Postpartum depression is one of the most serious problems in maternal health because it affects not only the mother but also her family. Postpartum depression disturbs maternal -infant interaction and attachment. However, most postpartum depression patients ignore this problem and do not seek treatment. Thus this study in conducted to development of a Home-Based Multimedia Tutoring System for postpartum depression management. With this computerized system, mothers in the postpartum periods can check the level of postpartum depression using a personal computer. This system will go through each mother's data and screen those who have abnormal values. In addition this system includes intervention programs -education for nutrition, hygiene care, sleep, postpartum exercise, methods of relaxation, deep breathing, visualization, music therapy and family therapy-to relieve postpartum depression. Using this system, a mother who has a minor level of depression can manage it by herself. Computer language used in this study were html 3.2, OS used was Microsoftware NT Server 4.0, the graphic tool was Adobe Photoshop 4.0, and the Webpage tool was Notepade. The results of this study are show at internet “URL : http : //203.241.225.42/”. Finally, the author suggests that this system could be adequately applied to assessing postpartum depression and as a intervention strategy for mothers during the postpartum period. Further this study contributes to designing an appropriate postpartum depression prevention strategy.

This study was performed as a part of survey for the operation and interim evaluation of the on-going pilot project of community health in Jeomdong-Myon, Yeoju-Gun, Kyunggi-Province since 1982. Seven Villages (Ri) were selected purposively for the survey from a total of 26 villages in Jeomdong-Myon, target area, under the consideration of the socio-economic background and the distribution of health resources. The target population was the women in age group of 15-44 years in that area and a total of 156 women were interviewed. This study was done by the interview survey using questionnaire which was composed with questions about MCH and FP. The results were as follows; 1) ln the status of maternal health, (1) the proportion of receiving antenatal care was relatively high (67.1%) in this area, but the starting time of antenatal care was delayed to 7 months or more of gestational period in 22 percent. Therefore some intervention would be necessary for advancing the time of antenatal care. (2) The proportion of home delivery was decreased according to the rank of birth-order. And more than 50 percent of delivery attendants were mothers-in-law or neighbors. The fact indicates that the health education for non-professional delivery attendents would be necessary for improving home delivery and the content of education would be preferable to focussing to the method of hygienic delivery attending. (3) The comprehensive health service and education would be recommended for the postnatal care, because of the extremely low rate of postnatal care in this area. 2) In the status of child health, (1) the pro-portion of breast feeding was 90.2 percent, and the weaning started after 12 months of birth in 12.7 percent. This indicates the necessity of education about the weaning program. (2) The proportion of completed basic vaccination was 39.6% and the main reason of missing vaccination was poor accessibility in terms of time. For that, the supply of vaccine to primary health care units (health subcenters) should be continuous, not infrequent nor occassional. 3) The proportion of respondants who were using contraceptive methods currently was 79.8 percent and in 44.7 percent they accepted the permanent contraceptive methods. Those results are distinct in this area. But it seems a serious health problem that 53.2 percent of respondants were experienced the induced abortion for birth control.

Many clinicians and researchers realiza there is a need to develop a postpartum depression scale. Thus, this study has been designed to development of a postpartum depression scale. Data were collected through a survey over a period of three months. Subjects who participated in the study were 167 Korean mothers in their postpartum period. The author used a convenience sampling method. The analysis of the data was done with SPSS PC/sup +/ for descriptive statistics, item analysis and factor analysis. Initially 62 items were generated from the interview data of eight postpartum depression patients and from a literature review. This preliminary scale was analyzed for reliability and validity. The results of this analysis are as follows. 1. Initially 62 items were analyzed through the Index of Content Validity(CVI) and 48 items were selected. 2. Seven factors were extracted through the principal component analysis, and these contributed 61% of the variance in the total score. Finally 46 items in the scale loaded .41∼ .84 on one of seven factors. 3. Each factor was labeled. Factor 1 was labeled 'emotional phenomena-emotional upset' and included 13 items, factor 2 was labeled' cognitive phenomena-self concept disturbance' and included seven items, factor 3 was labeled 'relationship to baby-negative feeling' and included six items, factor 4 was labeled 'relationship to baby- overload' and included eight items, factor 5 was labeled 'negative maternal identity' and included five items, factor 6 was labeled 'biophysiological phenomena-disturbance of physical functioning' and included four items, and factor 7 was labeled' interpersonal relationship phenomena-blamed others' and included three items. 4. Cronbach Coefficient Alpha for internal consistency was .95 for the total 46 items. Finally, the author suggests that this scale could be adequately applied in assessing the postpartum depression of mothers during the postpartum period. The results of this study can contribute to designing an appropriate postpartum depression prevention strategy.
