하악 과두 골절의 외과적 처치에 관한 임상적 연구 (CLINICAL STUDY ON SURGICAL MANAGEMENT OF MANDIBULAR CONDYLAR FRACTURES)

  • 1993년 1월부터 1995년 12월까지 원광 대학교 치과대학 구강악안면외과에 하악 과두 골절을 주소로 내원한 환자중 관혈적 정복술 적응증에 해당되는 79명의 환자에 대하여 관혈적 정복술을 시행하였다(이 중 25명은 Dr. 남씨법을 행한 환자임). 평균 환자 나이는 32.5세(8-65세)이었으며 환자 관찰기간은 약 18.4개월(3-28개월)이었다. 62명(81%)에서 편측 과두 골절 양상이었으며, 57명(72%) 환자가 하악골내 다른 부위와 연관되어 골절되었고 이중 47명(59%)에서 정중부와 관련되어 골절되었다. 환자 나이, 골절된 과두의 심한 정도, 임상 증상, 방사선 사진상 등을 통하여 수술 방법을 선택하였으며 과두부 골절이 아닌 경우 대개 골절 부위의 견고 고정 및 근심쪽 과두 골편 제거, 관절 성형술 및 관절원판 정복술, lag screw고정 등을 시행하였다. 술 후 악간 고정은 약 2주 정도 하였으며 이후 수동적 개구 운동 및 능동적 개구 운동을 2에서 4주, 4주이상에서 시행하였다. 술후 방사선학적 관찰시 과두 흡수 및 후방 부위로 근심 과두부 변위 등, 약 21.5%에서 술 후 방사선학적 변화를 가져왔으며 특히 Dr. 남씨법에서 더 많이 나타났다(Dr. 남씨법(32%), 다른 정복술(16%)). 술 후 임상 증상은 19%의 일시적 안면신경 마비, 개구시 하악 변위(16.4%), 악관절 동통(15.2%), 35mm 이하에서의 개구제한(10%), 기타 부정교합, 관절잡음, 가성 관절 강직 등의 합병증을 나타냈으며 이중 Dr. 남씨법에서 더 많은 합병증을 나타냈다. 과두 골절의 와괴적 처치는 가능한 견고 고정을 시행하는 것이 좋으며 이중 Dr. 남씨법은 그 사용에 있어 고려 해 보는 것이 좋다고 사료된다. 특히 과두 골절 처치에 있어 무엇보다도 술 후 계속적인 환자 관리 및 장기간 예후 관찰이 필수적이라 사료된다.

생체 흡수성 고정판을 이용한 하악골 골절치료의 예후 (THE PROGNOSIS OF FIXATION OF MANDIBULAR FRACTURES WITH BIODEGRADABLE PLATES AND SCREWS)

  • The efficacy of bioresorbable fixation has recently been described in the osseosynthesis of the oral & maxillofacial region. However, a liitle data exist regarding the use of biodegradable plates and screws for the internal fixation of human mandible fractures. The purpose of this study is to analyze and compare the treatment of mandibular fractures by using a bioresorbable fixation system with conventional titanium system in human mandible fractures. eighteen patients constituted the bioresorbable fixation group and twenty-five patients constituted the titanium fixation group. Both groups underwent open reduction and internal fixation by use of a bioresorbable system or a titanium fixation system. Panoramic radiograph were obtained preoperatively, immediately postoperatively after reduction, at 6 months and at 12 months postoperatively. In the bioresorbable fixation group, complication(infection) occurred in 1 patient(5.6%) and was resolved by incision & drainage, plate removal and antibiotics without untoward sequelae. 2 patients(8.0%) experienced complications in the titanium fixation group and were treated using conservative treatment. There was no statistical difference in complication rates between two groups. Our data supported the use of bioresorbale plate fixation in mandibular fractures as a means of avoiding the potential and well documented problems with rigid titanium fixation systems. In conclusion, the bioresorbable fixation system provide a reliable and sufficient alternative to conventional titanium plate system.

치조능확장골절단술을 이용한 임프란트 식립술의 유용성 평가 (Evaluation of the Availability of Implant placement using Ridge Expansion Osteotomy (REO))

