3.0 Tesla 자기공명영상시스템에서 고 해상도 나선주사영상 (High-resolution Spiral-scan Imaging at 3 Tesla MRI)

  • 목적 : 3.0 Tesla 고 자장에서 고 해상도 나선주사영상을 수행하였다. 나선주사영상은 초고속 영상기법의 하나로서, Echo Planar Imaging(EPI)에 비하여 eddy current 가 작게 발생하고, 경사자계 파형의 기울기가 완만하여 상대적으로 낮은 slew rate 를 가진 경사자계시스템으로 구현이 가능한 장점이 있다. 또한 고 자장 영상에서 고속스핀에코(Fast Spin Echo: FSE) 등의 rf 에코 기반의 고속영상방법에서 심각하게 대두되는 SAR 문제가 근원적으로 발생하지 않는 장점이 있어 고 자장에서의 초고속영상방법으로 주목을 받고 있다. 본 연구에서는 3.0 Tesla 에서 나선주사방식으로 고 해상도 영상을 얻어 고 자장 MRI에서 나선주사영상기법의 다양한 응용 가능성을 살펴보고자 한다. 대상 및 방법 : 3 Tesla 전신 자기 공명 영상 시스템에서 다양한 해상도의 나선주사영상 방법을 개발하였다. 고차(higher-order) shimming 을 통하여 영상의 화질을 개선하였고, 해상도에 맞게 interleaves 수를 조절하였다. 스핀에코 와 gradient에코 기반 나선주사영상방법을 구현하였고, 에코 time 과 repetition time, rf 회전 각도를 조절하여 영상의 대조도(contrast)와 신호대잡음비를 조절하였다. 결과 : 3 Tesla 전신 자기 공명 영상 시스템에서 나선 주사 방법을 이용하여 다양한 해상도의 영상을 얻었다. 고 자장에서 주 자장의 불균일도(inhomogeneity) 의 절대 크기가 커지기 때문에 이를 줄이기 위한 shimming 이 더욱 중요해진다. 한번의 스캔으로 axial, sagittal, coronal 방향의 불균일도 map을 구하여 spherical harmonics 분석으로 고차 shimming을 하였다. 팬텀과 in-vivo 두부 영상에서 single shot 나선주사 영상으로 $100{\times}100$ 정도의 영상과 6-12 정도의 interleaves 를 적용하여 $256{\times}256$ 의 고 해상도 영상을 얻을 수 있었다. 결론 : 신호대잡음비의 향상과 스펙트럼의 분리, 뇌기능영상에서 BOLD 효과 향상 등으로 고자장 영상에 대한 관심이 높아지고 있다. 그러나 고 자장 영상에서의 rf field 에 의한 SAR 증가는 중요한 제한 요소로 부각되고 있다. 나선주사영상은 SAR 문제가 근원적으로 발생하지 않고, EPI에 비하여 하드웨어 요구 조건이 낮아 고 자장에서의 고속영상방법으로 적합하다. 본 논문에서는 고차 shimming 을 통하여 불균일도를 개선하고, single shot 과 interleaving 을 적용한 multi-shot 나선주사영상 기법으로 $100{\times}100$에서 $256{\times}256$의 고해상도 영상을 얻어 고 자장에서 초고속영상기법으로 다양한 적용 가능성을 보였다.

SPECT/CT의 획득시간 증감에 따른 방사능농도 추정치의 변화 (Variation on Estimated Values of Radioactivity Concentration According to the Change of the Acquisition Time of SPECT/CT)

