Background: Achieving external access to and manual occlusion of the left atrial appendage (LAA) during minimally invasive mitral valve surgery (MIMVS) through a small right thoracotomy is difficult. Occlusion of the LAA using an epicardial closure device seems quite useful compared to other surgical techniques. Methods: Fourteen patients with atrial fibrillation underwent MIMVS with concomitant surgical occlusion of the LAA using double-layered endocardial closure stitches (n=6, endocardial suture group) or the AtriClip Pro closure device (n=8, AtriClip group) at our institution. The primary safety endpoint was any device-related adverse event, and the primary efficacy endpoint was successful complete occlusion of blood flow into the LAA as assessed by transthoracic echocardiography at hospital discharge. The primary efficacy endpoint for stroke reduction was the occurrence of ischemic or hemorrhagic neurologic events. Results: All patients underwent LAA occlusion as scheduled. The cardiopulmonary bypass and aortic cross-clamp times in the endocardial suture group and the AtriClip group were 202±39 and 128±41 minutes, and 213±53 and 136±44 minutes, respectively (p=0.68, p=0.73). No patients in either group experienced any device-related serious adverse events, incomplete LAA occlusion, early postoperative stroke, or neurologic complication. Conclusion: Epicardial LAA occlusion using the AtriClip Pro during MIMVS in patients with mitral valve disease and atrial fibrillation is a simple, safe, and effective adjunctive procedure.

Background: In Korea, the health gap widens due to the number of medical resources and access to medical services between metropolitan and rural. The purpose of this study is to identify the impact of residential migration on medical utilization and accessibility. Methods: This study extracted 528,516 claimed cases in the National Health Insurance Service-Cohort Sample Database from 2006 to 2015. Subjects were classified into two groups by the magnitude of the region, the metropolitan and the rural. The inversed probability weights were calculated for each group. And coefficients of the two-part model were estimated by generalized estimation equation. Results: Those who moved region from metropolitan to rural tend to increase the length of stay and inpatients with ambulatory care sensitive conditions (ACSC) disease. Contrariwise, those who moved areas from rural to metropolitan tend to decrease the total medical cost, the adjusted patient days, the number of outpatients and the number of outpatients and inpatients with ACSC disease. Conclusion: This study identified that between the residents who continued to reside in the region and the migrants, there were significant differences in the medical accessibility, quality of primary care, and unmet medical need.
Objectives : Galgunhaejutang extract (GHT) is a Traditional Korean medical herbal mixture that has been tradtionally used to treat alcohol consumption. This study was preformed to evaluate the effects of GHT on alcohol consumption in C57BL/6 mice. Methods : Sixty three C57BL/6 mice were alcohol dependence-induced by limited access paradigm. Water, GHT 0.688 g/kg (GHT-L), or GHT 3.45 g/kg (GHT-H) were administrated for 10 days. The amounts of alcohol consumption for 2 hours, water consumption for 22 hours, food intake for 24 hours and body weight were measured. Results : There weren't significant differences in 2 hours of alcohol consumption, 22 hours of water consumption, 24 hours of food intake and body weight for ten days between vehicle group and GHT-L or GHT-H group. Conclusions : Further studies employing multi-dose and long term administration of GHT (more than 10 days) might be of benefit.

