Changes in Nutritional Status according to Biochemical Assay, Body Weight, and Nutrient Intake Levels in Gastrectomy Patients (위절제 환자들의 혈액의 생화학적 지표, 체중 및 영양소 섭취 변화에 관한 추적 연구)

  • The purpose of this study was to investigate the changes in nutritional status of gastrectomy patients. The anthropometric and biochemical data were measured at pre-operation, at discharge, 1 month after discharge, and 3 months after discharge. Nutrient intake levels, nutrients adequacy ratio (NAR), mean adequacy ratio (MAR), and the proportion of patients with intake levels inferior to those of dietary reference intakes (DRIs) were analyzed at discharge, 1 month after discharge, and 3 months after discharge. Finally, the data on 23 patients (15 male and 8 female) were collected and used for statistical analysis. Fifteen patients underwent subtotal gastrectomy, and 8 patients underwent total gastrectomy. Compared to pre-operation, body weight and body mass index of subjects significantly decreased at discharge, 1 month after discharge, and 3 months after discharge (P<0.001). Serum albumin (P<0.001), total lympocyte count (P<0.001), total cholesterol (P<0.001), hemoglobin (P<0.001), hematocrit (P<0.001), and mean corpuscular hemoglobin concentration (P<0.05) were significantly different between pre-operation, at discharge, 1 month after discharge, and 3 months after discharge. The proportions of patients with lower nutrient intake levels than DRIs were substantial. MAR at discharge, 1 month after discharge, and 3 months after discharge were 0.70, 0.80 and 0.91, respectively. Especially, the NARs of folate, niacin, vitamin $B_2$, vitamin C, and zinc were all low. Considering the various nutritional problems of gastrectomy patients, systematic medical nutrition therapy is needed after gastrectomy.

Assessment of the Dietary Adequacy of Non-Insulin Dependent Diabetes Mellitus in Daegu Area (대구지역 인슐린 비 의존형 당뇨병 환자의 영양섭취 적정도 평가)

  • To assess the dietary therapy compliance of non-insulin-dependent diabetes mellitus (NIDDM) patients living in Daegu, we evaluated diet adequacy levels by index of nutritional quality (INQ), nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and dietary variety score (DVS) with food exchange system and Korean recommended dietary allowances (KRDA). One day dietary intake was measured by 24 hour recall method for 229 subjects over 20 years of age. Average daily energy intake was 1444 kcal. The relative ratio of carbohydrate, protein and fat in terms of energy intake was 69.4:14.5:16.0. The nutrient adequacy ratio (NAR) for Vitamin A, B$_2$, calcium were lower than 0.5 and MAR was 0.65. It appeared that the consumption of each food group as compared to prescribed food exchange unit was insufficient, in the order of dairy group (10.2 $\pm$ 25.2%), fruits group (58.2 $\pm$ 71.2%) and oils & fats group (42.9 $\pm$ 42.0%). The dietary variety score (DVS) was evaluated as an useful tool for diabetes' meal management when we accept minimum intake as 0.3 of food exchange unit. We found significant correlation between food groups and anthropometric indices: cereals group and % total body fat (r = 0.251, p < 0.01), meat group and waist circumference (r = 0.241, p < 0.01), vegetables group and WHR(r = 0.139, p < 0.05), and oils poop and WHR(r = 0.165, p < 0.05). from these results, we concluded that overall status of dietary intake of NIDDM patients in Daegu area was insufficient not only in quality but also in quantity. It is suggested that nutrition education for NIDDM patients should stress on balanced food consumption to meet proscribed amount by six food group to improve the inadequacy of dietary status.

Differential Absorption Analysis of Nonmagnetic Material in the Phantom using Dual CT

  • This study evaluates the change of computer tomography (CT) number in the case of the metal artifact reduction (MAR) algorithm, using the phantom. The images were obtained from dual CT using a gammex 467 tissue characterization phantom, which is similar to human tissues. The test method was performed by dividing pre and post MAR algorithm and measured CT values of nonmagnetic materials within the phantom. In addition, the changes of CT values for each material were compared and analyzed after measuring CT values up to 140 keV, using the spectral HU curve followed by CT scan. As a result, in the cases of N rod (trabecular bone) and E rod (trabecular bone), the CT numbers decreased as keV increasing but were constant above 90 keV. In the cases of I rod (dense bone) and K rod (dense bone), the CT numbers also decreased as keV increased but were uniform above 90 keV. The CT numbers from 40 keV to 140 keV were consistent in the cases of J rod (liver), D rod (liver), L rod (muscle), and F rod (muscle). For A rod (adipose), G rod (adipose), B rod (breast) and O rod (breast), the CT numbers increased as keV increased but were constant after 90 keV. The CT numbers from 40 keV to 140 keV were consistent in the cases of C rod (lung (exhale)), P rod (lung (exhale)), M rod (lung (inhale)) and H rod (lung (exhale)). Conclusively, because dual CT exhibits no changes in image quality and is able to analyze nonmagnetic materials by measuring the CT values of various materials, it will be used in the future as a useful tool for the diagnosis of lesions.

