강한 손바닥 압박 후 속발된 엄지두덩운동신경병증 1예 (A Case of Strong Palm Compression-Induced Thenar Motor Neuropathy)

  • Thenar motor neuropathy (TMN) is a compressive mononeuropathy of recurrent motor branch of median nerve. It is infrequent and may have different pathogenesis. It may be a unique entity of disease or considered a variant of carpal tunnel syndrome involving the motor branch only. We report a case of TMN induced by vigorous massage that applied strong digital pressure in the region of the base of palm and thenar muscles.

손목터널증후군 진단 시 F파의 유용성에 관한 연구 (Study on the Validity of F wave for Diagnosis of Carpal Tunnel Syndrome)

  • 본 연구는 초기 손목터널증후군으로 진단된 환자의 정중신경(median nerve)과 자신경(ulnar never)의 및 F파를 대조군과 비교 분석하여 초기 손목터널증후군의 진단에서 F파가 유용한 지표가 될 수 있는지 알아보고자 하였다. 신경전도검사 결과의 주요 지표인 말단잠복기, 말단진폭, 손바닥-손목 구간의 감각신경전도속도 및 F파 잠복기를 대조군과 t-test 검정으로 비교분석하고, 또한 주요 지표간의 상관성 분석을 위하여 상관분석을 실시하였다. 연구결과, 대조군과 초기 손목터널증후군 환자의 정중신경 말단잠복기 비교에서 두 군 간에 유의성이 높은 것으로 나타났다(p<0.001). 정중신경의 감각신경전도속도 비교에서는 대조군과 손목터널증후군 간의 유의성이 높은 것으로 관찰되었다(p<0.001). 후기반응검사인 F파 잠복기 분석에서 정중신경의 경우 대조군과 손목터널증후군간 높은 유의성(p<0.001)을 보였으나, 자신경의 경우는 차이가 없는 것으로 나타났다. 상관분석 결과는 감각신경전도속도와 말단잠복기, F파와 말단잠복기에서 모두 상관성이 있는 것으로 나타났다. 이러한 결과로, F파는 말단잠복기, 감각신경전도속도와 같이 손목터널증후군의 진단에 있어 유용한 지표가 될 수 있다고 사료된다.

Rare Tumors Causing Median Nerve Compression in Adults-A Narrative Review

  • The median nerve can be compressed due to a tumor along the course of the median nerve, causing typical compression symptoms or even persistence or recurrence after an operation. The aim of this review is to provide a comprehensive overview of rare tumors described in recent publications that cause median nerve compression and to evaluate treatment options. The PubMed, Embase, and Web of Science databases were searched for studies describing median nerve compression due to a tumor in adults, published from the year 2000 and written in English. From 94 studies, information of approximately 100 patients have been obtained. Results The rare tumors causing compression were in 32 patients located at the carpal tunnel, in 21 cases in the palm of the hand, and 28 proximal from the carpal tunnel. In the other cases the compression site extended over a longer trajectory. There were 37 different histological types of lesions. Complete resection of the tumor was possible in 58 cases. A total of 8 patients presented for the second time after receiving initial therapy. During follow-up, three cases of recurrence were reported with a mean follow-up period of 11 months. The most common published cause of median nerve compression is the lipofibromatous hamartoma. Besides the typical sensory and motor symptoms of median nerve compression, a thorough physical examination of the complete upper extremity is necessary to find any swelling or triggering that might raise suspicion of the presence of a tumor.

수근관증후군 뇌졸중 환자에서 경직정도에 따른 정중신경 단면적과 신경전도속도 및 상지기능의 비교 (Carpal Tunnel Syndrome in Stroke Patients According to the Degree of Spasticity in Median Nerve Cross-Sectional Area and Nerve Conduction Velocity and Comparison of Upper Extremity Function)