The Recently there has been a growing interest in health care due to the COVID-19 situation. In this paper, we intend to develop a healthcare monitoring system to provide users with smart healthcare systems in line with the healthcare 3.0 era. The system consists of a wireless network between various sensors, Android smartphones, and OLEDs using Bluetooth, and through this, a health care monitoring system capable of collecting user's biometric information and managing health by receiving data values of sensors connected to Arduino. In conclusion, the user's BPM value was calculated using the heart rate sensor, and the exercise intensity can be adjusted through this. In addition, a step derivation algorithm is implemented using an acceleration sensor, and calorie consumption can be measured using the step and weight values. As such, the heart rate, step count, calorie consumption data can be transmitted to a smartphone application through a Bluetooth module and output, and can be output to an OLED for users who are not easy to access the smartphone. This healthcare monitoring system can be applied to various groups and technologies.
Objective: The purpose of this study was to access quality of life and evaluate pre and post surgery management for persons with lower limb amputations. Method: This study was designed as a telephone survey. The subjects were patients who underwent their lower limb amputation between January 1994, and February 2005 at Asan Medical Center in seoul. sixty one of 203 subjects had granted consent and were studied. Results: The major cause of below knee amputations was vascular disease. Traumatic injuries were more common among subjects with above knee amputations. 93.4% of subjects were in use of prosthesis and 70.1 days were required to fit the prosthesis. 68.4% of subjects were able to ambulate with single point cane or without assistive devices. 80.3% of subjects complained phantom pain. Most subjects expressed the unsatisfactory result regarding the functional usage of prosthesis and education from hospital. Conclusion: Pre and post surgery rehabilitation program will benefit to promote better functional status and quality of life for persons with lower limb amputations.
For the patients' privacy, secure access control of medical images is essentially necessary. In this paper, two types of Region of Interest (ROI)-based selective encryption schemes are proposed, which concentrate on the security of crucial parts in medical images. The first scheme randomly inverts the most significant bits of ROI coefficients in several high frequency subbands in the transform domain, which only incurs little loss on compression efficiency. The second scheme employs a symmetric key encryption to encrypt selectively the ROI data in the final code-stream, which provides sufficient confidentiality. Both of two schemes are backward compatible so as to ensure a standard bitstream compliant decoder so the encrypted images can be reconstructed without any crash.

The development of health information technology enables people to access, view and acquire personal health record. But still, there have been a number of obstacles such as the absence of the standard to realize the ideal Personal Health Record(PHR) system. In this study, we proposed the service model that serves periodic Health Record Summary which is made by a medical specialist to people who are in the busy lives. Healthcare data from EMR in a hospital including people generate themselves at home is sent to a physician to make a medical opinion, and then it is changed into Health Level 7 Continuity of Care Document(CCD) format for interoperability. After a physician writes his opinion about patient's health condition, it will send to people by email. People who receive the health record summary data by email can save them into a USB device to view own PHR and medical comments of a physician through a computer. It will help people managing their own health condition with an opinion of a medical specialist.
Objectives : This study aims to : (i) characterize and differentiate between two different periods of baby boomers, (ii) study the utilization of their health care services, and (iii) establish effective ways of providing better health service utilization and preventive policy strategies for upcoming and older generations. Methods : A multiple regression analysis using descriptive statistics, frequency analysis, and dummy variables was utilized to access the presence of correlations between socio-demographic factors and health care service utilization. Results : Medical insurance type, marital status, and chronic disease were factors that influenced health care service utilization. Furthermore, the factors that influenced individual medical expenses were cohabitation, inpatient days, and chronic disease. Conclusions : Primary findings and exploratory statistics revealed that there were strong correlations and interaction among some of the predictor variables. Because of the chronologically limited nature of the sample data set gathered in 2012, it would be helpful to continue to develop or research related constructs that may capture relationships more effectively among extended populations.
Ma Gwa Hwe Tong, or 麻疹會通 is a general medicinal classic on measles which was an attempt to cope with the situation in the later Joseon Dynasty, when measles was prevailing all throughout the country. It was written in 1798 by Jung Yak Yong, who was a famous realist and a distinguished scholar, and in one of the chapters called Oh Gyun Pyun shows his thoughts. What he is trying to do through his writings, Ma Gwa Hwe Tong, was not reforming the current medicinal studies but to organize the classics on measles that he had access to with a system in order. The progressive thoughts shown in his other book, Eui-Ryung or 醫零, should be considered as his own thoughts only, and it is an overstatement to consider the book of Ma Gwa Hwe Tong to be his tryout in such thoughts.
