Study about the Causes of Muscle Force Mistake Occurrence from the Upper Limb Lifting Resistance Test in Manual Muscle Test (Manual Muscle Test 중 상지거상저항 검사 시 근력 오류 발생 원인에 대한 고찰)

  • Objectives: We investigated muscle force from the upper limb lifting resistance test to conform the objectivity in manual muscle test. Methods: We made standard method in upper limb lifting resistance test to compare with experiment method switching the lower limb position left & right. And resistance forces of upper limb of subject were checked to inspector with closing eyes. Results: 1. The lifting resistance of right upper limb was stronger when the lower limb of right and left were abducted. 2. The lifting resistance of right upper limb was weaken when the lower limb of right and left were adducted. 3. The lifting resistance of right upper limb was weaken when the lower limb of right and left were elevated. Conclusions: As the above results, the deltoid muscle force checked in the upper limb lifting resistance test is affected by the location of lower limbs, it suggested that the muscle force of some part in the body will be affected by the other parts. It will be useful to understand the symmetry principle of body in muscle function.

  • This study investigates the effects of instrument-assisted soft mobilization (IASTM) and manual myofascial release (MFR) on the muscle activity of the biceps brachii. This study was conducted on 10 men and women in their 20s, and all subjects participated in the experiment for 3 days and measured muscle activity of the upper forearm muscles. On the first day, the muscle activity value was measured before the intervention, and the remaining two days were measured for muscle activity after the intervention of each manual therapy in a random order. All muscle activity values were compared and analyzed through the dependent t test and the independent t test. The before and after comparison of muscle activity values before and after each intervention was verified by performing the dependent t test and comparing the values of muscle activity between groups between each intervention after intervention by performing an independent t test. According to the results of this study, both interventions significantly increased muscle activity of the biceps brachii before and after intervention, and there was no significant difference in muscle activity values between groups after intervention. Therefore, both manual therapy (IASTM, MFR) are thought to be effective in improving neuromuscular control ability.

  • Purpose: To measure the cervical isometric force generation capacity of men and women, IsoTract was used. Also the aim of this study was to compare manual muscle testing with isometric muscle strength in women. Methods: Forty seven healthy subjects(25 men, 22 women; age range, 18-29) were recruited in two sessions, one for flexion and the other extension. The cervical strength was measured using a force measuring device as IsoTract. Results: Cervical isometric strength was found to be measure for flexion(27.78 Ibs maximum and 24.52 Ibs average) and extension(33.22 Ibs maximum and 29.90 Ibs average) in mans. And isometic muscle strength for women exerted significantly higher in the grade 4 over than in the grade 3 in women. Conclusion: We concluded that there were difference in isometric muscle strength for the grade of manual muscle testing. So this findings may assist in the measure of whole body muscle strength.

  • Objectives: It has been reported to continue that temporomandibular joint balancing medicine (functional cerebrospinal therapy; FCST) is effective in treating incurable diseases in clinic recently. FCST is based on the results of restricted cervical rotation test, it means that the results of restricted cervical rotation test has a very high reliability on test results. Methods: This study has the aim to understand restricted cervical rotation test method and to use well with high technical skill. So manual muscle test method which was based on the upper limb lifting resistance test method are compared with restricted cervical rotation test method and had been discussed. Results: Results are that restricted cervical rotation test by using the passive motion of arrested persons (patients) have high the reliability and accuracy. Conclusions: It is concluded that restricted cervical rotation test is the new type of manual muscle tests and the results of test are very high the reliability and accuracy so that the acquirement of test method is helpful in clinic practically.

  • Spasticity has been defined as "a motor disorder characterized by a velocity-dependent increased in tonic stretch reflexes with exaggerated tendon jerks, resulting from hyperexcitability of the stretch reflex, as one components of the upper motorneuron syndrome". Spasticity is one of the common symptoms of stroke patients and frequently interferes with the motor functions such as gait, posture and activities of daily living. Therefore, its management is becoming a major issue in physical therapy. The purpose of this study was to determined the effects of reciprocal inhibition by isometric contraction of pre-tibia muscle on spasticity in hemiplegic patients through Hoffmann reflex. The subjects were consisted 45 patients who had hemiplegia due to stroke. All subjects randomly assigned to 3 group: manual reciprocal inhibition program group(manual group), neuromuscular electrical stimulation group(NMES group) and control group. The manual group received voluntary isometric contraction of pre-tibia muscle. The NMES group received neuromuscular electrical stimulation on tibialis anterior. The control group was not received any therapeutic intervention. Before and after experiments, Hoffmann reflex, M-wave and Modified Ashworth scale was measure in all patients. The data of 30 patients who complete experimental course were statistically analysed. Modified Ashworth scale were significantly decreased after experiment in manual group(p<.01). The Hmax/Mmax ratios were significantly decreased after experiment in manual group(p<.o1). There were no statistical difference between pre-test and post-test with modified Ashworth scale in NMES group(p>.01). There were no statistical difference between pre-test and post-test with Hmax/Mmax ratios in NMES group(p>.01). There were no statistical difference between pre-test and post-test with modified Ashworth scale in control group(p>.01). There were no statistical difference between pre-test and post-test with Hmax/Mmax ratios in control group(p>.01). The present results revealed that reciprocal inhibition which produced by voluntary isometric contraction of pre-tibia muscle can be reduce spasticity of gastrocnemius. Therefore, reciprocal inhibition is useful to improve functional activities in hemiplegic patient. Further study should be done to analyse the effects of intervention duration of reciprocal inhibition, appropriate muscle contraction, optimal time to apply the reciprocal inhibition in more long period.