  • 본 연구의 목적은 수근관증후군 뇌졸중 환자에서 경직정도에 따른 정중신경 단면적, 신경전도속도 및 상지기능 차이를 알아보고자 하였다. 연구대상은 성인 뇌졸중 환자 42명에서 CTS군 21명과 Non-CTS군 21명으로 선정하였다. 측정방법으로는 정중신경 단면적, 신경전도속도, GST, FMAS, CTS-FSS로 측정하였다. 연구결과, CTS군과 Non-CTS군 간 각 등급에서 정상측(p<.001)과 마비측(p<.001)의 정중신경 단면적, 정중 운동신경과 감각신경 기시잠시는 통계학적으로 유의한 차이가 있었다. CTS군과 Non-CTS군 간 각 등급에서 GST(p<.05), FMAS(p<.05), CTS-FSS(p<.001)는 통계학적으로 유의한 차이가 있었다. 본 연구는 수근관에 대한 병리역학적 내용을 제시함으로서 뇌졸중 환자의 상지기능훈련 시 고려해야할 내용 중 하나임을 제시하였다.

초음파에 의한 전완 국소 온도와 정중 운동 신경전도 속도의 변화 (Alteration of Forearm Local Temperature and Median Nerve Conduction Velocity by Therapeutic Ultrasound in Healthy Adult Subjects)

  • PURPOSE: Previous studies have documented the lack of ultrasound's non-thermal effects on nerve conduction using frequencies of 1 MHz and 870 kHz. The purpose of this study was to determine the biophysical effects of continuous ultrasound on median local forearm temperature and motor nerve conduction velocities using frequencies of 3.0 MHz. SUBJECTS: Twelve healthy subjects (6 males, 6 females, age $22.30{\pm}2.41$ yrs, weight $61.33{\pm}10.16$ kg, height $167.58{\pm}8.04$ cm) without a history of neurological or musculoskeletal injury to their dominant arm volunteered for this study. METHODS AND MATERIALS: Each subject received a total of five treatments, one each at .0, 0.5, 1.0, 1.5, 2.0 W/$cm^2$ of 3 MHz continuous ultrasound on the anterior surface of the middle area of dominant forearm for 10 minutes. Dependent measures for forearm local temperature and median motor nerve conduction velocity (MNCV) were taken pretreatment and immediately post-treatment. One-way ANOVA were used for each dependent measure. RESULTS: The posttreatment forearm local temperature were differed significantly (p<0.001) between intensities of ultrasound. The posttreatment forearm local temperature of the ultrasound treated with 1.0 w/$cm^2$, 1.5 w/$cm^2$ and 2.0 w/$cm^2$ were significantly higher than 0.5 w/$cm^2$ and 0.0 w/$cm^2$ of ultrasound (p<0.05). The posttreatment median MNCV were differed significantly from the respective pretreatment velocities (p<0.001). The MNCV of the ultrasound treated with 0.0 w/$cm^2$ and 0.5 w/$cm^2$ were significantly (p<0.05) slower than that observed pretreatment, while the three ultrasound intensities produced significantly increased posttreatment MNCV: 1.0 w/$cm^2$ and 1.5 w/$cm^2$ and 2.0 W/$cm^2$. The posttreatment MNCV at 2.0 w/$cm^2$ and 1.5 w/$cm^2$ was significantly faster than that at 0 w/$cm^2$, 0.5 w/$cm^2$ and 1.0 w/$cm^2$ (p<0.05), the MNCV at 1.0 w/$cm^2$ was significantly faster than that associated with 0 w/$cm^2$ and 0.5 w/$cm^2$ of ultrasound (p<0.05). CONCLUSIONS: The decreased median motor forearm local temperature and MNCV of the ultrasound treated with 0.0 w/$cm^2$ and 0.5 w/$cm^2$ were attributed to the cooling effect by ultrasound transmission gel. Local forearm temperature and nerve conduction velocity were directly related to the intensity of ultrasound. Alterations in MNCV from ultrasound on healthy nerves appeared to be related to temperature changes induced by thermal effects of ultrasound.

