  • Marine diesel engine production and refinements sought a continuous increase on mean effective pressure and thermal efficiency. These results in increased maximum combustion pressure within the cylinder and vibratory torque in crankshaft. As such, crankshaft should be designed and compacted within its fatigue strength. In this paper, the 8H25/33P($3,155ps{\times}900rpm$) engine for ship propulsion was selected as a case study, and tile strength analysis of its crankshaft is carried out by. simplified method recommended by IACS M53 and a detailed method with the crankshaft assumed as a continuous beam and bearing supported in its flexibility. The results of these two methods are compared with each other.

  • The present work is an experimental investigation on potential operating range using directly injected gasoline fuel in a single-cylinder compression ignition (CI) engine. The objectives of present study were to apply auto-ignited combustion to gasoline fuel and to evaluate potential operating range. In order to auto-ignite gasoline fuel in CI engine, the fuel direct-injection system and the intake air system were modified that a flow rate and temperature of intake air were regulated. The heat-release rate (HRR), net indicated mean effective pressure (IMEP), start of combustion (SOC), and combustion duration were derived from in-cylinder pressure data in a test engine, which has 373.33cc displacement volume and 17.8 compression ratio. The exhaust emission characteristics were obtained emission gas analyzer and smoke meter on the exhaust line system.

  • A wind tunnel test of a scaled-down model and field measurement were effective methods for elucidating the aerodynamic behavior of a chimney under a wind load. Therefore, the relationship between the results of the wind tunnel test and the field measurement had to be determined. Accordingly, the set-up and testing method in the wind tunnel had to be modified from the field measurement to simulate the real behavior of a chimney under the wind flow with a larger Reynolds number. It enabled the results of the wind tunnel tests to be correlated with the field measurement. The model surface roughness and different turbulence intensity flows were added to the test. The simulated results of the wind tunnel test agreed with the full-scale measurements in the mean surface pressure distribution behavior.

Effects of different wind deflectors on wind loads for extra-large cooling towers

  • In order to examine the effects of different wind deflectors on the wind load distribution characteristics of extra-large cooling towers, a comparative study of the distribution characteristics of wind pressures on the surface of three large cooling towers with typical wind deflectors and one tower without wind deflector was conducted using wind tunnel tests. These characteristics include aerodynamic parameters such as mean wind pressures, fluctuating wind pressures, peak factors, correlation coefficients, extreme wind pressures, drag coefficients and vorticity distribution. Then distribution regularities of different wind deflectors on global and local wind pressure of extra-large cooling towers was extracted, and finally the fitting formula of extreme wind pressure of the cooling towers with different wind deflectors was provided. The results showed that the large eddy simulation (LES) method used in this article could be used to accurately simulate wind loads of such extra-large cooling towers. The three typical wind deflectors could effectively reduce the average wind pressure of the negative pressure extreme regions in the central part of the tower, and were also effective in reducing the root of the variance of the fluctuating wind pressure in the upper-middle part of the windward side of the tower, with the curved air deflector showing particularly. All the different wind deflectors effectively reduced the wind pressure extremes of the middle and lower regions of the windward side of the tower and of the negative pressure extremes region, with the best effect occurring in the curved wind deflector. After the wind deflectors were installed the drag coefficient values of each layer of the middle and lower parts of the tower were significantly higher than that without wind deflector, but the effect on the drag coefficients of layers above the throat was weak. The peak factors for the windward side, the side and leeward side of the extra-large cooling towers with different wind deflectors were set as 3.29, 3.41 and 3.50, respectively.

Effects of One-time bamboo stepping on the Elderly's Blood Pressure and the Autonomic Neural System (일회성 대나무 밟기가 노인의 혈압과 자율신경계에 미치는 영향)

  • Background: The purpose of this study is to investigate the change of blood pressure and autonomic neural system through one-time bamboo stepping in hypertensive elderly people. Design: Randomized Controlled Trial. Methods: We recruited 26 elderly people aged 65 or older for study. They sat for 10 minutes in a chair, rested, and measured pre-exercise blood pressure and autonomic neural system. The blood pressure was measured three times and the mean value was used and the autonomic neural system was measured once. Bamboo stepping exercise was performed for 20 minutes. Blood pressure and autonomic neural system were measured 5 times immediately after exercise, and every 15 minutes at 60 minutes after bamboo stepping exercise. Results: There was a significant difference in the decrease of systolic blood pressure after one-time bamboo stepping exercise, the change of parasympathetic activity (HF), heart rate variability (HRV) and BPM measured immediately after exercise at 15 minutes after exercise, There was no significant difference in autonomic neural system change. Conclusion: One-time Bamboo stepping exercise is effective in decreasing systolic blood pressure of the hypertensive elderly.

