Simultaneous Medial and Lateral Discoid Meniscus in one Knee joint (한쪽 슬관절에서 내측 및 외측에 동시에 발생한 원판형 연골 - 1예 보고 -)

  • Most of discoid menisci are lateral and medial discoid meniscus is very rare. There is no report about simultaneous medial and lateral discoid meniscus in one knee joint. A 15-year-old male patient was diagnosed as having a complete medial discoid meniscus with horizontal tear and intact incomplete lateral discoid meniscus by means of magnetic resonance image and arthroscopy. The patient was treated by arthroscopic partial meniscectomy for both discoid menisci.

Study on the Acupoint Location of Sameumgyo (SP6) - Regarding the Theory of 8 Cun above the Tip of the Medial Malleoulus - (삼음교(SP6) 혈위에 관한 연구 - 족내과상 8촌설에 대하여 -)

  • Objectives : The objective of this study was to examine the acupoint location of Sameumgyo (SP6) from classic literatures. Methods : A review of SP6 acupoint location on 13 classics of acupuncture and moxibustion was performed. Results : Differences of SP6 acupoint location were found by classics of acupuncture and moxibustion. 9 classics of acupuncture and moxibustion said the acupoint of SP6 was located on 3 cun above the tip of the medial malleoulus, however acupoint location of SP6 was described on 8 cun above the tip of the medial malleoulus by 4 classics of acupuncture and moxibustion. Conclusions : Through a classic literature review, it was possible for the explanation that the location of SP6 was 8 cun above the tip of the medial malleoulus, but it was not intended to overturn the theory of 3 cun above the tip of the medial malleoulus regarding location of SP6. This study may be the starting point for future experimental or clinical studies for exploring new acupoint, and is expected to follow up.

Effects of Sphingosine-1-phosphate on Vestibular Nuclear Neurons

  • This study was designed to investigate the effects of sphingosine-1-phosphate on the neuronal activity of rat medial vestibular nuclear neurons. Sprague-Dawley rats aged 14 to 16 days were decapitated under ether anesthesia. After treatment with pronase and thermolysin, the dissociated medial vestibular nuclear neurons were transferred into a chamber on an inverted microscope. Spontaneous action potentials and potassium currents were recorded by standard patch-clamp techniques under current and voltage-clamp modes respectively. 15 medial vestibular nuclear neurons revealed excitatory responses to 1 and $5\;{\mu}M$ of sphingosine-1-phosphate. The spike frequency and resting membrane potential of these cells were increased by sphingosine-1-phosphate. The amplitude of afterhyperpolarization was decreased by sphingosine-1-phosphate. Whole potassium currents of medial vestibular nuclear neurons were decreased by sphingosine-1-phosphate (n=12). Sphingosine-1-phosphate did not affect the charybdotoxin-treated potassium currents. These experimental results suggest that sphingosine-1-phosphate increases the neuronal activity of the medial vestibular nuclear neurons by altering the resting membrane potential and afterhyperpolarization.

  • Meniscal cyst is an uncommon disease of knee joints, arising less frequently from medial meniscus than lateral. Most of they are accompanied with horizontal tear of meniscus. This is the case of 24 year old man who had 2 separate medial meniscal cysts with longitudinal tear on posterolateral side of medial meniscus without trauma. Treatment consisted of arthroscopic decompression of cysts and meniscal repair.

Four Species of the Family Erebidae (Lepidoptera), New to Korea

  • This paper reports four species of Erebidae from the southern part of Korea: two Boletobiinae, Honeyania ragusana (Freyer, 1845), Enispa masuii Sugi, 1982, one Hermininae, Adrapsa ablualis Walker, 1859, and one Arctiinae, Stictane rectilinea (Snellen, 1879). Honeyania ragusana can be distinguished by the following: a blackish dot on the apex of the forewing with strongly curved medial lines, a long slender uncus and a strongly distally bifurcated valva with a large distally expanded costa and a thumb-shaped process of sacculus on the male genitalia as well as a sclerotized antrum, strongly tapered ductus bursae, and large corpus bursae on the female genitalia. Enispa masuii, a lichen feeder, can be distinguished by the whitish medial lines of the grayish fore- and hindwings, and a pair of rounded ball-shaped lamella antevaginalis, long, sclerotized ductus bursae, and pear-shaped corpus bursae without signum of the female genitalia. Adrapsa ablualis can be distinguished by the whitish medial lines of the fore- and hindwings and large medial process of the costa and dentate distal margin of the valva of the male genitalia. Stictane rectilinea can be distinguished by the broad dark grayish medial band of the forewing and the long knife-shaped valva with a strongly hooked process on the distal part of the sacculus of the male genitalia.