  • SPECT/CT는 보급 초기에 뛰어난 보정방법과 융합영상을 기반으로 한 정성적 기능이 주목받았고, 최근 동반진단치료(Theranostics)등의 도입으로 그 정량적 기능에 대한 관심과 활용이 증가되는 추세이다. PET/CT와 달리 SPECT/CT의 절대 정량화는 조준기의 종류, 검출기 회전과 같은 조건들이 영상획득과 재구성 방법 등에 까다로운 요소로 작용하고 있다. 따라서 본 연구에서는 SPECT/CT 촬영조건 중 투영상수와 투영상당 획득시간에 따른 총 획득시간(검사시간)의 증·감이 방사능농도 추정치에 미치는 영향을 알아보고자 한다. 부피 9,293 ml의 원통형 팬텀에 멸균수를 가득 채운 후 99mTcO4- 91.76 MBq를 희석하여 총 획득시간 600 sec(10 sec/frame × 120 frames, matrix size 128 × 128)의 조건으로 기준영상을 촬영하였고, 체적감도와 교정인자를 확인하였다. 기준영상을 중심으로 총 획득시간을 60(-90%), 150(-75%), 300(-50%), 450(-25%), 900(+50%), 1200(+100%) sec/frame으로 증·감시켜 비교영상을 획득하였고, 각 영상별 세부조건은 투영상당 획득시간(sec/frame)을 1.0, 2.5, 5.0, 7.5, 15.0, 20.0 sec/frame(투영상수 120frames 고정)로, 투영상수를 12, 30, 60, 90, 180, 240 frames(투영상당 획득시간 10 sec/frame 고정)로 설정하였다. 획득된 각 영상에서 관심체적을 통하여 측정한 계수를 바탕으로 정성적 평가로서 CNR(Contrast to Noise Ratio)의 변동률(%)을 확인하였고, 방사능농도 추정치의 변동률(%)을 통해서는 정량적 평가를 시행하였다. 이때 방사능농도 추정치(cps/ml)와 실제 방사능농도(Bq/ml)의 관계는 회복계수(RC_Recovery Coefficients)를 지표로 비교·분석하였다. 투영상수 변화에 따른 결과[CNR 변동률(%), 방사능농도 추정치 변동률(%), RC]는 총 획득시간 증감률(%) -90%에서 [-89.5%, +3.90%, 1.04], -75%에서 [-77.9%, +2.71%, 1.03], -50%에서 [-55.6%, +1.85%, 1.02], -25%에서 [-33.6%, +1.37%, 1.01], +50%에서 [+33.7%, +0.71%, 1.01], +100%에서 [+93.2%, +0.32%, 1.00]이었으며, 투영상당 획득시간 변화에 따른 결과는 총 획득시간 증감률(%)-90%에서 [-89.3%, -3.55%, 0.96], -75%에서 [-73.4%, -0.17%, 1.00], -50%에서 [-49.6%, -0.34%, 1.00], -25%에서 [-24.9%, 0.03%, 1.00], +50%에서 [+49.3%, -0.04%, 1.00], +100%에서 [+99.0%, +0.11%, 1.00]이었다. SPECT/CT에서 총 획득시간의 증·감에 따라 획득된 총 계수와 그에 따른 영상품질(CNR)은 비례하여 변화하는 양상을 보였지만, 절대 정량화를 통한 정량적 평가에서는 모든 실험조건에서 5% 미만(-3.55에서 +3.90%)의 변화를 보여 큰 영향을 받지 않고 정량적 정확성(RC 0.96에서 1.04)을 유지하였다. 검사시간의 증가보다는 단축을 우선하여 고려하였을 때 총 획득시간 감소는 정성적 기능에 있어서 기존에도 배제할 수 없었던 사항이지만 정량적 기능은 큰 손실 없이 적용 가능하여 임상적으로 실효성이 있다고 판단된다. 다만 총 획득시간의 증·감 시 동일한 검사시간이라면 투영상수의 변경보다는 투영상당 획득시간의 변경이 정성적, 정량적으로 조건변화에 따른 변동 폭이 적은 것으로 나타났다.

