A STUDY ON THE RELATION BETWEEN DELAIRE'S IDEAL OCCLUSAL PLANE AND MASTICATORY MUSCLE ACTIVITY IN KOREAN NORMAL ANGLE CLASS I OCCLUSION INDIVIDUALS (한국성인 정상교합자에서 Delaire의 이상적 교합평면과 저작근 근활성도와의 관계에 대한 연구)

  • According to the functional matrix theory, Delaire proposes that individual occlusal plane was determined by variable effects of teeth, maxilla, mandible, cranium, cranial base and soft tissue matrix including the orofacial musculature. and that there is the ideal occlusal plane determined by the most proper spatial position of maxilla and mandible, functionally and esthetically. This study was designed to find out the relation between Delaire's ideal occlusal plane and muscle activity of masticatory muscles in individuals who have normal maxillo-mandibular relationships. Lateral cephalometric radiographs were taken and his/her individual occlusal plane and ideal occlusal plane were analyzed with Delaire's architectural and structural craniofacial analytic method. For evaluation of muscle activities of masticatory muscles, electromyography of anterior temporal muscle, superficial masseter muscle, and anterior belly of digastric muscle was recorded in fifty Korean normal Angle class I occlusion individuals. According to the average value of ideal occlusal plane, fifty normal Angle class I occlusion individuals were classified into three groups: Ideal occlusal plane group(I group), hyperrotation group(I+ group) and hyporotation group(I- group). The result of this study was as follows: 1. The results of Delaire's architectural and structural craniofacial analysis of lateral cephalography of the fifty Korean normal Angle class I occlusion individuals are that twelve persons(24%) have consistent or parallel with ideal occlusal plane and the average of angular difference was $1.22^{\circ}{\pm}3.69^{\circ}$. 2. There is no significant difference in muscle activities of masticatory muscles during resting(p<0.05), but significant increases of muscle activity of ipsilateral anterior temporal and masseter muscle, contralateral anterior belly of digastric muscle during unilateral chewing and of anterior temporal and masseter muscle during bilateral clenching(p<0.05). 3. To find out the effect of the angular difference between Delaire's ideal occlusal plane and real occlusal plane to muscle activity, muscle activities of masticatory muscles were compared with three groups in each other; I group, I+ group and I- group. The results were no significant differences during resting, unilateral chewing and bilateral clenching.(p>0.05) 4. Although there is no significant differences of masticatory muscle activities among the three groups, the fact that increasing tendency of masseter muscle activity of ideal occlusal plane group(I+) than those of any other groups(I+ and I-) during bilateral clenching was noted. There is only the implication that occlusal plane makes some effects on masticatory muscle activities, espacially that of masseter muscle during bilateral clenching. In conclusion, the hypothesis that occlusal plane is one of the factors which affect the muscle activities of masticatory muscles and that anyone whose occlusal plane consistent with Delaire's ideal occlusal plane has an extraordinary functional advantage in masticatory muscle function cannot be proven with electromyography methods.

Effect of Craniocervical Flexion Exercise with Masticatory Muscle Contraction on Deep Cervical Muscle Thickness and Neck Disability Index in Patient with Neck Pain (씹기근육 수축을 동반한 머리목굽힘운동이 목통증 환자의 깊은목뼈굽힘근의 수축두께와 목장애지수에 미치는 영향)

