REHABILITATION OF A PATIENT WITH A POST RADIOTHERAPY TRISMUS WITH AN OBTURATOR AND A MAXILLARY DENTURE USING MAGNET ATTACHMENTS

  • Construction of an obturator for rehabilitation of a patient who underwent a maxillectomy is vital. Routinely a constructed obturator includes denture portion. A patient who may present anatomical limitations due to surgical or radiotherapy complications often challenges the clinician. Purpose: This clinical report describes a patient with severe trismus after surgical resection and radiotherapy treatment of a tumor in the upper left maxilla. Conclusion: This report describes the concepts of using a rotational path insertion for an obturator and a separately constructed maxillary denture. The stability and retention of the obturator were obtained from anatomical features. Where as these were achieved through magnetic attachments and the remaining edentulous ridge to ensure esthetic and function of the prosthesis.

Solitary peripheral osteomas of the jaws

  • Osteoma is a benign osteogenic tumor composed of cancellous or compact bone, classified as peripheral, central, or extraskeletal. Peripheral osteomas are uncommon. Excluding the maxillary sinuses, the maxilla is a rare site for osteomas. The purpose of this report was to describe clinicopathological and radiological features of two peripheral osteomas occurring in the jaws, one located in the mandible and another in the edentulous maxillary alveolar ridge. The tumors were asymptomatic and were fully excised without any complications or recurrence. The lesions were submitted to histopathological analysis and diagnosed as peripheral osteoma, compact type.

Considerations for the Survival of Complete Arch Implant-Supported Zirconia Restorations; Status of Antagonistic Arches and Stress Distribution on Frameworks: A Case Report

  • This report describes two cases of complete arch implant-supported restorations. The first patient had seven dental implants in each arch with monolithic zirconia frameworks. At four weeks' follow-up, the one-piece maxillary framework was fractured, which was re-designed and re-fabricated using laser-sintered cobalt-chrome alloy. The second patient had four implants in the mandible only. A mandibular monolithic zirconia framework and a maxillary conventional complete denture were fabricated and delivered. At five years' follow-up, the patient reported no significant discomfort. Careful consideration and monitoring of the status of antagonistic arches and stress distribution on zirconia frameworks were suggested for complete arch implant-supported fixed restorations.

  • 저자들은 상악골 구치부에서 잔존 치조골의 흡수 및 상악동의 함기화로 인하여 통상적인 임플란트 시술이 어려운 증례에서 하악골의 하악지에서 자가골을 채취하여 이를 분쇄한 후 이종골과 부피비로 약1:1로 혼합하여 상악동 골이식술을 시행하고 동시에 임플란트를 식립하여 비교적 만족할만한 결과를 얻었기에 이를 문헌고찰과 함께 보고하며, 향후 이러한 증례들과 다른 골이식재를 사용한 증례들에 대하여 보다 장기적인 추적조사와 골이식된 부위의 골조직의 재형성, 식립된 임프란트의 보철적인 기능과 장기적인 예후에 대한 보다 체계적인 연구가 필요하리라 사료된다.

  • Backgrounds: Insufficient intermaxillary space is caused by non-restoration following tooth extraction in the past, and this involves eruption of the opposing teeth and changes of the arch structure. Such cases are difficult just by a simple prosthetic approach, and diversified treatment plans should be established. Among these, posterior maxillary segmental osteotomy (PMSO) is an efficient treatment option than extraction of opposing teeth as it surgically repositions multiple erupted teeth and alveolar bone. PMSO can preserve the natural teeth; therefore, it is being regarded as a treatment method which can improve insufficient intermaxillary space significantly. Case presentation: In this case report, the first patient received PMSO in order to place an implant in the mandibular edentulous space after decreased vertical dimension is restored, and the second patient received PMSO along with orthodontic treatment to obtain the intermaxillary space and balance the interarch molar width. Conclusion: PMSO is the treatment of choice when occlusion is compromised in the presence of decreased vertical dimension or arch length discrepancy.

상악동을 천공한 임플란트의 생존율에 대한 후향적 연구 (A Retrospective study on the survival rate of the sinus perforated implants)

  • Perforation of maxillary sinus is a common complication of implant placement in posterior maxilla. The purpose of this study was to evaluate the prognosis of sinus perforated implants placed in partially edentulous maxillae. Eighteen sinus perforated implants in 15 patients were examined for cumulative survival rate, radiographic preoperative bone level, and radiographic marginal bone level change. Twenty-two non-perforated implants in the same patients served as control. The results were as follows; 1. There was no statistically significant difference in cumulative survival rate between sinus perforated implants and non-perforated implants (P>0.05). 2. There was no statistically significant difference in the marginal bone level between sinus perforated implants and non-perforated implants (P>0.05). 3. There was no statistically significant difference in cumulative survival rate according to the preoperative bone level (P>0.05). These results suggests that perforation of maxillary sinus may not affect implant success in posterior maxillae.

