Background: This study was to examine the effect of applying leg exercise using visual informations on improving stroke patient's muscle power, balance and gait. Methods: The subjects of this study were hemiplegic patients by stroke, 30 patients were picked up, who were agreed with this research and were having hospital care for 8 weeks at RS, RB and HH medical centers. The study group was 15 and comparison group was 15, totally 30 patients were confirmed. leg exercise using visual informations was applied to study group, leg exercise without visual informations was applied to comparison group. These were proceeded for 8 weeks (5 days a week, 20 minutes a day), change of muscle power, balance and gait was checked to investigate the effect. Electromyography apparatus was used for checking muscle power, K-BBS (Korean version of Berg Balance Scale) and COP (center of pressure) were used for balance, then Functional Gait Assessment (FGA), timed up and go test (TUG) were used for checking the capacity of gait. Results: Vastus lateralis (p=.002), biceps femoris (p=.000), K-BBS (p=.000), COP (p=.007), FGA (p=.002), TUG (p=.009) are significant difference between the two groups. Conclusion: This study showed that visual informations impact change of muscle activity, balance and locomotor ability in stroke patients.

Objective: The purpose of this study was to investigate the relationship between physical features, strength, function, and upper extremity musculoskeletal pain during rehabilitation of manual wheelchair users with spinal cord injuries. Design: Cross-sectional study. Methods: The degree and frequency of upper extremity musculoskeletal pain were measured in persons with spinal cord injuries using manual wheelchairs with the use of questionnaires. The pain scores of the hand, wrist, and shoulder joints were calculated by multiplying the seriousness and frequency of pain. We collected data on the manual muscle test, Spinal Cord Independent Measure-III, and the Body Mass Index. Statistical analysis was performed by descriptive analysis and Pearson's correlation analysis. Results: A total of 47 patients participated in this study and the neurological level of the injuries ranged from C2 to S5. Pain in the shoulder joints was the most common in persons with tetraplegia and paraplegia. Pain was experienced as mild to moderate, and occurred one or more times a week. Of the 32 persons with paraplegia, the most common area of complaint was the shoulder. Of the 15 persons with paraplegia, the shoulder joints were the most common site of pain. The independence levels of the persons with spinal cord injuries were highly correlated to muscle strength levels (p<0.05). Conclusions: This study investigated upper extremity musculoskeletal pain during rehabilitation of manual wheelchair users with spinal cord injuries and the relationship between physical features, strength, and function. In most persons with spinal cord injuries, pain and frequency of shoulder joints were high and pain levels were also related to functional levels.
Objective: Stroke patients generally have problems with motor function, muscle weakness, and gait.This study was purposed toevaluate the effect of the treadmill training with kinesio taping of tibialis anterior (TKT) on muscle function, tibialis anterior, muscle strength, and gait ability in poststroke patients. Design: A randomized controlled design Methods: The participants were randomly divided in the TKT group (experimental group) and treadmill training with sham kinesio taping (control group), with 21 patients assigned to each group. Both groups receive treadmill training with kinesio taping and sham kinesio taping for 30 minutes per day, five days per week, for four weeks. The motor function was measured using the Fugl-Meyer assessment. A disital manual muscle test and G-walk were used to evaluate ankle dorsiflexor and gait ability.Evaluation was performed baseline and 4 weeks after the experiment. Results: Both groups showed significantly more improvement in muscle function, tibialis anterior muscle strength, cadence, gait velocity, and stridelength in pre-post intervention change(p<0.05).The experimental group showed significantly more improvement in motor function, muscle strength, cadence, gait velocity, and stridelength ability comparedto the control group(p<0.05). Conclusions: These finding show the benefits of treadmill training with kinesio taping for functional recovery in poststroke patients
Purpose : The purpose of this study was to investigate the effect of abdominal muscle strengthening exercise on abdominal muscle strength and respiratory function in stroke patients. Methods : The subjects were 14 stroke patients (10 males, 4 females) hospitalized at W rehabilitation hospital in Busan City and randomly assigned to 7 exercise groups and 7 control groups. Exercise was performed in combination with an upper and lower extremity pattern of proprioceptive neuromuscular facilitation. Measurements of abdominal muscle strength and respiratory function were made before intervention and 4 weeks after intervention. Abdominal muscle strength was assessed using a digital manual dynamometer, and respiratory function was assessed by spirometry. The collected data were analyzed with a paired t-test and independent t-test and the significance level was set as α =.05. Results : The results showed that applying abdominal muscle strengthening exercise to stroke patients showed a significant increase in abdominal muscle strength and a significant difference between groups (p<.05). Maximal-effort expiratory spirogram (MES) readings were significantly increased in forced vital capacity (FVC), and forced expiratory volume in one second (FEV1), in the exercise group, and there were a significant differences between the groups in terms of FEV1 (p<.05). Slow vital capacity (SVC) was significantly increased in vital capacity (VC), tidal volume (TV), inspiratory reserve volume (IRV), and expiratory capacity (EC), and there were significant differences between the groups in VC, TV, expiratory reserve volume (ERV), EC, and inspiratory capacity (IC) (p<.05). Conclusion : Abdominal muscle strengthening exercise was effective in the abdominal muscle strength of stroke patients, and it was confirmed to have a positive effect on the enhancement of respiratory function. Therefore, it seems that exercise programs for stroke patients with respiratory weakness should include abdominal muscle strengthening exercises.