  • PURPOSE: The purpose of this study was to determine the effect of craniocervical flexion exercise with masticatory muscle contraction in patients with neck pain. Patients with neck pain also experience muscle weakness and limitation of activities of daily living. Craniocervical flexion exercise with masticatory muscle contraction may increase the thickness of the deep cervical flexor muscles, thereby reducing pain and limitations on activities of daily living. METHODS: Twenty-six volunteers participated in this study. The subjects were randomly divided into an experimental group (n=13) and a control group (n=13). Both groups performed craniocervical flexion exercise using a pressure biofeedback unit; the experimental group performed craniocervical flexion exercise with masticatory muscle contraction for 20 minutes, 3 times a week for 4 weeks. Assessment tools included ultrasonography for measurement of muscle thickness and the Neck Disability Index for the level of pain and function. RESULTS: The rate of change in muscle thickness in both groups significantly increased (p<.05), with a significantly greater increase in the experimental group than in the control group (p<.05). The Neck Disability Index score significantly improved (p<.05) in both groups, with significantly greater improvement in the experimental group compared with the control group (p<.05). CONCLUSION: These results suggest that craniocervical flexion exercise with masticatory muscle contraction can be effective in increasing muscle thickness and improving the Neck Disability Index score in patients with neck pain.

Comparison of Satisfaction Before and After Dental Implant (치과 임플란트 시술 전·후 만족도 변화 비교)

  • This study was conducted to identify the factors affecting the satisfaction of patients undergoing dental implant surgery and to improve the satisfaction after the procedure. According to the study results of 178 dental implants patients who visited the dental hospital, regarding changes made before and after the implant procedure, 1.07 in terms of the masticatory function, while the social function evolved 0.7 points. In addition, the psychological function showed transformation 0.97 points. Furthermore, it will be necessary to make constant management through education and counseling regarding effective oral health management methods posterior to the implant procedure.

  • PURPOSE. To study the distribution of various occlusal schemes as well as associated static occlusal relationship among Thais. MATERIALS AND METHODS. The subjects'occlusal schemes on the left and right sides were classified as canine protected occlusion, group function, or unclassified according to the definition from Glossary of Prosthodontic Terms ($8^{th}$ edition). In addition, the presence of occlusal interferences during mandibular excursions were recorded. The measurement of the horizontal overlap and vertical overlap was also performed. Chi-square, One-way ANAVA and Mann-Whitney U statistical tests were used with level of significance set at P=.05. RESULTS. Total of 104 subjects were included in this study. The ratio for male to female was 1.8 to 1. Average age of the population was $25.01{\pm}6.87$ years old. The mean vertical overlap and horizontal overlap were $1.94{\pm}1.20$ and $2.41{\pm}1.32$ mm respectively. The majority of the populations (68.3%) possessed group function occlusal scheme. For the remaining, 17.3% possess canine protected occlusion and 12.5% possess combination of both occlusal schemes. We also found that occlusal interference was presented in 20.2% of the population. The most common was protrusive interference (57.14%), the second was balancing interference (38.1%) and the third was working interference (4.1%). CONCLUSION. Among Thais, the most common occlusal scheme was group function, however there were no significant occlusal factors related to any particular occlusal scheme.

Patients' Functional Satisfaction of Dental Implant Treatment (임플란트 시술에 대한 환자의 기능별 만족도)

  • This study was aimed to provide basic data which are fairly useful to improve satisfaction level of patients by analyzing factors related to each functional satisfactions of dental implant treatment. The subjects of this study were 120 patients who received implant treatment and analyzed with t-test, one-way ANOVA and pearson's correlation. The results of this study were as follows. 1. Differences of each functional satisfaction according to general characteristics was not reveal statistically significant. 2. A group to got periodic scaling and tooth brushed four or more times a day showed the highest satisfaction of social and esthetic function(p < .05). 3. A group to implantation in premolar area showed the highest satisfaction of masticatory function and a group to implantation in incisor to be high in satisfaction of social and esthetic function, but the differences were not statistically significant 4. A group to got implant treatment at a dental hospital showed the highest satisfaction of both social and esthetic function, and at a general hospital were found lowest(p < .05). 5. Masticatory function, social function, esthetic function, and overall satisfaction were found to be in a positive correlations, and especially, the correlation coefficient between the overall satisfaction and social function was found to be highest(p < .01).