Development of Functional Tea Products for Hypertension Patients with the Mixed Medicinal Herbal Extracts (고혈압 환자에 도움을 줄 수 있는 한방 다류 개발)

  • This study was carried out to investigate the effects of Eucommia ulmoides Oliver (Du-Zhong) as a main ingredient in a mixture of various medicinal herbal extracts on blood pressure, serum lipid concentrations, and hematological variables in hypertensive patients. After 4 weeks on drinking the mixed medicinal herbal teas, systolic blood pressure remarkably decreased from 167.14 to 134.86 mmHg and diastolic blood pressure decreased from 100.00 to 87.10 mmHg. In terms of serum lipid profiles, there were no differences in total-cholesterol, HDL-cholesterol, LDL-cholesterol level, and atherogenic index, However, the level of triglycerol was significantly reduced from 237.1 before the experiment to 145.00 mg/dL after 4 weeks on drinking the mixed medicinal herbal teas. Serum triglyceride were remarkably reduced from 237.1 to 145.00 mg/dL in the hypertensive patients who had taken the combined medicinal herbal extracts. Compared with data obtained from the patients before the experiment, there were no differences in hematological variables (RBC, WBC, hematocrit, hemoglobin, platlets and homocysteine) after the 4 weeks experiments, but mean corpuscular hemoglobin concentration was significantly increased by 2.70% (p<0.05) in the blood samples. Based on these findings, it can be presumed that the examined mixed medicinal herbal tea may be effective in lowering blood pressure and in increasing mean corpuscular hemoglobin concentration due to reduced serum triglyceride levels in hypertension patients.

Intracranial Pressure and Cerebral Blood Flow Monitoring after Bilateral Decompressive Craniectomy in Patients with Acute Massive Brain Swelling (급성 중증 뇌종창 환자의 양측성 감압개두술 후 뇌압 및 뇌혈류 측정)

  • Objectives : The management of massive brain swelling remains an unsolved problem in neurosurgical field. Despite newly developed medical and pharmacological therapy, the mortality and morbidity due to massive brain swelling remains high. According to many recent reports, surgical decompression with dura expansion is superior to medical management in patients with massive brain swelling. We performed surgical treatment on the first line of treatment, and followed medical management in case with refractory increased intracranial pressure(ICP). To show the quantitative effect of decompressive surgery on the intracranial pressure, we performed ventricular puncture and checked the ventricular ICP continuously during the decompressive surgery and postoperative period. Materials and Methods : Fifty-one patients with massive brain swelling, undergoing bilateral decompressive craniectomy with dura expansion, were studied in this study. In all patients, ventricular puncture was performed at Kocher's point on the opposite side of massive brain swelling. The ventricular pressure was monitored continuously, during the bilateral decompression procedures and postoperative period. Results : The initial ventricular ICP were varied from 13mmHg to 112mmHg. Immediately after the bilateral craniectomy, mean ventricular ICP decreased to $53.1{\pm}15.8%$ of the initial ICP(ranges from 5mmHg to 87mmHg). Dura opening decreased mean ICP by additional 36.7% and made the ventricular pressure $16.4{\pm}10.5%$ of the initial pressure (ranges from 0mmHg to 28mmHg). Postoperatively, ventricular pressure was lowered to $20.2{\pm}22.6%$(ranged from 0mmHg to 62.3mmHg) of the initial ICP. The ventricular ICP value during the first 24 hours after decompressive surgery was found to be an important prognostic factor. If ICP was over 35mmHg, the mortality was 100% instead of additional medical(barbiturate coma therapy and hypothermia) treatments. Conclusion : Bilateral decompression with dura expansion is considered an effective therapeutic modality in ICP control. To obtain favorable clinical outcome in patients with massive brain swelling, early decision making on surgical management and proper patient selection are mandatory.