Effects of Nutrition Education and Exercise Intervention on Health and Diet Quality of Middle-Aged Women (영양교육과 운동중재가 중년여성의 건강관련 요인과 식사의 질에 미치는 효과)

  • This study was conducted to investigate effects of nutrition education and exercise intervention on health and diet quality of middle-aged women. The subjects were 209 women who wanted to involve in the health promotion program run by Jincheon-Gun Health Center. Nutrition education was given once a week and dance sports were done twice a week for 3 months. Nutrient intakes were assessed by 24-hour recall method and food records. Dietary habits and nutrition knowledge were investigated by self-administered questionnaires before and after the intervention program. Body and blood compositions were measured before and after the program. The nutrient adequacy ratio (NAR) was significantly increased in energy, protein, vitamin A, vitamin B2 and calcium, mean adequacy ration (MAR) was significantly increased (p < 0.05), and also the index of nutritional quality (INQ) was significantly increased in protein (p < 0.01). Percent (%) RIs of protein (p < 0.01) and calcium (p < 0.05) were significantly increased and body weight (p < 0.001), body fat (p < 0.01), soft lean mass (p < 0.01) and BMI (p < 0.001) decreased. Over the 3-month study, there was a significant trend toward higher HDL-C and albumin levels and lower LDL-C level (p < 0.001). The results showed that nutrition education and exercise intervention could improve the body health and the diet quality.

QUALITATIVE PROPERTIES OF WEAK SOLUTIONS FOR p-LAPLACIAN EQUATIONS WITH NONLOCAL SOURCE AND GRADIENT ABSORPTION

  • We consider the following Dirichlet initial boundary value problem with a gradient absorption and a nonlocal source $$\frac{{\partial}u}{{\partial}t}-div({\mid}{\nabla}u{\mid}^{p-2}{\nabla}u)={\lambda}u^k{\displaystyle\smashmargin{2}{\int\nolimits_{\Omega}}}u^sdx-{\mu}u^l{\mid}{\nabla}u{\mid}^q$$ in a bounded domain Ω ⊂ ℝN, where p > 1, the parameters k, s, l, q, λ > 0 and µ ≥ 0. Firstly, we establish local existence for weak solutions; the aim of this part is to prove a crucial priori estimate on |∇u|. Then, we give appropriate conditions in order to have existence and uniqueness or nonexistence of a global solution in time. Finally, depending on the choices of the initial data, ranges of the coefficients and exponents and measure of the domain, we show that the non-negative global weak solution, when it exists, must extinct after a finite time.

A Study on the Evaluation of Nutrient Intake of Middle School Students in Kangneung (강릉지역 중학생의 영양소 섭취평가에 관한 연구)

  • To assess the nutrient intakes and diet quality of middle school students living in Kangneung area a dietary survey using th 3-day food record was conducted with 226 subjects. Mean daily energy intake was 2114$\pm$502 kcal with 61.6% of energy supplied by carbohydrate 16.4% by protein and 22.0% by fat. As well as insufficiencies in calcium and iron intakes the bioavailability of calcium and iron consumed is considered to low because most of them came from plant origins. For calcium iron vitamin A and vitamin B$_2$the proportions of subjects with intake levels less than 75% of RDA were all over 46%. The mean adequacy ratio(MAR) an index of overall dietary quality was 0.70 for males and 0.73 for females. The indexes of nutritional quality (INQ) were over 1 for most nutrients except 4 nutrients; 0.48 of calcium 0.64 of iron 0.73 of vitamin A and 0.79 of vitamin B$_2$. Nutrient intake levels less than 75% of RDA may be appropriate as a criteria for insufficiency of nutrient intakes. Based on these results it is evident that some of middle school students in Kangneung area did not consume enough nutrients especially calcium iron and vitamin A.

