The Role of Dynamic CT for the Differential Diagnosis of Solitary Pulmonary Nodule (고립성 폐결절의 감별진단에서 Dynamic CT의 역할)

  • Background : Malignant pulmonary nodules account for 30 to 40 percent of all solitary pulmonary nodules (SPNs). Therefore, characterization of SPNs is very important for treatment. Recently, dynamic CT has been widely used for tissue characterization and formation of differential diagnoses. The purpose of this study was to evaluate the ability of dynamic CT to formulate the differential diagnosis of SPNs. Materials and Methods : Nineteen patients with SPNs underwent dynamic CT (unenhanced scans, followed by a series of images at 20, 40, 60, 80, 100, 120, 140, 160, and 180 sec after intravenous injection of contrast medium). Diagnosis of SPN was performed based on pathologic findings in needle biopsy samples. Peak enhancement, net enhancement, slope of enhancement, and maximum relative enhancement ratio of the SPN were measured on dynamic CT, and Levene's test was performed to assess benignancy and malignancy. Results : Twelve SPNs were confirmed to have malignant pathology. There were no significant differences between benign and malignant nodules with respect to peak enhancement (p=0.787), net enhancement (p=0.135), or slope of enhancement (p=0.698). The maximal enhancement ratio was increased in malignancy compared to benignancy, but the difference was not statistically significant (p=0.094). Conclusion : In our study, the hemodynamic characteristics of dynamic CT were not significantly different between benign and malignant nodules. Therefore, long-term studies of larger patient samples are required to confirm our findings.

The usability of the MR Breast perfusion image and Time-Signal Intensity curve in Breast cancer patients (유방암 환자에서 MR Breast perfusion 영상과 시간-신호강도 곡선의 유용성)

  • The purpose of this study was to examine the usefulness of MR Breast perfusion image and time-signal intensity curve in patients diagnosed with breast cancer. We selected on 20 patients who were histologically diagnosed to have invasive ductal carcinoma (IDC) from March 2009 to December 2010. First, the Breast perfusion mapping image was reconstructed after obtaining the dynamic contrast enhancement image. The reconstructed image measured the slope, maximal relative enhancement, and time to peak on the detail including the lesion region, normal region, back ground region after obtaining the time-signal intensity curve. The lesion region and normal and slope of the back ground part were measured with the quantitive analytical method about the research and the average was compared and was analyze. In the qualitative analysis, the signal strength of each pixel was analyze with the macroscopic and being high it was low, the medium (2) performed the division of (a) by the three-point standard and the average was measured. The findings from the quantitative image analysis are the following: In the lesion region, the slope and maximal relative enhancement were the highestest among and the time to peak was the highestest in the back ground region. In the qualitative analysis, the breast perfusion image showed a diagnostic efficiency.

A Comparison of Iopamidol with Iopromide and Iohexol Contrast Media in Hepatic CT Angiography in Beagle Dogs (비글견의 간 CT 혈관조영상에서의 Iopamidol과 Iopromide, Iohexol 조명제의 비교실험)

  • The purpose of this paper was to compare the clinical efficacy of iopamidol and iopromide, iohexol nonionic contrast media in terms of their image quality in Beagle dogs with hepatic CT angiography and their application in veterinary clinics. With 9 Beagle dogs, contrast media of iopamidol (pamiray-$300^(R)$) and iopromide (ultravist-$300^(R)$, iohexol (omnipaque-$300^(R)$) were induced intravenously (600 mg I/kg, BW) and CT angiography was done under general anesthesia. CT scan included scout, pre-contrast and cine examinations. During CT angiography, peak HU (Hounsfield unit) and peak time were examined on each site (ROI; region of interest) of the aorta, caudal vena cava, potral vein and liver parenchyma. Any side effects were also examined. After experiments, it was found that there were no significant changes of HU and maximal enhancing time of each ROIs of aorta, caudal vena cava, portal vein and liver parenchyma between these contrast media. And any side effects were not noted. So it is concluded that iopamidol has similiar contrast enhancement like as iopromide and iohexol in hepatic angiography and and it is thought to be useful for evaluation of the abdominal organs by CT scan in veterinary clinics.
