A Case Report on the Immediate Effects of Cytoskeletal Manual Therapy on Pain, Muscle Thickness, and Pressure Pain Threshold in a Patient with Scoliosis

  • Objective: Patients with scoliosis complain of various symptoms such as muscle imbalance, dysfunction, back pain, abnormal posture and gait abnormality. The most basic treatment for scoliosis is to observe the progress based on conservative treatment. Therefore, in this case report, the effect of cytoskeletal manual therapy (CMT), a soft tissue mobilization technique, on pain intensity, muscle thickness, and pressure pain threshold (PPT) in a patient with scoliosis was investigated. Design: A case report Methods: A 25-year-old male diagnosed with scoliosis visited the Neuromusculoskeletal Science Laboratory with chronic back pain. In the laboratory, scoliosis was confirmed through the X-ray image used for his diagnosis, and it was confirmed again through Adam's forward bending test. Pain, pressure pain threshold and muscle thickness were measured to compare the immediate effects of CMT applied in the laboratory for 40 minutes. Treatments were visited two weeks after the first visit and outcome measures were assessed after a total of two visits. Results: After receiving CMT up to the second session, the pain intensity decreased by 4 points and the screening angle decreased by 15 degrees. Muscle thickness decreased in all but 10 mm on the dominant side of the thoracic spine. All of the PPTs increased, and the greatest increase was 3.1 lb on the dominant side of the thoracic spine. Conclusions: CMT showed positive improvement in pain during trunk flexion, spinal curvature, muscle imbalance, and pressure pain, which is considered as an ancillary treatment option for scoliosis management.

  • Interest in mannual therapy appears to continue to grow among physical therapists. The term 'mannual therapy' has traditionally been associated with physical therapists who examine and treat Patients who have disorders related to the neuro-muscular-articular system.. The manipulative means skilled and specialised use of manual and/or mechanically applied movement techniques, as a part of comprehensive orthopaedic physical therapy for disorders of the moving parts of body. Physical therapists use a large variety of manually applied examination procedures as part of the clincal decision-making process. The purpose of this article is to define and clarify the basic concepts of mannual therapy by comparing the geneal concepts and evaluation schemes of James Cyriax, John McM. Mennell, Geoffrey Maitland, Freddy Kaltenborn, Robin McKenzie, and Ola Grimsby. This article stresses the importance of developing stills in evaluation. Gimsby's approch is presented as a example of modem mannual therapy.

  • We reviewed literatures of western and oriental medicine about postoperative management of spine. Traditionally, with orthoses and bed rest, many physicians restricted activity of daily life and back exercises of patients who had taken spine surgery. More aggressive early mobilization, however, such as strengthening exercise and stretching after 4 weeks of surgery gets grounds nowadays. Physical therapies including manipulation, TENS, MENS, ultrasound and cryotherapy are being used as helpful treatment modalities of postoperative pain and swelling. Failed back surgery syndrome occurs due to wrong patient, diagnosis and surgery and is managed with conservative treatment or reoperation. In oriental medicine, treatments of musculoskeletal injuries including surgical wound are based on the balance of chi(氣) and blood(血).

  • Background: The purpose of this clinical case report is to confirm sacroiliac joint misalignment effects on the gait. Methods: A healthy women volunteered to participate in this case report. Measurement of the subject was performed two categories. One is physical examination of the pelvic by inspection, palpation, movement based tests and pain provocation tests. The other one is gait performance measurement by 3 dimentional motion analysis. Results: Diagnosis by physical examination of the subject was Lt. ilium posterior rotation. Pelvic, hip, knee and ankle in the 3 dimensional kinematic data, the most notable result was the ankle. Conclusion: To confirm the effect of the misalignment of the sacroiliac joint on the gait function, it must be evaluated by integrating the movement to the ankle from the lumbar.

  • 본 논문에서는 근골격계 질환 중에서 종종 발생하는 어깨 부위의 질환에 대하여 알아보도록 한다. 본 논문에서는 팔을 반듯이 위로 올렸을 때 통증이 발생하는 어깨 질환에 대한 도수치료법을 제시한다. 실험에서는 도수진단을 위하여 팔을 반듯이 위로 올릴 때 팔의 각도를 입력하고, 이상이 발생할 시 진단 및 치료법까지 시스템에서 제시한다.