Gestational diabetes mellitus (GDM) has become one of the major public health problems for both mothers and children globally. Internationally, the frequency of excess weight and obesity has risen dramatically in women of childbearing age. There seems to be a greater risk of having GDM in overweight or obese women, resulting in problems during pregnancy, birth and neonatal development. Hospital management is a problem for obese pregnant females with GDM and places extra burdens on the healthcare sector. GDM can result in possible risks to the wellbeing of the mother, fetus, and infant, as well as clinically significant negative effects on the mental health of the mother. For females and their developing babies, diabetes may cause problems during pregnancy. Unsatisfactory diabetes control enhances the risk of complications and other birth related issues during pregnancy. It may also cause a woman to suffer severe complications. Numerous maternal and fetal effects are associated with GDM and multiple detection and management methods are also pursued globally in order to reduce the burden of health. An overview of gestational diabetes treatment is given in this review.
The objective of this study is to review the role and function of the village health worker in the wake of the recent social changes in Korea. The stud}' also aimed at' elucidating' the characteristics of the village health worker organizations developed through community participation in this country. The study methods employed were: a review of literature and other existing information related to VHW's activities ; the use of Questionnaire and interview with VHW's on their activities in one selected county in 1992. The major findings obtained are as follows: 1. So far the community health care activities have been carried out by medical colleges, foreign aid agencies and religious organizations with a view to improving health care for the population in rural area. 2. The VHWs employed by the local community health services received remuneratiorin cash or in kind. However, presently no remuneration is paid to VHWs employed by the government village health projects. The training for VHWs is organized by County Health Center once a year for 1-2days regardless of pre-employment or refresher. 3. The needs of care for maternal and child health and family planning has been decline due to the decrease of the target population, but on the other hand, the needs of care by VHW for prevention of infectious diseases, health of the aging, management of geriatric diseases, prevention of poisoning of pesticides and environmental sanitation has beer increased. 4. It was felt strongly that the training for VHWs should be strengthened by means of developing a problem-solving oriented educational program in order to improve practical skills of VHWs.
Decentralization to local governments and amending of Health Center Law are to promote the efforts of health planning at the level of local agencies. In the health facility planning, it is important to take into account that what to be built, where to be located, how far should be service area and so forth, because health facilities are immovable, and require capital as well as personnel and consumable supplies. The aim of our study, answering to the question of 'where to be located?', is to determine the best location of urban health sub-center. At the local level, planning is the matter of finding the best location of specific facilitiy, in relation to population needs. We confine the accessibility, which is basic to location planning, to geographic one. Location-Allocation Model is used to solve the problem where the location is to maximize geographic accessibility. To minimize the weighted travel distance, objective function, $R_k=\sum{\sum}a_{ij}w_{i}d_{ij}$ is used. Distances are measured indirectly by map measure-meter with 1:25,000 Suwon map, and each potential sites, 10 administrative Dongs in Kwonson Gu, Suwon, are weighted by each number of households, total population, maternal age group, child age group, old age group, Relief for the livelihood, and population/primary health clinics. We find that Kuwoon-Dong, Seodun-Dong, Seryu3-Dong, according the descending orders, are best sites which can minimize the weighted distance, and conclude that it is reasonable to determine the location of urban health sub-center among those sites.
Purpose: The purpose of this study was examine levels of breastfeeding knowledge, attitude and number of breastfeeding problems in early postpartum period and rate of breastfeeding practice at 3 months. Method: The total numbers of the study subjects were 139 mothers at postpartum unit and followed by postpartum 1 week and 3 months. Result: 1. The mean score of knowledge and attitude and problems toward breast feeding scale were 22.83(SD=5.19), 20.99(SD=3.40), and 9.04(SD=3.59) respectively, indicating low to moderate level according to subjects characteristics, breastfeeding knowledge scores were significantly different by age (F=8.00, p<.001), breastfeeding experience (t=3.26, p<.001) and parity(t=2.39, p<.05), but no difference were found in rate of breastfeeding attitude and number of problems. 2. Rate of breastfeeding practice was 46.5% at three months of postpartum and the breastfeeding practice was significantly different by breastfeeding plan period($x^2$=13.33, p<.001) and monthly income ($x^2$= 3.80, p<.05). 3. Further, breastfeeding practice at 3 months was continuously influenced by number of problems of the breast-feeding(OR=.85) and breastfeeding plan period(OR=2.11) at early postpartum period. Conclusion: The findings suggest the necessity of maternal support during postpartum period to provide correct information about rate of breastfeeding knowledge and attitude to teach problem solving skills for any breastfeeding problems in order to increase rate of breastfeeding practice.