Low-dose intravenous ketamine versus intravenous ketorolac in pain control in patients with acute renal colic in an emergency setting: a double-blind randomized clinical trial

  • Background: This study was conducted to compare the effectiveness of low-dose ketamine versus ketorolac in pain control in patients with acute renal colic presenting to the emergency department (ED). Methods: This is a double-blind randomized clinical trial. The initial pain severity was assessed using the numerical rating scale (NRS). Then, ketamine or ketorolac was administered intravenously at a dose of 0.6 mg/kg and 30 mg respectively. The pain severity and adverse drug reactions were recorded 5, 15, 30, 60, and 120 min thereafter. Results: The data of 62 subjects in the ketamine group and 64 patients in the ketorolac group were analyzed. The mean age of the patients was $34.2{\pm}9.9$ and $37.9{\pm}10.6\;years$ in the ketamine and ketorolac group, respectively. There was no significant difference in the mean NRS scores at each time point, except for the 5 min, between the two groups. Despite a marked decrease in pain severity in the ketamine group from drug administration at the 5 min, a slight increase in pain was observed from the 5 min to the 15 min. The rate of adverse drug reactions, including dizziness (P = 0.001), agitation (P = 0.002), increased systolic blood pressure (> 140 mmHg), and diastolic blood pressure (> 90 mmHg) was higher in the ketamine group. Conclusions: Low dose ketamine is as effective as ketorolac in pain management in patients with renal colic presenting to the ED. However, it is associated with a higher rate of adverse drug reactions.

An Experimental Study on the Spray Characteristics by Twin-Fluid Atomizer for Wide Band Spray (광폭면 분무를 위한 2유체 노즐의 분무 특성에 관한 연구)

  • To develop the twin-fluid atomizer having the excellent performance of painting, the spray characteristics of how a wide area can be painted efficiently by one time spraying were studied in this paper. Spray phenomena are affected by the many factors determining the spray field such as the spraying pressure of gas, the spraying pressure and viscosity of liquid paints, the opening duration of needle valve, the design dimension of nozzle, and so on. As the results of experiments, these factors affecting on spray characteristics were suggested as followings; 1) The optimum spraying pressure of gas was $0.015{\sim}0.02\;kPa$, and the appropriate spraying pressure of liquid paint was 0.01kPa, In these situations, the setting up pressures must be compensated as much as the losing amount of pressure because a decompression occurred when operating valves. 2) The duration of opening the needle valve must be sustained for $1{\sim}2$ seconds to inject gas after spraying the liquid paint. This operating of the needle valve was necessary to avoid the affect on the changing of liquid column length, and to prevent the droplet deposit at the initial time of spraying. 3) The spray tip penetration was gained form the experimental equation, and the effective spraying angle was $85^{\circ}{\pm}5^{\circ}$ just at he appropriate spraying pressure of gas. The distribution of the area sprayed had the variation in $350{\pm}50\;mm$ because of the spraying pressure of gas, the its distance from the spray tip, and the lift of the needle valve.

Extra-Anatomic Ascending Aorta to Abdominal Aorta Bypass in Takayasu Arteritis Patients with Mid-Aortic Syndrome

  • Background: We evaluated the operative outcomes of an extra-anatomic bypass from the ascending aorta to the abdominal aorta in patients with type II or III Takayasu arteritis (TA) with mid-aortic syndrome. Methods: From 1988 to 2014, 8 patients with type II (n=2) or III (n=6) TA underwent an ascending aorta to abdominal aorta bypass. The mean patient age was $43.5{\pm}12.2years$ and the mean peak pressure gradient between the upper and lower extremities was $54.8{\pm}39.0mm\;Hg$. The median follow-up duration was 54.4 months (range, 17.8 to 177.4 months). Results: There were no cases of operative mortality. The mean peak pressure gradient significantly decreased to $-2.4{\pm}32.3mm\;Hg$ (p=0.017 compared to the preoperative value). Late death occurred in 2 patients. The symptoms of upper extremity hypertension and claudication improved in all patients. The bypass grafts were patent at $47.1{\pm}58.9months$ in 7 patients who underwent follow-up imaging studies. Conclusion: An extra-anatomic ascending aorta to abdominal aorta bypass could be an effective treatment option for severe aortic steno-occlusive disease in patients with type II or III TA, with favorable early and long-term outcomes.