  • Purpose : Ridge expansion osteotomy (REO) has been introduced when it is necessary to expand narrow crestal ridge with simultaneous implant placement. This study has designed to evaluate the clinical availability of REO. Materials and methods : Subject were patients who had visited Seoul National University Bundang Hospital from July. 2003 to December, 2005 for implant placement using REO by one surgeon. Intraoperative and postoperative complication, failure of initial osseointegration and marginal bone resorption were estimated using electronic medical record and periapical radiography. Twenty?three patients, 8 males and 15 females, mean age 51, ranged 18 to 72, were treated for mean 26 months, ranged from 16 months to 46 months. Results : Mean diameter and length of implants placed at upper anterior, were 3.72mm and 13.32mm each other. Guided bone regeneration and ridge splitting were accompanied in this study. Five cases of cortical bone fracture, three cases of crestal bone loss more than 2mm, 2cases of gingival recession, and 2 cases of infection were noted, but there were no implants removed because of disintegration. Success rate of implant was 91.7%, even if survival rate of implant was 100%. In addition, there were no statistical significance between the success rate of REO and bone graft(p>0.05). Conclusion : Based on the results of the present study, it can be concluded that REO technique is reliable for implant placement at atrophic ridge with adequate height compared to bone graft and other osteotomies for ridge expansion, but care should be taken of esthetic problem such as gingival recession because of crestal bone resorption from trauma by osteotome.

  • 관상접근법 후 발생한 측두부 함몰의 기전은 아직 명확하지는 않으나 중측두동맥의 손상에 의한 측두지방대의 소실과 변위, 측두근의 부피감소, 봉합시의 잘못된 위치에 의해 발생된다. 이에 저자등은 관상접근법 후 발생한 측두부 함몰을 빠른 혈관화와 골조직의 성장, 낮은 합병증을 보이는 $Medpor^{(R)}$를 이용하여 심미적인 개선을 얻을 수 있었고 이후 현재까지 양호한 경과를 보이기에 문헌고찰과 함께 보고하는 바 이다.

Removal of miniplates following facial trauma and orthognathic surgery: a 3-year study

  • The purpose of this study was to determine the cause and risk factors of removing bone plateby investigating and analyzing 359 patients who underwent reduction of fracture or orthognathic surgery with bone plate insertion over the past 3 years. Patients were evaluated with respect to age, smoking status, reason for insertion of plates, the numbers of inserted plates, sites of insertion, time between insertion and removal, reasons for removal of plates. The removal rate of bone plates was 33.1%. Of these, 17.0% of patients had clinical symptoms which led to remove plates. The removal rate of men was 29.9% and the rate of women was 39.2%. There were high removal rates from less than 20s (45.8%) and 20s (34.4%) those who are relatively young age group. On the other hand people in their 50s had a removal rate of 27.8% which was higher than those in their 60s with a rate of 20.7%. The removal rate of bone plate inserted into the mandible was 33.5%, and the removal rate of bone plate inserted into the maxilla was 34.7%. The mean period between the insertion and removal of bone plate was 10.9 months. The main reason for the removal of bone plate was the patient's requirement (44.5%). The most common cause of clinical symptoms was infection (22.7%). Infection was manifested within about a year and led to the plate being removed. Therefore, lowering the possibility of infection after surgery could decrease the removal rate of bone plate.

What do we know about uncommon complications associated with third molar extractions? A scoping review of case reports and case series

  • The current study aimed to explore the types and frequencies of uncommon complications associated with third molar extractions based on a scoping review of case reports and case series. The study used an electronic literature search based on PubMed and Embase up to March 31, 2020, with an update performed on October 22, 2021. Any case reports and case series that reported complications associated with third molar extractions were included. The types of complications were grouped and the main symptoms of each type of complication were summarized. A total of 51 types of uncommon complications were identified in 248 patients from 186 studies. Most types of complications were post-operative. In the craniofacial and cervical regions, the most frequent complications included iatrogenic displacement of the molars or root fragments in the craniofacial area, late mandibular fracture, and subcutaneous emphysema. In other regions, the most frequent complications include pneumomediastinum, pneumorrhachis, pneumothorax, and pneumopericardium. Of the patients, 37 patients had life-threatening uncommon complications and 20 patients had long-term/irreversible uncommon complications associated with third molar extractions. In conclusion, a variety of uncommon complications associated with third molar extractions were identified. Most complications occurred in the craniofacial and cervical regions and were mild and transient.

Evaluation of deep learning and convolutional neural network algorithms for mandibular fracture detection using radiographic images: A systematic review and meta-analysis

  • Purpose: The use of artificial intelligence (AI) and deep learning algorithms in dentistry, especially for processing radiographic images, has markedly increased. However, detailed information remains limited regarding the accuracy of these algorithms in detecting mandibular fractures. Materials and Methods: This meta-analysis was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Specific keywords were generated regarding the accuracy of AI algorithms in detecting mandibular fractures on radiographic images. Then, the PubMed/Medline, Scopus, Embase, and Web of Science databases were searched. The Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) tool was employed to evaluate potential bias in the selected studies. A pooled analysis of the relevant parameters was conducted using STATA version 17 (StataCorp, College Station, TX, USA), utilizing the metandi command. Results: Of the 49 studies reviewed, 5 met the inclusion criteria. All of the selected studies utilized convolutional neural network algorithms, albeit with varying backbone structures, and all evaluated panoramic radiography images. The pooled analysis yielded a sensitivity of 0.971 (95% confidence interval [CI]: 0.881-0.949), a specificity of 0.813 (95% CI: 0.797-0.824), and a diagnostic odds ratio of 7.109 (95% CI: 5.27-8.913). Conclusion: This review suggests that deep learning algorithms show potential for detecting mandibular fractures on panoramic radiography images. However, their effectiveness is currently limited by the small size and narrow scope of available datasets. Further research with larger and more diverse datasets is crucial to verify the accuracy of these tools in in practical dental settings.