The Change of Strength of Trunk Flexion and Extension by Intervention of Taping on Abdominal and Back muscles (배부근과 복부근에 대한 테이핑 적용이 체간 굴곡, 신전 근력에 미치는 영향)

  • Purpose : The purpose of this study was to measure the change of strength of trunk flexion and extension on abdominal and back muscles. Methods : Each taping group 1(n=14), taping group 2(n=14), taping group 3(n=14) and control group(n=15) were measured a muscle strength by Biodex at first test, second test, third test and forth test in 3 days. Results : The strength of trunk flexion and extension was significantly different among the experimental groups (p<.05). It also shows that significant increasing of muscle strength at 48 hours after application of taping (p<.05). Conclusion : In conclusion, we were found that taping technique base on taping type show immediately assistance of muscle strength and enduring effect for 48 hours.

Diagnostic Accuracy of Physical Examinations in Impingement Syndrome and Rotator Cuff Tear (충돌 증후군과 회전근 개 파열을 위한 이학적 검사들의 진단적 가치에 대한 분석)

  • Purpose: The aim of this study was to investigate the diagnostic accuracy of Neer, Hawkins provocative tests and supraspinatus manual muscle test for the assessment of impingement syndrome, partial tear and small complete tear of De rotator cuff. Materials and Methods: Seventy-one female and 115 male patients were included in the study. Patients were divided into four groups of no impingement, impingement without tear, partial tear and small complete tear of the rotator cuff, which were confirmed by sonogram, magnetic resonance imaging and surgery. Neer and Hawkins provocative tests and supraspinatus manual muscle test were performed respectively. SAS 6.12 version was used in statistical analysis. Results: We found that Neer test had 94% sensitivity, 54% specificity for impingement without tear and 89% sensitivity, 78% specificity for partial tear and 96% sensitivity, 23% specificity for small tear. Hawkins test revealed 95% sensitivity, 54% specificity for impingement without tear and 93% sensitivity, 78% specificity for partial tear and 100% sensitivity, 23% specificity for small tear. Supraspinatus manual muscle test revealed 27% sensitivity, 94% specificity for impingement without tear and 29% sensitivity, 82% specificity for partial tear and 48% sensitivity, 82% specificity for small tear. Conclusion: Neer and Hawkins tests have high sensitivity, low specificity for impingement syndrome, partial and small tear. Supraspinatus manual muscle test had low sensitivity and high specificity. However this test was not effective to differentiate the partial and small rotator cuff tear. We thought that more effective provocative test should be designed to detect the partial and small rotator cuff tear.

  • Objectives The purpose of this study was to investigate the possibility of spinal mobilization with leg movement (SMWLM) using Korean Medicine treatment on acute lower limb muscle weakness through four case studies. Methods SMWLM, with other Korean Medicine treatments, was performed on four patients who underwent treatment at OOOO Korean Medicine Hospital from August 2017 to July 2018. Additionally, results of the Numerical Rate Scale(NRS), EuroQol-FiveDimensions(EQ-5D),Oswestry lowback pain disability questionnaire(ODI), Straight Leg Raise(SLR) test, and Manual Muscle Test(MMT) were evaluated. Results There was a significant increase in NRS, ODI, and EQ-5D scales with im-rovements of SLR angle and MMT figures to a normal range. Conclusions This study suggested that Korean Medicine treatment combined with SMWLM may affect treating acute lower limb muscle weakness. Further clinical studies are needed to establish a definite conclusion.

  • Background: This study was conducted to investigate the effects of pressure belt during proprioceptive neuromuscular facilitation sprinter pattern training using a Thera-band on leg muscle strength and gait in stroke patients with stroke. Methods: Nine patients with stroke underwent training five times a week for four weeks, and changes in the muscle strength and walking ability of the paralyzed leg before and after training were measured. Muscle strength was measured using a Digital muscle tester, and walking ability was measured using a G-WAKER and the timed up and go (TUG) test. Results: Results showed that the quadriceps, hamstring, tibialis anterior, gastrocnemius, cadence, stride length, and stance phase significantly increased (p<.05). The swing phase, gait cycle duration, and TUG test results significantly decreased (p<.05). Conclusion: This study demonstrated that a pressure belt is a very useful tool for improving muscle strength and walking ability in patients with stroke.

  • Objectives : The present study reports seven foot drop patients with herniated intervertebral lumbar disc treated by Muscle Energy Technique and Korean traditional medicine therapy. Methods : Seven foot drop patients with herniated intervertebral lumbar disc were treated by Muscle Energy Rechnique and Korean traditional medicine therapy and were assessed by Manual Muscle Test, numeric rating scale, and oswestry low back pain disability index. Results : Among seven foot drop patients, two cases showed complete recovery, three cases showed recovery over good grade, and two cases showed recovery over good grade after temporary weakness. numeric rating scale and oswestry low back pain disability index of all cases showed improvement. Conclusions : Seven foot drop patients with herniated intervertebral lumbar disc were treated by Muscle Energy Rechnique and Korean traditional medicine therapy and showed improvement. It is anticipated that this report benefits the future in depth study and clinical treatments on the foot drop symptom in korean medicine.
