  • The medical sector actively changes and implements innovative features in response to technical development and revolutions. Many of the most crucial elements in IoT-connected health services are safeguarding critical patient records from prospective attackers. As a result, BlockChain (BC) is gaining traction in the business sector owing to its large implementations. As a result, BC can efficiently handle everyday life activities as a distributed and decentralized technology. Compared to other industries, the medical sector is one of the most prominent areas where the BC network might be valuable. It generates a wide range of possibilities and probabilities in existing medical institutions. So, throughout this study, we address BC technology's widespread application and influence in modern medical systems, focusing on the critical requirements for such systems, such as trustworthiness, security, and safety. Furthermore, we built the shared ledger for blockchain-based healthcare providers for patient information, contractual between several other parties. The study's findings demonstrate the usefulness of BC technology in IoHT for keeping patient health data. The BDSA-IoHT eliminates 2.01 seconds of service delay and 1.9 seconds of processing time, enhancing efficiency by nearly 30%.

Mindfulness Meditation Combined with Digital Health for Medical Personnel at a Korean Medicine Hospital in South Korea: A Case Series

  • Objectives: Medical personnel are professionals subject to stressful situations and psychological distress. This case series reports the results of a mindfulness meditation program combined with digital health for medical personnel at a Korean medicine (KM) hospital. Methods: An online mindfulness program was implemented in 2022 to improve the mental health of nursing staff in a KM clinic. The online mindfulness program, which was supplemented based on previous results and limitations, was applied to KM doctors and nurses working at a KM hospital in this case series. An important difference from the existing case series was the introduction of a smartphone application that promoted the daily routine of meditation. A total of 7 medical personnel, including 4 doctors and 3 nurses, participated in the program. Results: After participating in the program, an increase in deep acting and a decrease in surface acting, which are aspects of emotional labor, were consistent with the results of a previous case series. However, the patterns of change in burnout and hwa-byung symptoms differed depending on the occupation of the participants. Participants' satisfaction with this program and the smartphone application and willingness to recommend it to colleagues were high. Conclusions: As this study was only a small case series, the author plans to continue to expand and improve the program based on the findings.

Effects of the Tele-Monitoring With the Speech-to-Text Application on Occupational Balance in Healthy Adults : Feasibility Study (음성-텍스트 변환 어플리케이션을 이용한 원격 모니터링이 건강한 성인의 작업균형에 미치는 효과)

  • Objective : The COVID-19 pandemic has brought non-face-to-face healthcare service delivery system. Research into telehealth system and its efficacy remains unclear. Methods : Seven healthy adults participated in this study to investigate effects of tele-monitoring with the speech-to-text (STT) application to induce changes in occupational activities on occupational balance in healthy adults. Subjects were requested to choose occupational activities they wanted to have researched and then register them to the STT application. The STT application provided an alarm to check whether the pre-registered activities were performed on time, and whether the subjects performed it by their voice. The subjects were followed for 1 week, with assessments at baseline, and after 1-week's tele-monitoring. Results : Our findings showed that the subjects were willing to participate in tele-monitoring with the STT application with high adherence and satisfaction. In addition, there was a significant improvement in occupational activities related to health (p<.05). Specifically, adherence, satisfaction, and efficacy of the tele-monitoring with the STT application could successfully bring occupational balance in short-term periods. Conclusion : These findings highlight that tele-monitoring with a smartphone could be considered as one promising way to restore occupational balance in lockdown after the COVID-19 outbreak.

  • Objectives : The purpose of this study is to suggest how to interpret the 'practice of Korean Medicine', differentiating it from 'medical practice'. Methods : I analyze the legislations and precedents regarding the practice of Korean Medicine. Results : The Korean Medicine and Pharmaceutics Promotion Act defines 'practice of Korean Medicine' and it clearly differentiates it from the definition of 'medical practice'. However, the scope of this definition is somewhat restricting and it can violate doctors of Korean Medicine's right to equality and their academic freedom. Thus, the application of this definition of the 'practice of Korean Medicine' should be limited to the field of research and development. Meanwhile, criteria of distinguishing 'practice of Korean Medicine' from 'medical practice', which used to make a sharp distinction between Medicine and Korean Medicine by rigorously applying their academic standards, are now focusing more on protecting and improving health of the people. Discussions & Conclusions : I suppose that the distinction between the 'practice of Korean Medicine' and 'medical practice' will be more focused on public health rather than the academic stance of those two medical fields. Meanwhile, in accordance with dualistic medical system, the mutual usage of medical equipment in the area of 'treatment' should be limited while it should be allowed in the area of 'diagnosis' if it satisfies requirements suggested by the Constitutional Court.

