Restorative procedures can lead to weakening tooth due to reduction and alteraton of tooth structure. It is essential to prevent fractures to conserve tooth. Among the several parameters in cavity designs, cavity isthmus and depth are very important. In this study, MO amalgam cavity was prepared on maxillary first premolar. Three dimensional. finite element models were made by serial photographic method and cavity depth(1.7mm, 2.4mm) and isthmus (11 4, 1/3, 1/2 of intercuspal distance) were varied. linear, eight and six-nodal, isoparametric brick elements were used for the three dimensional finite element model. The periodontal ligament and alveolar bone surrounding the tooth were excluded in these models. Three types model(B, G and R model) were developed. B model was assumed perfect bonding between the restoration and cavity wall. Both compressive and tensile forces were distributed directly to the adjacent regions. G model(Gap Distance: 0.000001mm) was assumed the possibility of play at the interface simulated the lack of real bonding between the amalgam and cavity wall (enamel and dentin). When compression occurred along the interface, the forces were transferred to the adjacent regions. However, tensile forces perpendicular to the interface were excluded. R model was assumed non-connection between the restoration and cavity wall. No force was transferred to the adjacent regions. A load of 500N was applied vertically at the first node from the lingual slope of the buccal cusp tip. This study analysed the displacement, von Mises stress, 1 and 2 direction normal stress and strain with FEM software ABAQUS Version 5.2 and hardware IRIS 4D/310 VGX Work-station. The results were as follows: 1. G model showed stress and strain patterns between Band R model. 2. B model and G model showed the bending phenomenon in the displacement. 3. R model showed the greatest amount of the displacement of the buccal cusp followed by G and B model in descending order. G model showed the greatest amount of the displacement of the lingual cusp followed by B and R model in descending order. 4. B model showed no change of the displacement as increasing depth and width of the cavity. G and R model showed greater displacement of the buccal cusp as increasing depth and width of the cavity, but no change in the displacement of the lingual cusp. 5. As increasing of the width of the cavity, stress and strain were not changed in B model. Stress and strain were increased on the distal marginal ridge and buccopulpal line angle in G and R model. The possibility of the tooth fracture was increased. 6. As increasing of the depth of the cavity, stress and strain were not changed in B and G model. Stress and strain were increased on the distal marginal ridge and buccopulpal line angle in R model. The possibility of the tooth fracture was increased.

The placement of direct composite resin restorations has generally been found to have a strengthening effect on the prepared teeth. It is the purpose of this investigation to study the relationship between the cavity isthmus and the fracture resistance of a tooth in composite resin restorations. In this study, MO cavity was prepared on maxillary first premolar. Three dimensional finite element models were made by serial photographic method and isthmus(1/4, 1/3, 1/2 of intercuspal distance) were varied. Two types of model(B and R model) were developed. B model was assumed perfect bonding between the restoration and cavity wall and R model was left unfilled. A load of 500N was applied vertically at the first node from the lingual slope of the buccal cusp tip. This study analysed the displacement, 1 and 2 direction normal stress and strain with FEM software ABAQUS Version 5.2 and hardware IRIS 4D/310 VGX Work-station. The results were as follows : 1. Displacement of buccal cusp in R model occurred and increased as widening of the cavity, and displacement in B model was little and not influenced by cavity width. 2. There was a significant decrease of stress resulting in increase of fracture resistance in B model when compared with R model. 3. With the increase of the isthmus width, B model showed no change in the stress and strain. In R model, the stress and strain increased both in the area of buccal-pulpal line angle and the buccal side of marginal ridge, therefore the possibility of crack increased. 4. The stress and strain were distributed evenly on the tooth in B model, but in R model, were concentrated on the buccal side of the distal marginal ridge and buccal-pulpal line angle, therefore the possibility of fracture increased.
