Functional MR Imaging Using BOLD Technique in Patients with Brain Tumors (뇌종양 환자에서 BOLD 기법을 이용한 기능적 자기공명영상)

  • Purpose : To reveal clinical usefulness of functional MRI (fMRI) using sensorymotor and language stimuli for demonstrating anatomic relationship between sensorimotor or language cortices and lesions in the planning of brain tumor surgery. Materials and Methods : This study included 12 right-handed patients with brain tumors in or around sensorimotor or language cortices. Eleven patients were evaluated with primary motor and sensory stimuli. Of these patients, six patients were also evaluated with language stimuli. One patient was evaluated with language stimuli only. For fMR imaging, a 1.5T scanner was used and the EPI BOLD technique was employed. For postprocessing image, the SPM99 program and a program made by our department was utilized. We evaluated whether sensorimotor and language stimuli activate sensorimotor and language cortices. And also, clinical efficacy of revealing anatomic relationship between cerebral cortices and lesions for planning neurosurgical operation were evaluated. Finally, we compared post-operative neurologic function with pre-operative neurologic function in same patients. Results : The fMRI examination was successful in identifying the functional cortices and depicting anatomic relationship between functional cortices and lesions in all patients. In nine patients of 11 patients with identified sensorimotor cortices, postoperative grade of manual motor test was not changed, compared with preoperative grade. Whereas postoperative improved than preoperative grade in one patient of remaining two patients, postoperative aggravated than preoperative grade in the other. This result was due to atherosclerotic lacunar infarction, regardless of tumor resection. Postoperative deficit of language function was not found in seven patients with identified language cortices. Conclusion : fMRI could be a helpful method for determining the best approach to neurosurgical treatment in patients with brain tumors in or around sensorimotor or language cortices.

A Study on Anastomotic Complications after Esophagectomy for Cancer of the Esophagus : A Comparison of Neck and Chest Anastomosis (식도암 수술후 문합부 합병증에 관한 연구 - 경부문합과 흉부문합 간의 비교-)

  • Background: Leakage, stricture formation, and tumor recurrence at the anastomotic site are serious problems after esophagectomy for cancer of the esophagus or cardia. The prevalence of these postoperative complications may be affected by whether an anastomosis is made in the neck or in the chest, therefore a comparison was made between anastomoses made at these two sites. Material and Method: Between 1987 and 1998, 36 patients with cancer of the esophagus underwent transthoracic esophagectomy with cervical(NA, n=20) or thoracic anastomosis(CA, n=16). The tumors were staged postoperatively(stage IIA, n=13; s tage IIB, n=7; stage III, n=16) and were located in the middle thoracic(n=22) or lower thoracic esophagus and cardia(n=14). Result: The overall operative mortality was 8.3%(5% for NA group, 12.5% for CA group). The anastomotic leak rate for the NA group was 15.0% and 12.5% for the CA group. The anastomotic leak rate differed according to the manual(27.3%) or stapled(8.0%) techniques(p < 0.05). The median proximal resection margins in the NA and CA groups were 9.6 cm and 5.8 cm, and the corresponding rates of anastomotic tumor recurrence were 5.3% and 28.6%(p < 0.05). The prevalence of benign stricture formation (defined as moderate/severe dysphagia) was higher in the NA group(36.8%) than in the CA group(21.4%). When an anastomosis was made by the stapled technique, smaller size of the staple increased the prevalence of stricture formation - 41.7% with 25-mm staple and 9.1% with 28-mm staple(p < 0.05). Conclusion: Wider resection margin could decrease the anastomotic tumor recurrence, and the stapled technique could decrease the anastomotic leak. The prevalence of benign stricture was higher in the cervical anastomosis but the anastomotic leak and smaller size(25-mm) of the staple should be considered as risk factors.

  • The purpose of this study was to compare the shaping ability of the three $ProTaper^{(R)}$ instrumentation techniques in simulated canals. Thirty resin blocks were divided into 3 groups with 10 canals each. Each group was instrumented with manual $ProTaper^{(R)}$ (Group M), rotary $ProTaper^{(R)}$ (Group R), and hybrid technique (Group H). Canal preparation time was recorded. The images of pre- and post-instrumented root canals were scanned and superimposed. The amounts of canal deviation, total canal width, inner canal width, outer canal width and centering ratio were measured at apical 1, 2, 3, 4, 5 and 6 mm levels 1. Canal preparation time was the shortest in R group (p < 0.05). 2. The amounts of total canal width in R group was generally larger than the other groups, but no significant differences were observed except at the 1, 3 mm levels (p > 0.05) .3. The amounts of inner canal width in R group was larger than M group at the 1 mm level and H group was larger than R group at the 6 mm level (p < 0.05). The amounts of outer canal width in R group was larger than H group only at the 1 mm level (p < 0.05). 4. The direction of canal deviation in H, R group at the 1, 2, 3 mm levels was outward and that in M group at the 1, 2 mm levels was inward. The amounts of canal deviation in H group was larger than R group at the 6 mm level (p < 0.05). 5. The amounts of centering ratio in H group was larger than R group at the 6 mm level (p < 0.05).

