Development of a Health Bicycle for Improving the Muscle Strength of Lower Limb using MR Rotary Brake (MR 회전형 브레이크를 이용한 하지 근력 증진용 헬스 자전거 개발)

  • In this study, a new bicycle system was developed to improve muscular strength using the Magneto-Rheological(MR) rotary brake. The friction load of the MR rotary brake is adjusted according to muscle strength of the subjects. The characteristic of muscular strength was studied with various friction loads of MR rotary brake. The friction load was occurred with the current, applied to the MR. rotary brake. Experiments was composed of several cycling trials for various friction loads. In training programs involving muscle improvement, it is necessary to confirm muscle activity and fatigue. To measure the muscle activity and fatigue, EMG signals of rectus femoris (RF), biceps femoris (BF), tensor fasciae latae (TFL), vastus lateralis (VL), vastus medialis (VAS), gastrocnemius (GAS), tibialis anterior (TA) and soleus (SOL) muscles were collected with surface electromyography and analyzed into time and frequency domain. The experimental results showed that the muscle activity according to the applied current to the MR rotary brake was significantly different. The more the current was applied, the higher value of the integrated EMG (IEMG) was obtained. Especially, the magnitude of IEMG of the RF, BF, TFL and VL varied in direct proportion to the current. However, there was not significant in the median frequency as the cycling time continue.

Tumor Size as a Prognostic Factor in Gastric Cancer Patient

  • Purpose: The purpose of this study is to investigate the prognostic significance of tumor size for 5-year survival rate in patients with gastric cancer. Materials and Methods: A total of 1,697 patients with gastric cancer, who underwent potentially curative gastrectomy, were evaluated. Patients were divided into 4 groups as follows, according to the median size of early and advanced gastric cancer, respectively: small early gastric cancer (tumor size ${\leq}3$ cm), large early gastric cancer (tumor size >3 cm), small advanced gastric cancer (tumor size ${\leq}$ 6 cm), and large advanced gastric cancer (tumor size >6 cm). The prognostic value of tumor size for 5-year survival rate was investigated. Results: In a univariate analysis, tumor size is a significant prognostic factor in advanced gastric cancer, but not in early gastric cancer. Multivariate analysis showed that tumor size is an independent prognostic factor for 5-year survival rate in advanced gastric cancer (P=0.003, hazard ratio=1.372, 95% confidence interval=1.115~1.690). When advanced gastric cancer is subdivided into 2 groups, according to serosa invasion: Group 1; serosa negative (T2 and T3, 7th AJCC), and Group 2; serosa positive (T4a and T4b, 7th AJCC), tumor size is an independent prognostic factor in Group 1 (P=0.011, hazard ratio=1.810, 95% confidence interval=1.149~2.852) and in Group 2 (P=0.033, hazard ratio=1.288, 95% confidence interval=1.020~1.627), respectively. Conclusions: Tumor size is an independent prognostic factor in advanced gastric cancer irrespective of the serosa invasion, but not in early gastric cancer.

Development of Enzyme-Linked Immunosorbent Assay for Rapid and Sensitive Analysis of Biotin (Biotin의 분석을 위한 효소면역측정법(ELISA)의 개발)

  • In order to develop more rapid and reproducible analysis of biotin known as vitamin H, attempts were made to establish the condition for enzyme linked immunosorbent assay(ELISA) compared with traditional microbiological assay(MBA). Antibiotin and antiserum were obtained from the immunized rabbits injected with emulsion of biotin KLH conjugate and Freund's adjuvant. The antiserum showed cross reactivity on biocytin, a derivative of biotin, which is converted to biotin in intestine, at the rate of 177%(median inhibitory concentration(IC50)=12.58ppb), but not on other derivatives such as desthiobiotin, diaminobiotin and 2 imino biotin. Specific antibody for biotin was purified from the antiserum through protein A column and desalting column. The conditions of competitive direct ELISA (cdELISA) were established. Detection range of biotin concentration by cdELISA was 0.01∼300ng/ ml(ppb). In the spike test with milk, fruit flake and pine carrot juice, the correlation coefficient between two methods of MBA and ELISA was reliably consistent at the value of r=0.992. But detection of biotin by microbiological assay(MBA) was rather restricted in range and nonspecific. Detection range of biotin by MBA was 0.1∼0.5ng/ml(ppb). It showed cross reactivities on biocytin and desthiobiotin at the rate of 80.1% and 66.7%, respectively. In conclusion, ELISA revealed a significant improvement compared with MBA for the biotin detection in terms of sensitivity, detection range and cross reactivity. In addition, a variety of samples could be analyzed rapidly and conveniently at one time by using ELISA. These results strongly suggest that the ELISA is very promising for the practical application to detect biotin contents in a wide range of food stuffs.