Incidence of using esthetic composite resin in the posterior area is increasing but there were lots of inconsistent reports about their microleakage and marginal adaptation. The purpose of this study was to evaluate the differences of microleakage and marginal adaptation according to restorative techniques. 30 cavities with enamel gingival margin were prepared and restored with 3 types of composite resin [Z-100($Scotchbond^{TM}$ MP), AELITEFIL ($Onestep^{TM}$), Her culite XRV(Fuji BOND LC)] in direct technique and another 30 cavities were restored with preformed CR inlays and 3 different modern resin and resin-modified GI cements (Superbond C&B, Choice, Fuji Duet). Samples were chemically stressed in 75% ethanol for 24 hours and were thermocycled (5-$55^{\circ}C$(500 times. The degree of microleakage through proximal and gingival margins was examined by 1 % MB dye and the degree of marginal adaptation by examining the margins via SEM. The following results were obtained ; 1. In direct groups, Herculite XRV [Fuji BOND LC, 35.13 (15.50) %] group showed statistically different, less microleakage than Z-100 [$Scotchbond^{TM}$ MP, 72.91 (16.91 %] group and AELITEFIL [One-step, 93.73 (13.66) %] group (p<0.05). 2. In indirect groups, the degree of microleakage in Mean(S.D.) were: Super bond C & B [39.00 (24.35) %], Choice [57.19 (33.80) %], Fuji Duet [58.22 (40.36) %]. But there was no significant difference. 3. There was no significant difference between resin cement and resin-modified GI cement. 4. There were gaps at the interface with the tooth structure, but no gap was seen at the interface with restoration in all specimens. 5. In direct groups, Herculite XRV(Fuji BOND LC) group made little gap compared with other groups, but 40-$50{\mu}m$ thickness of bonding agent, Fuji BOND LC, looked like a cement used in indirect technique. 6. All indirect groups showed a variety of cement thickness, from less than $20{\mu}m$ to over $100{\mu}m$ and that dimension of buccal/lingual margin was less than that of gingival margin.
Clinical application of composite resin recently draw great concerns in dentistry. Especially due to advantages such as esthetics, adhesiveness, simple clinical procedures, various shapes and kinds of composite resins are widely being applied to prosthodontics, conservative dentistry, and orthodontics. But, clinical problems attributable to the polymerization shrinkage of composite resin have been proposed, and we have to regard clinical problems such as secondary caries, loss of restoration, fracture of the surrounding tooth structure, marginal discoloration, and tooth sensitivity, and many portions are remained to be overcome. Therefore, this study attempts to analyze stress distribution between resin and tooth structure which is generated during polymerization shrinkage of composite resin using three dimensional finite element method. Three dimensional finite element models with conventional box-shape cavity and erosion/abrasion type V-shape lesion cavity in upper central incisor were developed. These cavities were filled with four different types of placement techniques. (bulk filling, horizontal increment filling, oblique occlusal increment filling, oblique gingival increment filling) The stresses generated by polymerization shrinkage of composite resin were calculated. The results analyzed with three dimensional finite element method were as follows : 1. The increment filling technique showed the highest maximum normal stress in both conventional box-shape and V-shape cavities and showed a tendency to decrease after complete polymerization. 2. The bulk filling technique resulted in increased stresses during the curing process in both conventional box-shape and V-shape cavities and the highest maximum normal stress occurred after complete polymerization. 3. The bulk filling resulted in the lowest maximum normal stress in both box-shape and V-shape cavities 4. Regardless of placement method, in conventional box-shape cavity, the maximum normal stress increased in dentin floor, enamel, dentin sequence and in V-shape cavity, the maximum normal stress increased in enamel, dentin sequence.
In this study, we tried to evaluate the effect of Amalgambond liner application on the degree of marginal leakage and retention of amalgam restoration by comparing with that of the Copalite and All-bond 2. The results obtained from this experiment were as follows; 1. Tensile strength representing the bond between amalgam and tooth structure was the highest in Amalgambond, and All-Bond 2, Copalite in descending order. There were statistically significant difference between each group(P<0.05). 2. The degree of microleakage in Amalgambond was lower than that of All-Bond 2, and Copalite, but no stastically significant difference could be found (P>0.05). 3. The liner such as Amalgambond or All-Bond 2 were evaluated to be superior to the conventional Copalite in bond strength as well as in microleakage. But the result of this study could not show the superiority of one material over the others; Amalgambond and All-bond 2. Besides the results of the study, other factors, such as practical convenience, should be considered in determining the selection of material. The support of welldesigned clinical studies on this subject are in demand.