  • It needs to put into practice the systems analysis in the analysis of some operations and status of library for the purpose of systematizing the work of reforming in the new easier form to process, to storage, to retrieve and to make use of the increasing informations and data of library. In this study, some of systems which are generally using in every library was caught in the case study of K university library. Having analyzed them with the two methods of the flowcharting and mathematical analysis, we found the obstructive factors in operation. As the result of this research, it was gained the new system as the alternative one. A. Alternative System B. Advantages of alternative systems 1. In the reference room When it converts the present system into the new system, it can profit 6.771 won/user (13.815won-7.044won=6.771 won). Therefore, a half the average required cost of the present system can be saved. If this saving would be alloted for the cost 33,000won required to make the cataloging cards, it would be taken for 94 days (33,000 won ${\div}$ 6,771 won/user=4,874users. 4,874users ${\div}$ 52users/day=94days) to get it. The saving cost/year by the new system will be 95,417 won, and in the first year the initial cost (33,000won) reduces the saving cost to 62,417won. 2. In the periodical room The average required time for using the materials of the present system is 17 minutes/user and the average required cost/user is 23.775won, while the average required time of the new system is 4 minutes and the average required cost/user is 5.33won. Therefore, the new system has profit 4 times of the present system. Accordingly, it occurs when the dispersed periodical materials get together. 3. In the classification and cataloging When one processes - the oriental books - by the Linear Programming Technique, the maximum of the process can be increased from 11.6 volumes per librarian of the present system to 12 volumes per librarian of the new system increased 0.4 volume in a day, and cataloging by the manual printer can be shorten from 3 minutes per card of the present system to 1.5 minutes per card of the new system. Consequently, we can complete the other operations (books equipment, updating of cataloging cards, etc.) with 141 minutes which are saved in the course of the afore-mentioned works. 4. In the status of collections The average growth rate of 4 years from 1968 to 1971 is 9.825 %, and that of the purchased materials is 6.2% similar to the advanced nations, but it has the different position from 215,000 volumes by the Standard Degree for Establishment of College and University, and the difference between the total collections 151,671 volumes and Dunns' growth model ($N_t=N_oe^{-at}$) claimed by Leimkuhler 155,297 volumes in 1971 is 3,626 volumes, and for the purpose of compensation the difference, we found the fact that it needs to have the increased budget of 24~30% per year, Thus, if the budget of 24~30 % per year. Thus, if the budget would be increased per year as the rate of the afore-mentioned figure, it would be reached at the Standard Degree for Establishment of College and University in 1975, and thereafter, it can be decreased to the lebel which is able to maintain the growth rate of 5~6% per year.

A Study on Automated Lineament Extraction with Respect to Spatial Resolution of Digital Elevation Model (수치표고모형 공간해상도에 따른 선구조 자동 추출 연구)

  • The lineament is a linear or curved terrain element to discriminate adjacent geological structures in each other. It has been widely used for analysis of geology, mineral exploration, natural disasters, and earthquake, etc. In the past, the lineament has been extracted using cartographic map or field survey. However, it is possible to extract more efficiently the lineament for a very wide area thanks to development of remote sensing technique. Remotely sensed observation by aircraft, satellite, or digital elevation model (DEM) has been used for visual recognition for manual lineament extraction. Automatic approaches using computer science have been proposed to extract lineament more objectively. In this study, we evaluate the characteristics of lineament which is automatically extracted with respect to difference of spatial resolution of DEM. We utilized two types of DEM: one is Shuttle Radar Topography Mission (SRTM) with spatial resolution of about 90 m (3 arc sec), and the other is the latest world DEM of TerraSAR-X add-on for Global DEM with 12 m spatial resolution. In addition, a global DEM was resampled to produce a DEM with a spatial resolution of 30 m (1 arc sec). The shaded relief map was constructed considering various sun elevation and solar azimuth angle. In order to extract lineament automatically, we used the LINE module in PCI Geomatica software. We found that predominant direction of the extracted lineament is about $N15-25^{\circ}E$ (NNE), regardless of spatial resolution of DEM. However, more fine and detailed lineament were extracted using higher spatial resolution of DEM. The result shows that the lineament density is proportional to the spatial resolution of DEM. Thus, the DEM with appropriate spatial resolution should be selected according to the purpose of the study.