  • Objective : The Fibromyaigia Syndrome(FMS) is frequently used to call at clinic. There are not case reports on the treatment of FMS by oriental medicine. We got a effect on three patient with FMS by oriental medicine and this case is observed the clinical symptoms and scaled by VAS with chuna and acupuncture. Methods : We used to take chuna and acupunture, though classifcated by pathologic symptom and classification criteria of American College of Rheumatology. Result : Usually, most patients with FMS on the chronic stage have chronic prognosis. But symptoms of the patient on this report were improved after above treatments. Conclusion : This case showed oriental medicine enough could be applied on diseases like this syndrome as one of conservative therapies. Of course, more accurate diagnosis and research would be needed afterwards.

A Clinical Case Study of Osteoarthritis of Knee Joint treated with Traditional Korean Medical Therapy and Core Exercise (한방 치료와 코어 운동을 병행한 퇴행성 슬관절염 환자 1례에 대한 증례보고)

  • Objectives : The objective of this study is to report the effect of Traditional Korean medical therapy and core exercise on osteoarthritis of knee joint by observing a clinical case study. Methods : Patient with a diagnosis of osteoarthritis of knee joint, was treated by acupuncture, herb medicine, physical theraphy and core exercise. Results : After treatment, Visual Analogue Scale(VAS), Korean version of the Western Ontario and McMaster Universities(KWOMAC), Pain Rating Score(PRS) were reduced from 5 to 2, from 25 to 16, from 55 to 27, and CENTAUR test results were improved. Conclusions : This study showed that Traditional Korean medical therapy and core exercise has meaningful effect on osteoarthritis of knee joint, and more researches should be followed.

  • Objectives: This report aimed to provide an introduction to the analysis of the Biolinkage System. Methods: During diagnosis, the Biolinkage System was first classified into Type I & Type II according to the Patric's Test and the Pelvic Twist Analysis; the two types were then sub-classified into Step I, II, III according to the Thompson's sacrum test, Patrick's test, Scapular Fixation Test, and Pelvic Twist Analysis. During treatment, the sacrum-axis-temporomandibular joints were used in Step I, the hip joint-fourth cervical or scapular-occiput were used in Step II, and lastly the thoracic cage is used in Step III. Conclusions: The Biolinkage System is useful in the examination of somatic dysfunction.

  • In patients with a language developmental delay, it is necessary to make a differential diagnosis for autism spectrum disorders (ASDs), specific language impairment, and mental retardation. It is important that pediatricians recognize the signs and symptoms of ASDs, as many patients with language developmental delays are ultimately diagnosed with ASDs. Pediatricians play an important role in the early recognition of ASDs, because they are usually the first point of contact for children with ASDs. A revision of the diagnostic criteria of ASDs was proposed in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) that was released in May 2013. The autism spectrum describes a range of conditions classified as neurodevelopmental disorders in the fifth edition of the DSM. The new diagnostic criteria encompasses previous elements from the diagnosis of autistic disorder, Asperger disorder, childhood disintegrative disorder, and pervasive developmental disorder-not otherwise specified. An additional change to the DSM includes synthesizing the section on social and communication deficits into one domain. In ASD patients, the appropriate behavioral therapies and rehabilitation treatments significantly affect the prognosis. Therefore, this makes early diagnosis and treatment very important. In conclusion, pediatricians need to be able to recognize the signs and symptoms of ASDs and be attentive to them in order to make an early diagnosis and provide treatment.

Color Characteristic on Tongue Image of Malignant Neoplasm Patients (종양환자의 설 색상 특성에 관한 정량적 연구)

  • Tongue Diagnosis is the important traditional oriental medical diagnosis method that observes not only the general physiological state but also some kinds of disease. However, manual tongue diagnosis is much influenced by surrounding illumination. Therefore, Digital Tongue Inspection System(DigiTis) is needed for the quantification of objective tongue information, In this research, Tongue images of 98 malignant Neoplasm patients and 34 normal persons were collected by Digital Tongue Inspection System. Statistical analysis of tongue images and patient data indicates that cancer group has more blue-purple components in tongue body(舌質) and yellow components in tongue coating than normal group. Also, there are a lot of rose-pink components in the cancer group of second stage and blue-purple components in the cancer group of third or fourth stage. Our study shows that tongue image is a useful index for distinction between disease and health. Furthermore we need more extended research through the additional sampling and various disease.