한국산 전복과 2속 2종 및 4아종 (N. gigantea, N. madaka, N. discus discus, N. discus hannai, S. diversicolor diversicolor, S. diversicolor supertexta)을 대상으로 주사전자현미경 (SEM)을 이용하여 치설 관찰을 하였다. 그 결과 전복과 패류의 치설형은 비대칭적인 선설형 (Rhipidoglossate form)이며, 치식 (Radular formula)은 >50-5-1-5-50< 으로 중앙에 1열의 중치와 3열의 측치 그리고 50열 이상의 연치로 구성되어 있다. 이러한 치설 구조는 실험 대상종 모두 공통적인 결과로 종 또는 속 간 치설 구조의 차이는 나타나지 않았다. 그러나 각 종별 세부적인 치설 형태의 차이점은 인지되어, 이를 규명하기 위한 정밀하고 세밀한 관찰이 요구되었다.
The basic principles in the design of Class II amalgam cavity preparations have been modified but not changed in essence over the last 90 years. The early essential principle was "extension for prevention". Most of the modifications have served to reduce the extent of preparation and, thus, increase the conservation of sound tooth structure. A more recent concept relating to conservative Class II cavity preparations involves elimination of occlusal preparation if no carious lesion exists in this area. To evaluate the ideal ClassII cavity preparation design, if carious lesion exists only in the interproximal area, three cavity design conditions were studied: Rodda's conventional cavity, simple proximal box cavity and proximal box cavity with retention grooves. R option model was assumed non-connection between the restoration and cavty wall. A load of 500N was applied vertically at the first node from the lingual slope of the buccal cusp tip. This study analysed the displacement, 1 and 2 direction normal stress and strain with FEM software ABAQUS Version 5.2 and hardware IRIS 4D/310 VGX Work-station. The results were as followed. 1. Rodda's cavity form model showed greater amount of displacement with other two models. 2. The stress and strain were increased on the distal marginal ridge and buccopulpal line angle in Rodda's cavity form model. 3. The stress and strain were increased on the central groove and a part of distal marginal ridge in simple proximal box model and proximal box model with retention grooves. 4. With Gap option, Rodda's cavity form model showed the greatest amount of the stress on distal marginal ridge followed by proximal box model with retention grooves and simple proximal box model in descending order. 5. With Gap option, simple proximal box model showed greater amount of stress on the central groove with proximal box model with retention grooves. 6. Retention grooves in the proximal box played the role of supporting the restorations opposing to loads.