  • The quantification analysis of renal scintigraphy is strongly affected by the location, shape and size of region of interest(ROI). When ROIs are drawn manually, these ROIs are not reproducible due to the operators' subjective point of view, and may lead to inconsistent results even if the same data were analyzed. In this study, the effect of the ROI variation on the analysis of renal scintigraphy when the ROIs are drawn manually was investigated, and in order to obtain more consistent results, methods for automated ROI definition were developed and the results from the application of the developed methods were analyzed. Relative renal function, glomerular filtration rate and mean transit time were selected as clinical parameters for the analysis of the effect of ROI and the analysis tools were designed with the programming language of IDL5.2. To obtain renal scintigraphy, $^{99m}$Tc-DTPA was injected to the 11 adults of normal condition and to study the inter-operator variability, 9 researchers executed the analyses. The calculation of threshold using the gradient value of pixels and border tracing technique were used to define renal ROI and then the background ROI and aorta ROI were defined automatically considering anatomical information and pixel value. The automatic methods to define renal ROI were classified to 4 groups according to the exclusion of operator's subjectiveness. These automatic methods reduced the inter-operator variability remarkably in comparison with manual method and proved the effective tool to obtain reasonable and consistent results in analyzing the renal scintigraphy quantitatively.

The Effect of Rain on Traffic Flows in Urban Freeway Basic Segments (기상조건에 따른 도시고속도로 교통류변화 분석)

  • An earlier study of the effect of rain found that the capacity of freeway systems was reduced, but did not address the effects of rain on the nature of traffic flows. Indeed, the substantial variation due to the intensity of adverse weather conditions is entirely rational so that its effects must be considered in freeway facility design. However, all of the data in Highway Capacity Manual(HCM) have come from ideal conditions. The primary objective of this study is to investigate the effect of rain on urban freeway traffic flows in Seoul. To do so, the relations between three key traffic variables(flow rates, speed, occupancy), their threshold values between congested and uncontested traffic flow regimes, and speed distribution were investigated. The traffic data from Olympic Expressway in Seoul were obtained from Imagine Detection System (Autoscope) with 30 seconds and 1 minute time periods. The slope of the regression line relating flow to occupancy in the uncongested regime decreases when it is raining. In essence, this result indicates that the average service flow rate (it may be interpreted as a capacity of freeway) is reduced as weather conditions deteriorate. The reduction is in the range between 10 and 20%, which agrees with the range proposed by 1994 US HCM. It is noteworthy that the service flow rates of inner lanes are relatively higher than those of other lanes. The average speed is also reduced in rainy day, but the flow-speed relationship and the threshold values of speed and occupancy (these are called critical speed and critical occupancy) are not very sensitive to the weather conditions.

Jangdo(Small Ornamental Knives) manufacturing process and restoration research using Odong Inlay application (오동상감(烏銅象嵌)기법을 활용한 장도(粧刀)의 제작기술 및 복원연구)

  • In this research, literature research on the Odong material, mixture ratio, casting method and casting facility was conducted on contemporary documents, such as Cheongong Geamul. Also, a long sword was produced using the Odong inlay technique. The sword reproduction steps were as follows; Odong alloying, silver soldering alloying, Odong plate and Silver plate production, hilt and sheath production, metal frame and decorative elements, such as a Dugup (metal frame), production, Odong inlay assembly and final assembly. For the Odong alloy production, the mixture ratio of the true Odong, which has copper and gold ratio of 20:1, was used. This is traditional ratio for high quality product according to $17^{th}$ century metallurgy instruction manual. The silver soldering alloy was produced with silver and brass(Cu 7 : Zn 3) ratio of 5:1 for inlay purpose and 5:2 ratio for simple welding purpose. The true Odong alloy laminated with silver plate was used to produce hilt and sheath. The alloy went through annealing and forging steps to make it into 0.6 mm thick plate and its backing layer, which is a silver plate, had the matching thickness. After the two plates were adhered, the laminated plate went through annealing, forging, engraving, silver inlaying, shaping, silver welding, finishing and polishing steps. During the Odong colouring process, its red surface turns black by induced corrosion and different hues can be achieved depending on its quality. To accomplish the silver inlay Odong techniques, a Hanji saturated with thirty day old urine is wrapped around a hilt and sheath material, then it is left at warm room temperature for two to three hours. The Odong's surface will turn black when silver inlay remains unchanged. Various scientific analysis were conducted to study composition of recreated Odong panel, silver soldering, silver plate and the colouring agent on Odong's surface. The recreated Odong had average out at Cu 95.57 wt% Au 4.16wt% and Cu 98.04 wt% Au 1.95wt%, when documented ratio in the old record is Cu 95wt% and Au 5wt%. The recreated Odong was prone to surface breakage during manufacturing process unlike material made with composition ratio written in the old record. On the silver plate of the silver and Odong laminate, 100wt% Ag was detected and between the two layers Cu, Ag and Au were detected. This proves that the adhesion between the two layers was successfully achieved. The silver soldering had varied composition of Ag depending on the location. This shows uneven composition of the silver welding. A large quantities of S, that was not initially present, was detected on the surface of the black Odong. This indicates that presence of S has influence on Odong colour. Additional study on the chromaticity, additional chemical compounds and its restoration are needed for the further understanding of the origin of Odong colour. The result of Odong alloy testing and recreation, Odong silver inlay long sword production, scientific analysis of the Odong black colouring agent will form an important foundation of knowledge for conservation of Odong artifact.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm.