This study was performed to report the effect of oriental medical treatment and general biomechanic manipulation on a patient with upper limb palsy caused by postural brachial plexus injury. The patient was treated with acupuncture, herb medicine and general biomechanic manipulation. The effectiveness of treatment was evaluated with range of motion, visual analogue scale and manual muscle test. After 11 times treatment, motion and muscular force were progressed, pain was decreased. This result suggests that oriental medical treatment and general coordinative manipulation are effective to care the postural brachial plexus injury.
Objectives : The purpose of this study is to show the effectiveness of Chuna Cranio-Sacral Therapy for remaining pain and muscle strength decrease after herniated intervertebral disc surgery. Methods : Two patients with Failed Back Surgery Syndrome(FBSS) were hospitalized and treated by Korean medical treatments with Chuna Cranio-Sacral Therapy. The Range of Motion(ROM) of the lumbar spine and Manual Muscle Test(MMT) were adopted to measure the resulting motor recovery after treatment. And the Numerical Rating Scale(NRS) was adopted to measure changes of pain level. Two patients both had Chuna Cranio-Sacral Therapy category III disorder with short right leg. We applied category III blocking technique everyday for 20 minutes with Korean medical treatments. Results : In both cases, the range of motions were improved and the pain level decreased. And in case 1, muscle strength also showed improvements. Conclusions : We suggest Chuna Cranio-Sacral Therapy is effective for pain decrease and motor recovery of FBSS patients. Further studies are needed to set up an Korean medical protocol for FBSS.
A 36-year-old male patient developed diffuse low back pain. His past medical history was unremarkable and had no family history of neuromuscular disease. He had no bladder and bowel problems. Creatine kinase was 172 U/L (normal < 170). Other fluid and blood chemistry tests were normal. Manual muscle test grades of extremities and sensory examination were normal. Muscle stretch reflexes were normal. Fasciculations and myotonia were not detected. Straight leg raising test was negative. There was no spinal root compression, spinal stenosis, or signal intensity change of spinal cord on magnetic resonance imaging (MRI). Fatty change and atrophy of the cervical, thoracic and lumbar paraspinal muscles were noted on MRI. Nerve conduction studies were normal. Electromyography showed 1+ positive sharp waves in the lumbar paraspinal muscles. Electromyography of upper and lower extremity muscles revealed no abnormal spontaneous activity. We report a rare case of severe paraspinal muscle atrophy with fatty degeneration in a Young Adult.