The Association Between Masticatory Function Assessment and Masseter Muscle Thickness in the Elderly

  • Purpose: This study investigated the association between the objective indicator of masticatory function assessment and the masseter muscle thickness (MMT) using ultrasound imaging. Methods: A total of 99 subjects (males: 24, females: 75, mean age: 76) were analyzed. The maximum bite force (MBF) was measured with a pressure-sensitive sheet and an image scanner. The mixing ability index (MAI) was calculated by image analysis after asking the subjects to chew a wax specimen. The MMT during rest and clenching were obtained with a diagnostic ultrasound system, and the difference in MMT during rest and MMT during clenching was defined as the difference in masseter muscle thickness (DMMT). Multiple regression analysis was performed to determine the independent variables affecting MBF and MAI. Results: The MBF showed correlation with the number of remaining teeth (β=0.346, p=0.002) and DMMT (β=0.251, p=0.011). The MAI correlated with only the number of remaining teeth (β=0.476, p<0.001). Conclusions: The DMMT reflects the state of masseter muscle contraction, and can be used as a predictor as well as the number of teeth when assessing masticatory function.

A STUDY ON THE SATISFIED DEGREE OF ORAL FUNCTION IN GERIATIRIC PATIENTS WITH THE SHORTENED DENTAL ARCH (단치궁 노인의 구강 기능 만족도에 관한 연구)

  • The purpose of the present study is to inspect the satisfied degree of each oral function in geriatric patients with the shortened dental arch and when their prosthetic treatment is on schedule, provide some references to such treatment. For the approach to such study, 521 subjects were reviewed by grouping them according to the number of their remaining teeth, and masticatory function, phonetic function, facial change, and TMJ disorders were inspected and clarified through some questionnaires. Also through the questionnaires, the correlations between the geriatiric patients with the shortened dental arch and dentition and between the geriatiric patients with the Free-end RDP at the shortened dental arch and their oral function were found out with their satisfied degree of oral function. Results or findings from such study are as follows : 1. With regard to their satisfied degree of oral function, there was a significant difference of satisfaction between or among the group having only the anterior teeth and the group having the part of premolars and the group having even the part of molars, however no significant difference of satisfaction appeard between the group having 1st molars and the group having 2nd molars. 2. With regard to their satisfied degree of phonetic function, no significant difference appeared between or among the group having only the anterior part of teeth and the group having even the part of premolars and the group having even the part of molars, and with regard to their satisfied degree of facial change, no significant difference of satisfaction appeared between the group having the part of premolars and the group having even the part of molars. 3. With regard to their satisfied degree of masticatory function, phonetic function, TMJ disorders, and facial change, no significant difference appeared between the group having both the anterior part of teeth and the part of premolars and the group attached with the Free-end RPD on the same conditions of the afore-said group.

Discussion of Neurologic Factor Influencing on Chewing Ability of Implant (임플란트 저작능에 영향을 주는 신경학적 원인에 대한 고찰)

  • Many researches have been published about the numerous factors related to the chewing ability of implant prosthesis. Most respective studies have concluded that the chewing ability of implant prosthesis is mostly fine compared to other type of prosthesis. However, some patients are not satisfied with their chewing ability of implant prosthesis. Therefore the neurologic factor, one of the factors related to dissatisfaction of chewing ability was reviewed in this study to understand the mechanism of action of mastication. Data was searched using the keywords; 'implant chewing ability, masticatory ability' in Pubmed database and reviewed. Definitions of chewing ability, factors of chewing ability are reviewed and the neurologic factor, one of the factors influencing on chewing ability, is reviewed. Mechanoreceptor of Periodontal ligament(PDL) is providing the mastication information to brainstem. Due to the absence of mechanoreceptions of PDL in implant, masticatory ability is decreased especially when chewing hard food. Masticatory muscles and mechanoreceptor in TMD may compensate the lack of mechanoreceptor of PDL in implants. Furthermore sensitivity of nerve fiber around peri-implant tissues may support the mechanoreception and sensory reaction in the implant mastication. However, further studies should be conducted to prove the relationships between neurologic factors and mastication.