The Relationship between Clinical Sign and Radiographic Findings in Temporomandibular Disorders (측두하악장애의 임상적 증상과 방사선적 소견과의 관계)

  • 66 patients with temporomandibular disorders were selected for experimental group, and 45 normal subjects who were Dental students were selected for control group. Average age of experimental group was 30.5 years, Male to Female ratio was 2 : 3, and their age distribution were teen-ages to seventh decades. Transcranial radiography (TR) with Denar Accurad 100 was used for each group to get the values of width in joint spaces and to investigate the bony changes of articular surfaces and relative condylar position to articular fossa. In addition to TR, clinical interview and routine charting about amount of mandibular movements and occlusal variations were carried out in experimental group. The obtained results were as follows : 1. The mean values of joint space with in control group were 2.15mm to anterior, 2.98mm to superior and 2.29mm to posterior and the value of relative condylar position to the deepest portion of articular fossa was 0.21mm to anterior. In experimental group, those values were 2.01mm, 2.14mm 2.22mm and 0.12mm to posterior in sequence, respectively. Joint spaces in experimental group, therefore, were inclined to decreased, and relative condylar position was inclined to retrude. Joint space in control group showed symmetric condylar position, but in experimental group showed asymmetry. 2. Non-affected joints with no bony changes in experimental group showed the narrowest joint spaces which were thought to manifest the abnormal stress to non-affected side to dysfunctional state of contralateral affected joints. 3. Amount of mandibular movements in experimental group were within normal values in lateral movements and in protrusive movement but in opening movement with or without passive stretch, those were lower than normal values. Frequency of occlusal variation, for example, protrusive posterior contacts, open bite, median line shift to lateral side were inclined to increase with bony changes and with crepitus.

DETECTION AND RESTORATION OF NON-RADIAL VARIATION OVER FULL-DISK SOLAR IMAGES

  • Full-disk solar images are provided by many solar telescopes around the world. However, the observed images show Non-Radial Variation (NRV) over the disk. In this paper, we propose algorithms for detecting distortions and restoring these images. For detecting NRV, the cross-correlation coefficients matrix of radial profiles is calculated and the minimum value in the matrix is defined as the Index of Non-radial Variation (INV). This index has been utilized to evaluate the H images of GONG, and systemic variations of different instruments are obtained. For obtaining the NRV's image, a Multi-level Morphological Filter (MMF) is designed to eliminate structures produced by solar activities over the solar surface. Comparing with the median filter, the proposed filter is a better choice. The experimental results show that the effect of our automatic detection and restoration methods is significant for getting a flat and high contrast full-disk image. For investigating the effect of our method on solar features, structural similarity (SSIM) index is utilized. The high SSIM indices (close to 1) of solar features show that the details of the structures remain after NRV restoring.

Evaluation of the Fatigue Life for Carbon/Epoxy Composite Material by the Residual Strength Degradation Analysis (탄소섬유/에폭시 복합재료의 잔류강도 저하해석에 의한 피로수명 평가)

  • Fatigue tests have been carried out to measure the degradation of the residual strength and the fatigue life in carbon/epoxy (0/45/90/-45)$_{2s}$ composite materials. Theoretical predictions of residual strength and fatigue life were compared with experimental results. Distribution characteristics were studied using the probability of failure based on the cumulative distribution function and median rand. The static ultimate strength of carbon/epoxy composites used herein is observed to be relatively higher than that of existing similar composites ; while fatigue life is shorter due to the brittleness of matrix. The fatigue life obtained in these experiments is shorter than that estimated by residual strength degradation model when the stress level above 0.6 For the stress level of 0.6, the experimental value was abruptly increased. The cumulative distribution function for the static ultimate strength is well correlated to that for the strength converted from the measured fatigue life. Also, the predicted distribution of residual strength shows good agreement with the experimental results. Therefore, it is proven that the residual strength degradation model is reasonable.e.

Utility of FDG PET/CT Scans on Extracapsular Spread of Squamous Cell Carcinoma in the Larynx (후두 편평상피암종의 림프절 피막외 침범에 대한 FDG PET/CT 영상의 유용성)

  • Backgrounds : To evaluated the use of FDG PET/CT for the identification of extracapsular spread(ECS) with histologic correlation in laryngeal cancer. Methods : We reviewed 79 medical records of patients who underwent of FDG PET/CT for laryngeal cancer before surgery. Results : ECS was present in 41.9%(18/43) dissected necks and in 34.5%(20/58) dissected cervical levels. There was a significant difference in the SUVmax between cervical lymph nodes with ECS and without ECS($6.39{\pm}4.53$ vs. $1.19{\pm}1.64$, p<0.001). The cut-off value for the SUVmax for differentiating with ECS from without ECS was 2.8 with the sensitivity of 85.7% and the specificity of 85.6%. Conclusion : The median SUVmax cut-off values of FDG PET/CT higher than 2.8 was associated with greater risk cervical lymph node metastasis with ECS in patients with laryngeal cancer.

