and fluorescence microscopy. Apoptotic rates were analyzed by flow cytometry (FCM) and immunocytochemistry was used to examine Mcl1 expression of OS732 cells. Results: The survival inhibition rate of combined application of $100{\mu}g/ml$ TRAIL and $1{\mu}g/ml$ oxaliplatin on OS-732 cells was significantly higher than that of either agent singly (p<0.01). Changes of cellular shape and apoptotic rates also indicated apoptosis-inducing effects of combined application to be much stronger than those of individual application. Oxaliplatin had the effect of down-regulating Mcl1 expression and sensitizing OS cells to TRAIL-induced apoptosis. Conclusion: A combination of TRAIL and oxaliplatin exerts strong killing effects on OS-732 cells which might be related to down-regulation of Mcl1 expression.

  • The Korea retrospective dosimetry (KREDOS)-electron paramagnetic resonance (EPR) group undertook an intercomparison investigation utilizing hydroxyapatite. This analysis involved four institutions: the Korea Institute of Radiological and Medical Sciences, Dongnam Institute of Radiological and Medical Sciences, Korean Association for Radiation Application, and Radiation Health Institute of Korea Hydro & Nuclear Power. Following the irradiation of the hydroxyapatite sample, the recorded build-up was analyzed. To validate the reliability of the EPR dosimetry findings and enhance its operational performance, a hydroxyapatite dose-response curve was plotted and dosimetry was performed for a blind sample. The proficiency of each laboratory was assessed by employing an interlaboratory comparison methodology. This involved a comparative analysis of the measurement results by calculating the relative bias, z-score, and En value. The results submitted by the participating laboratories demonstrated satisfactory ratings for doses of 1.006, 3.999, and 6.993 Gy. Following the second intercomparison, efforts to optimize their hydroxyapatite-EPR dosimetry systems are underway in the participating laboratories. The current assessment of hydroxyapatite dose yielded the foundational data required to establish the parameters of dental dosimetry. In future, the third intercomparison experiment will be conducted for exploring other materials.

Office-Based Mitomycin-C Application to Prevent Postoperative Laryngeal Stenosis (술 후 후두협착을 예방하기 위한 외래에서의 Mitomycin-C의 반복 국소적용)

  • Background and Objectives: Topical administration of mitomycin-C (MMC) has been reported to reduce or delay scar formation in airway surgery. However, it is not infrequent to experience recurrent stenosis or adhesion of operative wound even after a meticulous MMC application during the laryngeal surgery. Therefore we aimed to evaluate the effectiveness of repeated postoperative MMC applications and the technical feasibility of MMC applications to the laryngeal wound at an outpatient clinic. Methods: We reviewed medical records of 13 consecutive patients who received office-based MMC applications after laryngeal airway surgery at Samsung Medical Center, Seoul, Korea. The patients were grouped into 3 categories according to the site of surgical wound and the purpose of MMC application; group I : supraglottic stenosis (n=5), group II : cordectomy and arytenoidectomy site granulation prevention (n=3), Group III : laryngeal web prevention (n=5). Outcomes in each group and adverse effects of repeated MMC applications were evaluated. Results: Office-based MMC application was successfully performed one to four times with a week interval for each patient. No significant complications were observed except slightly decreased mucosal wave in one female patient who received 4 times of MMC application at the anterior commissure of vocal fold. Repeated MMC applications at the outpatient clinic resulted in wide or acceptable supraglottic airway in group I, clean wound healing without granulation formation in group II, and negligible or no web formation at the anterior commissure in group III. Conclusion : Office-based topical administration of MMC to the larynx was technically feasible. Postoperative repeated MMC applications were effective to reduce recurrent stenosis or adhesion of supraglottic structures, to prevent granuloma formation after laser arytenoidectomy and glottic web formation after anterior commissure resection.

  • Absorption spectra of blood components have been measured for the purpose of predicting the total hemoglobin concentration. We obtained absorption spectra of major blood components from the visible to near-infrared of $400{\sim}2500nm$ region. In the near-infrared, water is the main absorbing constituent. The amount of water in the sample cell varies depending on the volume of solute concentration(water displacement). We acquired water-compensated spectra by considering the variation of water volume depending on the variation of analyze concentration. Those spectra show inherent absorption peaks of analyzes and linearity with respect to concentration. We selected wavelengths for non-invasive measurement of hemoglobin concentration considering the scattering effect of tissue and the interference of other blood components.

  • Researchers and industry managers highlight the emergence of mobile healthcare market given the rapid development of IT and the expansion of average life expectancy. Medical institutions, such as university hospitals, are aiming to achieve convenience and effectiveness of treatment by providing patient education through a mobile device. This study examines a case related to the development and usage of a mobile application based on Lean Canvas, namely, the Smart Explanation Service released by Samsung Medical Center. This application comprehensively analyzes the usage status of mobile application-based patient education and its application in clinical settings. This study also examines the level of satisfaction of patients and medical workers toward this application. The result of this study is expected to provide meaningful implication to medical institutions and related industries and enable them to create and offer prepare mobile healthcare services.

  • Health Monitoring System is a sophisticating technology and another way to the normal/regular management of the health of the patient. This Health Monitoring Mobile Application is a contribution from our side to the public and to the overall health industry in Pakistan. With the help of Health mobile application, the users will be able to store their medical records, prescriptions and retrieve them later. The users can store and keep track of their vital readings (heart rate, blood pressure, fasting glucose, random glucose). The mobile application also shows hospitals that are nearby in case the user wants to avail of any medical help. An important feature of the application is the symptoms-based disease prediction, the user selects the symptoms which he has and then the application will name certain diseases that match those symptoms based on relevant algorithms. The major advances and issues have been discussed, and as well as potential tasks to health monitoring will be identified and evaluated.