Outfracture osteotomy sinus graft technique의 유용성에 관한 임상적 고찰 (CLINICAL INVESTIGATION ON THE FEASIBILITY OF OUTFRACTURE OSTEOTOMY SINUS GRAFT TECHNIQUE)

  • Purpose: The purpose of this study was to evaluate the feasibility of the outfracture osteotomy sinus graft technique with the evaluation of 5-year survival rate of the implants placed in the atrophic edentulous posterior maxillary area. Materials and methods: One hundred and thirteen cases of 96 patients who visited our center from Aug 2004 to July 2009 and were diagnosed as atrophic edentulous maxillary alveolar ridge, were selected and underwent augmentation sinus surgery with outfracture osteotomy technique. Feasibility of the outfracture osteotomy technique was investigated with clinical and radiographic evaluation to assess the survival rate of the total dental implants in augmentation sinus surgery of this new kind. Total fixture number available in follow-up period was 179, in which the lost 10 patients were excluded out of 96 patients. Results: Five-year cumulative survival rate was 97.2% with 5 failures of total 179 fixtures. The average follow-up period was 29 and a half months, with the minimum and maximum follow-up periods of 4 months 21 days and 59 months 14 days, respectively. Conclusion: Traditional infracture technique is a popular method for an augmentation sinus surgery. The authors modified this classical method by outfracturing and readapting the bony window after sinus graft, with excellent treatment results evidenced by high survival rate of 97.2% (174 out of 179 fixtures), which proves the feasibility of the newly-designed outfracture osteotomy sinus graft technique.

Implant and root supported overdentures - a literature review and some data on bone loss in edentulous jaws

  • PURPOSE. To present a literature review on implant overdentures after a brief survey of bone loss after extraction of all teeth. MATERIALS AND METHODS. Papers on alveolar bone loss and implant overdentures have been studied for a narrative review. RESULTS. Bone loss of the alveolar process after tooth extraction occurs with great individual variation, impossible to predict at the time of extraction. The simplest way to prevent bone loss is to avoid extraction of all teeth. To keep a few teeth and use them or their roots for a tooth or root-supported overdenture substantially reduces bone loss. Jaws with implant-supported prostheses show less bone loss than jaws with conventional dentures. Mandibular 2-implant overdentures provide patients with better outcomes than do conventional dentures, regarding satisfaction, chewing ability and oral-health-related quality of life. There is no strong evidence for the superiority of one overdenture retention-system over the others regarding patient satisfaction, survival, peri-implant bone loss and relevant clinical factors. Mandibular single midline implant overdentures have shown promising results but long-term results are not yet available. For a maxillary overdenture 4 to 6 implants splinted with a bar provide high survival both for implants and overdenture. CONCLUSION. In edentulous mandibles, 2-implant overdentures provide excellent long-term success and survival, including patient satisfaction and improved oral functions. To further reduce the costs a single midline implant overdenture can be a promising option. In the maxilla, overdentures supported on 4 to 6 implants splinted with a bar have demonstrated good functional results.

Feasibility of the outfracture osteotomy sinus graft technique was investigated with clinical and radiographic evaluation to assess the survival rate of the total dental implants in augmentation sinus surgery. Total fixture number available in follow-up period was 320, in which the lost 15 patients were excluded out of 160 patients. Results: Eight point five year cumulative survival rate was 95.6% with 14 failures of total 320 fixtures. The average follow-up period was 28 months 16 days with the minimum and maximum follow-up periods of 4 months 5 days and 94 months 10 days, respectively. Conclusion: Traditional infracture technique is a popular method for an augmentation sinus surgery. 하악 임플란트 고정성 보철에 대합되는 상악 총의치 하방의 골흡수에 대한 고찰 및 증례보고 (Maxillary Resorption under Complete Dentures Opposing Mandibular Implant Supported Fixed Prosthesis: A Literature Review and Case Report)

  • 완전 무치악 환자를 총의치로 수복할 때 특히 하악의 경우 유지, 지지 면적이 상악보다 좁아 의치의 탈락, 불량한 지지 및 안정성, 통증을 유발하여 의치의 사용이 불편할 수 있다. 이런 환자에서 임플란트를 이용한 보철수복은 심미성, 안정성, 저작력을 향상시킬 수 있다. 반면 상악 완전무치악은 구개를 이용할 수 있으므로 총의치로 수복하였을 때 많은 환자들이 잘 적응하여 사용하고 있다. 이에 상하악 완전 무치악 환자의 치료 옵션으로 하악은 임플란트로 지지를 받는 고정성 보철물 또는 임플란트 오버덴쳐로 수복하고 상악은 연조직에 의해 지지를 받는 전통적인 총의치로 수복하는 전악 보철수복이 제시되었다. 이 때 상악 총의치에 대합하는 하악 임플란트 고정성 보철이 상악 잔존치조제의 골흡수에 어떠한 영향을 미치는지에 대한 문헌고찰과 함께 증례를 보고하고자 한다.