Analysis of Image Distortion on Magnetic Resonance Diffusion Weighted Imaging

  • The purpose of this study is to improve diagnostic efficiency of clinical study by setting up guidelines for more precise examination with a comparative analysis of signal intensity and image distortion depending on the location of X axial of object when performing magnetic resonance diffusion weighted imaging (MR DWI) examination. We arranged the self-produced phantom with a 45 mm of interval from the core of 44 regent bottles that have a 16 mm of external diameter and 55 mm of height, and were placed in 4 rows and 11 columns in an acrylic box. We also filled up water and margarine to portrait the fat. We used 3T Skyra and 18 Channel Body array coil. We also obtained the coronal image with the direction of RL (right to left) by using scan slice thinkness 3 mm, slice gap: 0mm, field of view (FOV): $450{\times}450mm^2$, repetition time (TR): 5000 ms, echo time (TE): 73/118 ms, Matrix: $126{\times}126$, slice number: 15, scan time: 9 min 45sec, number of excitations (NEX): 3, phase encoding as a diffusion-weighted imaging parameter. In order to scan, we set b-value to $0s/mm^2$, $400s/mm^2$, and $1,400s/mm^2$, and obtained T2 fat saturation image. Then we did a comparative analysis on the differences between image distortion and signal intensity depending on the location of X axial based on iso-center of patient's table. We used "Image J" as a comparative analysis programme, and used SPSS v18.0 as a statistic programme. There was not much difference between image distortion and signal intensity on fat and water from T2 fat saturation image. But, the average value depends on the location of X axial was statistically significant (p < 0.05). From DWI image, when b-value was 0 and 400, there was no significant difference up to $2^{nd}$ columns right to left from the core of patient's table, however, there was a decline in signal intensity and image distortion from the $3^{rd}$ columns and they started to decrease rapidly at the $4^{th}$ columns. When b-value was 1,400, there was not much difference between the $1^{st}$ row right to left from the core of patient's table, however, image distortion started to appear from the $2^{nd}$ columns with no change in signal intensity, the signal was getting decreased from the $3^{rd}$ columns, and both signal intensity and image distortion started to get decreased rapidly. At this moment, the reagent bottles from outside out of 11 reagent bottles were not verified from the image, and only 9 reagent bottles were verified. However, it was not possible to verify anything from the $5^{th}$ columns. But, the average value depends on the location of X axial was statistically significant. On T2 FS image, there was a significant decline in image distortion and signal intensity over 180mm from the core of patient's table. On diffusion-weighted image, there was a significant decline in image distortion and signal intensity over 90 mm, and they became unverifiable over 180 mm. Therefore, we should make an image that has a diagnostic value from examinations that are hard to locate patient's position.

핵의학 계측기기 및 감마카메라의 정도관리 연구 (A Study of Quality Control of Nuclear Medicine Counting System and Gamma Camera)

  • 목적: 최근 국내의 핵의학 계측기기 및 감마카메라의 정도관리 수행현황을 파악하고, 핵의학 계측기기와 감마 카메라의 정도관리를 수행하고자 하였다. 방 법: 최근 국내의 핵의학 계측기기 및 감마 카메라의 정도관리 수행현황은 총 53개 병원을 대상으로 설문조사방법을 이용하여 수행하였다. 이들의 정도관리 연구는 Capintec의 CRC-15 기종의 dose calibrator와 Tc-99m 35.52 MBq을 사용하여 2분 간격으로 정밀도를 측정하였다. Nucleus사의 기종의 Thyroid Uptake system은 Tc-99m 5.14 MBq을 이용하여 1분 간격으로 10초동안 정밀도를 측정하였다. 지름이 15 cm이고 높이가 각각 12 cm, 30 cm인 원통형 팬텀과 TC-99m을 이용하여 저에너지 고해상도 조준기가 부착된 CeraSPECT$^{TM}$의 예민도를 측정하였다. CeraSPECT$^{TM}$와 일반 평면카메라와의 예민도에 대한 특성비교를 위하여 Varicam (Elscint Ltd, Israel) 감마 카메라로 영상을 얻었다. CeraSPECT$^{TM}$로 획득한 자료로 각 슬라이스에 대한 보정상수를 계산하였다. Elscint 사의 Varicam 감마 카메라의 정도관리를 위해 저에너지 고해상도 조준기를 부착하고 140 keV 중심20% 에너지창, 256$\times$256 또는 512$\times$512 메트릭스 크기를 이용하여 시스템의 평면 예민도, 균일도, 계수율 및 공기중과 산란매질에서의 공간 분해능을 측정하였다. 결 과: 핵의학 계측기기 및 감마 카메라의 정도관리 수행율은 dose calibrator와 well counter의 경우 매우 저조한 수행율을 나타내었으며 그 외 감마 카메라 등은 대체로 양호한 수행율을 나타내었다. dose calibrator의 정밀도 측정은 $\pm$1.4%(<$\pm$5%)의 결과를 얻었고, thyroid uptake system의 정밀도 측정은 chi^2=29.7(>16.92)의 결과를 얻었다. Varicam 감마 카메라의 경우 슬라이스들간에 전반적으로 균일한 민감도를 보여주었으나 CeraSPECT$^{TM}$ 의 경우는 위쪽과 아래쪽 부분의 슬라이스들은 민감도가 두드러지게 떨어져 있었고 팬텀의 중심부분 슬라이스들은 민감도가 매우 높은 것으로 나타났다. 계산한 보정 상수를 이용하여 CeraSPECT$^{TM}$로 얻은 환자 자료를 보정하였을 때 보정전에 비하여 전반적으로 균일한 영상을 얻을 수 있었다. 감마 카메라의 시스템 평면 예민도 측정 결과는 4.39 CPM/MBq 이었으며, 시스템 균일도는 첫 번째 검출기와 두번째 검출기가 각각 2.14%, 3.79%로 나타났다. 시스템 계수율 측정의 경우 입력 계수율 R_20%가 각각 102,407 counts/sec (head 1), 113,427 counts/sec (head 2)일 때 20% 계수율 손실이 발생했을 때의 측정된 계수율 C_20%는 각각 81,926 counts/sec (head 1), 90,741 counts/sec (head 2) 이었다. 공기 중에서의 시스템의 공간 분해능은 FWHM이 8.16 m, FWTM이 14.85 mm이었고, 산란매질에서는 시스템의 공간 분해능은 FWHM이 8.87 mm, FWTM이 18.87 mm이었다. 결 론: 정확하고 신뢰도 높은 검사를 위해 정도관리는 필수이며, 이에 대한 명확한 인식과 실질적인 수행이 반드시 뒤따라야 할 것으로 사료되었다