Normal data on axonal excitability in Koreans

  • Background: Automated nerve excitability testing is used to assess various peripheral neuropathies and motor neuron diseases. Comparing these excitability parameters with normal data provides information regarding the axonal excitability properties and ion biophysics in diseased axons. This study measured and compared normal values of axonal excitability parameters in both the distal motor and sensory axons of normal Koreans. Methods: The axonal excitability properties of 50 distal median motor axons and 30 distal median sensory axons were measured. An automated nerve excitability test was performed using the QTRACW threshold-tracking software (Institute of Neurology, University College London, London, UK) with the TRONDF multiple excitability recording protocol. Each parameter of stimulus-response curves, threshold electrotonus, current-voltage relationship, and recovery cycle was measured and calculated. Results: Our Korean normal data on axonal excitability showed ranges of values and characteristics similar to previous reports from other countries. We also reaffirmed that there exist characteristic differences in excitability properties between motor and sensory axons: compared to motor axons, sensory axons showed an increased strength-duration time constant, more prominent changes in threshold to hyperpolarizing threshold electrotonus (TE) and less prominent changes in threshold to depolarizing TE, and more prominent refractoriness and less prominent subexcitability and superexcitability. Conclusions: We report normal data on axonal excitability in Koreans. These data can be used to compare various pathological conditions in peripheral nerve axons such as peripheral neuropathies and motor neuron disease.

수근관 증후군 치료결정의 한 요인으로 연령의 중요성 (The Importance of Age as a Factor of Carpal Tunnel Syndrome management)

  • Background : Carpal tunnel syndrome(CTS) is the most common entrapment neuropathy that refers to a group of signs and symptoms resulting from compression of the median nerve at the wrist. The course of CTS in older patients is different from the younger patients. This difference may be the result of different underlying mechanisms. The different nerve conduction studies of CTS may signify different approaches in management. This study was done to assess the differences in nerve conduction study of CTS in younger and older patients. Methods : This study involved 224 patients who visited Gachon Medical School, Gil Medical Center and was diagnosed by nerve conduction study from October 1997 to October 1999. We compared the results of nerve conduction study to age, especially in between those under 60 years and those 60 years or over CTS patients. Nerve conduction study consists of motor studies of both median nerves(terminal latency, compound action potential) and sensory studies(nerve conduction velocity, nerve action potential). And we also evaluated the variables between younger and older patients group. Those variables include sex, symptom period, laterality, abnormal physical findings and radiculopathy. Results : We found that a significant increase of terminal latency(p<0.1), but a decrease in compound motor action potential(p<0.05) in older patient's group. There was no significant differences in sensory nerve conduction velocity and action potential between those under 60 years and those 60 years or even patients. And also there was no significant difference in sex, symptom period, laterality, abnormal physical findings, radiculopathy between older and younger patients. Conclusions : This study showed a significant increase in the terminal latency and a decrease in compound action potential in older patients. The different nerve conduction studies of CTS by age effect may need different approaches in management.

일부 농약 폭로 농민들의 신경전도 검사에 관한 연구 (Nerve Conduction Velocity among Farmers Exposed to Pesticides)

  • 본 연구는 농약에 직업적으로 폭로되고 있는 농부들의 말초신경 기능을 평가하고자 농약살포 남성 농민 34명을 대상으로, 1998년 5월부터 1999년 2월에 걸쳐 실시하였다. 연구내용으로는 말초신경병변을 평가하기 위한 방법으로 현재 가장 많이 사용되고 있는 신경전도 검사를 우세 상하지의 청중신경, 척골신경, 비골신경, 후경골신경, 비복신경 대해 각 구간별로 나누어 실시하였다. 조사결과 조사대상자들은 평균 60세로서 농약살포 경력은 평균 33년이었다. 농사 유형은 주로 논, 밭, 고추 농사를 경작하고 있었으며 년간 평균 약 35일간 농약을 살포하였고 1일 살포시간은 약 9시간이었다. 또한 농약 중독 관련 증상을 경험한 적이 약 82%이상에서 보고되었다. 신경전도 검사상 조사원 모든 신정에서의 값이 정상범위에는 포함되었지만, 일부구간(척골감각 신경의 wrist-elbow, 후경골신경의 terminal latency)을 제외하고는 모두 참고치의 평균값보다 유의하게 낮은 것으로 나타났다. 또한 상대적 고폭로군과 저폭로군 간의 신경전도 검사치 비교에 의하면 정중감각 및 운동, 비골신경의 일부 구간 값이 고폭로군에서 높은 것을 제외하고는 유의한 차이를 발견할 수 없었다. 이것은 고폭로군의 연령이 저폭로군보다 유의하게 낮기 때문으로 판단되며, 조사 대상자들의 값이 참고치 평균보다 떨어진 것도 연령이 높음으로 인한 것임을 연령군별 비교에 의해 확인할 수 있었다. 따라서 향후 농약 폭로로 인한 미세한 말초신경 기의 이상여부를 조기에 파악하기 위해서는 감각역치 검사등 다론 신경학적 검사들을 함께 실시하는 것이 바람직하다고 판단되었다.