Tibial Plateau Leveling Osteotomy Combined with Tibial Tuberosity Transposition in a Dog with Medial Patellar Luxation and Cranial Cruciate Ligament Rupture

  • A 30 kg, 6-year-old spayed female Samoyed dog was referred with a history of intermittent weight-bearing lameness in her right hindlimb for 3 weeks. The patient hadsurgery to correct a medial patellar luxation in the same limb 3 years prior. Based on the physical examination and radiographic findings, MPL and CCLR of the right hindlimb were diagnosed. Pre-surgical arthroscopy examination was performed, revealing a complete rupture of the cranial cruciate ligament, medial caudal meniscal tears and fibrotic cartilagechanges on the trochlear groove. An arthroscopy-assisted partial meniscectomy was used to repair themedial caudal meniscus. To correct the tibial plateau angle and medial patellar luxation, a tibial plateau leveling osteotomy (TPLO) was performed. A tibial tuberosity transposition (TTT) was performed to realignthe quadriceps mechanism with the trochlear block recession followed by soft tissue reconstruction. The post-surgical recovery was uneventful, and the patient was weight-bearing with normal ambulation on the repaired limb. There were no complications, and the implants were well positioned at the last follow-up. The clinical outcome of the caseindicates that combining TPLO with TTTis a good surgical option for treatingconcurrent CCLR and MPL.

Closure Duration and Pitch as Phonetic Cues to Korean Stop Identity in AP-medial Position: Perception Test

  • The present study investigated some perceptual phonetic attributes of two Korean stop types, aspirated and lax, in medial position of an accentual phrase. The intonational pattern across syllables (Jun, 1993) is argued to depend on the type of stop (aspirated vs. lax) only in the initial position of an accentual phrase. In Kang & Dilley (2007), we showed that significant differences between aspirated and lax stops in medial position of an accentual phrase exist in closure duration, voice-onset time, and fundamental frequency (F0) values for post-stop vowels. In the present perception experiment, we investigated whether these phonetic attributes contribute to the perception of these two types of stops: The closure durations and/or F0's of post-stop vowels on accentual-phrase medial words were altered and twenty native Korean speakers then judged these words as beginning with an aspirated or lax stop. Both closure duration and F0 significantly affected judgments of stop identity. These results indicate that a wider range of acoustic cues that distinguish aspirated and lax Korean stops in production also plays a role in perception. To account for these results we suggest some phonetic and phonological models of consonant-tone interactions for Korean.

A Modified Stabilization of Medial Shoulder Luxation by Biceps Tendon Transposition in a Dog (개에서 상완이두건 변형 전위에 의한 내측 견관절 탈구의 안정화 증례)

  • A 10-year-old intact female Chihuahua weighing 2 kg was presented for complaint of forelimb lameness with non-weight bearing posture. Greater tubercle was palpated medial to its normal location on palpation and crepitation was intermittent with manipulation. On radiographic examination, left medial scapulohumeral joint luxation was found. Bilateral medial patellar luxations also were found and evaluated as grade III. Surgically, the medial scapulohumeral joint luxation was reduced with transposition of the tendon of the biceps brachii muscle. The patient's gait became normal and non-weight bearing posture was resolved at 6 weeks postoperatively.

Surgical Treatment of Using Acutrak Screw for Ankle Medial Malleolar Fracture (Acutrak 나사를 이용한 족관절 내과 골절의 치료)

  • Purpose: The purpose of this study is to evaluate the clinical and radiologic results of using Acutrak screws for treating ankle medial malleolar fracture. Materials and Methods: We reviewed 38 cases of ankle medial malleolar fracture treated with Acutrak screws from February 2005 to May 2008. Results: In clinical result, there were 30 exellent cases, 7 good cases, 1 fair case. In radiologic result, there is no case with reduction loss. Average union time is 10.5 weeks. Conclusion: We conclude that Acutrak screw fixation is a useful method for ankle medial malleolar fracture, there are many advantages in accurate anatomical reduction, small incision, short operative time.

Medial Partial Arytenoidectomy by $CO_2$laser for Bilateral Vocal Cord Paralysis (양측 성대마비 환자에 대한 $CO_2$레이저 피열연골 내측부분절제술)

  • Bilateral vocal cord paralysis can result in severe airway compromise. Over the years a variety of arytenoidectomy procedures have bee described, and one or more of these have been the gold standard for many years. A widely accepted treatment is endoscopic laser total arytenoidectomy. However, vocal results are usually poor. Objective : To evaluate the effect of treatment of endoscopic laser medial partial arytenoidectorny for bilateral vocal cord paralysis Material and Methods : We performed endoscopic medial partial arytenoidceomy with $CO_2$laser for 3 patients with bilateral vocal cord paralysis. The $CO_2$laser is operated with a continuous 7-watt beam in superpulse mode. We compared degree of dyspnea and glottic area of pre-operation with those of post-operation for 3 patients. We analysed aerodynamic study pre-operatively and post-operatively for 1 patient. Results The symptom of dyspnea was improved markedly and the glottic area was widened from 34% to 50% compared with that of pre-operation. The voice quality was slightly decreased. Tracheotomy was not necessary for not-tracheotomized patient and decanulation was possible for tracheotomized patient post-operatively. Cunclusion: The endoscopic laser medial partial arytenoidectomy is a convient and effective method for opening the posterior glottic airway.
