추나치료를 병행한 임신초기 급성요통 한의 치험 1례 (A Clinical Study of Acute Low Back Pain treated by Chuna & General Oriental Therapy during Pregnancy)

  • Objectives: The object of this study is to report a clinical effect of oriental medical treatments with Chuna for acute low back pain during the early stages of pregnancy.Methods: The patient in this case, 31 years-old female was admitted for 1days (20th/June/2016-30th/June/2016) due to the acute low back pain on the 7 weeks of pregnancy. We diagnosed as 挫閃腰痛, 傷筋 (Sprain and strain of lumbar spine) and treated with Chuna therapy and other conservative treatments including acupuncture herbal mixture. And we measured Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ).Results: After treatments, Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ) were significantly improved in case.Conclusion: Oriental medical treatments with Chuna manual therapy were associated with improvement of acute low back pain during the early stages of pregnancy.

요통 환자의 굴곡신연기법의 효과에 대한 임상적 연구 (The Clinical Study for the Effects of Flexion-Distraction Technique on Patients with Low Back Pain)

  • Objectives : The purpose of this study is to find out the effects of flexion-distraction technique on low back pain. Methods : The 40 patients were divided into 2 groups: group a was treated except flexion-distraction technique and group B was treated with flexion-distraction technique. Both groups were treated with acupuncture treatment, physical therapy and manipulation. Patients were evaluated by McGill pain questionnaire-short form(SF-MPQ), visual analogue scale(VAS), Oswestry disability index(ODI) and pain disability index(PDI). Results : 1. Both Groups were significantly decreased in VAS and ODI after 3 weeks of treatment. 2. Group B compared with Group A was significantly decreased in SF-MPQ, VAS, ODI and PDI after 3 weeks of treatment. Conclusions : We found out that flexion-distraction technique is considered to be effective and useful on low back pain.

  • Objectives : The purpose of this study is to find out the effects of muscle energy techniques on nuchal pain caused by traffic accidents. Methods : The 20 patients were divided into 2 groups: group A was treated except muscle energy techniques and group B was treated with muscle energy techniques. Both groups were treated with acupunture treatment, physical theraphy and herbal medication. Patients were evaluated by McGill Pain Questionnaire-Short Form(SF-MPQ), Pain Disability Index(PDI) and Neck Disability Index(NDI) in traffic accident patients. Results : 1. Both Group were significantly decreased in sensory of SF-MPQ, VAS, PDI and NDI after 7 days of treatment. 2. Group B compared with the Group A was significantly decresed in VAS, PDI after 7 days of treatment. Conclusions : We found out that muscle energy techniques is considered to be effective and useful on nuchal pain caused by traffic accidents.

Assessment of the Nature and Severity of Pain Using SF-MPQ for Cancer Patients at the National Institute of Oncology in Rabat in 2015

  • Background: Cancer is a worldwide health problem and pain is among the most common and unpleasant effects affecting well-being of cancer patients. Accurate description of pain can help physicians to improve its management. Many English tools have been developed to assess pain. Onkly a limited number of these are applied in Arab countries. Our aim was to assess the quality, the nature and the severity of pain using the short McGill Pain Questionnaire (SF-MPQ) on cancer patients in the National Institute of Oncology (NIO) in Rabat, Morocco. Materials and Methods: The tool used is the SF-MPQ inspired from the Arabic version of the MPQ. The subjects were cancer patients (N=182) attending the NIO, from 24th October 2015 to 8th January 2016, aging ${\geq}18$ years old, experiencing pain and coming to have or to update their pain medication. Results: The rate of participation was 96.3%. Eight patients had difficulties to express their pain using descriptors, but could use the Visual Analogue Scale (VAS) and the body diagram. The most frequent sensory descriptors were 'Throbbing', 'Shooting', 'Hot-Burning'. The most used affective descriptor was 'Tiring-Exhausting'. The mean VAS was 6.6 (2.4). The mean score of all items was 11.9 (7.8). The patients were suffering from severe pain. The internal consistency of the form was s acceptable. Conclusions: The findings indicate that most of the patients attending the pain center of the NIO could use the descriptors of the SF-MPQ to describe their pain. They indicate the usefulness of the SF-MPQ to assess the nature and the severity of pain in cancer patients. This tool should be tested in other Moroccan and Arabic contexts associated with other tools in clinical trials.

Methods : The 30 patients were divided into 2 groups : group A was treated except myofascial releasement and group B was treated with myofascial releasement. Both group were treated with acupuncture treatment, physical theraphy and herbal medication. Patients were evaluated by McGill Pain Questionnaire-Short Form(SF-MPQ), Visual Analogue Scale(VAS), Pain Disability Index(PDI) and Neck Disability Index(NDI). Results : 1. Both Group were significantly decreased in affective of SF-MPQ, VAS, after 10 times of treatment. 2. Group B compared with the Group A was significantly decreased in affective of SF-MPQ, NDI and PDI after 10 times of treatment. Conclusions : We found out that myofascial releasement is considered to be effective and useful on nuchal pain caused by traffic accidents.

황련해독탕 약침 치료를 병행한 임신 중 좌골신경통을 동반한 요통 환자의 증례보고 (A Clinical Case Report on Traditional Korean Medical Treatment Including Hwangryunhaedok-tang Pharmacopuncture for Low Back Pain with Sciatica during Pregnancy)

  • Objectives: This study was to report clinical effect and safety of Traditional Korean Medical Treatment including Hwangryunhaedok-tang Pharmacopuncture for pregnant woman with low back pain with sciatica. Methods: The patient who suffered from sudden low back pain with sciatica was treated with Hwangryunhaedok-tang Pharmacopuncture, acupuncture, moxibustion, herbal medication and cupping therapy during admission. Visual Analog Score (VAS), Oswestry Disability Index (ODI), McGill pain questionnaire-short form (SF-MPQ) were checked to measure the outcome. Results: After treatment, Visual Analog Score (VAS), Oswestry disability index (ODI), McGill pain questionnaire-short form (SF-MPQ) were considerably improved in this case. Conclusions: The results indicate that Traditional Korean Medical Treatment including Hwangryunhaedok-tang Pharmacopuncture is the effective therapy for low back pain with sciatica during pregnancy.