치료법을 달리한 상악골절후 부정유합 2예 (THE CARE OF DELAYED MALUNION AFTER MAXILLARY FRACTURE BY DIFFERENT METHODS:REPORT OF THREE CASES)

  • 현대 사회에서 외상의 특성은 다발성 손상이 빈발하는 것이며 상악골절시 생명을 위협하는 다발성 전신손상이 동반되면 수술이 지연되어 골절부의 지연된 부정유합을 보일 수 있다. 또한 골절된 양상에 따라서는 관혈적 정복술의 적용여부가 고려되는데 특히 골절편의 분쇄가 심하면 수술시 골절부위의 이개 후 정상위치로의 이동이 어렵고 설사 골절편의 이동이 이루어졌다고 하여도 정상위치로의 고정이 용이하지 않을 뿐만 아니라 분쇄골절편의 상실량이 많아지면 골이식까지 시행해야 하는 등 외과적이술식의 적용에 많은 난관이 예상된다. 따라서 상악골절 후 통상 6주일 간의 시간경과가 없다면 오히려 상악골절편의 이동방향을 예측하여, 전방견인용 headgear 또는 headcap 을 사용한 교정적인 치료를 시도함이 바람직할 수도 있다. 더우기 reverse headgear를 이용한 치료법은 골절된 경우가 아니더라도 상악골의 전방이동에 유용함이 실험적으로나 임상적으로 입증된 만큼 이를 상악골절후 후퇴된 상악골의 전방견인에 적용할 경우 교정치료의 최대장애인 cortical bone의 연속성이 골절로 인해 끊어지고 섬유성 유착상태로 남게되어 교정력의 효과가 치아이동 뿐만 아니라 골편의 이동에 매우 유익하리라 사료된다. 이에 저자는 상악골적이 분쇄양상인 한 환자에서 headgear와 headcap을 이용한 교정치료법을 5개월간 적용해 양호한 결과를 얻었고, 골절양상이 분쇄형이 아닌 증례에서는 외상후 5주간 경과 되었지만 관혈적 정복술을 시행하여 정상교합을 회복했으며, 심한 두부 손상으로 6개월만에 의식 회복을 한 LeFort I, II, III, 환자에서는 두부방사선 계측학적 분석, 교합기 상에서의 모형분석, 전산화 단층촬영 검사 등을 시행한 후 새로운 골면에다 LeFort I -osteotomy with iliac bone graft를 시행해 정상교합 및 안모추형을 개선시켰기에 이를 보고한다.

의도적 재식술을 위한 비외상성 안전 발치법 (Atraumatic Safe Extraction for Intentional Replantation)

  • Intentional replantation is useful for failed cases of conventional dental treatment - including root canal treatment - to restore the tooth in question. Based on a recent study, it is relatively very successful; prognosis is good for a long period. On the other hand, a tooth that becomes an indication of intentional replantation is often severely weakened throughout several treatments. Moreover, with multi-rooted teeth, extracting without root fracture is difficult. Safe extraction that is free of coronal or root fracture is important, but little information is known as to a concrete, safe way of extraction. There are a few considerations for safe extraction. First, a tooth with orthodontic extrusion force is easier for extraction due to its increased mobility; it increases the amount of the periodontal ligament, which is essential for re-attachment. As a safe way of extraction, the use of physics forceps has been introduced recently; it minimizes damage to the gingiva and alveolar hone. This paper reports the good result of using atraumatic safe extraction via both orthodontic extrusion and physics forceps$^{(R)}$.

Radiographic and computed tomography monitoring of a fractured needle fragment in the mandibular branch

  • Some complications can arise with the usage of local anesthesia for dental procedures, including the fracture of needles in the patient. This is a rare incident, usually caused by the patient's sudden movements during anesthetic block. Its complications are not common, but can include pain, trismus, inflammation in the region, difficulty in swallowing, and migration of the object, which is the least common but has the ability to cause more serious damage to the patient. This report describes a case in which, after the fracture of the anesthetic needle used during alveolar nerve block for exodontia of the left mandibular third molar, the fragment moved significantly in the first 2 months, before stabilizing after the third month of radiographic monitoring.