Objects: This study is designed to analysis improvement and maintaince of trunk stability targeting patients who need post operative rehabilitation exercise after undergoing opened microscopic laser discectomy(OMLD) due to HNP. Method: Between March 2004 and February 2005 a total sixty patients who underwent OMLD due to $L_4/L_5$, $L_5/S_1$ HNP and who agreed to the experiment were subject for this study. Experimental group consisted of 18 subjects, and they underwent 45 minutes dynamic lumbar stabilization exercise And control group consisted of 18 subjects who conducted conservative physical therapy based on the use of physical modality for 45 minutes except to exercise. Results: Experimental group that was lumbar extensor's isometric peak torque, weight distribution of both leg, trunk muscle balance and Oswestry LBP disability index increased during 12 weeks in a statistically significant manner compared to before exercise (p<.05). When re-test was tried, isometric peak torque (p>.05) and Oswestry LBP disability index(p<.05) maintained 12th week level or improved. Weight distribution rate of both leg and trunk muscle balance maintained the level of 8th week of exercise execution(p<.05). Control group that was lumbar extensor's isometric peak torque, weight distribution rate of both leg and trunk muscle balance aggravated during 12 weeks compared to pre-test(p>.05) But, Oswestry LBP disability index increased in a statistically significant level compared to pre-test(p<.05). When re-test was tried, isometric peak torque increased slightly compared to pre-test, but decreased when at least $60^{\circ}$ flexion angle(p>.05). Weight distribution rate of both leg and trunk muscle balance also aggravated compared to pre-test(p>.05), and Oswestry LBP disability index was similar to the 4th week of physical therapy execution(p>.05). Conclusion: Successive postoperative Especially, Application of dynamic lumbar stabilization exercise program is important than traditional lumbar strengthening exercise program in the maintaince of spinal stability.
Background: This study was aimed to determine the effects of deep abdominal muscle exercises (DAME) and thoracic mobility exercises (TME) on pulmonary function. Methods: This study was conducted with 22 college students who are in their 20s and have no problem carrying out activities of daily living. All subjects were randomly assigned to either the DAME group (11) or the TME group (11) to undertake the exercises for 2 weeks. To measure pulmonary function of subjects, forced vital capacity (FVC), forced exploratory volume in 1 second (FEV1) and peak expiratory flow (PEF) were measured using chest graph. Chest expansion of subjects was also measured with tape ruler. These measurements were performed on the first day before the exercise program started and on the next day after the 2-week exercise program was completed. A paired-t test was performed to compare the differences in pulmonary function before and after the exercise program, and an independent t-test was performed to compare the two groups. Results: The results of this study were as follows: 1) In comparison of pre- and post-exercise changes in the DAME and TME groups, both groups showed significant increase in chest expansion and PEF after the exercise program, compared with the baseline data (p<.05). Both groups also demonstrated improvements in FVC and FEV1 after the exercise program, compared with the baseline data. However, the differences were not statistically significant (p>.05). 2) The comparison of the DAME and TME groups revealed no significant differences in chest expansion, FVC, FEV1 and PEF (p>.05). Conclusion: It is therefore concluded that both DAME and TME were effective in improving pulmonary function.
Background: Patients with neck pain develop instability due to muscle imbalance, decreased proprioception, and balance disorders. Studies have examined various exercise methods as treatment methods, but few studies have compared the effects of cervical stabilization exercise and dynamic balance exercise. The purpose of this study was to investigate the effects of dynamic balance exercise on pain, functional level, and psychosocial level in patients with non-specific chronic neck pain. Methods: Thirty-four non-specific chronic neck pain patients were randomly assigned to the experimental group (EG, n=17) and control group (CG, n=17); the cervical stabilization exercise and dynamic balance exercise program were applied to the EG; and only the cervical stabilization exercise program was applied to the CG. The intervention was conducted twice a week, for six weeks. Assessment items evaluated pain, dysfunction (Korean version neck disability index), range of motion, craniocervical flexion test, cervical deep flexor endurance test, and psychosocial level. Data analysis was performed using intention-to-treat analysis as assigned. To analyze differences in the items assessed in the two groups, we used a repeated measures analysis of variance with an interaction between group (EG, CG) and time point (baseline, 6 weeks, 12 weeks). Results: The endurance of the cervical flexor muscles between the group and the measurement point after intervention (p<.05). Both groups showed significantly improved endurance between time points after the intervention (p<.05), with the EG showing a greater change than the CG. None of the other measurement items differed in the pattern of change between measurement points. Conclusion: In conclusion, the EG applying a cervical stabilization exercise and a dynamic balance exercise experienced a significant difference in muscle endurance improvement compared to the CG. We propose an exercise intervention program that includes stabilization exercises and dynamic balance exercises for patients with chronic cervical pain who lack muscle endurance.