경추추간판 탈출증환자의 감압치료 병행효과에 대한 임상적 연구 (The Clinical Studies for Non Surgical Spinal Decompression Treatment on Cervical Disc Herniation)

  • Objectives: The purpose of this study is to make a survey of the effectiveness of the non surgical spinal decompression treatment in patients with cervical disc herniation documented on magnetic resonance imaging(MRI). Methods: We studied about 27 cases of cervical disc herniation which were treated with non surgical spinal decompression and other medical treatment (acupuncture, interferential current therapy and ice poultice). Each patient has been treated with spine decompression and other medical treatment. And degree of improvement has been evaluated by short form-McGill pain questionnaire(SF-MPQ), visual analogue scale(VAS) and neck disability index(NDI) before treatment and 5th, 10th times after treatments. Results: 1. The VAS SF-MPQ and NDI scores showed statistically significant improvement after 5th and 10th treatment. 2. The VAS, SF-MPQ and NDI scores after 5th treatment showed better improvement than those of after 10th treatment. 3. The VAS, SF-MPQ and NDI scores in Subacute stage showed more statistically significant improvement among the other stages. Conclusions: As a result, non surgical spinal decompression treatment has clinical effects of pain reduction on patient with cervical disc herniation.

침치료로 호전되지 않은 퇴행성 슬관절염 환자의 도침치료 1례 (Miniscalpel Acupuncture Treatment on a Knee Degenerative Osteoarthritis Patient, Who does not Responded to Acupuncture Treatment)

  • Objectives : This study was done to show the clinical effect of miniscalpel acupuncture treatment on osteoarthritis of the knee which is refractory to acupuncture treatment. Methods : A patient was treated with acupuncture for three weeks, non-treated(wash out period) for two weeks, and treated with miniscalpel acupuncture for three weeks. The effect of treatments were measured with Visual Analogue Scale(VAS), Range of Motion(ROM), Short form McGill pain questionnaire(SF-MPQ), Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC). Results : During the three weeks of acupuncture treatment, VAS, SF-MPQ and WOMAC improved, but after two weeks of the wash out period each score worsened. During the three weeks of miniscalpel acupuncture VAS, SF-MPQ, and WOMAC improved while ROM improved remarkably. Conclusion : These results suggest that miniscalpel acupuncture might be a therapeutic option for knee degenerative osteoarthritis patients who does not responded to acupuncture treatment.

보존적 치료로 호전되는 않은 상완이두근 건염에 대한 화침을 포함한 한방 치료 1례 (A Case report on the Biceps Tendinitis Treatment with Fire needling Therapy)

  • Objective : The purpose of this study is to evaluate the effect of the fire needling therapy treatment on the biceps tendinitis. Methods : Fire needling therapy was administered five times to the patient complaining with biceps tendinitis. The improvement of clinical symptom was evaluated by McGill Pain Questionnaire -Short Form(SF-MPQ), Visual Analogue Scale(VAS) and Shoulder Pain, and Disability Index (SPADI). Result : After fire needling therapy to the biceps tendinitis patient five times, the patient's SF-MPQ score, VAS, and SPADI improved from 24 to 11, from 9 to 4, and from 33.6 to 14.8 respectively. Conclusions : Fire needling for biceps tendinitis should be tested in future randomized clinical trials.

Severity of Musculoskeletal Pain and Its Effect on Psychosocial Factors in Veterans With Posttraumatic Stress Disorder

  • PURPOSE: The aim of this study is to investigate the relationship between psychosocial factors and the severity of musculoskeletal pain in veterans with posttraumatic stress disorder (PTSD). METHODS: A total of 60 subjects were recruited from among the veterans with musculoskeletal pain at D Veterans Hospital. PTSD was evaluated by using the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition; severity of pain was measured by using the short-form McGill Pain Questionnaire (SF-MPQ); depression and anxiety were measured by using the Symptom Checklist-90-Revision; and the quality of sleep was measured by using the Pittsburgh Sleep Quality Index. All data were analyzed using SPSS 18.0 software for Windows. RESULTS: The averages cores of pain intensity ($7.48{\pm}1.67$), SF-MPQ-sensory ($13.84{\pm}7.52$), SF-MPQ-affective ($4.41{\pm}3.79$), depression ($19.30{\pm}11.37$), anxiety ($13.39{\pm}7.99$), and quality of sleep ($10.05{\pm}5.89$) were obtained in veterans with PTSD. SF-MPQ-sensory measures sleep quality (r=0.346, p<0.01), SF-MPQ-affective measures depression (r=0.318, p<0.01) and anxiety (r=0.404, p<0.01), and these showed a statistically significant positive correlation in veterans with PTSD. Pain levels were observed to be higher in veterans with PTSD. Moreover, in these subjects, physical pain had a significant influence on the anxiety variable among the psychosocial factors. CONCLUSION: These findings suggest that musculoskeletal pain provides meaningful information about depression, anxiety, and sleep disorder in veterans with PTSD. Our data suggest that musculoskeletal pain may need to be addressed as part of the health management process of veterans.