When restoring a tooth, the dentist tries to choose the ideal material for existing situation. One criterion that is considered is its suitability for restoring coronal strength. As more tooth structure is removed, the cusps are weakened and susceptible to fracture. Further, this increased deformation may cause the formation of intermittent gaps at the margin between the hard tissue and the restoration, facilitating marginal leakage. The improvements in ceramic materials now make it possible for alternatives to amalgams, composites, and cast metal to be of offered for posterior teeth. Of the materials used, ceramics most closely approximates the properties of enamel. The introduction of computer-aided design/computer-aided manufacture(CAD/CAM) systems to restorative dentistry represents a major technological breakthrough. It is possible to design and fabricate ceramic restorations at a single appointment. Additionally, CAD/CAM systems eliminate certain errors and inaccuracies that are inherent to the indirect method and provide an esthetic restoration. The aim of this investigation was to study the loading characteristics of CAD/CAM ceramic inlay and to compare the stress distribution and displacement associated with different designs of cavity(the isthmus width and cavity depth). A human maxillary left first premolar was prepared with standard mesio-occlusal cavity preparation, as recommended by the manufacturer Ceramic inlay was fabricated with CEREC 2 CAD/CIM equipment and cemented into the prepared cavity. Three dimensional model was made by the serial photographic method. The cavity width was varied $\frac{1}{3}$, $\frac{1}{2}$ and $\frac{2}{3}$ of intercuspal distance between buccal and lingual cusp tip. The cavity depth was varied 1.5mm and 2.3mm. So six models were constructed to simulate six conditions. A point load of 500N was applied vertically onto the first node of the lingual slope from the buccal cusp tip. The stress distribution and displacement were solved using ANSYS finite element program(Swanson Analysis System). (omitted)
청소년기 외상으로 인한 상악 전치부의 복잡 치관 파절은 기능적, 심미적 문제를 야기할 수 있다. 치수 노출을 동반한 치관 파절은 남아있는 치질의 양에 따라 다양한 수복 방법을 고려할 수 있다. 직접 레진 수복은 가장 일차적으로 고려되는 전통적이고 효과적인 방법이나 시간이 지남에 따라 변색되고 깨질 가능성이 높다. 전장관 수복은 성장기의 치아 이동으로 변연 부조화에 의한 재수복 가능성이 높으며 광범위한 치관 파절 시 주로 시행하는 포스트 수복은 치근 천공 및 치근 파절의 위험성을 증가시킨다. 그러나 Endocrown은 치수강 공간으로부터 유지력을 얻을 수 있는 일체형 구조로 생체역학적 측면에서 효과적인 재건이 가능하고 기능과 심미성의 회복에 유리한 이점을 제공한다. 따라서 Endocrown은 청소년기 외상으로 인한 복잡 치관 파절의 수복 방법으로 고려할 수 있다.
Adhesion of composite resin to tooth structure has been of tremendous signgicance in clinical dentistry. Due to the lack of adhesion between composite restorative resins and enamel and dentin, microleakage occurs at the tooth/restoration interface. This may lead to discoloration, secondary caries, marginal breakdown, postoperative sensitivity, and even pulpal pathology. According to extensive use of composite resin, every effort on improving bonding strength and reducing microleakage between a tooth and composite resin has been continued. This study was conducted to determine the difference in microleakage in enamel and dentin treated with air-abrasion, acid etching and combination when restored with composite resin. Class V cavities were prepared on 30 premolars. The specimens were divided into following groups. group 1:air-abrasion+Scotchbond Multi-purpose group 4 :air-abrasion+All-Bond 2 group 2:acid etching+Scotchbond Multi-purpose group 5 :acid etching+All-Bond 2 group 3:combination+Scotchbond Multi-purpose group 6 :combination+All-Bond 2 #combination:air-abrasion + acid etching The specimens were filled with Z-100 after application of Scotchbond Multi-purpose and All-Bond 2. Thermocycling was conducted by alternately dipping the specimens in $5^{\circ}C$ and $55^{\circ}C$ water for 30 seconds 500 times. 1% methylene blue was applied and the specimens were left for 24 hours at $37^{\circ}C$. After washing out the dye, the tooth was sectioned buccolingually along the axis. The sectioned surface was observed with stereoscope for dye penetration. The author has measured the microleakage in teeth prepared with air-abrasion, acid ethching and combination to study the difference in microleakage following different methods of tooth surface treatment and has come to following results. 1. In comparing microleakage between groups, group 1 and 4 showed statistically significant difference from group 2, 3, 5 and 6(p<0.05). There was no significant difference among group 2, 3, 5, 6(p>0.05) nor between group 1 and 4(p>0.05). 2. In comparing microleakage among tooth surface treatment methods, Air-abrasion group showed significantly more microleakage than acid etching group and combination(airabrasion + acid etching) group(p<0.05). Combination(acid etching+air-abrasion)group tended to show lesser microleakage than acid etching group, but this was not statistically significant(p>0.05). 3. In comparing microleakage between bonding agents, there was no statistically significant difference between Scotch bond Multi-purpose and All-Bond 2(p>0.05).