Keyhole 방법을 이용한 MR 온도감시영상의 시간해상도 향상기법 (Time Resolution Improvement of MRI Temperature Monitoring Using Keyhole Method)

  • 목적 : 본 연구는 PRF(Proton Resonance Frequency)를 이용한 MR 온도감시 영상에서 시간 해상도를 keyhole방법 적용으로 향상시키고자하였다. 제시된 keyhole방법과 기존 온도영상 방법 사이의 비교를 위해 온도 값에 대한 RMS(Root Mean Square) 오차와 SNR(Signal to Noise Ratio)을 비교하였다. 대상 및 방법 : PRF 방법과 GRE(Gradient Recalled Echo)를 이용하여 MR 온도영상을 구현하였으며 장비로는 임상용 1.5T MRI 장치를 이용하였다. 인체모사 조직인 2% 한천 젤 팬텀과 돼지 근육조직으로 실험을 수행하였다. 2.45GHz대역의 마이크로파 발생장치로 MR호환 동축 슬롯 안테나를 구동하여 MRI장치 내에서 대상 조직과 팬텀을 5분간 가열하였다. 가열 직후 10분 동안에 순차적으로 MR 원 데이터를 획득하였다. 획득된 원 데이터는 PC로 전송되어 전체 위상을 부호화하여 얻은 원 데이터의 바깥영역과 K-space의 중앙 영역을 각각 128, 64, 32, 16으로 위상부호화된 데이터로 keyhole영상을 재구성하였다. 256개로 전체 부호화된 자체-참조 온도영상과 RMS 오차를 비교하였으며, zero-filling 영상과 SNR비교를 하였다. 결과 : keyhole 온도 영상에서 위상부호화 수가 128, 64, 32, 16으로 줄어들수록 RMS 오차로 산출한 온도의 차이가 0.538, 0.712, 0.786, 0.845$^{\circ}C$ 만큼 증가하였으나 SNR 값은 keyhole의 위상부호화 수가 줄어도 유지되었다. 결론 : 본 연구는 고정된 매트릭스 크기에 keyhole 방법 적용을 이용하여 온도 감시에서의 시간해상도 증가와 SNR 값을 유지하는 결과를 도출하여 성공적인 적용을 보여 주었다. 본 연구를 기반으로 한 다음 연구에서는 최적화된 변수를 이용한 keyhole 방법 적용으로 최소 온도 오차의 실시간 MR 온도 감시가 가능할 것이라 예상된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The correlation between glucose concentration and parameters was the best at 610nm. 30535 T00401030535 ^x Radiation-induced fibrosarcoma tumors were grown on the flanks of C3H mice. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. On this base, we composed of the radial pulse diagnosis system in korean traditional medicine. The system composed of a radial pulse wave detection system and a radial pulse diagnosis system. To synthesize microspheres for biomedical application, "two stage shot growth method" was developed. The uniformity ratio of synthesized microspheres was always smaller than 1.05. And the surface charge density (or the number of ionizable functional groups) of the microspheres synthesized by "two stage shot growth method" was 6~13 times higher than that of the microspheres synthesized by conventional seeded batch copolymerization. The system based on PC-486DX was designed to acquire medical images produced by medical imaging instruments and related audio information, that is, doctors' reporting results. Input information was processed and analyzed, then the results were presented in the form of graph and animation. All the informations of the system were hierarchically related with the image as the apex. Processing and analysis algorithms were implemented so that the diagnostic accuracy could be improved.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