A Study on the Research Methodology in Korean Medical Classics - Focused on Abductive Reasoning - (원전학(原典學) 연구방법론에 대한 고찰 - 귀추적 추론을 중심으로 -)

  • Objectives : In this paper, characteristics of research methodology used in the field of Korean Medical Classics and its application was studied, with a focus on abductive reasoning that takes place in such methodology. Methods : First, the properties of the Korean Medical knowledge system, production, circulation and consumption of Korean Medical knowledge, methodology of knowledge production, reasoning of hypothesis, Medical Classics research methodology and its examples were examined. Afterwards, the relationship between Medical Classics research and Korean Medical Doctors's competence was studied. Results : The knowledge system of Korean Medicine, formed by a knowledge production group changes continuously not unlike a living organism. Knowledge is produced through Sang (象) within human consciousness that lies in an existential relationship between the knowledge producer and subject, through means of abductive reasoning. Conclusions : Creative knowledge production through abductive reasoning in the field of Korean Medical Classics will hopefully contribute to production of highly useful knowledge in clinical settings, complement and make change in the current Korean Medical knowledge system. Various teaching methods based on this research methodology will contribute to strengthening Korean Medical Doctors's competence as well.

Factors affecting intent to use of Respiratory management mobile application (호흡기질환 관리 모바일 애플리케이션 사용의향에 미치는 요인)

  • Purpose: To analyze the results of an online survey conducted to develop a user-friendly respiratory disease management mobile application. Methodology: The questionnaires were conducted from July 26, 2018 to October 23, 2018 for 90 days. A total of 267 respondents were used for the analysis. Chi-square test, t-test, and multivariate logistic regression were used for statistical analysis. Findings: As a result of the analysis, preference for functions related to medical services was high in all ages and the odds of positive intention to use respiratory disease management application was 4.76 times higher than 40 years old compared with less than 40 years old. The group with more than one effort was found to be significantly higher than those who did not. Practical Implication: It is expected that the functions derived from the main results in this study will be helpful for the effective health management of patients with chronic respiratory disease.

A Study on the Clinical Application of Intelligent Replenishment System of Automatic X-ray Film Processor Based on Film Density (자동현상 지능화 보충방식의 임상적응에 관한 연구)

  • To inquire its usefulness of the clinical application of intelligent replenishment system of automatic X-ray film processor based on film density, we processed the serial 300 sheets of radiographic film of chest [$14{\times}14"$, HR-C type] and bone [elbow & ankle($8{\times}10"$), skull($10{\times}12"$), hand & foot($11{\times}14"$), pelvis($14{\times}17"$), HR-G type, 68, 70, 77, 85 sheets respectively]. We analyzed the characteristic corves, relative speeds, average gradients and base plus fog densities every twenty five sheets. We also evaluated the developer and fixer replenishment volumes every that time. In the chest and bone radiograph two all, the characteristic curves were little change, and the relative speeds, average gradients and base plus fog densities were within the maximum control limits. The average developer replenishment volumes were about 43m1/sheet and 39m1/sheet respectively. It brings decreased results about 29% in comparison with the conventional replenishment system. In our experiences, we conclude that the intelligent replenishment system of automatic X-ray film processor based on film density maintains image quality consistently, decreases also the replenishment volumes. Therefore, this system will be resulted in economic and environmental effects, and solve problems of over and low replenishment volume.

Reduction of inappropriate revisits to the emergency department 72 hours after being discharged by 'Discharge explanation report' ('퇴원설명문'에 의한 72시간내 부적절한 응급센터 재방문의 감소)

  • Background : Patients who were discharged from the emergency department(ED) may revisit. These patients are divided into two groups; one is expected scheduled condition, the other is unexpected condition. These patients of inappropriate revisits to the ED would be unsatisfied, difficult to make rapport and take legal action as a result of additional medical charges. The purpose of this study was to reduce inappropriate revisits to the ED with a new method which was developed by analyzing inappropriate revisits in 2002. Methods : This study was conducted in a tertiary hospital consisting of 1,278 beds. The most common cause of inappropriate revisits was the medical team's lack of explanation about a disease. Thus we decided that the effective method was to offer full explanations to patients to understand the clinical pathway of a disease. We made four types of stickers to explain most common 4 diseases in 2003. An emergency physician completed 'discharge explanation report' and explained it to patients in 2004. Results : In 2002 inappropriate revisited patients were 164, patients with four diseases were 79. During the same period of 2003, inappropriate revisited patients were 56 (-65.9%), four disease patients were 6 (-92.4%) and in 2004 inappropriate revisited patients were 52, four disease patients were 19. Causes of revisits were lack of explanation about a disease in 35 patients (44.3%) in 2003, and 5 patients (83.3%) in 2003, and 16 patients (84.2%). Conclusions : Application of 'explanation stickers' at discharge reduced inappropriate revisits from 34.5% in 2002 to 15.9% in 2003. Application of 'Discharge explanation report' by emergency physician reduced inappropriate revisits from 15.9% in 2003 to 13.5% in 2004. Reduction of inappropriate revisits elevated the quality of medical treatment, and decreased patients' dissatisfaction in ED.