Comparative Analysis and Evaluation of Dietary Intakes of Koreans by Age Groups: (1) Nutrient Intakes (서울 및 근교에 거주하는 한국인의 연령별 식생활 비교 및 평가: (1) 영양소 섭취 비교)

  • It is necessary to compare the dietary characteristics among different age groups because different nutritional problems require different dietary applications. This study was performed to identify the nutritional characteristics of Korean diet among different age groups. The study subjects consisted of preschool children(PC), elementary school children(EC), high school students(HS), college students(CS), and adults over 30 years old living in Seoul and the surrounding areas. The subjects over 30 years old were classified to 30-49 years, 50-69 years, and over 70 years. A dietary survey was conducted using 24-hour recall method and data were collected from 2392 subjects. Energy intake was as much as 1771kcal for PC, 2139kcal for EC, 2475kcal for HS, 2276kcal for CS, 1860kcal for 30-49 years, 1715kcal for 50-69 years, 1326kcal for over 70 years. The subjects of PC, EC, HS, and CS groups got about 25% of energy from fat, but the subjects in 30-49 years, 16%, those in 50-69 years, 15%, and those over 70 years, 13%. The energy consumption from protein was about 15% in each age group. Percent of energy intake as Korean RDA was greater than 100% for PC, EC, HS, and male CS groups. The percent intake for RDA of calcium, iron, vitamin A, and vitamin B$_2$was low in most age group. Iron intake was inadequate in female of PC and CS groups, and subjects in 30-49 years and over 70 years. Mean adequacy ratio(MAR) was 0.9 in PC, and decreased with increase of age. Therefore MAR did not reach to 0.7 among subjects over 70 years. Index of nutritional quality(INQ) for calcium, iron, vitamin A, and vitamin B was below 1 in most age groups. From these results, major nutritional problems of each age group were overall inadequate intakes of calcium and iron, excess consumption of protein and fat for PC, EC, HS, and CS groups, and inadequate nutrients intake of subjects over 70 years old. Effects of Nutrient Intake of College Women in the Daegu Area on Body Content and Bone Mineral Density (대구지역 여대생의 영양소 섭취 상태가 체성분 및 골밀도에 미치는 영향)

  • Body content bone mined density and nutrient intake status of 129 college women in Daegu area are analyze4 The mean age was 23.1 years. Bone mineral density was measured in their right heel by SONOST-2000 ultrasound bone densitometer, and nutrient intake was assessed by a Questionnaire. Diet intake data were obtained by using a 24 hours recall method to evaluate the subject's usual diet. The average energy, Ca, Ee, Zn and folate intakes were 64.2, 47.5, 54.7, 62.9 and $57.4\%$ of RDA, respectively. Ca/P and Ca/Protein ratio were 0.46 and 6.7, respectively. The mean adequacy ratio(MAR), an index of overall dietary quality wag 0.6. The index of nutritional quality was under 1.0 for protein(0.7), vitamin A(0.9), E(0.7), $B_1(0.8)$, niacin(0.8), $B_6(0.6)$ and phosphorus(0.6). The less the protein and energy intake, the less the bone mineral density(p<0.05). The intakes of protein(p<0.05) and that of phosphorus(p<0.001) were positively correlated with bone quality index(BQI). The calcium intake was positively correlated with Z-score(p<0.05). The phosphorus intake was positively correlated with T-score(p<0.001) and Z-score(p<0.0001). In conclusion, this study indicates that nutrient intake of the college women is considerably lower than the RDA. Also the BQI is lower than standard.

A Study on Nutritional Intake of the Rural People in Korea- Comparison of the Nutritent Intake by Areas and Age - (농촌지역 주민의 영양섭취 실태 조사 (I) -지역별.연령별 영양섭취 실태 비교-)

  • This study was conducted to investigate the nutritional intake of people in rural areas using 24-hour recall method. Kyungki Yeoju, Kangwon Whengsung, Chungbuk Chungju, Kyungnam Ulju, and Junbuk Jungup were selected for this study. The results were as follows 1) The mean daily energy intake was 2055.7kcal for male, 1703.2kcal for female. Fat intake were 43.0g for male, 32.7g for (female and protein intake was 74.5g for male 61.5g for female. Nutrient intake of Ulju residents were the highest and that of Whengsung and Yeoju residents were the lowest. Nutrient intake decreased significantly in elderly over 70 years. 2) Nutrients consumed below 80% of Korean RDA were Ca, Vit A, riboflavin and Vit E. Ulju residents consumed significantly higher protein, Ca, P compared to other area residents. P, Fe, Vit C intake of adults male were significantly higher than those of elderly over 80 years. Vit A, thiamin and riboflavin consumption of adults female were also significantly higher than those of elderly over 80 years. Ca, Vit A, riboflavin and Vit I were consumed below the Korean RDA in both male and female. 3) Average CPF ratio of energy intake was 69.2 : 14 : 16.8. Energy intake ratio from carbohydrate was significantly lower in Ulju residents compared to Yeoju and Jungup residents. Energy intake ratio from protein was significantly higher in Ulju area than the other areas. Carbohydrate dependency was increased with age, meanwhile fat dependency was decreased with age. 4) The mean adequacy ratio(MAR), an index of overall dietary quality, were significantly different among areas : the highest was 0.81 in Ulju, the lowest was 0.65 in Whengsung. Adults of 20-49 years showed the highest MAR of 0.76 and the lowest MAR was 0.52 in over 80 years group. The indexes of nutritional quality(INQ) was higher in 01;u area than the other areas, and INQ showed the tendency to decrease with increasing age. Especially, there were significant decrease in INQ of all nutrients except Vit A with age. Based on these results, it is evident that people in rural area did not consume enough nutrients. Specially, dietary intake of Vit A, Vit B$_2$, Vit E and Ca were not adequate. (Korean J Nutrition 31(9) : 1468-1480, 1998)