수근관증후군 수술 전후 임상증상과 전기생리학적 검사소견의 변화: 25손을 대상으로 한 예비연구 (Clinical and Electrophysiological Changes after Open Carpal Tunnel Release: Preliminary Study of 25 Hands)

  • Background: Electrophysiological study has been known as a useful method to evaluate the therapeutic effect of operation in idiopathic carpal tunnel syndrome (CTS). The purpose of this study was to evaluate the clinical and electrophysiological changes after carpal tunnel release (CTR) compared to the preoperative results. Methods: We analyzed the changes of nerve conduction study (NCS) before and after minimal open carpal tunnel release in 18 patients (25 hands) with CTS. Follow-up study was performed over 6 months after operation. Results: Clinical improvement was seen in all cases after CTR. In contrast, electrophysiological improvement was various depending on the parameters; the mean median sensory latency and nerve conduction velocity (NCV) improved significantly (p = 0.001). The mean median motor latency also improved, but NCV and compound muscle action potential (CMAP) amplitude did not change. The extent of improvement was evident in moderate CTS, but not in severe CTS. Conclusions: In this preliminary study, all subjects who underwent CTR achieved a clinical relief along with a significant improvement of electrophysiological parameters such as median sensory latency, sensory NCV and median distal motor latency. After CTR, a number of cases with mild to moderate CTS showed a prominent improvement of clinical and electrophysiological parameters, while fewer improvements were seen in severe CTS, although it did not reach the statistical significance.

Associations of nerve conduction study variables with clinical symptom scores in patients with type 2 diabetes

  • Background: Diabetic peripheral polyneuropathy (DPN) is associated with a variety of symptoms. Nerve conduction studies (NCSs) are considered to be the gold standard of nerve damage assessments, but these studies are often dissociated from the subjective symptoms observed in DPN patients. Thus, the aim of the present study was to investigate the correlations between NCS parameters and neuropathic symptoms quantified using the Michigan Neuropathy Screening Instrument (MNSI). Methods: Patients with type 2 diabetes mellitus (T2DM) with or without symptoms of neuropathy were retrospectively enrolled. Demographic data, clinical laboratory data, MNSI score, and NCS results were collected for analysis; DPN was diagnosed based on the MNSI score (${\geq}3.0$) and abnormal NCS results. Pearson's correlation coefficients were used to evaluate the relationships between MNSI score and NCS variables. Results: The final analyses included 198 patients (115 men and 83 women) with a mean age of $62.6{\pm}12.7$ years and a mean duration of diabetes of $12.7{\pm}8.4$ years. The mean MNSI score was 2.8 (range, 0.0-9.0), and 69 patients (34.8%) were diagnosed with DPN. The MNSI score was positively correlated with the median motor nerve latency and negatively correlated with the median motor, ulnar sensory, peroneal, tibial, and sural nerve conduction velocities (NCVs). When the patients were categorized into quartiles according to MNSI score, peroneal nerve conduction velocity was significantly lower in the second MNSI quartile than in the first MNSI quartile (p = 0.001). A multivariate analysis revealed that the peroneal NCV was independently associated with MNSI score after adjusting for age, sex, and glycosylated hemoglobin A1c (HbA1c) levels. Conclusions: The present results indicate that a decrease in peroneal NCV was responsible for early sensory deficits in T2DM patients.