Analysis of Variability Factors in Establishing Pesticide Residue Limits on Food Crops (농산물중 농약잔류 허용기준 설정시의 변이계수 분석)

  • In establishing legal limits of pesticide residues, various default values are employed to compensate for unavoidable variability in data. Because permissible errors in residue analysis reach ${\pm}30-40%$ RSD, maximum residue limits (MRLs) follow geometric progression. According to 5-yr period JMPR reports, variability factor (ratio of highest to median values) in field residue trials was 3.8-fold in 486 crop-pesticide combinations and round-up effort from highest residue to MRL was 1.5-fold, whereas regulatory margin (ratio of MRL to highest residue) used in Korea was 4.8-fold in 822 crop-pesticide combinations; Korean MRLs will be set at higher levels as compared with Codex limits if these margins are employed. Validation studies to compare and harmonize Korean and Codex MRLs of pesticide residues on food crops should be undertaken.

Phase Angle Using Bioelectrical Impedance Analysis for Predicting Survival in Patients with Metastatic Cancer (전이암 환자의 예후 판단 인자로서 생체전기 임피던스 분석을 이용한 위상각의 역할)

  • Objectives: The aim of this study was to identify the validity of phase angle as a prognostic factor for survival in patients with metastatic cancer. Methods: Data of patients with metastatic cancer who visited the Korean medicine cancer center at Kyung Hee University Hospital in Gangdong from April 2016 to February 2018 were collected for this study. The values of phase angle (PhA) and blood hemoglobin (Hb), C-reactive protein (CRP), and serum albumin levels were also investigated by bioelectrical impedance analysis (BIA). Results: In total, 86 patients were analyzed. The cut-off value of the phase angle was determined as 5.0. Univariate and multivariate analysis revealed that phase angle (HR 3.15, 95% CI 1.79-5.54) and CRP (HR 2.15, 95% CI 1.24-3.73) predicted survival with statistical significance. The median overall survival of the low PhA group ($PhA{\leq}5.0$) was 1.8 months (95% CI 0.6-2.9 months) and 7.2 months (95% CI 5.2-9.2 months) in the high PhA group (PhA>5.0)(p<0.001). Conclusions: The phase angle could be an independent prognostic factor for patients with metastatic cancer. Further research is required to confirm these findings and their correlation with other indexes.

Preliminary Risk Assessment of Several Major Pharmaceutical Products In Aquatic Ecosystem

  • Acute toxicities of five pharmaceutical products were evaluated with aquatic microbes, invertebrates, and fish. The test pharmaceuticals, i.e., cimetidine, carbamazepine, diltiazem, acetaminophene, and metformin have been often detected in aquatic environment, but theire cological hazard on receptors of various trophic levels has seldom been evaluated. In the present study, we conducted acute toxicity assays with a marine bacterium, Vibrio fischeri, an invertebrate, Daphnia magna, and a fish, Japanese medake (Oryzias latipes). In general, D. magna, showed the most sensitive response to the test chemicals. Diltiazem exhibited the lowest EC50 value after 96 hr of exposure at 7.6 mg/L, followed by cimetidine >acetaminophen > metformin = carbamazepine in an order of decreasing susceptibility. With the fish, diltiazem and carbamazepine showed the 96 hr EC50 values at 14.1${\sim}$35.4 mg/L while acetaminophen, cimetidine, and metformin did not cause 50% mortality at 100 mg/L. Similar pattern was noted with the Microtox Assay, with which the median effective concentrations for acetaminophen, cimetidine, and metformin were found at the range between 301.8 and 755.4 mg/L. Carbamazepine and diltiazem exposure to the microbes resulted in EC50 values around 50 mg/L. Predicted no effect concentrations (PECs) of these pharmaceuticals derived from the EC5O values obtained from this study, and predicted environmental concentrations (PECs) obtained from available literatures were utilized to estimate ecological risks of the test compounds. No test pharmaceuticals resulted in risk quotients (PEC/PNEC) greater than 1, which suggests no serious potential ecological concerns. It should be noted however that further studies including the refinement of PEC derivation, identification and toxicity assessment of the metabolites and/or their interactions with other stressors may be warranted to better understand the environmental consequences of the residual pharmaceutical discharge to the waterway.