도시저소득층노인들의 급식유무에 따른 영양상태 비교

  • 우리나라 급식서비스프로그램에서는 도시저소득층 재가노인을 대상으로 사회복지관등을 통하여 몇 개의 지정된 장소에서 1일 1끼니(점심정도)의 집단급식서비스(congregate site service)를 제공하고 있다. 도시저소득층노인들의 경우 영양소 섭취상태가 매우 저조한 것으로 나타나 이러한 급식서비스는 노인들의 영양상태에 많은 영향을 끼칠 것으로 생각된다. 따라서 본 연구에서는 서울 수서지역에 거주하는 노인 273명(남: 61명, 여: 212명)중 적어도 최근 1년 동안 일주일에 5회 이상의 식사에 참여한 군을 급식춘(N=137)으로 하고 나머지를 비급식군(N=136)으로 하여 영양상태를 비교하였다. 급식군은 비급식군에 비해 독거노인의 비율이 유의하게 높았으며 교육수준이 낮고 식품비 사용액수가 적었다. 24시간 회상법으로 조사한 결과 급식군은 비급식군에 비해 동물성단백질(p < 0.001), 동물성지방(p < 0.01), 동물성칼슘(p <0.01), 동물성 철분(P <0.001), 아연(p <0.01), 비타민B$_2$ (p < 0.001), 비타민B$_{6}$ (p < 0.01), 콜레스테롤(p <0.001)섭취량이 유의하게 많은 것으로 나타났으며 MAR도 유의하게 높았으나(P <0.01) 급식, 비급식군의 MAR이 각각 0.59, 0.53으로 모두 식사의 질이 낮았다. 급식군은 점심 급식을 통하여 하루 총섭취량 중 에너지는 39.7%, 당질33.7%, 단백질 43.8%, 지방 52.0%, 칼슘 35.2%, 철분은 36.9%, 비타민E 55.7%를 섭취하고 있어 하루 총섭취량의 33-56%를 급식에 의존하고 있었다. 반면에 비급식군은 하루 총섭취량 중 18-31%의 영양소를 점심으로 섭취하고 있어 대조를 보였다. 급식군은 비급식군에 비해 키가 유의하게 작은 것(p<0.001)외에는 다른 신체 계측치에서는 유의차가 없었다. 급식군은 혈중 철분지표 중에서 유의하게 낮은 MCH(P < 0.05)와 유의하게 높은 TIBC를 보였으나(p <0.05) hemoglobin 수치에 의한 빈혈비율(여 < 12.0d/㎗, 남 < 13.5g/㎗)은 급식, 비급식군 각각 20.2%, 23.8%로서 유의한 차이가 없었으며 12문항을 사용하여 조사한 우울점수에서도 군 간의 유의한 차이가 없었다. 이상으로 보아 급식군은 비급식군에 비해 사회 경제적 수준이 낮으나 점심급식을 통하여 하루 필요영양소의 1/3-1/2를 섭취함으로써 비급식군에 비해 영양소섭취량이 높거나 비슷한 섭취량을 유지하고 있었으며 신체계측치와 빈혈비율 등에서 급식군에 비해 유의한 차이를 보이지 않았다. 그러나 급식과 비급식군 모두에서 MAR이 낮아 아직도 대부분의 섭취영양소가 권장량의 75%미만으로 나타나 저소득층노인에게서 급식의 확대와 함께 급식의 질을 좀 더 높일 수 있도록 정부와 자치단체에서 의 각별한 관심과 지원이 절실히 필요하다고 생각된다.