Death is one of the population movement phenomena used as an important health index in most society. Especially it is regarded as group phenomenon in a specific group rather than individual one and considered important in public health field because the level and cause of death is related to health of public. The auther examined the changes of the Korean mortality pattern to evaluate the status of the Korean public health by studying mortality indicator using the population census and other materials from 1930 to 1980. The results are as follows: First, the Korean crude death rate was reduced to one third in 1980 compared to that in 1930, but the crude birth rate was constant from 1930 to 1960 causing the increase of population. So the population pattern is changing from the classic pyramic shape to bell shape and the dependency ratio was reduced from 78 in 1930 to 61 in 1980. Second, the infant mortality rate decrease rapidly. In 1980 it was one seventh of that in 1930 which was proved by the change of the age-specific death rate curve from U to J shape. Third, the male mortality reduction after the age of forty was much less than that of female, which explained the specific death pattern of high mortality in Korean middle and oldaged males. Fourth, the main cause of death was changed from infectious, parasitic, respiratory and digestive system disease to circulatory of tumorous diseases. Considering the above results, Korean health problem is now changing from the infant infection to geriatric chronic regressive disease. That naturally the direction of health service should be turned from the infant stage maternal and child health to the health problems of old people.
That's an attempt to study about knowledge, attitudes and practice of the residents with regard to maternal child health, family, planning, and tuberculosis in rural area which was chosen to be a demonstration area for nursing students of College of Nursing, Yonsei University for their public health nursing practice. In addition, it is designed to collect information on population of the area such as age and sex distribution, mobility, and other characteristics also be sought by doing a census. The purpose of study is to obtain data which are needed for planning student learning experiences in the community. The information which was obtainable through this study is as follows; 1. In demographic aspect a. Population structure was a typical rural type in terms of population pyramid. b. Distribution of age and sex of residents showed more female in the older ages. c. Population has not moved much. Majority of them have been living in the area since their births. 2. In sociological aspect a. The level of education of female was comparatively lower than male. b. The residents of the area did not have any special religion. c. The economic status was found low or middle class. 3. In health practice In general health knowledge, attitudes and practice appeared to be poor. As a result of this study, it could be summararized that there are a great public health nursing needs in the community. According to these needs found and to it's nature, severity and massiveness, the nurse instructors with students were able to set up a priority system in services. Meanwhile student case load can be logically figured out in consideration of variety of experiences need for each student. The study helped student not only to find out resident characteristics and their health problems in terms of family and community problem but also to provide a good learning experience by participating in the interviewing. In sum this type of study is necessary for planning of student field experience when no data are available where students are going to have field experiences in public health nursing.

Diabetes represents a serious health problem. In the diabetic state, alterations in metabolism, increased susceptibility to infections and immunological changes occur. The suppression of the immune response has been identified as a relevant factor that contributes to the increase in the rate of infections in these patients. At the same time, breast cancer is the most frequent malignant tumor in women. The molecular and cellular mechanisms underlying cancer development have revealed that immune cells functionally regulate epithelial cancer development and progression. Breastfeeding has been hypothesized to reduce the risk of breast cancer. However, early systematic reviews have not yielded consistent findings for this association. The demand for human milk is increasing due to the promotion and consumer acceptance of the health benefits of consuming a natural product rich in bioactive components. However, due to changes in glucose metabolism, the components of the milk from diabetic women are modified depending on the time of evaluation. In this literature review, we summarize important new findings revealing the paradoxical role of breastfeeding in preventing the onset of breast cancer in diabetic mothers. We hypothesized that the milk component production in diabetic mothers is affected by changes in glucose metabolism. Therefore, adequate maternal glycemic control and an adequate duration of breastfeeding for diabetic mothers are crucial to ensure that the immunity components are able to confer protection against breast cancer.
